Objective This research was designed to reveal the influence of the new rural cooperative medical system (NRCMS) on anticoagulant therapy of warfarin in atrial fibrillation (AF) patients in Gaochun district,Nanjing city.The reasons of not attending NRCMS and the relationship between NRCMS and the reasons of not correctly anticoagulant therapy were also investigated.Methods We collected outpatients and inpatients in Gaochun district in our hospital who suffered with AF and did not attend any medical insurance except NRCMS.Patients' data of whether enrolled in NRCMS,whether anticoagulated with warfarin,whether measure international normalized ratio and then confine it in therapeutic extent and fundamental data of gender,age etc.were collected by questionnaire.Then,the data were analyzed.Results A total of 1 094 patients were included in our research of which 896(81.9％) patients were enrolled in NRCMS.198 subjects did not attend NRCMS.The most common reason was not getting the message of NRCMS,which covered 104 patients (52.5％).Among the patients without counter indications of warfarin,801 patients did not be anticoagulated,of which 637 were covered by NRCMS and 164 were not insured by NRCMS,which accounted for 71.1％ and 82.8％ of their total respectively.The difference was statistically significant (x2 =11.387,P =0.001).Similar to the nonNRCMS,the most common reason of not accepted anticoagulation in patients covered by NRCMS was taking substitutions,which was hold by 42.9％ patients.All the differences of proportions of patients had recorded reasons with and without NRCMS had no statistical significance except for the proportion of patients who hold the reason of not knowing the necessity of anticoagulation,having difficulty in daily life,and compound reasons.Among the anticoagulated patients,202 cases did not restrict INR in therapeutic range,of which 170 cases were covered by NRCMS and 32 cases were not insured by NRCMS,which accounted for 65.6％ and 94.1％ of their total respectively.The difference was statistically significant (x2 =11.385,P =0.001).The most common reason of not restricting INR in patients covered by NRCMS was did not know the necessity of restricting INR,which was hold by 40.6％ patients.The proportions of patients who did not take anticoagulation with the reason of having difficulty in daily life had statistical difference,while the difference of other reasons did not meet statistical significance.Conclusion There were many patients did not attended NRCMS.NRCMS can improve the proportion of anticoagnlation therapy with or without correct ways.However,there were many patients did not treated with anticoagulation and there were many patients did not restrict INR in therapeutic rainge.Propaganda,health education and directions of health information are needed.

Although tumor immunotherapy has been proposed for many years,the consensus denoting it as an essential approach for fighting against cancer is reached only in recent years. Tumor immunotherapy can be categorized as active and passive ones. In order to successfully cure cancer,safe and efficient active immunotherapy is required. Dendritic cells (DCs)are not only the bridge linking innate and adaptive immunity,but also the key determinants of the quality of adaptive immunity:immunity versus immune tolerance. Therefore,the safe and efficient DC-based tumor-specific and broad-spectral tumor vaccine has an irreplaceable important position in tumor immunotherapy. Because of the high heterogeneity of DCs, the research on DC-based tumor vaccine has encountered a bottleneck. Here,we reviewed the progress in research on DC-based tumor vaccine and related problems needed to be resolved with the incorporation of our experiences.

Objective To study the relationship of gastric carcinoma with serum trace elements and helicobacter pylori(Hp).Methods The contents of 6 serum trace elements and helicobacter pylori were determined in gastric carcinoma,chronic gastritis patients and normal control.Results The serum levels of Mg,Ca,Zn and Cu in the patients with gastric carcinoma were significantly lower than those in normal control Mg (0.71?0.15)mmol/L vs (0.97?0.26)mmol/L,Ca (1.68?0.28)mmol/L vs (2.03?0.31)mmol/L,Cu (12.27?5.91)mmol/L vs (16.57?3.55)mmol/L,Zn (16.18?4.77)mmol/L vs (21.78?7.37)mmol/L.The infection rates of Hp in the patients with gastric carcinoma and normal control were 71.4% vs 27.3%(?2=6.201,P=0.013),there was significant diversity.Conclusion Obvious changes of serum Mg,Ca,Cu,Zn and infection Hp may be related to carcinogenesis of gastric carcinoma.

Objective To investigate the effect of pvdQ(PA2385)gene on Pseudomonas aeruginosa swarming motility.Methods The plasmid pME6032 with pvdQ gene was constructed and identified,then transformed into Pseudomonas aeruginosa PAO1 using the eleetroporation to build pvdQ-overexpression strain.The pME6032-PAO1 strain was constructed with the same method.The cloning plasmid pEX18Gm containing sacB was successfully used to construct unmarked deletion mutant of pvdQ gene and pvdQ mutant strain. Bacteria were inoculated in LB and were cultured overnight.The clones were measured for the diameter of the swarming zone.The statistical analysis was done using one-factor ANOVA.Results Strains of pvdQ-overexpression and pvdQ-mutant were successfully constructed and confirmed by polymerase chain reaction(PCR).The four strains were compared for the swarming motility by changes in diameter:PAO1(20.52±1.80)mm,pME6032-PAO1 strain(19.39±2.10)mm,pvdQ-overexpression strain(51.20±2.16)mm,pvdQ-mutant strain(3.30±0.55)mm.The diameter of pME6032-PAO1 strain was not significantly different from that of wild strain PAO1(t=-0.1493,P>0.05).However,the diameter of pvdQ(Q-mutant strain was significantly shorter than that of wild strain PAO1(t=2.8525,P<0.05).while the diameter of pvdQ-overexpression strain was longer than that of the wild strain PAO1(t=1.4230,P<0.05).Conclusions pvdQ gene may be involved in regulating the swarming motility of Pseudomonas aeruginosa,which can promote Pseudomonas aeruginosa swarming motility.

