1.The value of plasma 8-iso-prostaglandin F2 alpha determination in the neonatal hyperunconjugated bilirubinemia

Objective To investigate the changes of plasma 8-iso-prostaglandin F2 alpha (8-iso-PGF2α) before and after treatment in the neonatal hyper-uneonjugated bilirubinemia, so as to confirm theperoxidation damage of serum tolal bilirubin (TBil). Methods One hundred and twenty-six neonates who were 2-7 days old were divided into three groups according to TBil concentration: group hyper-bihrubinemia A (HBE-A)(51 cases), group hyperbilirubinemia B (HBE-B)(25 cases), and healthy control group (50 cases). The serum TBil and unconjugated bilirubin contents were measured by automatic biochemical analyzer. The level of 8-iso-PGF2α in plasma was measured by enzyme linked immunosorbent assay. Results Plasma 8-iso-PGF2α level in healthy control group was (13.84±6.13)ng/L, 95% confidence interval was 10.88-18.02 ng/L.Plasma 8-iso-PGF2α level before treatment and 7 days after serum TBil recovering to be normal in group HBE-A and group HBE-B were (19.56±2.74), (14.13±5.93) ng/L and (22.30±6.33), (14.88±6.44) ng/L, respectively. Plasma 8-iso-PGF2α level increased with serum TBil increased. Plasma 8-iso-PGF2α level decreased with serum TBil decreased. Conclusions The neonatal hyper-unconjugated bilirubinemia can induce peroxidation damage.The determination of plasma 8-iso-PGF2α level may accurately evaluate the bilirubin- induced peroxidation in early stage and progression of jaundice, suggesting the antioxidants should be added to treat neonatal hyper-unconjugated bilirubinemia.
2.Preparation of leukoreduced platelet concentrates from pooled buffy coats in a mixture of solutions for medical use

Objective To develop a method to produce leukoreduced platelet concentrates(LR-PCs) from pooled buffy coats in additive solution(a mixture of solutions for medical use).Methods LR-PCs were made from 6 pooled buffy coats(12 whole-blood units) and 220g additive solution,including 90% multiple electrolytes injection solution,8% ACD-A and 2% 50g/L NaHCO3 injection solution.After centrifugation,the PCs were leukoreduced with a filter and stored in a 600ml platelet storage bag.LR-PCs were prepared in a closed system.Results Routinely produced LR-PCs(n=30) contained(2.96?0.31)?1011 platelets with a volume of(270? 32)ml.The WBC and RBC count were(1.3?0.2)?106 and(5.8?1.1)?109 per unit,respectively.The CD62P expression was(22.5? 10.6)%.After 8-day storage,the in vitro quality of LR-PCs,including pH,hypotonic shock response(HSR) and CD62P expression were 7.14?0.04,(54.0?8.2)% and(45.7?13.8)%,respectively.Conclusion In terms of the in vitro quality of LR-PCs,the method of preparing LR-PCs from pooled buffy coats in mixing solutions is feasible.
3.Clinical characteristics, esophageal motility and acid exposure in elderly patients with gastroesophageal reflux disease

Objectlve To review the clinical characteristics,esophageal motility and acid exposure in elderly patients with gastroesophageal reflux disease (GERD).Methods Two hundred and sixty patients aged 18 -81 years were diagnosed as gastroesophageal reflux disease from 2005 to 2011 in outpatient clinic,the patients were divided into elderly group ( ≥ 60 y) and control group ( < 60 y).The incidence of esophagitis,severity of esophagitis,esophageal motility and esophageal acid exposure were analyzed and compared between two groups.Results The incidence of esophageal hiatal hernia,non-typical reflux symptoms and extra-esophageal symptoms in elderly group were higher than that of control group(45.1% vs.30.6%,36.3% vs.27.2% and 37.1% vs.25.2%,P =0.016,0.005 and 0.037,respectively).There was no significant difference in esophagitis incidence between two group (55.8% vs.50.3%,P =0.250).The incidence of esophagitis graded as LA-B to LA-D were higher in elderly group( 19.5% vs.10.2% and 8.8% vs.2.7%,P =0.034 and 0.030,respectively).The percentage of upright acid clearance time,supine reflux time of pH <4 and the DeMeester score of elderly group were higher than those of control group (P =0.000,0.006 and 0.001,respectively).Lower esophageal sphincter pressure (LESP) of elderly group was lower than that of control group ( 13.36 ±6.39 vs.15.57 ±7.01,P =0.009).Conclusion In comparison with young and middle-aged patients,the elderly GERD patients have lower esophageal motility,longer esophageal acid exposure time and higher frequency of severer esophagitis.
4.A study of esophageal function and reflux characteristics of gastroesophageal reflux disease in patients presenting with chronic cough

