1.Reconstructing anterior cruciate ligament in rabbits using autologous tendon combined with artificial ligament

BACKGROUND: Autologous tendon and artificial tendon are both the important grafts that can reconstruct the ruptured anterior cruciate ligament, but each has their defects and excellents. OBJECTIVE: To investigate the biomechanical change and histological rejection of reconstructing the anterior cruciate ligament of the rabbits with autologous tendon and artificial ligament. DESIGN, TIME AND SETTING: A controlled observation was performed from September 2007 to February 2008 in the Orthopedics Laboratory and the Biological Laboratory of Zhengzhou University (Zhengzhou, Henan, China). MATERIALS: Twenty-eight adult New Zealand rabbits of SPF grade were offered by Zhengzhou University Animal Experimental Center, weighing 2.4-3.1 kg with a mean of 2.8 kg, irrespective of genders. MB66 braid was produced by Smith & Nephew Medical (Shanghai) Limited. METHODS: The tendons of extending toe were harvested in both lower extremities, one tendon was wrapped with artificial ligament, while the other was only processed into the terminal wove dovetail. Anterior cruciate ligament was excised from bilateral knee joints in rabbits through the parapatellar incision, then femoral and tibial bones were drilled to implant the autologous tendon and artificial ligament on one side (serving as combined tendon group), whereas only autologous tendon on the other side (serving as pure tendon group). Both sides were sutured through bone bridge. Every 7 rabbits were killed under anesthesia at 2, 4, 6, and 8 weeks postoperatively. MAIN OUTCOME MEASURES: Gross observation and histological determination were carried out in the reconstructed anterior cruciate ligament of rabbits. The maximal breaking force of both grafts were detected. RESULTS: At 2 and 4 weeks postoperatively, the cells of reconstructed grafts disappeared and showed necrosis. Inflammatory cells infiltrated between autologous tendon and artificial ligament, there were cells growing from the edge to the center. At 6 weeks postoperatively, the autologous tendon had a little loose and disfiguration. The synovial membrane and cells hyperplasia were obvious. The synovial membrane filled the combined tendon and the infiltrative inflammatory cells reduced. The proliferative cells remarkably trended to the center. At 8 weeks postoperatively, the grafts were mostly substituted by new cells, and the artificial ligament was completely parceled by the synovial membrane, but there were still some gaps; The combined tendon exhibited no loose or disfiguration, the infiltrative inflammatory cells disappeared. The cells in grafts were close to normal cells in anterior cruciate ligament, but fibrocytes arranged in disorder and fibers showed a verticality. 2.Determination of Emodin and Chrysophanol in Shutongantai Capsules by HPLC

Objective To establish a quality standard of Shutongantai Capsules. Methods Thermo C18 column was used with methanol-0.1% phosphoric acid as mobile phase. The flow rate was 1.0 mL/min, detection wavelength was 254 nm, and column temperature was 30 ℃. Results Emodin showed a good linear relationship in the range of 0.016-0.080 μg (r=0.999 6). Chrysophanol showed a good linear relationship in the range of 0.032-0.160 μg (r=0.999 8). Conclusion The established method is simple, feasible and reproducible, and can be used for the quality control of Shutongantai Capsules.
3.Content Determination of Berberine, Epiberberine, Coptisine and Palmatine in Shutongantai Capsules by HPLC

Objective To build a method for determination of berberine, epiberberine, coptisine and palmatine in Shutongantai Capsules by HPLC.Methods Thermo C18 column (5μm, 4.0 mm× 250 mm) was used with acetonitrile-0.05 mol/L potassium dihydrogen phosphate solution (50∶50, 0.4 g sodium dodecyl sulfate was added in each 100 mL solution, to adjust the pH 4.0 with phosphoric acid) as mobile phase;the flow rate was 1.0 mL/min;detection wavelength was 345 nm;column temperature was 30℃.Results Berberine hydrochloride showed a good linear relationship in the range of 0.180 34-0.901 68μg (r=0.999 8).Conclusion The method is simple, accurate and reproducible, and can be used for the quality control of Shutongantai Capsules.
4.Comparing three types of operation for treating cholecystolithiasis with choledocholithiasis

