1.Application of metoprolol combined with trimetazidine in the treatment of coronary heart disease and heart failure

Objective To explore the clinical effect of metoprolol combined with trimetazidine in coronary heart disease patients with heart failure and improvement of cardiac function.Methods 160 coronary heart disease patients with heart failure were selected,and they were randomly divided into research group and control group according to the digital table,80 cases in each group.The research group was treated with metoprolol combined with trimetazidine,and the control group was only treated with metoprolol therapy.The clinical curative effect,adverse reaction,heart rate(HR),plasma brain natriuretic peptide(BNP),left ventricular end-diastolic diameter(LVEDD),left ventricular end systolic diameter(LVESD) and left ventricular ejection fraction(LVEF) of the two groups were observed.Results The effective rate of the research group was 92.50%,which of the control group was 78.75%,the difference was statistically significant between the two groups(x2=6.14,P<0.05).The HR and BNP of the two groups before treatment were (95.24±5.71)beats/min,(291.42±16.34)pg/mL,(96.04±5.64)beats/min,(293.23±17.23)pg/mL respectively,the differences were not statistically significant between the two groups(t=0.89,0.68,all P>0.05),and after treatment,the HR and BNP were (72.53±4.24)beats/min,(221.42±11.42)pg/mL,(82.53±5.01)beats/min,(248.25±14.23)pg/mL respectively,the differences were statistically significant between the two groups(t=13.63,13.15,all P<0.05).The LVEDD,LVESD and LVEF of the two groups before treatment were (64.24±3.12)mm,(45.73±2.43)mm,(48.23±5.34)%,(64.67±3.22)mm,(45.64±2.47)mm,(48.29±5.38)% respectively,the differences were not statistically significant between the two groups(t=0.86,0.22,0.07,all P>0.05);After treatment,the LVEDD,LVESD and LVEF of the two groups were (51.94±3.01)mm,(34.93±2.53)mm,(60.23±4.53)%,(58.23±3.21)mm,(41.21±2.49)mm,(53.23±4.64)% respectively,the differences were statistically significant between the two groups(t=12.78,15.82,9.66,all P<0.05).The incidence rates of adverse reactions in the research group and the control group were 6.25%,11.25% respectively,the difference was not statistically significant between the two groups(x2=1.25,P>0.05).Conclusion The clinical effect of metoprolol combined with trimetazidine in the treatment of coronary heart disease patients with heart failure is obvious,and can improve heart function,it is worthy of clinical promotion.
2.Effect of Hyzaar on serum uric acid in essential hypertension patients with hyperuricemia

Objective To observe the efficacy and safety of Hyzaar on hyperuricemia patients with essential hypertension.Methods 178 patients with essential hypertension and hyperuricemia were randomly divided into control group and observation group with each of 89 cases.Patients in the control group were treated with enalapril, 10 mg qd ,and hydrochlorothiazide, 12.5mg qd.Patients in the observation group were given 1 piece of Hyzaar tablet (5mg losartan and 12.5mg hydrochloride per tablet) per day for a course of 8 weeks.Blood pressure,serum uric acid level before and after the treatment were observed.Results Antihypertensive efficacy of the observation group and control group were 95.5% and 93.3% ,respectively( P > 0.05 ).Serum uric acid level of the observation group was significantly decreased than before treatment( P <0.01 ) ,while the serum uric acid of the control group showed no significant changes.The incidence of dry cough was higher in control group.Conclusion Hyzaar showed good efficacy in decreasing blood pressure and uric acid,and good safety in treating patients with primary hyperuricemia and hypertension.
3.Preliminary study on the pharmacognosy and toxicology of Mallotus panicutus (lam) Muell.Arg