ObjectiveTo investigate the characteristics of gastroesophageal reflux disease (GERD) with chronic cough by the results of combined multichannel intraluminal impedance and high-resolution manometry (MII-HRM) procedure and 24-hour muhichannel intraluminal impedance combined pH (MII pH) monitoring.Methods From March 2010 to November 2010,consecutive patients of GERD with chronic cough (more than 8 weeks) admitted to Beijing Chaoyang Hospital of Capital Medical University underwent 24-hour MII-pH monitoring and MII-HRM procedure with symptom association probability(SAP) over 95%.Data of lower esophageal sphincter (LES) pressure,LES relaxation,LES residual pressure,esophageal body peristalsis function and swallow bolus transit (liquid/viscous) was collected and the result of 24-hour MII-pH monitoring was analysed by the computer software containing reflux episode activity(acid/nonacid,upright/recumbent),proximal extent,acid exposure and mean acid/bolus clearance time.Seventeen patients of GERD with typical reflux symptom were selected as the control group.ResultsComparing with patients of GERD with typical reflux symptom,patients of GERD with chronic cough showeddecreased upper esophageal sphincter pressure (UESP) [( 122.55 ± 60.48 )mm Hg vs ( 86.37 ± 41.35 ) mm Hg,P < 0.05,1 mm Hg =0.133 kPa ],higher percentage of abnormality peristalsis [(9.47 ± 15.63 ) % vs ( 22.16 ± 17.45 ) %,P < 0.05 ],degraded esophagus transmittability of liquid substance [( 88.82 ± 12.23) % vs ( 71.68 ± 23.06 ) %,P < 0.05 ],more reflux episode activity ( acid/nonacid ) in supine position and proximal reflux episode (nonacid) in supine position (P < 0.05 )and longer mean bolus clearance time( P < 0.05 ).Conclusion Decrease of the UESP,increase of the reflux episode activity (acid/nonacid) in supine position and proximal reflux episode (nonacid) in supine position,lengthened mean bolus clearance time in supine position and the degraded esophagus clearance ability may all correlated with the pathogenesy of GERD with chronic cough.
5.Influence of jiaweibugan decoction on VEGF expression of sciatic nerve in experimental diabetic rats

Objective To observe the effect of jiaweibugan decoction on VEGF expression of sciatic nerve in experimental diabetic rat and explore the preventive and therapeutic effects of iiaweibugan decoction on peripheral neumpathy in~xperimental diabetic rats.Methotis The diabetic rat model was established by streptozotocin(STZ).The rats were killed on the 4th or 8th week from the beginning of treatment respectively,and VEGF mRNA of sciatic nerve was detected by reverse-transcriptase polymerase chain reaction(RT-PCR).Resuits The results of RT-PCR showed that VEGF expression ofthe model group and treated group on the4th or8th week WaS higherthan that of the normal control group(P<0.05).Compared with the model group,VEGF expression in the diabetic rats treated byjiaweibugan decoction increased markedly at the end of4th week(P<0.05),while decreaSed significantly at the end of8th week(P<0.05).Conclusion Jiaweibugan decoction has a proventive and therapeutic effect on peripheral neumpathy in experimental diabetic rats.
6.The relationship between gastroesophageal reflux disease and idiopathic pulmonary interstitial fibrosis

