Extensive functions can be provided to hospital management,medical treatment and teaching when massive data are reasonably processed and analyzed. Knowledge mined from sequence of doctor's advice for medicine can not only evaluate treatment quality but also provide necessary basis for establishing a safety and effective medicine treatment plan. This paper analyzes some common information of doctor's advice for medicine,mines the doctor's advice for medicine by using sequence pattern mining method,and explains the mining results.

3.Strengthening Management of Hospital's Network by Means of Active Directory

Objective To solve the problems in the network management of the hospital's information construction.Methods The client workstations and the users of the hospital network were designed and organized by means of active directory.They are deployed and managed by means of setting the type of users and issuing the group policy.Results The application of active directory could manage the hospital's network uniformly,agilely and safely.Conclusion The management level is enhanced by means of active directory.It will have broad prospects in the hospital's network management.
4.Influence of Yes-associated protein-1 on proliferation and metastasis of gallbladder carcinoma

Objective To investigate the expressions of Yes-associated protein-1 (YAP1) in gallbladder mucosal epithelium of normal persons,in patients with simple/calculous cholecystitis,and in patients with gallbladder carcinoma;and to study the mechanism of YAP1 in gallbladder carcinoma development.Methods Immunohistochemistry was used to detect the expression and distribution of YAP1 protein in 50 persons with normal gallbladder,101 patients with simple cholecystitis/calculous cholecystitis and 100 patients with gallbladder carcinoma.RT-PCR and western-blot were used to detect the mRNA and protein levels of YAP1 in normal and malignant gallbladder mucosal epithelium cells.siRNA was used to shut down the expression of YAP1 in SGC996 cells.MTT was used to test cell vitality.Flow cytometry was used to measure cell cycle.Results Immunohistochemistry revealed the expression rates of YAP1 in the gallbladder carcinoma group,the cholecystitis/gallstone group and the control group to be 87.0% (87/100),56.4% (57/101) and 5.0% (1/20),respectively (P < 0.01).The YAP1 protein levels were higher in gallbladder carcinoma tissues and cells when compared to normal tissues and cells.RT-PCR showed the mRNA levels of gallbladder carcinoma cells to be 12.5 ± 1.2 times of normal gallbladder mucosal epithelial cells (P < 0.05).After using siRNA to shut down the YAP1 expression,EMT associated proteins were down-regulated,cell vitality was decreased,and cell cycle was arrested in the S-phase.Conclusions YAP1 is closely related to cell proliferation and metastasis of gallbladder carcinoma.It may promote tumor progression through epithelial-mesenchymal transition.transition;Tumor progress
5.Efficacy comparison of lumber disc herniation treated with mild moxibustion at Yaoyangguan (GV 3) under different conditions.

OBJECTIVETo compare the difference in clinical efficacy on lumber disc herniation (LDH) treated with Yaoyangguan (GV 3) between mild moxibustion under thermosensitive condition and that under non-thermo-sensitive condition.
METHODSFifty-seven LDH patients were selected as the study objects. Mild moxibustion at Yaoyangguan (GV 3) was applied for 45 min each time. Additionally, the conventional acupuncture was given, once a day, for 20 days. At the end of treatment, two groups were classified. A thermosensitive acupoint group (thermosensitive group) was composed of the cases with thermo-sensitization at Yaoyangguan (GV 3) and presenting for > or =4 times in the entire treatment. A non-thermosensitive acupoint group (tranquilization group) was composed of the cases without thermo-sensitization or the frequency of thermo-sensitization <4 times in the entire treatment. The modified Japanese orthopedics association scoring system (M-JOA) was adopted to observe the cases before and after treatment and 6 months after treatment in the two groups. The efficacy was compared between the two groups.
RESULTSAfter treatment and in 6 months after treatment, the score of M-JOA was apparently reduced as compared with that before treatment in the two groups (all P<0.05). The score reducing in the thermosensitization group was more obvious than that in the tranquilization group (both P<0.01). After treatment and in 6 months after treatment, the curative rate and remarkably effective rate were 89.7% (26/29) and 79.3% (23/29), which was better than 71.4% (20/28) and 60.7% (17/28, both P<0.05) in the tranquilization group separately.
CONCLUSIONMild moxibustion at acupoint under thermosensitive condition achieves the better short-term and long-term effects as compared with that under non-thermosensitive condition.

