1.Study on immunological responses to eukaryotic expression vector containing PRRSV ORF5 and CpG motifs

Objective:To enhance the immune efficiency of porcine reproductive and respiratory syndrome virus(PRRSV)DNA vaccine,the ORF5 genes were used as candidate genes to construct the recombinant plasmids CpG-pVAX1-ORF5 by pVAX1 as eukaryotic expression vector.The aim is to analyze the immune responses and the effects of CpG-ODN induced by GP5 recombinant plasmids of PRRSV.Methods:Piglets were immunized with recombinant DNA plasmids which expressed PRRSV GP5 for three inoculations.The PRRSV antibody in serum,the concentration of IL-2 and the lymphocyte proliferation test(MTT) in peripheral blood of vaccinated piglets were detected.The vaccinated pigs were challenged intranasally with PRRSV SD2.Results:GP5 DNA immunization with CpG resulted in the production of both PRRSV antibodies and cellular immune(a significant enhancement of a lymphoproliferative response ).The CpG-pVAX1-ORF5 was showed significantly better protection from the PRRSV challenge compared with the control plasmid.This immune response was characterized by a significantly decreased frequency of viraemia with decrease of 80% compared with the control group.There were little clinical symptoms and protection in some extent from lung damage.Conclusion:The results indicate that CpG-ODN could be used as immune adjuvant of PRRSV DNA vaccine.
2.Long-term clinical and rehabilitation effect of Xingnaojing injection with hyperbaric oxygen in the stroke patients

Objective To observe long-term clinical and rehabilitation effect of Xingnaojing injection with hyperbaric oxygen on the stroke patients with hemiplegia.Methods100 patients with cerebral apoplexy hemiplegia from May 2012 to April 2014 in Wuzhong City People's Hospital Affiliated to Ningxia Medical University during the period.According to the stochastic indicator method, the object of this study were divided into experimental group (50 cases) and control group (50 cases).For all patients with conventional treatment, daily aspirin 0.5 g, nim horizon each time, three times a day oral treatment, control group patients on the basis of the joint 20 mL Xingnaojing injection, once a day.Patients in the experimental group were given hyperbaric oxygen treatment on the basis of the combination, for an hour every day.Treatment for 2 months after hospital discharge, 18 months after treatment to return of two groups of patients, compared two groups of patients before and after treatment action, ability of daily life, the quality of life as well as the secondary injury, and the patient's neurologic deficits were evaluated.ResultsIn 18 months after treatment, the treatment group patients walk rate was 68%, significantly higher than that of control group 42%(P<0.05), nerve function defect assessment in experimental group patients was (10.43±2.19) scores,was significantly better than control group (16.04±3.23) scores, daily life ability, life quality and improve secondary injury effect in experimental group was significantly better than control group, difference has statistical significance (P<0.05).ConclusionXingnaojing injection combined hyperbaric oxygen treatment of stroke patients with hemiplegia, can improve long-termdaily life skills, and improve quality of life with rehabilitation, the long-term clinical effect is remarkable, less adverse reaction.
3.Risk factors to induce cerebral infarction during hyperbaric oxygen treatment: A 6-case report

BACKGROUND: It is commonly believed that hyperbaric oxygen (HBO)plays a neuroprotective role in the prevention and treatment of cerebral infarction (CI), however hyperbaric oxygen therapy (HBOT)-induced CI was also reported.OBJECTIVE: To investigate the risk factors of HBOT-induced CI.DESIGN: Case analysis with patients as subjects.SETTING: At the Department of Hyperbaric Oxygen of Guangzhou General Hospital of Guangzhou Military Command Area of Chinese PLA.PARTICIPANTS: Totally 192 inpatients receiving HBOT were selected from the Department of Hyperbaric Oxygen of Guangzhou General Hospital Guangzhou Military Command Area of Chinese PLA between December 1996 and March 1998, including 127 males and 65 females with age ranging from 9 to 78 years old. Totally 6 patients, 3 males and 3 females, suffered from CI during HBOT, with the age from 51 to 76 yeas.METHODS: The background factors of 192 patients who HBOT METHODS and 6 cases with induced CI were analyzed.MAIN OUTCOME MEASURES: Symptoms, signs and skull CT or MRI.RESULTS: Data of 192 patients were dealt with intentionally and all were statistically analyzed without miss. ① The incidence of CI during HBOT was 0.3% (6/192). ② The risk factors of HBOT-induced CI included: age of ≥ 60 years, hypertension, hyperlipidemia, primary CI or cerebral hemorrhage, blood hyperviscosity and diabetes, and the incidence would greatly increased in those who has 4 or more than 4 risk factors, but seldom with less than 4 risk factors.CONCLUSION: The cluster level of risk factors was closely related with the incidence of HBOT-induced CI.
4.A questionnaires study on cochlear implantation in patients with white matter changes.

