Pulmonary electrical impedance tomography (EIT), a noninvasive, continuous, dynamic, and radiation-free bedside imaging technique for monitoring pulmonary ventilation, is now widely utilized in the diagnosis and management of critically ill patients. Beyond ventilation monitoring, hypertonic saline contrast-enhanced EIT for bedside pulmonary perfusion assessment has recently garnered significant attention. This article describes the application of hypertonic saline contrast-enhanced EIT to evaluate pulmonary perfusion in two patients following pulmonary endarterectomy, providing a reference for its perioperative application in such patients.
2.Effects of Jiawei Jingqin Decoction (加味荆芩汤) on Inflammatory and Immune Responses in Rosacea Model Mice

ObjectiveTo investigate the potential mechanisms of Jiawei Jingqin Decoction (加味荆芩汤) in the treatment of rosacea. MethodsFifty 8-week-old female BALB/c mice were randomly divided into normal control group, model group, and Jiawei Jingqin Decoction low, medium, and high-dose groups, with 10 mice in each group. Except for the normal control group, mice in the other groups were injected with 40μl of antimicrobial peptide LL-37 at a concentration of 320 μmol/L on the back, repeated once every 12 hours for a total of 4 injections, to induce the rosacea mouse model. The mice in the normal control group were injected with 40 μl of PBS solution at the same time. After successful modeling, the mice in the Jiawei Jingqin Decoction low, medium, and high-dose groups were orally administered Jiawei Jingqin Decoction at doses of 4.35, 8.71, and 17.42 g/(kg·d) respectively, while the mice in the normal control group and the model group were orally administered 20 ml/(kg·d) of normal saline. All groups were treated once daily for 5 days. The changes in skin lesions were observed, and the erythema area was measured and scored. Skin tissue at the injection site was collected for histopathological examination using HE staining, and the number of mast cells was detected using toluidine blue staining. The levels of interleukin-6 (IL-6), interleukin-1β (IL-1β), and tumor necrosis factor-alpha (TNF-α) in the skin lesion tissues were measured using the ELISA method. The mRNA expression levels of key factors in the innate immune response pathway of rosacea disease mechanism, including Toll-like receptor 2 (TLR2), downstream molecules serine protease kallikrein 5 (KLK5) and matrix metalloproteinase 9 (MMP-9), as well as related inflammatory factors (IL-1β, IL-6, TNF-α), and adaptive immune CD4+ T cell-related factors interferon-gamma (INF-γ) mRNA were detected using RT-qPCR. Immunohistochemical staining was performed to detect the protein expression of KLK5, MMP-9, and the number of CD4+ T-positive cells. Western blot analysis was used to determine the protein expression levels of CD4+ T cell-related factors including IFN-γ, CC chemokine receptor 5 (CCR5), signal transducer and activator of transcription 1 (STAT1), signal transducer and activator of transcription 3 (STAT3). ResultsThe skin at the injection site on the back of mice in the normal control group appeared normal without any skin lesions or erythema, and histopathological examination showed no infiltration of inflammatory cells. The model group had significantly higher erythema scores and larger erythema areas compared to the normal control group (P<0.05), and histopathological examination revealed a significant infiltration of inflammatory cells, with some signs of deep connective tissue damage. Compared to the model group, the Jiawei Jingqin Decoction medium and high-dose groups showed a significant decrease in erythema scores and erythema areas (P<0.05), as well as a reduction in inflammatory exudates and inflammatory cell infiltration. However, the Jiawei Jingqin Decoction low-dose group did not show significant difference (P>0.05), but there was a slight reduction in inflammatory cell infiltration compared to the model group. When compared to the normal control group, the rosacea skin tissues of the model group showed an increase in the number of mast cells, levels of IL-1β, IL-6, TNF-α, mRNA expression of IL-1β, IL-6, TNF-α, TLR2, INF-γ, KLK5, MMP-9, protein expression of KLK5, MMP-9, and the number of CD4+ T-positive cells (P<0.05). Compared to the model group, the Jiawei Jingqin Decoction medium and high-dose groups showed a decrease in the number of mast cells, levels of IL-1β, mRNA expression of IL-1β, TNF-α, and the number of CD4+ T-positive cells. The Jiawei Jingqin Decoction high-dose group showed a decrease in IL-6, TNF-α levels, mRNA expression of IL-6, TLR2, INF-γ, MMP-9, protein expression of MMP-9, KLK5, and the number of CD4+ T-positive cells, as well as a decrease in IFN-γ and STAT1 protein expression (P<0.05) compared to the Model group. ConclusionJiawei Jingqin Decoction has significant improvement effect on the skin symptoms of roseacne in a mouse model. Its mechanism might be related to regulating skin immune inflammatory reactions, thereby reducing the release of pathogenic factors and inflammatory factors.