6.Treatment of equinovarus accompanied with ulcer using Ilizarov technology and limited operation.

OBJECTIVETo explore the surgical method and clinical curative effect of Ilizarov technique combined with limited surgical treatment for neurotrophic malnutrition equinovarus accompanied with weight-bearing area ulcer.
METHODSFrom July 2004 to December 2011, 21 cases of neurological disorders equinovarus with skin ulcer in weight-bearing area were treated including 14 males and 7 females with an average age of 21.3 years old ranging from 8 to 32 years. Among them,19 cases with talipes equinovarus were on account of spina bifid and 2 cases on account of spinal cord injury of lumbar sacral segment. Nine cases were on the left foot and 12 cases were right foot. The ulcer area in out-below side of the fifth metatarsal bone was in 6 cases, in basement of the fifth metatarsal bone in 5 cases,in lateral of calcaneocuboid joint 2 cases, and in lateral of subtalar joint in 8 cases. Surgical procedure included plantar medial release technique, tendo-chilles lengthenning, and three-joint partial osteotomy. During operation,the skin and soft tissue of ulcer were resected and the incision was sutured when the ulcer was in the lateral of subtalar joint, and ankle Ilizarov external fixation for extension was installed after correcting talipes equinovarus partially. For the legacy skin ulcer in weight-bearing area,the Shenrg-ji cream (Chinese characters) was used after operation for one time per day until the wound healed. Adjusting Ilizarov external fixation for correcting residual deformity until it is satisfaction. During this process weight-bearing walking was keeping.
RESULTSAll cases were followed up from 6 to 52 months with an average of 28 months. All ulcers were healing and no recurrence. The ulcer healing time was 14 to 36 days postoperative (26 days in average). Eighteen deformity feet were corrected satisfactorily, and recovered fully plantar foot, 3 feet appeared mild deformity after 1 year. Ankle joint activity appeared limited in 15 cases and got completely rigidity in 6 cases. The AOFAS increased from preoperative 34.0±7.2 to 86.0±8.5 postoperatively; 8 feet got excellent, 10 good,3 fair.
CONCLUSIONThe clinical effect of Ilizarov technique combined with limited operation and the Sheng-ji cream in correction of the talipes equinovarus with skin ulcer in weigh-bearing area on account of neurotrophic malnutrition is satisfactory, the surgical method is simple and the treating course in security, and serious complications can be avoided.

7.Replantation of fingertip amputation in lack of availability of intravenous anastomosis.

OBJECTIVETo discuss the replantation of fingertip amputation in lack of availability of intravenous anastomosis.
METHODSFrom November 2009 to November 2010, 86 patients (104 fingers) with fingertip amputation were treated with replantatioin, including 64 males and 22 females, with an average age of 26 years ranging from 2 to 64 years. The time from injury to therapy was from 30 min to 12 h, time of broken finger ischemia was from 2.5 to 12 h. Preoperative examination showed no obvious abnormalities. Four different replantation methods were selectively applied to these 104 amputated fingertips of 86 cases: (1) replantation with anastomosis of single or bilateral proper digital artery in 37 fingers; (2) replantation with arteriovenous bypass in 27 fingers; (3) replantation with exclusive anastomosis of digital artery in 24 fingers; (4) replantation with removing the palmar pocket method in 16 fingers.
RESULTSOne hundred and two of 104 amputated fingertips were survived. Among these survived fingers,75 cases (92 fingers) were followed-up for 6 to 24 months. According to the assessment standard of Chinese Medical Association of Hand Surgery, the results were excellent in 52 cases, good in 19, poor in 4.
CONCLUSIONIt benefits to expand the indications and improve the survival rate of replantation of fingertip amputation with the correct choice of different replantation methods according to the injury situation of the broken fingertip artery after debridement under the microscope.

