1.Effect of pure carbachol or combined with dietary fiber on intestinal mucosal barrier of rats after diffuse brain injury

Objective To study the protective effect of pure carbachol or combined with dietary fiber on intestinal mucosal barrier of rats after diffuse brain injury (DBI).Methods An adult male Wistar rat model of DBI was reproduced by gravitational shock method. The rats injured and survived after resuscitation were divided into three groups: model group (n = 40), carbachol group (n = 40) and carbachol combined with dietary fiber group (combined group,n = 32). In addition, a control group was established by simply an incision performed on the scalp, and the rats could drink freely (n = 5). In the experimental groups, 2 hours after resuscitation the rats began to receive gavage, 6 hours once, the liquid amount 15 mL/kg should be assured in every 6 hours, and if insufficient, normal saline was supplemented. In model group, normal saline 90 mL/kg was given, in carbachol group, carbachol 300μg/kg was administered and in combined group, carbachol 300μg/kg combined with dietary fiber 60 mL/kg was supplied. At 3 (combined group being excluded), 6, 12, 24 and 48 hours after resuscitation, the rats were anesthetized to collect samples and detect the plasma levels of D-lactate and activity of diamine oxidase (DAO) respectively, and the changes of villus height of small intestine were examined by a light microscope.Results The plasma D-lactate levels and the activities of DAO at any time point in the experimental groups were significant higher than those in control group (allP < 0.01). Along with the prolongation of time, the levels of plasma D-lactate and DAO activities in carbachol and carbachol plus diatary fiber groups were gradually lower than those of the model group, and at 48 hours after injury they reached their valley values [D-lactate (ng/L): 6.32±0.79, 7.46±1.67 vs. 17.65±1.53, DAO activity (kU/L): 0.76±0.01, 0.86±0.01 vs. 2.23±0.15]. Under light microscopy, compared with control group, the villus height of small intestinal mucosa at any time point in any experimental group was gradually lowered, and reached the valley values at 12 hours, then gradually increased , and peaked at 48 hours, the villus height in carbachol group and combined group was higher than that in model group (μm: 265.36±10.20, 261.54±10.38 vs. 247.51±9.39, bothP < 0.05).Conclusion When only carbachol is administered into the rat intestine early after diffuse brain injury in rats, beginning from 6 hours after injury, the protective effect of intestinal mucosal barrier is shown, representing decrease of plasma D-lactate level and DAO activity, amelioration of intestinal mucosal damage and protection of intestinal mucosal barrier; under the same above situation, the carbachol combined with dietary fiber was applied, showing the similar above carbachol protective effects.

OBJECTIVE:To evaluate the curative efficacy of Yixinshu capsule(YXSC)for patients with chronic heart failure(CHF)and its effects on plasma levels of angiotrnsionⅡ(Ang Ⅱ)and tumor necrosis factor-alpha(TNF-?).MET-HODS:60 patients with CHF were randomly assigned to either treatment group(n=30)or control group(n=30):the control group received conventional symptomatic treatment with western medicines including anti-infection therapy,relieving airway obstruction,continuous low-flow oxygen inhalation,cardiac and diuretic therapy,vasodilator,correcting acid-base imbalance and electrolyte disturbances etc;the treatment group received YXSC in combination with the above conventional symptomatic treatment.Plasma levels of AngⅡ and TNF-? in two groups before and after treatment were detected;and the cardiac function and the indexes of echocardiography(LVEF,A/E)of all cases were recorded in detail before and after treatment.Meanwhile,10 healthy subjects served as normal control.RESULTS:After treatment,plasma levels of AngⅡ and TNF-? in both group decreased remarkably(P

4.Application of digital three -dimensional model of abdominal and pelvic vessels in selecting intravascular catheterization method