Hereditary hemochromatosis is a rare autosomal genetic disorder that can cause multi-organ dysfunction in the liver,pancreas,spleen,heart and pituitary gland,with diverse clinical manifestations,make the diagnosis difficult.In recent years,with the deepening of clinical understanding and the development of genetic diagnosis tools,the diagnostic rate of this disease has increased significantly.In this paper,we report a case of hereditary hemochromatosis type 3 involving multiple organs and complicated by severe heart failure,aiming to improve the clinicians'understanding of this disease and reduce the leakage and misdiagnosis.
4.Relationship between coronary flow reserve by CZT SPECT and invasive coronary fractional flow reserve and its potential clinical value

Objective:To evaluate the relationship between regional coronary flow reserve (CFR) obtained from cadmium-zinc-telluride SPECT(CZT SPECT) myocardial functional perfusion imaging (MFPI) and invasive fractional flow reserve (FFR) measured during coronary angiography (CAG) and its clinical value in guiding coronary interventions.Methods:Forty-two patients (30 males, 12 females, age (63.3±9.8) years) who completed CZT SPECT MFPI in the First Affiliated Hospital of Nanjing Medical University from June 2022 to September 2022 and underwent CAG within 3 months were included retrospectively. The concordance of CFR and FFR for diagnosing myocardial ischemia (CFR<2.0 and FFR<0.8) was calculated at the vascular level. The diagnostic efficacy of coronary stenosis≥70% for decreased myocardial blood flow (CFR<2.0) was calculated. Kappa test was used to analyze the data. Results:A total of 126 major coronary arteries were identified in 42 patients, of which 30(23.8%) had a CFR<2.0 by CZT SPECT and 33(26.2%) had stenosis≥70% in CAG. A total of 32 coronary vessels were performed with MFPI CFR and FFR measurements, of which 6 were both decreased and 21 were both normal, so the concordance rate was 84.4%(27/32)( Kappa=0.612, P<0.001). Among 33 coronary vessels with stenosis≥70%, 13 were with CFR≥2.0. Among 30 coronary vessels with CFR<2.0, 10 were with stenosis<70%. When using stenosis≥70% to diagnose CFR decreasing, the sensitivity was 66.7%(20/30), specificity was 86.5%(83/96), positive predictive value was 60.6%(20/33), negative predictive value was 89.2%(83/93), and accuracy was 81.7%(103/126). Conclusions:The concordance between CFR and FFR for the diagnosis of myocardial ischemia is good. Nearly 1/3 of the coronary arteries with decreased CFR have stenosis<70%, whereas nearly 40% of the coronary arteries with stenosis≥70% are not result in myocardial ischemia. Regional CFR determined by CZT SPECT may have potentially significant clinical value in the diagnosis of coronary artery disease and decision-making of coronary intervention.
5.Association of cytomegalovirus infection with T cell senescence and cardiovascular diseases in maintenance hemodialysis patients

