1.Application Practice of Quality Control Circle in the Management Improvement of Dismounted Drug in Outpatient Pharmacy of Our Hospital

OBJECTIVE:To reduce the types of drugs which was not consistent with the accounts in monthly checking of out-patient department,reduce the wastage of dismounted drugs and improve dismounted drug management. METHODS:Through qual-ity control circle(QCC),analyzing the reasons for inconformity of accounts and drugs,adopting relevant countermeasures of staff training,cipher prescription,counter storage,managed by designated person,expiry data management and so on,the process of dismounted drug management was formulated and implemented. The types of drugs which was not consistent with the accounts and activity growth value of circle members were analyzed statistically before,during and after QCC. RESULTS:The types of drugs which was not consistent with the accounts were 33.6,21.2 and 13.6 before,during and after QCC,decreasing by 59.5%. The ac-tivity growth value of circle members were all positive value. CONCLUSIONS:QCC of outpatient pharmacy can effectively reduce the types of drugs which is not consistent with the accounts in monthly checking,standardize dismounted drug management and im-prove pharmaceutical care quality.
2.Radioprotective effect of calorie restriction in Hela cells and SD rats

Objective To explore the effect of low calorie metabolism on the survival of HeLa cells exposed to X-rays,and the influence of starvation on the antioxidative factors in the blood of rats after irradiation.Methods MTT method was used to evaluate the impact of different concentration glucose on the proliferation of HeLa cells.Colony formation assay was employed to detect the influence of glucose ( 1,5,10 and 25 mmol/L) on radiosensitivity of HeLa cells. Flow cytometry assay was used to analyze distribution of cell cycle and apoptosis.60 male SD rats were randomly divided into 6 groups with 10 rats each.Rats in every two groups were fed ad libitum,fasted for 24 h and fasted for 48 h,respectively.Rats in one group of each approach were respectively exposed to whole-body X-rays at 11 Gy. At 2 h after irradiation,all of rats were sacrificed and their venous blood was collected.Elisa kits were used to detect superoxide dismutase (SOD) and total antioxidant capacity (T-AOC).Results An increased viability was observed in HeLa cells treated with the glucose at low concentration ( <25 mmol/L),while HeLa cell growth was inhibited by glucose at doses of >25 mmol/L. Relevant to cells treated with 1 mmoL/L glucose,SERs (sensitive enhancement ratio) in cells exposed to 5,10 and 25 mmol/L glucose were 1.07,1.10 and 1.23,respectively. A reduction of G2/M and S arrests and apeptosis caused by 6 Gy X-ray irradiation were observed [(49.68 ±1.88)% and (35.54±1.45)% at G2/M phase,(16.88 ±1.22)% and (10.23 ±1.65)% atS phase,t=10.42,5.61,P<0.05]and in the cells treated with 1 mmol/L glucose compared with cells treated with 25 mmol/L glucose [ ( 25.50 ± 0.95 ) % and (7.56 ± 1.07 ) %,t =21.72,P <0.05 ].Without irradiation,calorie restriction exhibited a negligible influence on SOD and T-AOC in rats.However,after 11 Gy irradiation,compared with rats fed ad libitum,the levels of SOD and T-AOC were significantly increased in rats with calorie restriction ( t =40.32,42.78, P < 0.05 ).Conclusions Calorie restriction has a certain radioprotective effect in vivo and in vitro.
3.The accuracy of intravesical prostatic protrusion for diagnosing bladder outlet obstruction: A meta-analysis