8.Identification and risk monitoring of potential risk areas of schistosomiasis in Dongting Lake area

Objective To identify and monitor potential risk areas of schistosomiasis in Dongting Lake area. Methods With remote sensing (RS)and geographic information system(GIS) technology, potential risk areas of schistosomiasis in Dongting Lake area were extracted by using spectral characteristics and environmental factors. Based on epidemiological data and land use data, the regional epidemic index and environmental susceptibility index were fused with potential epidemic risk areas in the grid system of Dongting Lake area to form the epidemic risk surveillance map of schistosomiasis in Dongting Lake area from 2006 to 2016. Results From 2006 to 2016, the high risk areas of schistosomiasis epidemic in Dongting Lake area were concentrated in the central and Northern lakeside areas of Changde City (Annual average epidemic index R-=0.330 5, Regional average susceptibility index Y-=2.458 2), the sensitive land types were Lake area (Sensitivity coefficient β=1.236 6), the surrounding area of Datong Lake in Nanxian and Northern Yuanjiang City (R-=0.426 0, Y-=0.836 8), paddy field area (β=0.202 1) and other construction land (β=0.308 0). The extremely high risk areas were concentrated in some lake-facing areas in Jinshi (R-=0.449 1, Y-=3.917 0) and the sensitive land type is woodland(β=1.234 5). The risk of river basin from high to low was respectively the Lishui River, the Yuanjiang River, the Zijiang River and the Xiangjiang River. Conclusions The high-risk and extremely high-risk areas should be the focus of future epidemic control. The snail control and environmental renovation of epidemic-sensitive areas such as sparse woodland, grassland with high coverage, reservoir pits and ponds should be done well in Dongting Lake area.
9.Effects of NBS1 in MRN complex on gallbladder carcinoma cells

Objective To study the expression of MREll-RAD50-NBS1 complex in normal gallbladder tissues,simple cholecystitis,calculous cholecystitis and gallbladder carcinoma.Methods The expression of MRN complex in different gallbladder lesion tissues were detected by immunohistochemistry.Western blot,Methyl thiazolyl tetrazolium(MTT) assay and flow cytometry were used to detect the influence of apoptosis and proliferation induced by NBS1.Results The differences between the expression of MRE11,RAD50 and different gallbladder lesion tissues were not statistically significant (respectively x2 =2.724,1.697,all P > 0.05).The expression of NBS1 in GBC tissues [15.3% (9/59)] was prominently lower than that in the normal gallbladder tissues [84.7% (50/59)],simple cholecystitis [87.8% (36/41)],calculous cholecystitis [61.2% (30/49)] (x2 =87.388,P < 0.01).The Western blot results reveal that NBS1 can mediate apoptosis by up-regulate Bax and down-regulate Bcl-2.The MTT assay results showed that the relative absorbance of treatment and control group after 24,48,72 h were[(1.120 ± 0.006)vs.(1.350±0.009),(1.600±0.004)vs.(1.99±0.01),(1.83±0.01)vs.(2.260±0.003)(F=7.659,P <0.01).The apoptosis rate in treatment group(17.23% ± 0.56%)was higher than that in the control group (4.13% ± 0.67%) (t =9.133,P < 0.01).Conclusion NBS1 is closely related to gallbladder carcinoma.NBS1 can inhibit the proliferation and promote apoptosis of GBC-SD cells.
10.Waist circumference distribution of Chinese school-age children and adolescents

Objective To study the distribution of waist circumference (WC) in a representative group of Chinese children for establishing the Chinese WC cut-off points. Methods Cross-sectional data obtained from 15 mainland provinces and Hong Kong were collected. There were 178 865 children and adolescents, aged 7-18 years-old in total. All subjects were divided into five regional groups- costal city, non-coastal city, northern rural, southern rural, and Hong Kong, for analyzing the distribution of WC. Results Large variations in WC distribution were found among the five groups for both sexes. The coastal city group had the highest P85 WC values consistently from the early school age to 18-year-old, when comparing with other groups. In contrast, southern rural group had P85 values consistently lagged behind, in both city group and in the north rural group. Hong Kong group had a high P85 WC at early ages, but because of the smaller increments of WC during adolescence, the P85 curve was gradually exceeded by the northern rural groups. Catch-up trend was found in the two rural groups since peak puberty. These disparities were caused not only by factors related to socioeconomic and urban-rural but also to the geographic differences between the noah and the south. Because of the regional variation of WC, a "gradient" prevalence of central obesity was prospected among these groups. Closed associations were found between WC and height, weight and BMI in all age period. Comparison on the WC distributions between people from China, U.S.A or the Netherlands demonstrated the necessity of setting up China's own WC cutoffs.Sex and age-specific percentiles were obtained and smoothed by using the LMS method.Conclusion Data obtained from this study could be used to develop the national WC cutoff points for Chinese children and adolescents.
