2.Relationship between the ways of nutritional support and recovery of immunological function after PTCD in patients with malignant obstructive jaundice

Objective To investigate the factors affecting the recovery of immunological function after bilirubin decreasing interventional therapy in patients with obstructive jaundice.Methods 67 patients with malignant obstructive jaundice were investigated.The patients were randomly divided into the enteral nutrition (EN) group (n =34)and the parenteral nutrition (PN) group (n =33).TBIL,DBIL,TSP,ALB,PA,TF,CD3 +,CD4 +,CD8 +,CD4 +/CD8 + were determined before PTCD and at the 7th day after the procedure.Results Except for 20 patients,the levels of TBIL,DBIL,CD8 + significantly declined,and the levels of TSP,ALB,PA,TF,CD3+,CD4+,CD4+/CD8 + significantly increased in the 2 groups of patients.There was no significant difference in the recovery of liver function and immunological status between the PN and the EN groups.Conclusions The recovery of immunological function was significantly associated with reduction of serum bilirubin in patients with malignant obstructive jaundice.There was no relationship with the ways of nutrition support.

Knee osteoarthritis (KOA) is a common degenerative joint disease. Biomechanics of lower extremity plays an important role in KOA. Foot progression angle, with the advantages of non-invasive, convenience, has been attached great importance by people gradually. Changing foot progression angle may effect the knee adduction torque, tibia rotation, muscle activity, which leads to change the load of the knee joint, and alleviate the pain, improve the function of knee joint and the quality of life of the patients.
5.CT findings of melamine caused urinary calculi

Objective To analyze the CT findings of melamine induced urinary calculi.Methods Nineteen children with a history of ingestion of melamine contaminated infant formula milk were studied, including 12 males and 7 females, age ranged from 50 days to 5 years.Results CT demonstrated renal pelvic and ureteral stones in 13 cases, with urinary obstruction in 9 of them.The size of the stones ranged from 0.3 cm × 0.3 cm to stag-horn calculus.The density of the stones measured from a low of 40-70 HU up to a high of 410 HU with an average density of 160 HU.Conclusion CT scan is an excellent modality in demonstrating urinary tract calculi caused by melamine. It is the method of choice when ultrasound examinations are equivocal.

In addition, the indicators system and appraisal model were called into play to explore the use of hospital brands, offering scientific references for hospital management.
7.Follow-up of vertebral artery stent in origin segment by color Doppler flow imaging

Objective To evaluate and follow-up the effection of vertebral artery origin stenting (VAOS) by color Doppler flow imaging(CDFI) and assess the rate of restenosis after stenting. Methods One hundred and thirty-five patients with stenosis of vertebral artery origin segment underwent stent input,135 pieces of stent was input in vertebral artery origin segment with moderate or severe stenosis,in which 70 bare metal stent(BMS) and 65 drug-eluting stent(DES). The diameter of VAOS, peak systolic velocity (PSV),end diastolic velocity (EDV) and resistence index(RI) of segment at origin and cervical vertebral were evaluated before and every 1,3,6, 12 month after VAOS by CDFI. The incidence of restenosis were calculated,and the factors of restenosis were analysed by COX regression. Results The diameter of VAOS was improved from (1.20±0.38) mm to (2.61±0.49) mm after stent procedure ( P = 0. 000), PSV and EDV also decreased from (296.02 ± 113.86)cm/s to ( 113.47 ± 36.35 )cm/s and (90.08 ± 47. 59)cm/s to (32. 21 ± 12. 69)cm/s respectively(P=0.000). The PSV and RI in cervical segment were increased from (46. 88 ± 17.46)cm/s to (67.79 ± 24.31 ) cm/s and 0. 54 ± 0. 10 to 0.62 ± 0.09 respectively( P = 0. 000).Over a median 7 months follow up(range 1 to 12 months) ,the cumulative restenosis rate at 3,6,12 month were 7.9% ,16. 9% and 25.0% respectively. DES was the only one negtive predictor of restenosis(OR=0. 388,95% CI:0.162-0.931, P = 0.034),and the factor of residual stenosis contributed to the occurance of restenosis after stenting(OR = 3.758,95% CI:1.498-9.427, P=0.005). Conclusions CDFI is a sensitive and noninvasive examination to follow-up VAOS and detect in-stent restenosis immediately. VAOS has a high rate of restenosis. DES is effective to prevent in-stent restenosis.
8.Optimized expression, preparation of human papillomavirus 16 L2E7 fusion protein and its inhibitory effect on tumor growth in mice.

