1.Self efficacy theory applied in pulmonary rehabilitation in residence in elderly patients with chronic obstructive pulmonary disease

Objective To explore the effect of self efficacy combined with mobile health information service applied in pulmonary rehabilitation in residence in elderly patients with chronic obstructive pulmonary disease. Methods Sixty-eight elderly patients in stable situation were divided into the control group and the intervention group with 34 cases in each group accoridng to the random digit table. In the control group, the nursing intervention included health education and telephone follow-up. The intervention group adopted self efficacy theory combined with mobile health information service. The items such as persistence in pulmonary rehabilitation, Exercise Self-Regulatory Efficacy Scale (Ex-SRES) and health condition were assessed. Results After intervention for 12 weeks, the scores of persistence in pulmonary rehabilitation in residence in the intervention group scored 5.35 ±1.42, which was higher than that in the control group (2.03±1.40), the difference was significant (t=160.43, P<0.01). Ex-SRES of the intervention group scored 62.06±13.10,91.21±11.12, which were higher than those of the control group, 50.38±18.03,42.56±18.23, the difference was significant (t=9.32,176.88, P<0.01) . The interaction between treatment effects and time effect was significant in COPD Assessment of Tool (CAT) and Ex-SRES respectively(F=489.95,P=0.00;F=134.80, P=0.00). Comparing value of CAT in baseline (24.97±4.74,24.62±5.39) and at the end of the twelfth week(24.94±4.74,24.65±5.35), the difference was not significant in two groups respectively(P>0.05). The improvement of CAT was significant at the end of the twenty-forth week (21.90±4.67) in the intervention group (t=-12.09, P<0.01). Conclusions Self efficacy theory and the application of mobile health information services can help patients persist in long-term rehabilitation exercise training.
2.Application of Short Message Service for Community-based Pulmonary Rehabilitation

Objective To apply short message service (SMS) for pulmonary rehabilitation in residence. Methods A total of 102 old pa-tients with chronic obstructive pulmonary disease (COPD) in stable situation were equally divided into three groups randomly. They all ac-cepted a two-week lesson of exercise project, and then took in the project for 22 weeks. Meanwhile, group A phoned to their experts once a week, group B was phoned by their experts once a week, and group C accepted SMS once a day and sent back to the experts. They were in-vestigated with Exercise Self Regulatory Efficacy Scale (Ex-SERS), modified British Medical Research Council Scale (mMRC) and COPD Assessment Test (CAT) before and after intervention. Results The score of Ex-SERS was the most in group C (F=179.53, P<0.001), with the interaction of time (F=387.09, P<0.001). While the scores of mMRC (F=8.754, P<0.001) and CAT (F=11.32, P<0.001) were the least in group C. Conclusion Application of SMS in the integrated management for pulmonary rehabilitation in residence may improve the self regu-latory efficacy of exercise, release the dyspnea and the influence on living in patients with COPD.
3.Chinese Hans population study of fluorescent labelled 8 STR multiplex system.

OBJECTIVE:
Chinese Hans population study of 8 STR loci was carried out in 350 unrelated individuals.
METHODS:
The 8 STR loci(vWA, TH01, TPOX, CSF1PO, D5S818, D13S317, D7S820, D16S539) and a sexual distinction locus Amelogenin, were amplified simultaneously in one PCR reaction tube, the products were separated by electrophoresis with PE-377 DNA sequencer, and analyzed by Genescan analysis software, then the accurate PCR fragment length and designation of genotypes were finished automatically by the sequencer.
RESULTS:
63 alleles and 2 sex determination alleles were found in Hans population altogether; the minimum allele frequency in Chinese Hans population at these loci was 0.17%, the maximum was 50.17%; the fragments were 119-327 base pairs long; the TDP of this system reached 99.99999998%; this system inherited steadily, and was accord with Mandel inheritance rule, no gene mutation was found, all loci met Hardy-Weinberg equilibrium, but the frequencies of Hans population differed from those of African-Americans, Caucasion-Americans and Hispanic-Americans.
CONCLUSION
This method is good for identification of little amount and degraded DNA samples from blood stain, semen stain mixed with vaginal excretion, hair root, cigarette root, muscle tissue and ancient bone. It can also make a foundation of establishing the National DNA Database.