Objective To identify Mallotus panicutus (lam) Muell.Arg and explore the acute toxicity.Methods Pharmacognostic identification and chemical reaction methods were adopted to the identification.The maximum concentration and maximum volume of ethanol extracts of M.panicutus (lam) Muell.Arg were given via intragastric administration in mice to observe the toxic and side effects after 14 days.Results The chemical reaction showed M.panicutus(lam) Muell.Arg concluded the constituents such as flavone and saponin and there was no acute toxicity in mice.After 14 days,the growth rate of mice weight of blank group and drug group were 73.5% and 62.9% (P > 0.05).Conclusion The results could be taken as the reference for analyzing the quality of this drug and guiding the clinical medication.
4.Significance of ERK/MAPK signal transduction pathway in the targeting treatment of gastrointestinal cancer

Extracellular signal-regulated kinase/mitogen-activated protein kinase(ERK/MAPK) signaling pathway plays an important role in gastrointestinal tumorigenesis and development.ERK/MAPK signal transduction pathway has three important molecular targets: the small G protein Ras,Raf kinase and MEK1/2 and ERK1/2.At present,there are three kinds of approach which can inhibit ERK / MAPK signal transduction pathway:⑴ destroying the structure and(or) function of the target protein,⑵adopting the deficit strategy,⑶damaging the interaction between proteins.These approaches can provide new ideas for the treatment of gastrointestinal cancer.
5.Study on TCM Syndromes of 170 Cases of HIV/AIDS Patients by Intraveneous Drug

Objective To investigate the characters of TCM syndromes of HIV/AIDS patients by intraveneous drug. Method Based on the TCM theories,related literature and experts' experience,the questionnaire was compiled and revised,170 patients were investigated one by one,and the data was analysed. Result The frequency lines in turn:spleen qi deficiency,liver depression with qi stagnation,kidney yin deficiency,phlegm-heat congestion of lung,disharmony of liver and stomach,lung qi deficiency,kidney deficiency of both yin and yang,qi deficiency,phlegm-damp congestion of lung,liver fire,heart yin deficiency. The excess syndromes lies in the liver and the lung. The deficiency syndromes lies in kidney,spleen and stomach,and lung. Conclusions The frequent syndromes in the study distinguished from the other congener study. It may be caused by many different contribution factors of patients and researchers.
6.Effect of benazepril combined with atorvastatin in treatment of chronic heart failure and its effects on level of plasma NT-proBNP

Objective To study curative efficacy of benazepril combined with atorvastatin in treatment of chronic heart failure and its effects on level of plasma N-terminal pro brain natriuretic peptide(NT-proBNP). Methods 90 patients of chronic heart failure who received therapy from January 2013 to January 2016 in the first people's Hospital of Xiangshan, Zhejiang. According to random number table, those patients were divided into the observation group and the control group with 45 cases in each group, on the basis of routine treatment, the control group was treated with benazepril, while the observation group was treated with atorvastatin on the basis of control group. After 2 months, the treatment effect was compared. Results After treatment, the left ventricular end diastolic diameter (LVIDd), left ventricular end systolic diameter (LVIDs) in the observation group were lower than the control group, left ventricular ejection fraction (LVEF) was higher than the control group, the difference was statistically significant (P<0.05), the level of NT-proBNP in the observation group was lower than the control group, the difference was statistically significant (P<0.05), 6 minute walking distance was better than the control group, the difference was statistically significant (P<0.05), the total effective rate in the observation group 91.11%(11/45) was higher than the control group 71.11%(32/45), the difference was statistically significant (P<0.05), during the follow-up of 6 months, the recurrence rate of heart failure in observation group 2.22%(1/45) was lower than the control group 20.00%(9/45), the difference was statistically significant (P<0.05). Conclusion Benazepril combined with atorvastatin is well for chronic heart failure, which can effectively reduce the level of plasma NT-proBNP and prognosis.
7.Application of anatomical isometric technique in arthroscopic reconstruction of anterior cruciate ligament and quadruple hamstring tendon autograft