HPV16 L2E7 is a fusion protein used for therapeutical vaccine targeting HPV virus. To increase its expression in Escherichia coli, we optimized the codon usage of HPV16 l2e7 gene based on its codon usage bias. The optimized gene of HPV16 sl2e7 was cloned into three different vectors: pGEX-5X-1, pQE30, ET41a, and expressed in JM109, JM109 (DE3) and BL21 (DE3) lines separately. A high expression line was selected with pET41a vector in BL21 (DE3) cells. After optimization of the growth condition, including inoculation amount, IPTG concentration, induction time and temperature, the expression level of HPV16 L2E7 was increased from less than 10% to about 28% of total protein. HPV16 L2E7 protein was then purified from 15 L culture by means of SP Sepharose Fast Flow, Q Sepharose Fast Flow and Superdex 200 pg. After renaturing, HPV16 L2E7 protein with ≥ 95% purity was achieved, which was confirmed via SDS-PAGE gel and Western blotting. The combined use of purified HPV16 L2E7 and CpG helper has shown clear inhibition of tumor growth in mice injected with tumor cells, with six out of eight mice shown no sign of tumor. This study lays a solid foundation for a new pipeline of large-scale vaccine production.

9.Changes of glucagon and somatostatin levels during oral glucose tolerance test in patients with type 2 diabetes mellitus

The fasting and 2 h levels of glucagon, somatostatin ( SS), and C-peptide during 75 g oral glucose tolerance test in 60 patients with type 2 diabetes and 34 normal subjects were determined. Compared with control group, the fasting levels of glucagon and SS and 2 h levels of SS after glucose loading significantly decreased,while the fasting and 2 h levels of C-peptide increased in diabetes group. The 2 h levels of these hormones were significantly higher than the fasting levels in two groups. Compared with control group, the increased folds of glucagon ( 1.40±0.48 vs 1.20±0. 30, P<0. 05 ) and SS( 2.79±2. 17 vs 1.14±0. 22, P<0. 01 ) levels after glucose loading were higher and that of C-peptide level ( 3.58 ±3. 10 vs 8. 33 ± 6. 99, P<0. 01 ) was lower in diabetes group. The levels of fasting glucagon were positively correlated with that of fasting SS in two groups( both P<0. 01 ). These results suggest that disturbance exists in hormones from α and δ cells besides the dysfunction of β cells in patients with type 2 diabetes.
10.Relationship between infants cow's milk protein allergy and gastroesophageal reflux disease

Objective To investigate the relationship between cow's milk protein allergy(CMPA)and gastroesophageal reflux disease(GERD)and the prognosis of GERD combined with CMPA.Methods Fifty patients(24 boys and 26 girls)with GERD were enrolled in this study from January 2015 to June 2016 at Department of Pediatrics,Tangdu Hospital of the Fourth Military Medical University.All children were treated with serum milk protein soluble IgE(sIgE)and milk protein avoidance test,and those with positive results of children's milk protein by provocation test and those with milk serum protein sIgE negative by milk protein provocation tests were diagnosed as CMPA children with GERD according to the CMPA diagnostic criteria and received diet therapy for 1 month and then their blood eosinophil ratio and 24-hour esophageal pH were monitored.Results Twenty-three cases(46%)of 50 children with GERD were diagnosed as CMPA.There was significant difference in clinical symptoms between GERD group and GERD combined with CMPA group(x2=22.78,P<0.05),but there existed cross-symptoms among individual patients,so clinical accurate diagnosis turned out to be difficult.There was no significant difference in family history of allergy between GERD group and GERD combined with CMPA group(x2=3.19,P>0.05).For children with GERD combined with CMPA,the patients received dietary treatment for 1 month.There was significant improvement in vomiting,runny nose/wheezing/cough and diarrhea(P<0.05).However,because the treatment of eczema was long and it could relapse easily,there was no significant change after 1 month of therapy(P>0.05).The proportions of blood eosinophils were decreased after treatment compared with those before treatment [(2.7±1.8)%vs.(8.2±2.7)%,t=10.006,P<0.01].The results of 5 children's 24-hour esophageal pH monitoring showed that the reflux index and the number of acid GERD episodes were lower than before,and the difference was all statistically significant before and after(all P<0.05).Conclusions The occurrence of GERD in infants is partly related to CMPA,and the treatment of CMPA can relieve the clinical symptoms of GERD.
