1.Application of patients with specific behaviors in the facial trauma clinical teaching

Objective To detect the feasibility of patients with specific behaviors in the facial trauma clinical teaching. Method 47 medical students were randomly divided into experimental group ( n = 22 ) and control group(n=25). Students from experimental group at least dealed with 5 ~ 10 facial trauma patients with specific behaviors, while specific behaviors patients in control group were not more than 2. Teaching results were compared. Results There was no significant difference in operation test scores between the two groups ( (84.68 ±3.41 ) vs ( 83.96 ± 3.46 ), P = 0. 476 ). Composite score of teachers in the experimental group was significantly higher than that in control group( (90.09 ± 5.69 ) vs (83.48 ± 5.19), P < 0.01 ). The results of questionnaire survey showed that two in five self-assessment results were higher in the experimental group(P < 0. 01 ). Conclusion Medical students are more appropriately involved in treating special behaviors patients, which can improve the overall quality of students. Teaching with special behaviors patients has good effect, and is feasible.
2.Reduction mammaplasty with pure liposuction

Objective With the social and economic advancement, the morbidity of breast hypertrophy is increasing. Inverted T technique was the traditional breast reduction mammaplasty method. It left severe scar after operation. The aim of this study was to introduce our experience about breast reduction with pure liposuction that significantly reduced the operative scar after operation. Methods After using tumescent anesthesia, a small incision about 5 mm was performed at inframammary fold for proper selected patients, The entire breast and subcutaneous fat were thoroughly suctioned with small caliber cannulas for breast reduction. No surgical excision was applied. Compression garment was applied after the operation. Results A total of 11 patients were successfully treated. Only one patient had a unilateral seroma that was responded to multiple aspiration. All the patients were satisfied with the results. The morphology was good. The texture of the breast was improved. Conclusion Reduction mammaplasty with pure liposuction is simple and effective for those who had mild to moderate breast hypertrophy with no severe ptosis and more fatty tissue. The scar is invisible. It deserves to be recommended.
3.Treatment of concealed penis

Objective To introduce the clinical manifestations and treatment methods of concealed penis, which is relative rare in plastic surgery. Methods Vertical preputiotomy was performed. The hypoplastic tissuely was excised. After that, there was no prepuce redundant, so that no prepuce excision was needed. Results 7 patients were treated. All were satisfied with the results. Conclusion Concealed penis should be distinguished from phimosis. Circumcision should be avoided in concealed penis patient.
4.The influence of disposing the shoulder by the dental pneumatic ultrasonic hand-piece on the edge sealing prop-erty of the zirconium dioxide crown

15 extracted premolars were selected and randomly divided into 3 groups, with 5 in each group. MANI TF-13 bur、TR13-EF bur and the dental pneumatic ultrasonic hand-piece were respectively used to dispose the shoulder. 15 Zirconium dioxide full crowns were made. The shoulder marginal microleakage was observed by staining method. Dental pneumatic ultrasonic hand-piece showed the best anti-mic-roleakage effect in shoulder refinement.
5.Vertical incision scar reduction mammaplasty with medial-superior pedicle based on Würinger's horizontal septum

Objective The vertical mammaplasty improves the breast morphology,and reduces the operative scar.However,the nipple-and-areola complex (NAC) has the risk of necrosis on those severe patients.In this report,we performed the vertical incision scar reduction mammaplasty based on Würinger's horizontal septum to reduce the NAC necrosis possibility.Methods Typical Lejour mosque-dome design was performed.The inferior part of glandular tissue and skin were excised.The NAC was elevated to normal position with medial-superior pedicle based on the horizontal septum.The breast morphology was modified with lateral and medial glandular pillar suturing using nonabsorbent thread.For reducing the vertical scar,the lower breast flap was thinned to induce the skin contraction.Results 27 patients with breast hypertrophy were operated from May 2009 to October 2012.1 patient had unilateral breast reduction,and 6 patients with 8 breasts had partial wound dehiscence.They were treated conservatively with topical dressings.No complete NAC necrosis occurred.Conclusions The vertical reduction mammaplasty based on Würinger's horizontal septum is a safe and effective approach,and the NAC necrosis possibility is reduced with this procedure.
6.Quality control and parameter optimization of coronary angiography with 64 multi-slice CT

Objective To control the image quality and to optimize the scan protocols of 64 multi-slice CT coronary imaging by analyzing factors that influence the image quality.Methods Seventy-eight patients with known or suspected CAD underwent CT coronary angiography using 64 multi-slice CT.The image quality in those cases was analyzed retrospectively to derive the optimized scan protocols.The application value of the override function was discussed.The dependency of cardiac reconstruction algorithms and corresponding image quality on patient heart rates was analyzed.In addition,the relationship between heart rate and the contrast-enhancement effect was studied.Results(1)Correct use of override function could improve the quality of cardiac images.(2)For patients with heart rates below 70 beats per minute,the image quality of single-segment reconstruction was satisfactory.For patients with heart rates between 70 and 79 beats per minute,the scores of images reconstructed with two-segments were better than that of single-segment reconstruction.(3)In the group with averaged heart rate of 60.7 beats per minute,the forecast contrast quantity of 70 ml was judged insufficient in 65.7%(23/35)cases;while for the group with averaged heart of 71.1 beats per minute,52.9%(18/34)cases were enhanced well.Conclusion The use of override function,correct segment mode and appropriate contrast quantity could be selected according to the heart rate,which could improve the image quality and reliability.
7.Effects of stress and ovariectomy on behavior activity in mice and intervention of TCM complex Ⅰ

