1.Dynamic study of the effect of hyperbaric oxygenation comprehensive treatment on the biochemical parameters of blood in patients with head injury

Objective To observe the changes of malondialdehyde (MDA), superoxide dismutase (SOD-1) in serum and plasma endothelin (ET)and angiotensin II(AⅡ) in patients with severe closed head injury before and after being treated with hyperbaric oxygenation (HBO). Methods Fifty patients were recruited. Thirty-two patients were treated with HBO in addition to the conventional management (HBO group) and the other eighteen patients without HBO except for conventional management served as the controls (control group). Thirty healthy volunteers served as normal controls (normal group). Activities of MDA ,SOD-1,ET and AII were measured with spectrometry and radioimmunoassay in all the subjects, respectively, during the study. Results Before treatment, there was a significant difference between the normal group and HBO group as well as the control group in regard to the activities of the 4 parameters(P0.05) as the 4 parameters were concerned. The patients scored 3-5 with GCS in the control group were significantly different from those scored the same with GCS in the normal group in terms of the SOD-1 and MDA. Higher GCS was found in patients with low activities of MDA, ET and AII but high SOD-1 levels. OBJECTIVE:To probe into the status quo and the trend of the utilization of the oral stasis-eliminating Chinese patent medicine in our hospital in order to provide scientific basis for rational use of this medicine.METHODS:The consumption amount and consumption sum of the oral stasis-eliminating Chinese patent medicine in our hospital during 2006-2008 were reviewed in respect of the DDDs and DDC using DDDs ranking method.RESULTS:During 2006~2008,the consumption amount of the oral stasis-eliminating Chinese patent medicine in our hospital had a steady increase,and the DDDs showed a good synchronism.CONCLUSION:The use of stasis-eliminating Chinese patent medicine is rational in our hospital.
3.Highly sensitive chemiluminescence technology for protein detection using aptamer-based rolling circle amplification platform

A robust,selective and highly sensitive chemiluminescent (CL) platform for protein assay was presented in this paper.This novel CL approach utilized rolling circle amplification (RCA) as a signal enhancement technique and the 96-well plate as the immobilization and separation carrier.Typically,the antibody immobilized on the surface of 96-well plate was sandwiched with the protein target and the aptamer-primer sequence.This aptamer-primer sequence was then employed as the primer of RCA.Based on this design,a number of the biotinylated probes and streptavidin-horseradish peroxidase (SA-HRP) were captured on the plate,and the CL signal was amplified.In summary,our results demonstrated a robust biosensor with a detection limit of 10 fM that is easy to be established and utilized,and devoid of light source.Therefore,this new technique will broaden the perspective for future development of DNA-based biosensors for the detection of other protein biomarkers related to clinical diseases,by taking advantages of high sensitivity and selectivity.

5.Development of extravascular leakage of 6% hydroxyethyl starch 130/0.4 when used for acute hemodilution in pediatric patients undergoing open heart surgery

Objective To evaluate the development of extravascular leakage of 6% hydroxyethyl starch (HES) 130/0.4 when used for acute hemodilution in the pediatric patients undergoing open heart surgery.Methods Forty-eight American Society of Anesthesiologists physical status Ⅰ or Ⅱ pediatric patients,aged 2-12 yr,weighing 12-53 kg,scheduled for elective surgical repair of ventricular or atrial septal defect,were divided into 2 groups (n =24 each) according to age:preschool group (2 yr ≤ age ≤ 6 yr) and school-age group (6 yr<age ≤ 12yr).After anesthesia induction and endotracheal intubation,a volume of 6% HES 130/0.4 equivalent to 10% of the blood volume was infused via the central veins at 0.5 ml · kg-1 · min-1 in two groups.Immediately before infusion of HES (T0) and at 15 and 30 min after the end of infusion (T1,2),blood samples were collected fron the central vein for determination of plasma colloid osmotic pressure (COP) and hemnoglobin (Hb) concentrations.The concentrations of 6% HES 130/ 0.4 in plasma at T1 and T2 and in urine at T2 were measured by the anthranone colorimetric method.Results Compared with the baseline value at T0,the concentrations of Hh in plasma were significantly decreased at T1,2 in preschool group,and the concentrations of Hb in plasma were significantly decreased and plasma COP was increased at T1,2 in school-age group (P<0.05).There were no significant differences in plasma Hb concentrations or COP at each time point between two groups (P>0.05).The plasma 6% HES 130/0.4 concentrations were significantly lower at T2 than at T1 in two groups (P>0.05).Compared with school-age group,the plasma 6% HES 130/0.4 concentrations were significantly decreased at T1,2 in preschool group (P<0.05).There was no significant difference in 6% HES 130/0.4 concentrations in urine between the two groups (P>0.05).Conclusion When 6% HES 130/0.4 is used for acute hemodilution,extravascular leakage happens after acute hemodilution and is more obvious in the preschool pediatric patients undergoing open heart surgery.
6.Prognostic value of pretreatment serum hemoglobin level in early-stage extranodal nasal-type NK/T-cell lymphoma