OBJECTIVE:
This article evaluates the auditory and speech perception outcomes of sensorneural hearing loss with cerebral white matter abnormalities after cochlear implantation.
METHOD:
A retrospective analysis was performed on the cochlear implantatees with cerebral white matter abnormalities by using the categories of auditory performance and speech intelligibility rating, and compared with the non-syndrome patients with sensorineural hearing loss. Paired t-test was used for statistical analysis.
RESULT:
There was statistical difference between normal group and white matter changes group with sensorineural hearing loss after 6 month. No statistical difference was found after 12 month and 24 month.
CONCLUSION
In the short term, the cochlear implant can be performed safely in patients with white matter changes. After a formal rehabilitation training,no significant difference in auditory or language ability was found between normal group and white matter changes group with sensorineural hearing loss.

5.THE METABOLIC BALANCE OF ZINC AND COPPER IN DIABETES MELLITUS

Zinc(Zn) and copper(Cu) balance tests were conducted in patients with diabetes mellitus. The results revealed that the negative balance of Zn and excessive positive balance of Cu were much improved after the management of diabetes mellitus. In addition, a close relationship was found between serum glucose concentration and the disorder of Zn and Cu balance, i.e. serum Zn level, apparent intestinal absorption rate and balance status of Zn were reversely related to serum glucose concentration, whereas urinary Zn, serum Cu level and apparent intestinal absorption of Cu were positively correlated to serum glucose concentration. Poly-factor analysis suggested that hyperzincuria and malabsorption of Zn were the main factors concerning negative Zn balance.
6.Clinical Value of ~(131)I SPECT/CT Fusion Imaging in Differentiated Thyroid Cancer

Purpose The clinical advantages of ~(131)I SPECT/CT fusion imaging over planar imaging of differentiated thyroid cancer(DTC) were studied. Materials and Methods 97 DTC accepting ~(131)I therapy after thyroid surgery underwent planar ~(131)I whole-body scan(WBS) and SPECT/CT fusion imaging. The diagnostic performance of ~(131)I-WBS and SPECT/CT were comparatively analyzed. Results The patients had totally 251~(131)I-WBS and 102 SPECT/CT imaging. In comparison to WBS, SPECT/CT fusion images were proven of a few advantages, such as: (1)Fused and 3D images being obtained by SPECT/CT;(2)finding more focus;(3)correctly confirming the position of lesions;(4)locating some ~(131)I negative lesions;and (5)distinguishing physiological or polluted activity. Conclusion ~(131)I SPECT/CT fusion imaging could clearly reveal the shape、sizes、biological status and relation to surrounding tissues, thus providing additional information to ~(131)I-WBS in DTC.
7.The clinical significance of real-time fluorescence PCR quantification of hepatocyte growth factor mRNA expression in acute leukemia

Objective To detect quantitatively hepatocyte growth factor(HGF)mRNA expressions of bone marrow mononuclear cells(MNCs)in acute leukemia(AL)and investigate its clinical significance.Methods Total mRNA of quantitated bone marrow MNCs isolated from 67 de novo AL cases was extrated and then cDNA was synthesized.Expression of HGF mRNA was quantified absolutely using real-time fluorescence quantification PCR(FQ-PCR).Results Expressions of HGF mRNA in a group of AL were higher significantiv than these in a control group(6.936 ±1.613,0.407 ±0.170,P<0.001),but there was similafitv between a group of acute myeloid leukemia(AMI,)and group of acute lymphoblastic leukemia (ALL)(7.127±1.911,6.635±0.934,P>0.05).In AL subtypes,the expression of M5(9.998 4±1.454)was higher than that of M2,M3,M4,L1,L2 and L3(P<0.001),but there ware no differences among the latters(P>0.05). Meanwhile,there was no statistical significance on the expressions of HGF mRNA between different age and sex(P>0.05).In addition,expressions of HGF mRNA in the remission group were lower than these in the non.remission group(6.393±1.165,8.041±1.848,P<0.005).Conclusions There are statistical significances of the expressions of bone marrow MNCs HGF mRNA among the AL group and control group.As to AL subtypes,there are no statistically significant differences between AML and ALL as well as between different age and sex.Besides,lower HGF mRNA level is correlated with better curative effect.It is suggested that HGF mRNA is a suitable index for AL diagnosis and treatment.
8.Establishment of real-time fluorescence quantitative polymerase chain reaction analysis assay quantification for hepatocyte growth flactor mRNA expression and its clinical relevance in lymphoma