AIM : To observe the effect of stress and ovariectomy on behavior activity in mice and the prevention and therapy of the TCM complexⅠcomposed of spine date seed and common anemarrhena rhizome. METHODS : The effect of stress and ovariectomy were investigated on bedtime, survival time under hypoxia, climbing time on the string and number of times of the activity in mice, and the prevention and therapy of TCM the complexⅠwere observed. RESULTS : The effect of stress, ovariectomy, stress and ovariectomy were different on bedtime, survival time under hypoxia and climbing time on the string and the number of times of the activity in mice. The effect of stress and ovariectomy on previous parameters was stronger than that of stress or ovariectomy lonely in mice. The stress and ovariectomy resulted in sleep impairment, the decreasing of the survival time under hypoxia and climbing time on the string, and the increasing of the number of times of the activity in mice. CONCLUSION : The TCM complex I can alleviate the high sensitivity to the environment in mice with ovariectomy and stress, which is similar to the mechanism of human climacteric syndrome.
8.Method for Automatic Tube Current Selection for Obtaining a Consistent Image Quality and Dose Optimization in a Cardiac Multidetector CT.

OBJECTIVE: To evaluate a quantitative method for individually adjusting the tube current to obtain images with consistent noise in electrocardiogram (ECG)-gated CT cardiac scans. MATERIALS AND METHODS: The image noise from timing bolus and cardiac CT scans of 80 patients (Group A) who underwent a 64-row multidetector (MD) CT cardiac examination with patient-independent scan parameters were analyzed. A formula was established using the noise correlation between the timing bolus and cardiac scans. This formula was used to predict the required tube current to obtain the desired cardiac CT image noise based on the timing bolus noise measurement. Subsequently, 80 additional cardiac patients (Group B) were scanned with individually adjusted tube currents using an established formula to evaluate its ability to obtain accurate and consistent image noise across the patient population. Image quality was evaluated using score scale of 1 to 5 with a score of 3 or higher being clinically acceptable. RESULTS: Using the formula, we obtained an average CT image noise of 28.55 Hounsfield unit (HU), with a standard deviation of only 1.7 HU, as opposed to a target value of 28 HU. Image quality scores were 4.03 and 4.27 for images in Groups A and B, respectively, and there was no statistical difference between the image quality scores between the two groups. However, the average CT dose index (CTDIvol) was 30% lower for Group B. CONCLUSION: Adjusting the tube current based on timing bolus scans may provide a consistent image quality and dose optimization for cardiac patients of various body mass index values.

9.Skin-sparing Mastectomy and Immediate TRAM Breast Reconstruction

Purpose To introduce our experience about the skin-sparing mastectomy and immediate breast reconstruction using transverse rectus abdominis myocutaneous flap(TRAM). Methods For early stage breast cancer,we preserved the breast skin,performed the subcutaneous glandular excision and axillary dissection,immediately reconstructed the breast using TRAM flap. Results 9 patients were treated by skin-sparing mastectomy and immediate TRAM breast reconstruction.The morphology of reconstructed breasts was excellent. Conclusions The skin-sparing mastectomy and immediate TRAM breast reconstruction is safe and effective for treating early stage breast cancer.The incision is relative small and more aesthestic.
10.Successful prenatal diagnosis following elimination of maternal cell contamination in a family with recessive dystrophic epidermolysis bullosa

Objective To perform a DNA-based prenatal diagnosis in a family with recessive dys-trophic epidermolysis bullosa, and to develop a strategy to eliminate matemal cell contamination in arnniotic fluid samples. Methods Amniocentesis was carried out at gestation week 16, amniotic fluid culture was used to separate fetal cells from maternal blood cells. Peripheral blood was obtained from the proband, and her parents. Genomic DNA was extracted from peripheral blood and aminotic cells. Subsequently, PCR and direct sequencing were performed to detect pathogenic mutations in the COL7A1 gone. Karyotype analysis was used to confirm paternal information in amniotic fluid. Linkage analysis between micro-satellite markers was performed to confirm the fetal genotype. Resulta Centrifugation showed visible contamination of aminotic cells by blood cells. Direct sequencing revealed that the proband was a carrier of both maternal mutation, R525X in exon 12, and paternal mutation, R2610X in exon 105, while the fetus only carried the maternal mutation, R525X. The second direct sequencing and hapiotype analysis after elimination of mater-nal blood cells by amniotic fluid culture confirmed that the fetus was a carrier of maternal mutation with nor-real phenotype. The pregnancy continued and a clinically unaffected girl was born at gestation week 40.Conclusion The accuracy of DNA-based prenatal diagnosis could be improved by the combination of direct sequencing, amniotic fluid culture, karyotype analysis and linkage analysis, etc.