Objective:To evaluate the influence of cytomegalovirus (CMV) infection on T cell senescence and cardiovascular disease (CVD) in maintenance hemodialysis (MHD) patients.Methods:It was a single center cross sectional study. Patients aged over 18 years old and received hemodialysis for at least 6 months at the Blood Purification Centre of the Department of Nephrology of Zhongshan Hospital Affiliated to Fudan University from January 2021 to April 2021 were enrolled. Demographic, hematological, nutritional and inflammatory markers were obtained. Anti-CMV-IgM and IgG antibodies were detected using the Roche Elecsys assay. CD28 - T cell was evaluated by flow cytometry. Mann-Whitney U test or Kruskal-Wallis H test was used for anti-CMV-IgG comparison among groups. Spearman correlation and linear regression were used to assess the relationship between anti-CMV-IgG and CD28 - T cell compartment. Logistic regression was used to assess the relationship between anti-CMV-IgG and CVD. Results:A total of 438 MHD patients (270 men and 168 women) were enrolled in the study. The median age was 62 (51, 70) years. The median time on hemodialysis was 57 (21, 100) months. The primary diseases included chronic glomerulonephritis [213 cases (48.6%)], diabetic nephropathy [82 cases (18.7%)], polycystic kidney disease [34 cases (7.8%)], hypertensive renal disease [34 cases (7.8%)], etc. Of these patients, 430 (98.2%) were seropositive for anti-CMV-IgG, 206 (47.0%) had anti-CMV-IgG titers exceeding the upper limit of 500 U/ml. Patients aged over 70 years old were 100% seropositive for anti-CMV-IgG. Patients on HD for more than 5 years had a higher seropositive rate of 99.1% than those with shorter HD duration, although these results were not statistically significant. Spearman correlation analysis showed that the anti-CMV-IgG titers in MHD patients were positively correlated with the proportion of CD4 + CD28 - T cells and CD8 + CD28 - T cells ( r=0.316, P<0.001; r=0.272, P<0.001). Multiple linear regression analysis showed that after adjusting for age and gender, lg[CD4 + CD28 - T cells(%)] and lg[CD8 + CD28 - T cells(%)] were positively correlated with lg[anti-CMV-IgG titers (U/ml)], respectively ( β=0.455, t=8.315, P<0.001; β=0.412, t=7.282, P<0.001). In analyzing the relationship between anti-CMV-IgG titers and CVD, patients were divided into six groups according to age and anti-CMV-IgG level. 6.Selection Mode of Essential Medicine in South Africa and Its Enlightenment to China

OBJECTIVE:To provide reference for further standardizing and improving the selection of essential medicine in China. METHODS :The selection mode of essential medicine in South Africa was introduced ,involving organizational structure and responsibilities ,selection criteria ,selection procedure ,dynamic adjustment mechanism and implementation effect. The suggestions were put forward for improving selection mode of essential medicine in China. RESULTS & CONCLUSIONS : Pharmaceutical and Therapeutics Committees at all levels in South Africa were responsible for collecting applications for essential medicine and submitting them to the Rational Selection Group for preliminary screening ,which was finally decided by the National Essential Drugs List Committee ;the selection criteria was mainly based on WHO essential medicine selection criteria ;the selection basis of essential medicine in South Africa was based on standard treatment guidelines ;a bottom-up normalized user feedback mechanism was established to dynamically adjust the list ,and more attention was paid to drug evidence and evidence level in the review process ;the review points tended to focus on evidence and the level of evidence. In general ,the selection of essential medicines in South Africa had high recognition and good application. It is recommended that China appropriately refer to the practice of South Africa ,take clinical guideline as one of the selection criteria ,establish daily gradual feedback channels , strengthen the review of evidence ,unify Chinese evidence grading system ,and combine Chinese national conditions to finally explore a selection model for essential medicine with Chinese characteristics.
7.Reform Course and Driving Factors of American Medicare Payment Mode and Implication for China

OBJECTIVE:To provide reference for deepening the reform of medical insurance payment mode in China. METHODS:By analyzing the specific reform process and driving factors of American Medicare ,and considering the background of current payment reform in China ,then some suggestions were put forward to promote the reform of medical insurance payment mode in China. RESULTS & CONCLUSIONS :The payment mode of Medicare in the United States had undergone three stages , which were post-payment system ,pre-payment system and value-based payment system. The payment modes included payment by service items ,payment by disease diagnosis related groups (DRGs)and payment by service value. The change was the result of the comprehensive effect of the three systems of technology ,politics and social culture in the United States. The demand for reasonable treatment and control fees drove the change from post-payment system to pre-payment system ,while the crisis of service quality , the rise of service cost and the contradiction between doctors and patients drove the change to value-based payment. Payment mode reform had a positive impact on Medicare in the United States ,reducing medical expenditure and improving the quality of service. It is suggested that China should draw lessons from the experience of the United States in reforming the prepayment system nationwide on the basis of the current DRGs pilot projects. Meanwhile ,in order to avoid the medical quality crisis in the later period of the United States ,it is necessary to introduce the concept of value-based payment ,establish incentive and restraint mechanisms and strengthen the construction of the regulatory supporting system for the whole process.
8.Study on the Implementation of American Post-marketing Re-evaluation of Drugs Legal System and Its Enlightenment to China