Objective To investigate the effect of pretreatment serum hemoglobin (Hb) level on the prognosis of early-stage extranodal nasal-type NK/T-cell lymphoma.Methods A retrospective analysis was performed on the clinical data of 175 patients with stage Ⅰ or Ⅱ extranodal nasal-type NK/T-cell lymphoma who were admitted to The Tumor Hospital Affiliated to Shanxi Medical University from 2000 to 2015.The inclusion criteria included Ann Arbor Ⅰ/Ⅱ stage, the primary tumor located in the upper aerodigestive tract, without other malignant diseases, and complete clinical information and follow-up data.Of the 175 patients, 67 received chemotherapy alone, 8 received radiotherapy alone,100 received radiotherapy and chemotherapyed.The survival rate was calculated using the Kaplan-Meier method.The log-rank test was used for univariate prognostic analysis.The Cox regression model was used for multivariate prognostic analysis.Results The univariate analysis showed that pretreatment serum Hb level (≥120 g/L), lactate dehydrogenase (LDH) level (normal), Eastern Cooperative Oncology Group (ECOG) score (0-1), Ann Arbor stage (IE), and radiotherapy were associated with significantly improved progression-free survival (PFS) and overall survival (OS)(P=0.000-0.046).The multivariate analysis showed that pretreatment serum Hb level, LDH level, ECOG score, and Ann Arbor stage were independent prognostic factors for PFS and OS (P=0.000-0.040).Conclusion Patients with a high pretreatment serum Hb level (≥120 g/L) have a better prognosis than those with a low pretreatment serum Hb level (<120 g/L).
7.Mini-puncture hematoma scavenging technique, traditional craniotomy operation, or medicine expectant treatment for patients with hypertensive intracerebral hemorrhage

Two hundred and five patients with hypertensive intracerehral hemorrhage (HICH) received mini-puncture hematoma scavenging technique (MPST; n=80), traditional craniotomy operation (TCO; n=78), or medicine expectant treatment (MET; n=47), respectively. Clinical data demostrated that consciousness level and Glasgow Coma Scale (GCS) was more obviously improved in the MPST group,while postoperative complication rate was relatively lower. The MPST group and TCO group saw no significant difference in over-all matality, although both were significantly lower than the MET group. Severe disability rate in the MPST group was significantly decreased. In comparison with the TCO or MET group, 1 to 3 class activity of daily life score, language function recovery and response rate in the MPST group significantly improved (all P<0.05). We suggest that because of lower severe disability rate, less postoperative complications, and improved quality of life, MPST should be a better treatment of choice for HICH patients.
8.Polymorphisms of the CYP1A1 and GSTM1 genes and their combined effects on individual susceptibility to lung cancer in a Chinese population

Objective To investigate the polymorphisms of CYP1A1 and GSTM1 genes as well as their separate and combined effects on individual susceptibility to lung cancer. Methods This study was conducted in Jiangsu Province. In the matched 106 pairs of patients with lung cancer and non-cancer control persons,genomic DNA were prepared from peripheral blood samples. The genotypes of CYP1A1 Ile/Val and GSTM1+/0 polymorphisms were detected by the allele-specific (AS)-PCR and multi-differential(MD)-PCR. Results Individuals with genotype Val/Val of CYP1A1,genotype 0/0 of GSTM1, combined genotypes GSTM1 0/0 and CYP1A1 Val/Val, or combined genotypes GSM1 0/0 and CYP1A1 Ile/Val had higher relative risk than those with the corresponding common genotypes and combined genotypes; their odds ratios were 4.02(P=0.03), 1.92(P=0.019), 9.38(P=0.04) and 3.27(P=0.01), respectively. Conclusion There is a synergy of susceptible genotypes GSTM1 0/0 and CYP1A1 Val/Val or CYP1A1 Ile/Val to enhance the individual susceptibility to lung cancer.
9.Feature extraction and classification of EEG for mental tasks based on wavelet packet analysis.

This paper explores the use of wavelet packet analysis to extract features from spontaneous electroencephalogram (EEG) during three different mental tasks. Artifact-free EEG segments are transformed to multi-scale representations by dyadic wavelet packet decomposition channel by channel. Their feature vectors formed by energy values of different sub-spaces EEG components are used as inputs of a radial basis function network to test the classification accuracies of three task pairs. The results indicate that the classification accuracies of the wavelet packet analysis method are significantly better than those of autoregressive model method. Wavelet packet analysis would be a promising method to extract features from EEG signals.