Objective To understand the features of cognitive impairment in patients with subclinical hypothyroidism. Methods The cognitive function of the patients with subclinical hypothyroidism and the healthy controls was tested using Mini-Mental State Examination (MMSE) and Wechsler Memory Scale (WMS). The change in cognitive function was observed and analyzed before and after L-T4 treatment. Results Fifty-seven patients with subclinical hypothyroidism and the same number of healthy controls were enrolled in this study. There were no significant differences between two groups in biochemical indexes and MMSE score (all P＞0.05). But there were significant differences between two groups in WMS score, including pictures, memories, learning, touch, understanding memory and back several test scores (all P＜0.05). There existed certain degree of cognitive dysfunction mainly displayed in memory, attention, etc. After three months of L-T4 treatment, all had significant improvement (all P＜0.05). Conclusions Patients with subclinical hypothyroidism have cognitive dysfunction, when the thyroid stimulating hormone is restored to normal level, cognitive function is also improved.

Objective To study imageology anatomy concerned of sellar turica and explore its potential role on growth of pituitary adenomas. Methods According to classification of sinus sphenoidalis,103 normal imageology data of sellar turica region were analyzed, including 83 cases with entire saddle type sinus sphenoidalis and 20 cases anti-saddle type sinus sphenoidalis. Furthermore, the clinical data of 45 cases of pituitary adenomas with extension into sinus sphenoidalis were retrospectively studied. Results The relationship between sellar floor's shapes and sinus sphenoidals'types was that the larger the sinus sphenoidalis was, the deeper the floor was(98.8%). The relationship between sellar floor's shapes and the superior surface of hypophysis was that most of depressed sellar floors were companied with depressed superior surfaces of hypophysises(93.8%). The anteroposterior shapes of sellar turica in midsagittal can be divided into 4 types such as bag-type, kettle-type, tube-type, and sifter-type. 45 patients suffering from pituitary adenoms with extension into sinus sphenoidalis all had entire saddle type sinus sphenoidalis. Conclusions The imageology anatomy concerned of sellar turica could explain the phenomena of pituitary adenomas with extension toward sellar floor and sinus sphenoidalis. Referring to the literatures concerned, it indicated that the imageology anatomy was the key factor of affecting the growth of pituitary adenomas.

Objective · To observe the vascular structure before autogenous arteriovenous fistula (AVF) construction in patients with end-stage renal disease (ESRD) and analyze the risk factors of the pre-existing venous neointimal hyperplasia. Methods · The 8 vein samples were screened from 20 ESRD patients at their first time of the AVF construction (non-stenosis group), and the other 8 vein samples were screened from 15 ESRD patients at their at least second time of the AVF repair operation (stenosis group). Sections were prepared and stained with hematoxylin & eosin (H-E) or Masson's trichrome for observation. The intimal thickness was measured by the cellSens software, and its correlation with patients' renal function, calcium-phosphorus metabolism, iron metabolism and inflammatory reaction in the non-stenosis group were analyzed. Results · In the non-stenosis group, there were varying degrees of intimal hyperplasia in 5 (62.5%) cases, loss of endothelial cell layer in 3 (37.5%) cases, and vascular wall replacement by collagenous with atrophy or loss of muscle layer in 5 (62.5%) cases. In the stenosis group, almost all vein samples showed general thickening of the wall and 2 (25.0%) totally lost the muscle layer. Avg It of those two groups were statistically significant (P<0.01). In the non-stenosis group, both of average I/M thickness and average I/M area were negatively related to glomerular filtration rate (GFR) (P<0.05) and positively related to serum phosphorus and calcium-phosphorus product (P<0.05). Conclusion · Some apparently normal vein of ESRD patients showed varying degrees of intimal hyperplasia before AVF construction. The intimal hyperplasia had a remarkable correlation with GFR or calcium-phosphorus metabolism. Early intervention of the intimal hyperplasia prior to AVF construction may be a new prevention and control means.

AIM: To study the protective effects of intravenous (iv) CGRP on myocardial injury in rat. METHODS: Establish a rat myocardial ischemic injury model by subcutaneous injection of single dose of isoproterenol (ISO), and treat the model with single dose of iv CGRP. Two hours later, serum CK, LDH, MDA and SOD levels were measured, MDA and SOD in myocardial tissue were tested, and myocardial tissue structures were observed. AIM: To explore the activative effect of water extract from cultured mycelium of Paecilomyces cicadae (P. cicadae) on peritoneal macrophages (PM?) and alveolar macrophages (AM?) of rats.METHODS: The rats were randomly divided into four groups: normal control group(Ⅰ), cyclophosphamide (Cy) group (Ⅱ), P. cicadae group(Ⅲ), Cy+ P.cicadae group(Ⅳ).The rats were bred in the same circumstance and were administered with corresponding drugs by subcutaneous injection for 26 days. PM? and AM? of rats were irrigated by normal saline and collected by centrifuge and incubated in a humidified incubator for 2 h at 37 ℃. The activities of acid phosphatase (ACP) and lactate dehydrogenase (LDH) in the PM? and AM? were determined by biochemical methods, and the capability of PM? and AM? in phagocytosis of neutral red were measured by colorimetric method too. RESULTS: After administering P. cicadae to rats, the activities of ACP and LDH in PM? and AM? of normal rats were elevated significantly,and the reduction of the activities of ACP and LDH in PM? and AM? of rats due to Cy was notably antagonized, and the capability of PM? and AM? phagocytizing the neutral red were strengthened significantly.CONCLUSION: P. cicadae can activate the PM? and AM? of rats.