Objective To evaluate the different effeotions among laparoseopic choleeysteotomy (LC)combined with bile duct exploration (LCBDE)and stone removal,LC with endoacopic sphincterotomy (EST),and open-choleeysteetomy (OC)with exploration of common bile duct (ECBD)for treating choleeys-tolithiasis with choledocholithiasis.Methods Among 289 cases of choleeystolithiasis and choledocholithla-sis,132 patients were treated by OC with ECBD,36 cases by LC with LCBDE,121 cases by EST combined with LC.The stone residual rate and leakage of bile rate,operation time,the loss of blood in operation,re-covery time of gastrointestinal function,length of hospital stay of the three groups were compared.Results There was no statistical difference in the stone residual rate and leakage of bile rate among the three groups,but OC with ECBD group had significantly longer operation time,more loss of blood in operation,later re-covery time of gastrointestinal function and longer hospital stay than the other groups.Conclusions There are respective indications,advantages and disadvantages in the three groups.We found that the operation of LC with LCBDE is the better choice for the patients of cholecystolithiasis with choledocholithiasis,single choledocholithiasis;However,the patients with obviously infection of biliary tract or acute pancreatitis is not suitable for this method.
5.Preparation and optimization of fibrin-gel-coated vancomycin alginate beads

Objective To prepare and optimize fibrin-gel-coated vaneomycin alginate beads (FG-Vanco-AB)and investigate their possible use in treatment of osteomyelitis or prevention of infection.Methods Vancomycin alginate beads were produced by dropping vancomycin and alginate mixed liquor into calcium chloride solution.Beads including high vancomycin content were prepared and chosen by optimizing different concentrations of vancomycin solution and alginate solution.These beads were coated with fibrin gel formed by different concentrations of fibrin and the same concentration thrombin.The optimized beads were selected based on available release time,when vancomycin in medium could kill Staphylococcus aureus(ATCC25923). Results Higher content of vancomycin in bead resulted in increase of vancomycin concentration and alginate concentration in mixed liquid.The highest vancomycin content beads were prepared by 16%alginate and 50 mr/ml vancomycin,up to(27.36±0.90)%.The further results showed that vancomycin concentrations from beads coated with fibrin at 75 mg/ml and thrombin at 400 IU/ml could kill Staphylococcus aureus and remained above the breakpoint sensitivity for 19 days.Conclusion The available release time is prolonged,and the possibility of clinical use is conspicuously increased after vancomycin beads are optimized by adjusting the rate of mixed component and fibrin gel coat.
6.Long-term clinical observation of anterior allogenic iliac graft in treatment of thoracolumbar fractures

Objective To evaluate the long-term clinical efficacy of iliac bone block allograft in anterior reconstruction of thoracolumbar fractures.Methods A follow-up study was carried out on 93 patients with thoracolumbar fractures treated by anterior decompression and interbody fusion with iliac bone block allograft from February 2004 to March 2007.The radiographic and clinical outcomes were retrospectively assessed.Results Sixty-six patients were followed up for 60-97 months (mean,78 months).The effective interbody fusion rate was 100%,with the fusion time of 6-12 months (mean,10 months).The Cobb' s angle corrected from pre-operative (21.6 ± 8.3)° to post-operative (5.8 ±5.2)°,but when the infusion became effective,the angle showed average loss of 2.4°.All the 66 patients were associated with various degrees of neurological deficiency,but the patients with incomplete nerve dysfunction obtained different degree of recovery.Conclusions Iliac bone block allograft is effective in reconstruction of spinal anterior-middle column stability with the aid of internal fixation instruments.Thereby,it may be a potential alternative to autograft in the treatment of thoracolumbar fractures and can attain early and late stage persistent spinal stability.
7.Apoptosis of K562 Cells Induced by 3,7-Dinitrodibenzobromonium Salt

Objective: To investigate the antitumor activities and apoptosis mechanisms of 3,7-dinitrodibenzobromonium salt (cBr) on human chronic myelogenous leukemia K562 cell line in vitro. Methods: The antitumor activities of cbr were tested by Trypanblau exclusion test, MTF coloretric assay and clonal forming test, and the mechanisms were studied by morphological analysis (microscopy, fluoreseencemicroscopy, electron microscopy) and flow cytometry analysis. Resuits: cBr could inhibit of the proliferation K562 cells and kill them significantly. The effects varied with different time and doses, and IC50 of cell colony forming rate was 0. 497 mmoL/L. Apoptosis rate tested by flow cytometry was 70.34%,and apoptotic characteristics could be found by microscopy and electron microscopy. Conclusion: cBr could inhibit K562 cell proliferation greatly, and could induce K562 cells into apoptosis.
8.Risk factors analysis of white matter lesions of different degrees in the elderly aged 80 years and over