Objective To construct quantitative standard for quantification of hepatocyte growth factor (HGF) mRNA and establish its real-time fluorescence quantitative(FQ)-PCR assay to estimate its clinical relevance in lymphoma.Methods Recombinant plasmid Was constructed with target cDNA obtained from isolated total RNA by RT-PCR After PCR products were identified and purified,recombined plasmids were quantitated and then acted as quantitative standard.A new real time FQ-PCR analysis system Was established with the second pair of primers and the probe after amplification condition and the concentrations of components were optimized.HGF mRNA expressions in 47 lymohoma cases[11 Hodgkin disease(HD) cases,36 non-Hodgkin lyphoma(NHL)cases.among these patients,36 patients in remission while 11 patients without remission ] were analyzed quantitatively,and its specificity and sensitivity for lymphoma diagnosis were evaluated by receiptor operation character(ROC)curve method.Results HGF mRNA quantitative standard was constructed successfully.and its real time FO.PCR analysis system Was established combined with hot.start PCR and down.touch PCR technique. According to slope of standard curve (-3.513)and correlation cofficient(0.999),amplification efficiency of the system was 92.6%.Coefficient variation of intra-assay,intra-day and inter-day-assay were 2.1%,4.0% and 6.8%,respectively.Sensitivity of FQ-PCR Was 2 eopies/μl.Expressions of HGF mRNA in lymphoma group Was higher than that in control group(6.425±2.172 and 0.317±0.192,respectively,t=15.883,P<0.001),and its expressions in remission group was lower than no remission group(6.157±1.712 and 7.59l ±1.184,respectively,t=2.768,P<0.05).However,there Was not difference of HGF mRNA level between group HD and group NHL(P>0.05).According to ROC analysis,its sensitivity and specificity were 93.6% and 100% when cutoff value for lymphoma clinical diagnosis Was 3.136.Conclusion HGF mRNA'8 quantitative standard and its real time F9-PCR analysis system have been successfully constructed,and it can be used for quantitative detection of its mRNA expression in lymphoma.
9.Multiple predictor models for diagnosis of sepsis in trauma patients

Objective To set up a warning diagnostic model by using the commonly used clinical indicators in an attempt to provide a basis for the early,fast and accurate diagnosis of posttraumatic sepsis.Methods Based on the presence of sepsis,165 patients were grouped into sepsis group (n =45) and non-sepsis group (n =120).Body temperature,respiration,heart rate,C-reactive protein(CRP),white blood cell,blood platelet count(PLT),activated partial thromboplastin time (APTT),acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ) score and sepsis-related organ failure assessment(SOFA) score were tested to identify the independent predictors of sepsis.Warning diagnostic models of unweighted score (unwScore) and weighted score (wScore) for posttraumatic sepsis were constructed by combining the independent variables.Receiver operation characteristic curve (ROC) was used to evaluate the independent predictor and warning diagnostic models for posttraumatic sepsis.Results Body temperature,respiration,heart rate,CRP,APACHE Ⅱ score and SOFA score were significantly different between the two groups(P < 0.05).Multiple analysis showed body temperature,CRP and APACHE Ⅱ score were independently associated with sepsis.With the ROC analysis,areal under the curve (AUC),sensitivity,specificity,positive predictive value and negative predictive value of unwScore (0.915,0.87,0.85,69.64% and 94.50%) and wScore (0.931,0.96,0.78,63.24% and 97.85%) were better than these of body temperature (0.855,0.84,0.78,59.38% and 93.07%),CRP (0.761,0.64,0.80,55.77% and 85.84%) and APACHE Ⅱ (0.884,0.84,0.82,64.41% and 93.40%).Conclusions Body temperature,CRP and APACHE Ⅱ score are independent predictors of sepsis.Models combining body temperature,CRP and APACHE Ⅱ score demonstrate high performance in diagnosing sepsis in trauma patients.
10.Construction and evaluation of models for early diagnosis and prognosis assessment of trauma sepsis risks

Objective To investigate the value of muhiple inflammatory cells and clinical score in early diagnosis and prognosis assessment of trauma sepsis risks.Methods This retrospective control study enrolled 209 severe trauma patients admitted from January 2010 and May 2016.White blood cell count,lymphocyte count and percentage,monocyte count and percentage,neutrophil count and percentage,ratio of neutrophil to lymphocyte count (N/L),acute physiology and chronic health evaluation (APACHE) Ⅱ score,sequential organ failure assessment (SOFA),improved early warning score (MEWS),Glasgow coma score (GCS),multiple organ dysfunction syndrome (MODS) score and lactic acid (LAC) were collected on the day of admission and 3,5,7 days after trauma.These data were applied to construct weighted and biological score models for early diagnosis and prognosis of traumatic sepsis.Receiver operating characteristic curve (ROC) was performed and area under the curve (AUC) was calculated to measure the value of the two models in early diagnosis and prognosis of sepsis.Results AUC of the weighted model combined by APACHE Ⅱ score,SOFA score and MEWS was 0.729 on the day of admission.AUC of the weighted model combined by inflammatory cells was 0.680 and AUC of the biological score model was 0.800 3 days after trauma (P < 0.05).AUC of the weighted models combined by inflammatory cells was 0.798 and AUC of the biological score model was 0.812 5 days after trauma (P < 0.05).AUC of the weighted models combined by inflammatory cells was 0.706 and AUC of the biological score model was 0.713 7 days after trauma (P > 0.05).AUC of the biological score model had significant difference 3 days and 5 days after trauma (P < 0.05).Of the weighted model combined by APACHE Ⅱ score,MODS score,GCS and LAC to evaluate the prognosis of sepsis,the AUC showed significant difference on the day of admission (0.838),3 days after trauma (0.878),5 days after trauma (0.947) and 7 days after trauma (0.936) (P < 0.05).Conclusions Biological score possesses better effect on early diagnosis of sepsis 3 days after trauma.Weighted model combined by APACHE Ⅱ score,MODS score,GCS and LAC can effectively predict the prognosis of sepsis 5 days after trauma.