Objective By using digital three-dimensional (3D) model of abdominal and pelvic vessels to investigate the relationship between the angle/position of the bifurcation of bilateral common iliac arteries and the length of the common iliac artery, thus to guide the endovascular interventional catheterization manipulation. Methods A total of 439 patients, who received abdominal and pelvic CTA at authors’ department during the period from Sep. 2011 to May 2013, were included in this study. By using digital 3D reformation technique, the 3D model of the abdominal and pelvic vessels as well as the lumbosacral vertebrae was established in all patients. With the help of mimics and geomagic software the angles between the left and right common iliac arteries and the lengths of common iliac arteries were determined, and the lumbar vertebra corresponding to the bifurcation of common iliac arteries was recorded. Using statistical software, the relationship between the bifurcation angle of bilateral common iliac arteries and the corresponding lumbar vertebra as well as the length of common iliac arteries was analyzed. Results The digital 3D model of the abdominal and pelvic vessels as well as the lumbosacral vertebrae was successfully reconstructed in all 439 patients. Both the angle between the left and right common iliac arteries and the corresponding lumbosacral level of bifurcation angle were clearly demonstrated on the model. The average angle was 49.1° ± 12.4°. The bifurcation was located at L3 -L4 level in 38 cases, at upper one-third level of L4 in 63 cases, at middle one third level of L4 in 89 cases, at lower one-third level of L4 in 135 cases and at L4 -L5 level in 114 cases. The average length of the left and right common iliac artery was (45.6 ± 15.6) mm and (43.3 ± 15.4) mm respectively. Pearson correlation analysis indicated that a negative correlation existed between the angle and the position level of the left and the right common iliac artery (r = -0.172, P< 0.05), and a negative correlation also existed between the angle and the length of the common iliac artery (left: r = -0.171, P<0.05; right: r = -0.164, P < 0.05). Conclusion The lower the opening position of the left and right common iliac artery is, and the bigger the intersection angle is, the shorter the length of corresponding common iliac artery will be. In this case, catheterization via femoral artery to contralateral iliac artery is a short path, and the catheterization procedure is easy to manipulate with higher success rate. On the contrary, the catheterization will be difficult, and the use of loop-technique will be needed. The digital 3D model of the abdominal and pelvic vessels as well as the lumbosacral vertebrae can help select the optimal catheterization path.
5.The protection of carbachol on intestinal barrier function in patients with trauma

Objective To observe the protection of carbachol on intestinal barrier function in patients with trauma. Methods A prospective randomized controlled trial was conducted. Seventy patients after trauma with a definite diagnosis of multiple organ dysfunction syndrome(MODS)from Department of Critical Care Medicine in Hebei United University Affiliated Hospital were included. According to random number table,the patients were divided into a carbachol treatment group(37 cases)and a mosapride citrate treatment group(33 cases),and all the patients in the two groups were treated by antibacterial drugs,supportive agents for organ function,surgery, etc symptomatic treatment. Based on the conventional treatment,in the carbachol treatment group,carbachol was administered through a stomach tube at the dose of 0.2 mg/kg,twice a day,and the dose was doubled if no exhaust or defecation persisted for 3 days after treatment,while in the mosapride group,mosapride citrate was given at the dose of 5 mg once and thrice a day,the therapeutic course of both groups being 7 days. On the 1st,3rd,5th, 7th day after admission,peripheral venous fasting blood in early morning was collected,the activity of diamine oxidase(DAO),expression rates of CD11b+and CD18+in polymorphonuclear neutrophil(PMN),contents of tumour necrosis factor-α(TNF-α)and interleukin-10(IL-10) were detected,and the clinical curative effects were observed. Results Compared to the mosapride citrate treatment group,the total effective rate was significantly higher in the carbachol treatment group on the 7th day after treatment〔70.3%(26/37)vs. 45.5%(15/33),P<0.05〕. The activity of DAO,expression rates of CD11b+and CD18+in PMN,contents of TNF-αand IL-10 in the carbachol treatment group were decreased with the extension of time,and reached valley values on the 7th day,the differences were statistically significant in the comparisons with those in mosapride citrate treatment group at the same time point〔DAO(mg/L):3.21±0.52 vs. 3.93±0.51,CD11b+:(14.89±2.16)% vs.(28.92±1.59)%,CD18+:(53.67±2.44)% vs. (72.46±4.08)%, TNF-α(ng/L):111.44±16.42 vs. 129.73±18.74, IL-10(ng/L):67.71±38.83 vs. 121.45±40.23,all P<0.05〕. At the various time points,the above indexes had no obvious changes in mosapride citrate treatment group. Conclusion Carbachol can ameliorate the ischemic/reperfusion(I/R)injury in patients with intestinal barrier dysfunction after trauma,decrease the release of inflammatory cytokines in vivo,and promote peristalsis of intestinal tract,therefore carbachol has clinical value of protecting intestinal barrier function.