OBJECTIVE: To provide reference for the establishment and improvement of the post-marketing re-evaluation of drugs (shorted for re-evaluation) legal system in China. METHODS: Through sorting out American re-evaluation system, this paper focused on the current situation and procedures of the implementation of American re-evaluation system and put forward the suggestions for improving drug re-evaluation system in China. RESULTS & CONCLUSIONS: American re-evaluation system takes enterprises as the main body of execution and the government as the main body of supervision. It has the characteristics of highly informatized and transparent process. The work includes the report of ADR implementation of monitoring systems, the periodic reporting system and the post-listing clinical trials and research systems. The implementation process is to find clues, FDA preliminary review and notification, enterprise further self-examination and review, corporate actions and accept FDA supervision. It is suggested that when establishing the legal system of re-evaluation system in China, the main role of patients should be highlighted, and risk communication should be guided by the public. For example, the Medwatch voluntary reporting system of FDA can be imitated. The unified data collection, storage system and scientific data processing methods can be established. Continuously strengthen the main responsibility consciousness of pharmaceutical enterprises in testing and reporting, and constantly reduce the risk of drug use of patients.
9.Study on the Legal System of Drug Reevaluation in America,European Union ,Japan and the Enlightenment to China

OBJECTIVE: To provide reference for the establishment and improvement of drug reevaluation system in China. METHODS: The design and relevant laws of drug reevaluation system in America, European Union and Japan were reviewed and summarized. The shortcomings of drug evaluation in China were analyzed and suggestions for improvement were put forward. RESULTS & CONCLUSIONS: America, European Union and Japan have formulated relevant laws and systems for post-marketing reappraisal of drugs, as well as specific implementation norms and technical support plans. The legal system of drug reevaluation in America can be divided into ADR monitoring and reporting system, post-marketing drug research system and active drug safety risk monitoring system. The European Union mainly has a monitoring and evaluation system for conventionally marketed drugs, drugs licensed under exceptional circumstances or conditions, and a re-registration system for drugs. Japan’s legal system of reevaluation mainly consists of reexamination system, reevaluation system and other systems. However, there are still some shortcomings in the drug reevaluation in our country, such as weak system, weak legislation, inadequate operability and deterrence, ineffective links between the implementation and application of drug evaluation results, and ineffective allocation of resources. It is suggested that we should integrate the existing reevaluation system to improve the legal system of drug reevaluation in China,starting from the systematic construction of the legal system of drug reevaluation, the construction and standardization of the legislative level, the connection of the legal system of drug reevaluation and the allocation of resources.
10.Relationship between disease activity and choroidal thickness in patients with inflammatory bowel disease

Objective To investigate the relationship between inflammatory bowel disease (IBD) activity and choroidal thickness,and evaluate the utility of a choroidal thickness measurement in assessing IBD activity.Methods A total of 100 eyes of 50 patients of IBD with different disease activity,including 23 patients of ulcerative colitis,27 patients of Crohn's disease (CD).Ninety-six eyes of 48 healthy volunteers were recruited as control group.Choroidal thickness was measured using enhanced depth imaging(EDI)optical coherence tomography.Results Compared with the subfoveal choroidal thickness (294.37 ± 35.04) μm in healthy volunteers,the subfoveal choroidal thickness (349.28 ± 76.57) μm in UC patients with severe disease activity,the subfoveal choroidal thickness (326.71 ± 59.71) μm and (354.24 ± 66.34) μm,respectively,in CD patients with moderate and severe disease activity were found to be increased significantly (all P < 0.05).Conclusion Choroidal thickness should be considered as a potential marker to assess the disease activity in patients with IBD,especially in patients with CD.