protrusion measured by ultrasound for diagnosing bladder outlet obstruction.Methods A literature search of medline (1966.1-2011.6),embase(1984.1-2011.6),CNKI (1994.1-2011.6) and WEIPU Data (1989.1-2011.6) from 1999 to 2009 was performed by two reviewers independently.QUADAS items was applicated to assess trial quality.Golden standard was BOOI measured by urodynamics (BOOI more than 40 indicates bladder outlet obstruction).Heterogenous studies and meta-analysis were conducted by Meta-Disc 1.4 software.Results Totally 6 studies was included at last,involving 682 subjects.No threshold effect was found,but there was heterogeneity due to other factors.The meta-analysis showed that the sensitivity was 70.8 %,specificity was 87.6 %,positive LR was 5.132,negative LR was 0.303,the diagnostic OR was 22.18,the area under SROC curve was 0.8723 and Q index was 0.8028.Conclusions Intravesical prostatic protrusion measured by ultrasound is a good index for diagnosing bladder outlet obstruction in patients with benign prostate hyperplasia when intravesical prostatic protrusion is equal or more than 10mm.
4.Effect of ionizing radiation and cisplatin on the expression of TOB1 in human lung cancer cell lines

Objective To explore the effect of ionizing radiation and cisplatin on the expression of Transducer of erbB2,1 ( TOB1 ) in human lung adenocareinoma SPCA-1 and LTEP-α-2 cell lines.Methods SPCA-1 and/or LTEP-α-2 cells were respectively irradiated with 2,4,6,8,and 10 Gy X-rays generated by a linear accelerator (with the source skin distance of 100 cm and dose rate of 200 cGy/min).The cell molecular samples were subtracted at 6,12,18,and 24 h after 6 Gy irradiation.For X-rays and cisplatin combination treatment,cells were divided into radiation group (6 Gy X-rays),cisplatin group (20 mol/L cisplatin),and combination group (6 Gy X-rays + 20 mol/L cisplatin).SPCA-1 and LTEP-α-2 cells without any treatment were used as control group.The relative levels of TOB1 mRNA and protein expression were detected by rigorously controlled semi-quantitative RT-PCR and Western blot analysis with quantitation of the mRNA/protein bands by densitometry.Results The expression level of TOB1 was significantly increased in both mRNA and protein level after radiation exposure (t =8.25-24.48,P <0.05).For time-dependent induction of TOB1,the expression was significantly increased 6 h after X-rays exposure (t =14.23-15.82,P < 0.05 ).More significant increase of TOB1 expression was identified after the combination treatment of X-rays and cisplatin,compared to that of the radiation and/or cisplatin treatment alone (t =11.21-13.67,P <0.05).Conclusions lonizing radiation and cisplatin could upregulate the expression of TOB1 in lung cancer cells in both mRNA and protein levels.TOB1 may be a molecular target for ionizing radiation and cisplatin treatment in lung cancer and it may have potential implication in evaluating the curative efficiency of chemo-and radio-therapy.
5.Impacts of electroacupuncture combined with ultraviolet therapy on cytokines of herpes zoster at the acute stage in patients.

OBJECTIVETo observe the effect of electroacupuncture (EA) combined with ultraviolet therapy on herpes zoster at the acute stage and the impacts on serum interleukin 2 (IL-2), interleukin 6 (IL-6) and interleukin 10 (IL-10) in the patients.
METHODSThirty-four patients of herpes zoster were randomized into a medicine group and a combined therapy group, 17 cases in each one. In the medicine group, the intravenous drops with acyclovir injection, muscular injection with cobamamide and the topical with acyclovir ointment were applied. Additionally, TDP was radiated locally. In the combined therapy group, on the basis of the treatment as the medicine group, EA and ultraviolet therapy were supplemented. The duration of treatment was 10 days in the two groups. Before and after treatment, blister relief, incrustation time and the visible analogue scale (VAS) were recorded in the two groups. The clinical efficacy was assessed in the two groups and the levels of serum IL-2, IL-6 and IL-10 were determined in the two groups.
RESULTSIn the combined therapy group, the time of blister relief and incrustation was earlier apparently than that in the medicine group (both P<0.05). VAS score after treatment were reduced as compared with that before treatment in the two groups (both P<0.01), and the reducing amplitude in the combined therapy group was larger than that in the medicine group (P<0.01). The total effective rate was 94. 1% (16/17) in the combined therapy group, higher than 76.4% (13/17) in the medicine group (P<0.05). After treatment, IL-2 levels were increased as compared with those before treatment in the two groups (both P<0.05), the levels of IL-6 and IL-10 were reduced obviously as compared with those before treatment in the two groups (all P<0.01). After treatment, the levels of IL-6, IL-10 were reduced much more apparently in the combined therapy group as compared with those in the medicine group (both P<0.05).
CONCLUSIONEA combined with ultraviolet irradiation more rapidly and effectively relief the symptoms of herpes zoster, significantly relief pain, shorten the duration of sickness, improve the body immunity and reduce nerve injury.