Objective To explore the application of 3D printing technology in individual pelvic structure of female and its value in obstetrics and gynecology based on the CT datasets of the pelvic structure and digital three-dimensional reconstruction. Methods CTA image dataset of a patient from gynecology department was obtained for constructing three-dimensional models of each organ using the digital three-dimensional reconstruction technology , then the digital 3D model with the same size as the model displayed was printed with Z510 3D printer. Results 3D models of patient′s lumbosacral vertebrae, aorta artery, common iliac artery, internal and external iliac artery , postcava , common iliac vein , internal and external iliac vein , pelvis ureter , uterus and uterine artery were printed out in the same size replica of the virtual reality model. Conclusion 3D printed model has all the features of 3D vision and can be touched and felt by people , which can provide new insights for medical education, clinical and medical research.
7.The application of the digital three-dimensional reconstruction on assessing risk in the uterine artery embolization surgical approach

Objective To explore the correlation between the incidence of abdominal and pelvic artery distortion and age using dig-ital three-dimensional reconstruction,in order to assess risk in uterine artery embolization surgical approach.Methods Collecting the CTA datasets of 475 patients,group A aged 11-27 (n=40),group B aged 28-44(n=218),group C aged 45-61(n=197), group D aged 62-78(n=20).The 3D model of pelvic arteries was reconstructed by mimics software,and the arterial distortion was recorded by rotating and observing the 3D model,which was further classified into three categories.One was the distortion of the aorta only,the second was the distortion of iliac artery,and the third being distortion of both sturctures.the number of cases appeared arterial distortion in every age group was calculated and the ratio of the total cases to each age group was counted.Results There were 128(26.9%)cases appeared arterial distortion,the last 347(73.1%)were not found distortion in any artery.Of the 128 cases,the occurrence rates of arterial distortion in each age group were 2.5%、11.9%、44.7%、65% for group A,B,C and D respec-tively.Pearson linear correlation analysis showed that the incidence of arterial distortion positively correlated with age (r=0.404,help of three-dimensional reconstruction.The occurrence rate of artery distortion is increased with age and the iliac artery distortion occurrence is higher than that of the aorta artery.
8.Construction of evaluation index system for full-time scientific researchers in Level-three general hospital

Objective:To establish a set of comprehensive appraisal indicator system for the full-time scientific research personnel in Level-three general hospitals, improve the management of full-time scientific research personnel at hospital.Methods:Firstly, an initial indicator system was established by literature review and in-depth interviews with experts. Secondly, Delphi method was used to screen the indicators to confirm the indicator system. Finally, Analytic Hierarchy Process (AHP) was used to determine the weights of indicators at all levels, consistency tests were also conducted.Results:A set of comprehensive appraisal index system for full-time scientific research personnel in Level-three general hospitals was established, including four first-level indicators, which covered the comprehensive quality, scientific research capacity, scientific research performance and academic impact, as well as other 17 secondary indicators and 53 third-level indicators. Among these indicators, the scientific research performance has the largest weight value (0.5224), and according to the consistency test results, CR was less than 0.1. Besides, through the consistency test, the weight assignment is reasonable.Conclusions:The appraisal index system of full-time scientific research personnel in Level-three general hospitals is reliable. It can be used as a tool for evaluation of full-time scientific research personnel, which also provide reference for other hospitals to improve the management of full-time scientific research personnel.
9.Quantitative MRI of SD-rat adipose derived stem cells labeled with a new type paramagnetic nanoparticles in rats in vivo

Objective To explore the feasibility of tracking migration and distribution of SD-rat adipose derived stem cells (ADSCs) labeled with Polyethylene Glycol/Polyehthyleneimine modified superparamagnetic iron oxide (PEG/PEI-SPIO) in rats with chronic cerebral ischemia using MRI.Methods Thirty female SD rats underwent permanent occlusion of bilateral common carotid arteries 6 months were divided into PEG/PEI-SPIO labeled group and unlabeled group (each n =15).Labeled or unlabeled ADSCs suspension was injected into the right ventricle of rats in two groups,respectively.MR scans were performed at the 7th,14th and 21st day after transplantation for each 5 rats.T2 value of T2mapping sequence in hippocampus,cortex and cerebellum were measured.Then the rats were scarified,and the brains were obtained,and Prussia dyeing was performed.Under high magnification,blue dye cells at each time points and brain area were counted.T2 values and blue dye cells were statistically analyzed.Results Class round hypointensity areas were detected in temporol-parietal cortex and hippocampus in both groups on T2WI,T2* WI and SWI.T2 value of the right temporolparietal cortex and hippocampus in the labeled group was shorter than those of the unlabeled group on the 14th day after transplantation (P=0.013,0.045).T2 value of the right temporol-parietal cortex in the labeled group was shorter than that of the unlabeled group on the 21st day after transplantation (P=0.007).The number of blue dye cell of the right temporol-parietal cortex on the 14th and 21th day,hippocampus on the 14th day in the labeled group were more than those of the unlabeled group after transplantation (P=0.029,0.032,0.043).Conclusion ADSCs labeled with PEG/PEI-SPIO transplanted into lateral ventricle of SD rat could migrate to the damaged areas caused by chronic cerebral ischemia.It is possible to use quantitative MRI to track migration and distribution of ADSCs labeled with PEG/PEI-SPIO in rat brain.