Objective To determine the prevalence of gastroesophageal reflux disease (GERD) in patients with idiopathic pulmonary interstitial fibrosis (IPIF). Methods From December 2006 to January 2008, 24 consecutive patients with IPIF admitted to Beijing Chaoyang Hospital underwent 24-hour esophageal pH monitoring and esophageal manometry. Meanwhile, 23 patients with diffuse parenchymal lung disease (DPLD) (excluding IPIF) admired to the hospital in the same period served as a control group. Comparison of the prevalence of pathologic esophageal acid exposure GERD symptoms, and ineffective esophageal motility (IEM) between the two groups was made. In this study, nocturnal acid exposure is defined as acid reflux episodes occurring from 10pro to 6am. Results (1) 16 out of the 24 (66. 7%) patients with IPIF were demonstrated to have pathologic esophageal acid exposure; the prevalence of GERD in IPIF patients was significantly higher than that in other DPLD patients, whose prevalence was 26. 1% (P<0.05); (2) 87.5% patients with IPIF and GERD (GERD-IPIF) had nocturnal acid exposure episodes; (3) only 37.5% of the GERD-IPIF patients was found to have typical GERD symptoms such as heartburn and regurgitation; (4) The prevalence of IEM was similar in IPIF and other DPLD patients, being 42.9% and 39. 1% respectively (P >0. 05). Conclusions IPIF patients have higher prevalence of GERD and most of them usually do not show typical reflux symptoms. It is hereby suggested that IPIF patients should be screened with pH monitoring for GERD.
7.Biological characteristics of umbilical cord mesenchymal stem cells in different conditioned media

BACKGROUND:The growth of mesenchymal stem cells in vitro in different conditioned media is different evidently. So it is necessary to choose a more suitable medium. OBJECTIVE:To contrast and observe the proliferation of human umbilical cord mesenchymal stem cells in three kinds of media and to check the immunophenotype and differentiation ability of mesenchymal stem cells. METHODS:Human umbilical cord mesenchymal stem cells were col ected by explant method in sterile conditions. After subculturing by T75 incubation bottles, the third generation of mesenchymal stem cells were cultured in Dulbecco’s modified Eagle’s medium/Ham’s nutrient mixture F-12 medium containing 5%fetal bovine serum, Dulbecco’s modified Eagle’s medium/Ham’s nutrient mixture F-12 containing 10%fetal bovine serum and Mesen PRO RSTM medium. After 1, 3, 5, 7 days of culture, the cells were counted to draw a growth curve. Immunophenotype of the third generation of umbilical cord mesenchymal stem cells were determined by flow cytometry and the ability of osteogenic and adipogenic differentiation was also detected. RESULTS AND CONCLUSION:The third generation of cells cultured highly expressed CD44, CD73, CD90, CD105, but did not express CD29, CD31, CD34, HLA-DR. The oil red O staining showed a lot of little red lipid drops after adipogenic induction;alizarin red staining showed osteoblast-like cells after osteogenic induction, indicating umbilical cord mesenchymal stem cells in vitro have the potential of multi-directional differentiation. After observation and counting, the colony and shape of cells cultured in Dulbecco’s modified Eagle’s medium/Ham’s nutrient mixture F-12 containing 10%fetal bovine serum were superior to those cultured in the other two media. Therefore, it is concluded that Dulbecco’s modified Eagle’s medium/Ham’s nutrient mixture F-12 containing 10%fetal bovine serum is preferred for cellsubculture.
8.Meropenem effects on biological characteristics of umbilical cord-derived mesenchymal stem cells during umbilical cord collection