4.Effect of Kechuanning Oral Liquid on Signal Transduction Pathway of PI3K in Asthma Rats Induced by Virus

Objective To observe the effect of Kechuanning Oral liquid on the expression of P13K in CD4+and CD8+T cells of asthma rats induced by respiratory syncytial virus (RSV), and explore its mechanism of preventing and treating asthma induced by virus. Methods SD rats were randomly divided into phosphate-buffered saline (PBS) control group, model group, dexamethasone and salbutamol group (positive drug group), and Kechuanning oral liquid group. Ovalbumin asthma rats model was built and induced by RSV. CD4+and CD8+T cells were separated by magnetic beads method. The expressive level of PI3K-110αin T cells was detected by immumofluorescence method with flow cytometry. Results Compared with PBS control group, positive drug group and Kechuanning oral liquid group, the CD4+and CD8+T cells percentage and the expression level of PI3K-110α in CD4+ and CD8+T cells of model group were significantly higher. The differences were statistically significant (P<0.01). The CD4+and CD8+T cells percentage was not associated with the expression level of PI3K-110α in CD4+ and CD8+T cells of each group by Pearson correlation analysis. Conclusion The CD4+ and CD8+T cells involve in the asthma immunity balanced modulation, and the level of PI3K expressed by CD4+ and CD8+T cells increase distinctly. Kechuanning oral liquid can restrain the expression of PI3K in CD4+ and CD8+T cells, which might be one of the immunologic mechanisms of treating asthma.
5.Prokaryotic expression of Neisseria gonorrhoeae NspA and its antigenicity

Objective To develop fast detection techniques for the diagnosis of gonococcal infections.Methods Prokaryotic expression vector for Neisserial surface protein A (NspA) was constructed using the NspA gene cloned by PCR.Mice were immunized with the renatured recombinant NspA (rrNspA)to prepare antibodies against NspA.Western blot and ELISA was used to analyze the binding of NspA antibodies to lysate of gonococcal cells or to intact gonococci.Results NspA antibodies that were prepared by the rrNspA expressed in E.coli could bind to rrNspA,the natural NspA existing in gonococcal cells,or intact gonococci.Conclusion RrNspA and its antibodies have potential value in developing fast diagnostic kits for gonococcal diseases.
6.Observation on the therapeutic effect of scraping therapy on perimenopausal syndrome.

OBJECTIVETo observe the clinical effects of scraping therapy on perimenopausal syndrome.
METHODSTwenty women with perimenopausal syndrome were treated with scraping therapy and the dorsal course of the Governor Vessell and the Urinary Bladder Meridian of Foot-Taiyang were scraped, especially on the Back-shu points and Ashi points. The clinical symptoms were observed and compared with a modified Kupperman score before and after treatment.
RESULTSIn all the 20 patients, 3 cases were cured, 6 cases were markedly effective, 9 cases were effective, 2 cases were ineffective, and the total effective rate was 90.0%. The Kupperman total score after treatment of (10.4 +/- 7.5) was significantly lower than the score before treatment of (25.0 +/- 5.3) (P < 0.001), in which, hot flushes and sweating, insomnia, fatigue, paresthesia, anxiety/irritability, hypaphrodisia, urinary system infection, tinnitus, dizziness, memory deterioration and headache were eased significantly (P < 0.001, P < 0.05).
CONCLUSIONThe scraping therapy has a good clinical effect on perimenopausal syndrome and can significantly improve the clinical symptoms.

7.The correlation of asymmetrical dimethylarginine level and oxidative stress to the onset of Alzheimer's disease.

This study is to investigate the influence and mechanism of action of asymmetrical dimethylarginine (ADMA) and the induced oxidative stress level on Alzheimer's disease (AD) incidence. ADMA concentration, nitric oxide, Abeta(40)/Abeta(42) ratio, inducible NO synthase (iNOS) activity and the concentrations of the induced free radicals including malondialdehyde (MDA), 3-nitrotyrosine (3-NT) and peroxynitrite (ONOO-) in the cerebrospinal fluid (CSF) from 34 neurologically normal controls and 37 AD patients were quantitatively determined and statistically compared. The results showed that the ADMA concentration significantly decreased in AD patients, and it showed negative correlation with the NO, iNOS activity, and showed positive correlation with MMSE score. ADMA concentration was negatively correlated with Abeta(40)/Abeta(42) ratio (P<0.01) with the observation that Abeta(40)/Abeta(42) ratio increased while ADMA level decreased in CSF in AD patients. The concentration levels of MDA, 3-NT and ROS significantly increased compared with the control with all the P values less than 0.05. These findings suggested that the ADMA disorder and the oxidative damage effect of the induced free radicals in CSF of AD patients are an important mechanism of AD incidence, and their joint regulation may provide new idea for the prevention and clinical treatment of AD.