6.The correlation between expression of carcinoembryonic antigen mRNA in preoperative peritoneal lavage fluid and prognosis of operation in patients with gastric cancer

Objective To investigate the correlation between expression of carcinoembryonic antigen (CEA) mRNA in preoperative peritoneal lavage fluid and prognosis of operation in patients with gastric cancer.Methods The expression levels of CEA mRNA in preoperative peritoneal lavage fluid of 68 patients with gastric cancer and 30 patients with gastric benign tumor were measured by reverse transcription polymerase chain reaction.The correlation between the expression level of CEA mRNA in preoperative peritoneal lavage fluid and clinicopathological features,prognosis of postoperative were analyzed.Results The expression level of CEA mRNA in preoperative peritoneal lavage fluid of patients with gastric cancre was 1.74 ± 0.25 and with gastric benign tumor was 0.19 ± 0.04,and there was statistical difference (P < 0.05).The expression level of CEA mRNA in preoperative peritoneal lavage fluid had a correlation with the tumor infiltration depth,tumor differentiation,lymph node metastasis,distant metastasis and TNM stage (P < 0.05).There was statistical difference in the recurrent / metastasis rate and survival rate 1 year after operation between low (34 cases) and higher (34 cases) expression level of CEA mRNA in preoperative peritoneal lavage fluid of gastric cancer patients:47.06% (16/34) vs.82.35% (28/34) and 79.41% (27/34) vs.55.88% (19/34),P < 0.05.Conclusion The expression of CEA mRNA in preoperative peritoneal lavage fluid has close correlation with the clinicopathologic features,and the high expression of CEA mRNA indicats poor prognosis,indicating that CEA mRNA in preoperative peritoneal lavage fluid could be used as a prognostic marker in patients with gastric cancer.
7.Study on the protective effect of lycium barbarum polysaccharide on light aging resistance in rats

Objective To study the mechanism of the protective effect of lycium barbarum polysaccharide on light aging resistance rats by using the metabolic profile and metabolic target analysis technique.Methods Ultraviolet irradiation induced Wistar rats were induced to produce skin photo aging model and 24 rats were randomly divided into three groups, including control goup, model group, Lycium barbarum polysaccharide group (LBP). After modeling for 24 hours, LBP group was conducted with Lycium barbarum polysaccharide solution of 10 mg/kg. Control group and model group were given same volume of stroke-physiological saline solution for 14 days. The biochemical indexes such as rat serum antioxidant activity of related substances and MDA were measured in model group and drug group; the urine metabolomics study was also investigated for the mechanism of lycium barbarum polysaccharide against light aging band.Results Compared with the model group, the LBP group rats total superoxide dismutase activity (301.51 ± 42.56 U/mgvs.93.41 ± 56.31 U/mg), hydroxyproline (8.91 ± 5.78μg/mgvs.4.74 ± 1.54μg/mg) content significantly increased (P<0.05), but the malondialdehyde (8.54 ± 6.41 nmol/mgvs.21.31 ± 6.58 nmol/mg) decreased without any statistics difference (P<0.05). The urine metabonomics results showed that LBP could regulate the skin photoaging of multiple metabolic pathways and key metabolic enzymes in the process, such as peanut four acid, tyrosine, taurine, citric acid and hippuric acid, L-cysteine, inositol, threonine etc.Conclusions In the process of skin photoaging in rats, multiple metabolic pathways in vivo were disordered, and Lycium barbarum polysaccharide could play a protective role by regulating the key metabolic enzymes in the network.
8.Analysis of the volatile oil in fructus lycii(Lycium barbarum L.) by GC-MS and HELP method