Objective To investigate the independent risk factors of cerebral white matter lesions (WML) of different degrees in the elderly aged 80 years and over,and provide the evidences for forecasting the prognosis of WML.Methods Brain magnetic resonance images (MRI) findings in 151 people aged 74 to 93 years were collected and analyzed.According to the severity of WML in brain MRI using the Fazekas Scale,the persons were divided into non-WML (control) group,mildWML (grade 1 WML) group and moderate-to-severe WML (grade 2 WML) group.The cognitive score,vascular risk factors,cerebral hemodynamic and arteriosclerotic index,and radiological features were compared among the three groups.Subsequent one-way ANOVA and multivariate logistic analysis were performed to determine the statistically significant factors and the independent risk factors among groups.Results The statistically significant factors with one-way ANOVA analysis among the three groups were cognitive performance (F = 48.595,P = 0.000),hypertension (x2 =7.052,P=0.029),cigarette history (x2 = 19.476,P= 0.000),cholesterol (TC) (F= 3.086,P=0.049),Crouse score (F=3.968,P=0.021) and multiple cerebral atrophy indexes.When compared with control group,cigarette history (OR 2.031,95%CI 1.244-1.317),lacunar infarction (LI)numbers (OR 2.031,95%CI 1.316-4.015) and cholesterol (OR 1.610,95%CI 0.972-2.668) were independent risk factors in grade 1 WML group (all P<0.05).The independent risk factors between grade 1 and 2 WML group were cognitive performance (OR 0.276,95%CI 0.143-0.532),cigarette history (OR 2.262,95% CI 1.260-4.059),and sylvian fissure ratio (SFR) (OR 1.954,95% CI 1.013-3.768) (all P<0.05).The independent risk factors between the grade 2 WML group and control group were cognitive performance (OR 0.091,95%CI 0.030-0.273),bicoudate ratio (BCR)(OR 2.511,95%CI 1.147-5.499),Crouse score (OR 2.304,95%CI1.127-4.712)and LI numbers (OR 2.200,95%CI 1.028-4.707) (all P<0.05).Conclusions Mild WML patients have no significant abnormalities in cognition,brain atrophy and cerebral atherosclerosis.Moderate to severe WML patients manifest remarkable cognitive disorder,cerebral atherosclerosis and brain atrophy.Compared with the controls,cognitive performance,BCR,Crouse score,LI numbers were the independent risk factors for moderate-severe WML patients.
9.Patent-blue-liposomes for lymphography and the detection of their biological property

Objective To prepare patent-blue-lipesomes(PB-LPS),a lymph-mapping developer.Methods PB was encapsulated in the liposomes using the passive loading method.The female Wistar rats were injected subcutaneously with the PB-LPS,PB,blank liposomes or isotonic rmtrium chloride at the heel solos,to study the stained situation of the lymph nodes and chemical analysis oftheir ALT,AST,BUN,Cr.Results The lymph nodes were obviously blue staining after injected PB-LPS or PB,the blue smimng ratios were 40%,20%(P<0.05),and the effect of PB-LPS on tracing of lymphy nodes was better than PB.No significant difference was found in ALT,AST,BUN and Cr.Conclusion The PB-LPS can be successfully prefabricated with such advantage as avirulence and high entrapment efficiency by passive loading method.
10.Prevalence survey of metabolic syndrome and its components in the elderly at Tanggu Community in Tianjin

Objective To investigate the prevalence of metabolic syndrome (MS) and its components among the elderly at Tanggu community in Tainjin. Methods The elderly inhabitants were drawn out randomly from one community at Tanggu in Tianjin to examine the MS-related indexes. There were 623 samples with complete data aged (67.4±6.0) years, including 261 males with mean age of (68.3±6.1) years and 362 females with mean age of (66.9±6.0) years. Diagnosis of MS was done by criteria of Chinese Medical Association Diabetes Branch in 2004. Results The standardized prevalence of MS for the elderly in the community was 18.5% with the highest risk group aged 60~69 years. Standardized prevalences of hypertension, high triglycerides, high blood sugar, overweight, obesity, low HDL-C were 49.5%, 38.8%, 25.3%, 42.0%, 4.2%, 3.0% in the elderly. Conclusions The prevalence of MS in the elderly is lower in Tanggu community in Tianjin than in Shanghai, Foshan in Guangdong, Qingdao in Shandong and Beijing city. The condition in female is more severe than in male, the highest prevalence occurs in hypertension, and overweight, high triglycerides and high blood sugar follow it, but few of them are obesity, low HDL-C is fewer too.