BACKGROUND:Due to the difficulty in the control of delivery and col ection process, antibiotics are often added into the preservation fluid in order to avoid bacterial contamination but not affect cellgrowth and proliferation. OBJECTIVE:To observe the effects of meropenem on the proliferation and differentiation potential of umbilical cord-derived mesenchymal stem cells. METHODS:There were two groups in this experiment:control group, preservation fluid with penicil in-streptomycin (final concentration of 100 U/mL);experimental group, preservation fluid with meropenem (final mass concentration of 1.0 mg/L). 100 umbilical cord samples were col ected in each group, and the positive rate was calculated. After isolation and culture, the passage 3 cells were used to draw a growth curve, flow cytometry analysis was used for phenotype determination, and osteogenic and adipogenic differentiation of cells were detected. RESULTS AND CONCLUSION:The contamination rates were 3%(3/100) in the experimental group and 20%(20/100) in the control group, indicating that meropenem can obviously reduce the contamination rate. In the experimental group, the morphology, differentiation potential and cellphenotype of the passage 3 cells were al normal. The proliferation ability of cells showed no difference between the two groups. Therefore, meropenem can be added to the preservation fluid.
9.Alginate/chitosan microspheres containing cationic beta-cyclodextrin polymer/insulin complex

BACKGROUND: Protein drugs such as insulin are usually used in the form of injection due to its low oral administration bioavailability. For the purpose of higher bioavailability, the common method is to encapsulate the drug into algitnate/chitosan microspheres, which introduced polysaccharides to accelerate biological absorption or reduce demolition of protease. OBJECTIVE: To construct alginate/chitosan microspheres containing cationic?-cyclodextrin polymer (CP?CD) /insulin complex for insulin oral delivery system, in addition, to inspect the influence of the charge density of cationic ?-cyclodextrin polymers on insulin release performance in vitro. DESIGN, TIME AND SETTING: The contrast observation experiment was performed at the Laboratory of Department of Polymer Science and Engineering, Sichuan University from November 2006 to July 2008. MATERIALS: ?-cyclodextrin, epichlorohydrin was supplied by Tianjin Bodi Chemical Industry Co., Ltd. Choline chloride and alginate was provided by Kelong Reagent Company, chitosan was purchased from Golden-shell Biochemical Co., Ltd. And insulin was produced by Wanbang Biochemical Pharmaceutical Company. METHODS: Cationic ?-cyclodextrin polymer was synthesized through a one-step polymerization of ?-cyclodextrin, epichlorohydrin and choline chloride. It was combined with insulin and then encapsulated into alginate/chitosan microspheres. In vitro insulin release behavior of the microsphere was studied in simulative gastrointestinal fluid. MAIN OUTCOME MEASURES: The entrapment efficiency, as well as insulin release in simulative gastrointestinal fluid. RESULTS: The charge density of CP?CD had a great impact on the insulin association efficiency of microspheres. The maximum association efficiency could reach (76.3?1.5)%. It showed that cumulative insulin release of microspheres containing CP?CD was less than that of the control group in simulated gastric fluid. The insulin released was 5.8 IU in simulated intestinal fluid, which was higher than the control group. CONCLUSION: Alginate/chitosan microsphere containing CP?CD/insulin complex is a promising system for improving insulin oral delivery efficiency.
10.Gonadotropin releasing hormone analogues in treating idiopathic central precocious puberty in girls

Objective To observe the clinical effect of gonadotropin releasing hormone analogues in treatment of the girls with idiopathic central precocious puberty(ICPP). Methods Twelve girls with ICPP received therapy with GnRH-A.Before and after the end of treatment for 6 months,secondary sexual characteristics and growth velocity were observed,pelvic ultrasonic examination,X-ray bone age(BA),serum level of E_2 and LHRH stimulating test were detected,and the predicted adult height(PAH) was analysed. Results After treatment,the size of breasts,uterus and ovaries were significantly reduced.The basic and peak levels of serum LH and FSH by LHRH stimulation were also significantly reduced.The ratio of BA/CA(chronology age)was decreased.The PAH was increased. Conclusions GnRH-A can effectively depress the sexual characteristics,reduce the maturation of BA,and also improve the PAH in girls with ICPP.