[Objective]To study the feasibility and short-term effects of the arthroscopic reconstruction of ACL with LARS artificial ligaments and quadruple hamstring tendon autograft usirg anatomical isometric technique.[Method]Arthroscopic ACL reconstruction was carried out in 20 patients with ACL injuries by using the LARS artificial ligaments and quadruple hamstring tendon autograft.Bone tunnels of the femur and the tibia were prepared with ACL anatomical isometric technique.20 patients were divided into LARS artificial ligaments group(11 patients) and quadruple hamstring tendon group(9 patients).The artificial tendon group was inserted into the tunnels and fixed with two interface screws after being tightened.Quadruple hamstring tendon group was fixed with endobutton plate.[Result]The operation time were 30~80 min(mean,56 min) in LARS artificial ligaments group and 80~120 min(mean,100 min) in quadruple hamstring tendon group.No postoperative complications such as synovitis and ligament rupture and movement restriction occurred iF two groups.A follow-up check up was made in the 20 patients for 6~12 months(mean,9 months).The Lysholm scores improved from a preoperative score of 22~65(36.72~15.54) to 80~98(90.45~4.68) at follow-up(t=10.535,P
8.Arthroscopic suture fixation with cannulated screw and washer for bony avulsion of the posterior cruciate ligament

[Objective]To investigate the feasibility and short-term effects of arthroscopic suture fixation with cannulated screw and washer for bony avulsion of the posterior cruciate ligament.[Method]Arthroscopic suture fixation with cannulated screw and washer was carried out in 30 patients with bony avulsion of the posterior cruciate ligament.The posterior cruciate ligament and avulsion bone fragment were held together with two Ethibond 5 polyester threads,which were first wrapped around the posterior cruciate ligament from the anterior to the posterior,and then tied behind the posterior cruciate ligament and over the bone fragment.The threads were pulled out through tibia tunnels,tighted and fixed on cannulated screw and washer under the tibia tunnels 2 cm.[Result]The operation time were 60-90 min.A follow-up was made in 10 patients for 6-12 months.All fractures were united without displacement at 6 weeks after operation.No postoperative complications occurred.The Lysholm scores were 83-95(89.6?4.03).[Conclusion]Arthroscopic suture fixation with cannulated screw and washer for bony avulsion of the posterior cruciate ligament technique is simple to perform and gives a satisfactory short-term results.
9.Clinical Observations on Acupuncture at Specific Points plus Kidney-reinforcing Chinese Herbal Medicine for the Treatment of Cervical Spondylotic Vertebral Arteriopathy

ObjectiveTo investigate the clinical efficacy of acupuncture at specific points plus kidney-reinforcing Chinese herbal medicine in treating cervical spondylotic vertebral arteriopathy (liver-kidney deficiency type).MethodNinety patients with cervical spondylotic vertebral arteriopathy (liver-kidney deficiency type) were allocated, using a random number table, to a treatment group of 45 cases and a control group of 45 cases. One case dropped out in the control group. The treatment group received acupuncture at Baihui(GV20), Dazhui(GV14), Houxi(SI3), Sanyinjiao(SP6), Taixi(KI3) and Xuanzhong(GB39) plus oral administration of six ingredients with rehmanniaplus radix puerariae formula and the control group, oral administration of six ingredients with rehmannia plus radix puerariae formula alone. The clinical therapeutic effects were evaluated after eight consecutive weeks of treatment.ResultThe total efficacy rate and the cure rate were 91.1% and 62.2%, respectively, in the treatment group and 77.3% and 34.1%, respectively, in the control group; there were statistically significant differences between the two groups (P<0.05). The therapeutic effect was better in the treatment group than in the control group. After treatment, the difference values ofthe vertigo severity, frequency and duration, headache, neck and shoulder pain, and life and work scores on the scale werehigher in the treatment group than in the control group (P<0.05).ConclusionAcupuncture at specific points plus Chinese herbal medicine is more effective than Chinese herbal medicine alone in treating cervical spondylotic vertebral arteriopathy (liver-kidney deficiency type).