10.Computed tomography and magnetic resonance imaging features of IgG4-related pancreatitis

Objective To summarize the computed tomography (CT) and magnetic resonance imaging (MRI) features of IgG4-related pancreatitis.Methods The retrospective and descriptive study was conducted.The clinical data of 23 patients with IgG4-associated pancreatitis who were admitted to Sir Run Run Shaw Hospital of Zhejiang University School of Medicine from November 2012 to May 2018 were collected.There were 21 males and 2 females,aged from 45 to 83 years,with an average age of 63 years.Patients underwent upper abdominal enhanced CT and enhanced MRI examinations.Observation indicators:(1) imaging examination of the patients;(2) imaging characteristics on CT and MRI examinations;(3) follow-up.Follow-up using outpatient examination including laboratory and imaging examination was performed to detect clinical symptoms and signs once a month within 3 months postoperatively,once every 3 months within 3 months to 1 year postoperatively,once every 6 months with 1-2 years postoperatively,and once a year after 2 years postoperatively up to August 2018.Measurement data with skewed distribution were represented as M (range).Count data were described as absolute numbers.Results (1) Imaging examination of the patients:19 of 23 patients underwent upper abdominal enhanced CT combined with upper abdominal enhanced MRI scanning,3 underwent upper abdominal enhanced CT combined with upper abdominal MRI plain scanning,and 1 underwent only upper abdominal enhanced MRI scanning.(2) Imaging characteristics on CT and MRI examinations:pancreatic manifestations included 7 aspects.① Location of lesion and morphological classification:of 23 patients,17 were diffuse type,showing sausage-like appearance;4 were focal type,including 2 with mass in the pancreatic head and 2 with mass in the pancreatic body and tail;2 were multiple type,including 1 with mass in the pancreatic head,body and tail,and 1 with mass in the pancreatic body and tail.② Density of lesions on CT examination:of 23 patients,22 received CT plain scanning,including 10 with equal density and 12 with slightly low density;1 didn't receive CT plain scanning.③ Signal of lesions on MRI examination:on T1 weighted imaging,16 of 23 patients showed homogeneous slightly low signal,4 showed isointensity signal,2 showed mixed slightly low signal,and 1 showed slightly high signal.On T2 weighted imaging,21 of 23 patients showed homogeneous slightly high signal,1 showed isointensity signal,and 1 showed mixed slightly high signal.Of 23 patients,19 underwent diffuse weighted imaging (DWI) and 4 didn't undergo DWI.There were 17 patients with slightly high signal and 2 with high signal on DWI.On apparent diffusion coefficient imaging,10 patients showed slightly low signal,and 9 showed low signal intensity.④ Calcification:of 23 patients,2 had multiple calcifications including 1 of diffuse type with calcification located at pancreatic head,and 1 of focal type with calcification located at pancreatic body and tail;20 had no calcification;1 without CT plain scanning cannot be judged calcification.⑤ Enhancement pattern:23 patients showed progressive delayed enhancement of pancreatic lesions on enhancement scanning,homogeneous or heterogeneous enhancement in the arterial phase,and further enhancement in the portal venous phase and delayed phase (enhancement degree of focal type and multiple type was basically consistent with that of normal pancreas).⑥ Halo sign surrounding lesions:of 23 patients,7 had no halo sign,including 2 of diffuse type,3 of focal type,and 2 of multiple type;16 had halo sign (15 of diffuse type and 1 of focal type),including 11 with halo sign surrounding pancreatic body and tail,3 with halo sign surrounding whole pancreas,and 1 with halo sign posterior pancreatic body and tail,and 1 with halo sign anterior pancreatic head.⑦ The main pancreatic duct:14 of 23 patients had irregular stenosis of the main pancreatic duct,7 had mild dilatation of the main pancreatic duct,and 2 had no dilatation of the main pancreatic duct.Extra-pancreatic manifestations included 3 aspects.① Bile duct dilatation:10 of 23 patients had simple intra-hepatic and extra-hepatic bile duct dilatation,8 had no intrahepatic or extra-hepatic bile duct dilatation,and 5 had intra-pancreatic common duct stenosis with upstream bile duct dilatation.② Extra-pancreatic organ involvement:18 of 23 patients had extra-pancreatic organ involvement (5 with gall bladder involvement,4 with intra-hepatic and extra-hepatic bile duct and gall bladder involvement,3 with intra-hepatic and extra-hepatic bile duct involvement,1 with lung involvement,1 with spleen involvement,1 with common duct involvement,1 with kidney involvement,1 with lung and intra-hepatic and extra-hepatic bile duct involvement,1 with fibrosis of mesenteric root),and 5 had no extra-pancreatic organ involvement.③ Retroperitoneal lymph nodes:2 of 23 patients had enlarged retroperitoneal lymph nodes,21 had no enlarged retroperitoneal lymph nodes.(3) Follow-up:of 23 patients,19 were followed up for 3-74 months,with a median time of 22 months.Sixteen of 19 patients had various degrees of improvement in clinical symptoms,laboratory and imaging manifestations after standardized hormonotherapy,and 3 had no response to hormonotherapy.Conclusions CT and MRI examinations have important diagnostic value for IgG4-related pancreatitis.The pancreas mainly show diffuse swelling and halo sign surrounding lesions is a typical imaging characteristic.