8.Elevated Arsenic Exposure Is Associated with an Increased Risk of Chronic Hepatitis B Virus Infection: NHANES (2003-2014) in U.S.Adults

Studies concerning the association between arsenic exposure and hepatitis B virus (HBV) infection have been lacking.The present study aimed to examine the association between total urinary arsenic (TUA) and infection of HBV.A total of 5186 participants from National Health and Nutrition Examination Survey (NHANES) 2003-2014 were included in the analysis.We used logistic regression to evaluate the association.We defined two measures of TUA.TUA1 was the sum of arsenous acid,arsenicacid,monomethylarsonic acid and dimethylarsenic acid.TUA2 was defined as TUA minus arsenobetaine and arsenocholine.The results showed that the weighted overall prevalence of HBV infection was 6.08%.For NHANES 2003-2014,the medians (interquartile range) of TUA1 and TUA2 were 5.60 μg/L (3.97-8.09 μg/L) and 4.91 μg/L (2.36-9.11 μg/L),respectively.Comparing the highest quartile to the lowest quartile after multivariable adjustment showed that the odds ratios (ORs) and 95% confidence intervals (CIs) for TUA1 and TUA2 were 2.44 (1.40-4.27) and 2.84 (1.60-5.05),respectively.In conclusion,elevated urinary arsenic was associated with the risk of HBV infection.Further studies,especially prospective studies,are needed to confirm the causal relationship between arsenic exposure and HBV infection.
9.Clinical characteristics of refractory Mycoplasma pneumoniae pneumonia in children.

OBJECTIVETo provide a basis for early diagnosis and treatment of refractory Mycoplasma pneumoniae pneumonia (RMPP) in children by comparing the clinical characteristics of RMPP and general Mycoplasma pneumoniae pneumonia (MPP).
METHODSChildren with MPP hospitalized between October 2015 and December 2016 were selected as study subjects. According to the diagnostic criteria, children were divided into RMPP group (n=152) and MPP group (n=551). The differences between the two groups in the basic situation, clinical manifestations, infection parameters and myocardial enzymes were compared.
RESULTSThere were no significant differences in gender and age between the RMPP and MPP groups (P>0.05). The peak temperature in the RMPP group was significantly higher than that in the MPP group on the first day of admission (P<0.01). The percentage of children with augmentation in the RMPP group was lower than that in the MPP group (P=0.009). The percentage of neutrophils [Ne(%)] and serum procalcitonin (PCT) levels in the RMPP group were both higher than those in the MPP group (P<0.05), while the percentage of lymphocytes was significantly lower in the RMPP group (P<0.05). The serum levels of aspartate aminotransferase (AST) and lactate dehydrogenase (LDH) in the RMPP group were also higher than those in the MPP group (P<0.05). Binary logistic regression analysis showed that the peak temperature and LDH were closely related to RMPP in children (P<0.05). Receiver operating characteristic (ROC) curve analysis showed that the area under the curve (AUC) of the peak temperature and LDH for the diagnosis of RMPP was 0.647 and 0.637 respectively. In children ≤2 years old, when the threshold value of LDH was 400 U/L, the diagnostic sensitivity was 52.63% and the specificity was 54.84%. In children above 2 years old, when the threshold value of LDH was 335 U/L, the diagnostic sensitivity was 69.92% and the specificity was 51.55%.
CONCLUSIONSThe children with RMPP have a high fever in the early stage. Meanwhile there are abnormal laboratory test results in these children. Elevated serum LDH levels have a high clinical value of early diagnosis of RMPP, especially in children above 2 years.
10.Shear stress and vascular smooth muscle cells modulate the proliferation of endothelial cells via TGFβ1 and p-Akt pathways

Objective To investigate the effects of shear stress and vascular smooth muscle cells (VSMCs) on the proliferation of endothelial cells (ECs) and the molecular mechanism involved in. Method Using parallel-plate flow chamber system, normal shear stress of 15 dyn/cm2(1 dyn=10-5 N) was applied to ECs cultured singly and co cultured with VSMCs respectively. Then, the expression of PCNA, a molecule representing cell proliferation ability, and phosphorylation of Akt were analyzed by Western blotting in order to investigate the roles of shear stress and VSMCs in EC proliferation. Under the static condition, the expressions of PCNA and p-Akt were analyzed in ECs co-cultured with VSMCs with and without physical contact. And then TGFβ1 neutralizing antibody was employed to demonstrate the contribution of TGFβ1 in VSMCs induced EC proliferation. Results Normal shear stress decreased EC proliferation and Akt phosphorylation. VSMCs increased EC proliferation and Akt phosphorylation in both co-culture conditions with and without physical contact. Normal shear stress partly reversed the increase of proliferation and Akt phosphorylation in ECs with physical contact to VSMCs, and TGFβ1 neutralizing antibody exerted the similar effects in ECs without physical contact to VSMCs. Conclusions Normal shear stress is a protective factor with its inhibitory effect on EC proliferation. VSMCs induced EC proliferation via TGFβ1 and p Akt pathways by paracrine model.
