Objective To study the expression of interleukin-6 (IL-6) and hepcidin in patients with diffuse large B-cell lymphoma (DLBCL) and their significance in anemia. Methods 45 DLBCL patients with or without anemia were analyzed. Peripheral blood samples were collected during diagnosis, and the concentrations of IL-6, hepcidin, serum ferritin and hemoglobin (Hb) were measured. 24 healthy volunteers were collected as controls. Results The levels of plasma hepcidin and IL-6 in patients with DLBCL were (347±171)μg/L and 0.27 ng/L (0-9.61 ng/L), respectively, and compared with those [(175 ± 92)μg/L] and 0 ng/L in healthy controls, the differences were statistically significant (both P<0.001). Plasma hepcidin levels in patients with high lactate dehydrogenase (LDH) (P=0.003), B symptoms (P=0.040) or age-adjusted international prognostic index (IPI)>1 (P=0.010) were increased. The levels of IL-6 in patients of male (P=0.003), stage Ⅲ-Ⅳ (P=0.008) or IPI>1 (P=0.004) were significantly higher. The level of hepcidin was highly correlated with serum ferritin (r=0.77, P<0.001), weakly correlated with IL-6 (r=0.31, P=0.030), and not correlated with Hb (r=-0.12, P=0.3). There was a negative correlation between IL-6 expression and Hb (r=-0.35, P=0.009). Multivariate analysis showed that IL-6 could predict anemia (P=0.03), whereas hepcidin could not (P=0.89). Conclusion The elevated hepcidin level is frequent in DLBCL, and the elevated IL-6 plays the major role in the development of anemia.
6.Last critical step for transanal total mesorectal excision: anastomosis.
Chinese Journal of Gastrointestinal Surgery 2019;22(3):238-241
Colorectal anastomosis remains a key aspect in mid- and lower-rectal surgery, particularly in patients who are male, obese, status post neoadjuvant chemoradiation, or possess narrow pelvic anatomy. Transanal total mesorectal excision (taTME) and transabdominal total mesorectal excision (TME) both primarily allow position of the tumor to dictate whether circular stapling or hand-sewing is utilized to achieve anastomosis. Given that taTME only requires a single circular stapler to achieve anastomosis, it consequently decreases the risk of jeopardizing blood supply to the anastomotic site. Should transanal hand-sewing be pursued, numerous points in performing the purse-string suture warrant particular attention: (1) Before suturing, separate the distal rectum to be anastomosed from the surrounding tissues, and ensure the full-layer suture of the distal purse-stitched suture. (2) It is recommended that beginners complete the suturing process under direct visualization without removing the transanal platform. (3) Commonly used anastomosis designs include end-to-end, side-to-end, J-pouch, or coloplasty, depending on patient characteristics and surgeon preferences. Our single-institution clinical experience suggests that taTME patients who underwent neoadjuvant chemoradiation or transanal hand-sewn anastomosis should still have a temporary colostomy created. To date, literature has yet to demonstrate the superiority of taTME anastomosis outcomes. This publication aims to point out anastomosis techniques and safety pointers, as well as clinical experiences.

OBJECTIVE:To study the effects of 3 extracts of Acanthopanax sessiliflorus fruits on the proliferation and apoptosis of human hepatocarcinoma cells SMMC-7721,and to provide reference for confirming the mechanism of anti-tumor effect. METHODS:MTT assay was adopted to investigate the effects of low-mass concentration,medium-mass concentration and high-mass concentration of ethanol extract(0.92,1.84,3.68 mg/mL),crude polysaccharide extract(0.06,0.12,0.24 mg/mL)and refined polysaccharide extract (0.04, 0.08, 0.16 mg/mL) from A. sessiliflorus fruits on the proliferation and apoptosis of SMMC-7721 cells after treated for 24,36,48 h,respectively. Flow cytometry was used to investigate the effects of 1.84 mg/mL ethanol extract,0.24 mg/mL crude polysaccharide extract and 0.16 mg/mL refined polysaccharide extract on cell cycle and cell apoptosis after treated for 24 h. The above tests were all negative control(only adding cells without drugs). RESULTS:Compared with negative control,3 extracts of A. sessiliflorus fruits could significantly inhibit the proliferation of SMMC-7721 cells (P<0.01),could significantly decrease the percentage of SMMC-7721 cells in G0/G1 and G2/M phase(P<0.01),could significantly increase the percentage of SMMC-7721 cells in S phase (P<0.01) and the apoptosis rate of SMMC-7721 cells (P<0.05);especially the effects of ethanol extract from A. sessiliflorus fruits were the most obvious. CONCLUSIONS:Three extracts of A.sessiliflorus fruits can inhibit the proliferation of human hepatocarcinoma SMMC-7721 cells, block SMMC-7721 cells in S phase and induce the apoptosis of SMMC-7721 cells.