Objective Gas chromatography-mass spectrometer (GC-MS) technology combined with Heuristic evolving latent projections (HELP) method were used to qualitatively analyze the volatile oil of fructus lycii (Lycium barbarumL.)Methods With the best temperature-programmed chromatographic condition, overlapping peaks among the total ion chromatogram were separated. Then an automatic mass spectral deconvolution and identification system (AMDIS) was used to identify volatile components comprehensively. Finally, the pure chromatographic peaks were matched with NIST 05a mass spectra library for giving the precise qualitative result. Results Totally, 29 compounds in fructus lycii (Lycium barbarum L.) were identified accurately.Conclusions Compared with the traditional qualitative analysis method, the new established method which GC-MS technology combined with HELP could get tremendous advantages. It not merely enhanced the accuracy of identification but also solved the bottleneck of handling the inseverable compounds.

10.Clinical significance of different stratification of platelet count in primary acute myeloid leukemia in children

Objective:To analyze the clinical characteristics of primary acute myeloid leukemia (AML) (non-M3 type) in children suffering from different levels of platelet count(PLT).Methods:In the Tumor Hospital of Zhengzhou University from January 2014 to December 2018, laboratory and clinical data of 247 de novo primary AML pediatric patients were retrospectively reviewed.According to the PLT before treatment, patients were divided into very low platelet group (VLG), low platelet group (LG) and non-lowing platelet group (NLG), with<50×10 9/L, ≥50×10 9/L but <125×10 9/L and ≥125×10 9/L as the boundaries.All patients were followed up until June 30, 2019.Meanwhile, the follow-up data was obtained by consulting medical records or by telephone.SPSS 17.0 software was applied for data analysis. Results:In general clinical features, a different group of hemoglobin (Hb) content, fusion gene AML- ETO and clinical risk stratification were statistically significant in different PLT groups ( χ2=11.270, 12.115 and 12.848, respectively, all P<0.05). However, the differences of other indicators in different groups of PLT were not statistically significant (all P>0.05). There were no statistically significant differences in terms of 3-year disease-free survival(DFS) rate (59.3%, 36.3%, 50.4%) among the 3 groups (all P>0.05). The median total survival(OS)time(40.5 months)and 3-year OS rate(41.0%) of NLG patients were significantly higher than those of VLG(23.1 months, 30.1%)and LG(14.1 months, 18.2%)patients, with statistically significant differences( χ2=7.798 and 6.553, respectively, all P<0.05). The univariate analysis of gender, white blood cell(WBC), Hb, PLT, lactic dehydrogenase(LDH), FLT3-ITD, NPM1, DNMT3A, CEPBA, C-KIT, AML-ETO, molecular genetic prognosis, complete remission(CR), and hemopoietic stem cell transplantation(HSCT) displayed that DNMT3A mutation was an adverse factor that affects patients′ OS ( χ2 =5.834, P<0.05), and the positive factors that influences OS were non-reducing PLT before treatment, and obtaining CR and subsequent HSCT ( χ2=7.798, 79.168, and 31.337, respectively, all P<0.05). Multi-factor analysis revealed that the independent protective factors that affect patients′ OS were the non-reducing PLT before treatment, and obtaining CR and subsequent HSCT( Wald=42.760, 15.918, and 10.183, respectively, all P<0.05). Conclusions:Before treatment, non-reducing PLT is a protective factor for primary childhood AML patients, and the prognosis is satisfying.
