Medical Picture Archiving & Communication System (PACS) and Radiology Information System (RIS) are the two embranchments of medical informatics developing most rapidly. The correlative legal issues of medical PACS and RIS should be paid attention to.

Objective To evaluate the effect of intra-aortic balloon pumping(IABP)in treating serious coronary heart disease. MethodsA retrospective analysis was performed on 19 patients who suffered from serious coronary heart disease and accepted IABP therapy,the differences of mean arterial pressure before and after treatment were compared.In order to compare the in-hospital mortality,the patients were divided into 2 groups:6 of 19 patients accepted single IABP therapy,13 of 19 patients attempted IABP and revascularization(thrombolytic/percutaneous coronary intervention/coronary artery bypass graft)therapy. ResultsBedside success rate of IABP operation was 100%without complication.Effective rate was 89.5%(17/19),2 patients who were irreversible phase of cardiogenic shock,were an ineffective treatment.The patient's mean arterial pressure increased from(52.1 ± 18.4)mm Hg to(78.3 20.8)mm Hg after using IA BP for 30 minutes(P＜0.01).The in-hospital mortality was significantly lower in patients received revascularization therapy in addition to IABP compared with patients who had IABP support alone 7.7% vs 83.3%(P＜0.01). ConclusionIABP in treating serious coronary heart disease was safe and effective.IABP treatment before irreversible phase of shock and revascularization therapy following IABP are the key to decrease in-hospital mortality.

BACKGROUND:Bone marrow is the main source of mesenchymal stem cells(MSCs)at present,but its application has been limited,because of some reasons such as inconvenience of isolation,and the quantity of cells decreases with human increased age.Umbilical cord as a new source of MSCs has been widespread concerned recently.OBJECTIVE:To explore the approach of isolating and culturing MSCs from Wharton's jelly of human umbilical cord,and the methods of identifying the surface antigens and the differentiation potential.METHODS:MSCs were isolated and amplified via tissue-cultivation,and cultured by FasGrow medium.Morphology of MSCs from Wharton's jelly of human umbilical cord was observed under the optical microscope.Its immunophenotypes were detected using immunohistochemistry.The differentiation of MSCs into the osteoblasts was determined utilizing Gomori calcium-cobalt alkaline phosphatase staining,von Kossa calcium node staining,and tetracyclinefluorescence labeling.The differentiation of MSCs into the adipocytes was detected using oil red O staining.RESULTS AND CONCLUSION:MSCs were easily obtained from Wharton's jelly of human umbilical cord via the proposed approach.The primary cells grew up to 70%-80% confluence after 12-16 days of culture,and meanwhile the undifferentiated state was maintained and proliferation was stabilized after passage.The cell cycle of double increase was about 2 days,and proliferation in vitro reached twenty generation above.Surface antigen analysis showed that CD44,CD105,CD133,MHC-I were positively expressed,while CD34,CD45 were negatively expressed.Experiments of differentiation in vitro indicated that the obtained cells were capable of differentiating into fat,osteoblast and nerve-like cells.

Objective To investigate the prevention and management strategies of in-hospital complications in the hip replacement in elderly patients(≥80 years old) Methods The data of 42elderly patients with hip replacement between Oct.2004 and Dec.2006 were collected and analyzed.Their age ranged from 80 to 96 years with an average of 84.6 years.Twenty-four cases were associated with severe osteoporosis(Dorr Ⅲ),13 with coronary heard disease,10 with arrhythmia,7with diabetes,5 with cerebral paralysis.Twenty five cases had a cemented stem bipolar femoral head replacement,2 had uncemented stem bipolar femoral head replacement,7 had cemented total hip replacement,3 had revision of total hip replacement. Results No death occurred during operations,1 died postoperatively due to dislocation at the 5th day postoperation.One case had coma for 1 week after general anaesthesia.5 had temporary cognitive handicap,38(90.5%)were able to walk at the aid of walker with easement of pain before discharging. Conclusions To gain good results in elderly patients with hip arthroplasty,less invasive and quick anaesthetic technique and operative procedures are encouraged.Meticulous preoperative preparation.active preventing and treating the intraoperative and postoperative complications should be emphasized.

Objective To explore the correlations of expressions of gastric mucosa water channel aquaporin AQP3 and AQP4 and different gastritis types. Methods The gastric mucosa was mounted under gastroscope. The types of gastric mucosa pathology and activity were tested by the common pathohistology. The expressions of AQP3 and AQP4 were determined by immunohistology. Results The expressions of AQP3 and AQP4 of chronic superficial gastritis were significantly higher than those in the non-gastritis group and chronic atrophic gastritis group (P < 0.01), especially in the activity period of chronic superficial gastritis. The expressions of AQP3 and AQP4 of chronic atrophic gastritis group were reduced when compared to those in the non-gastritis group, in spite of no statistical differences between them. While compared to the non-gastritis group , the expression of AQP3 of chronic atrophic gastritis group during the active stage was remarkably decreased (P < 0.05). Conclusion The expressions of AQP3 and AQP4 of gastric mucosa in chronic gastritis in various pathological types are different. AQP3 and AQP4 may be the targeted point , which could be used for the differential diagnosis and treatment of chronic gastritis of different pathological types.

The effects of DHAA-urea,a novel dehydroabietylamine(DHAA) derivatives,on cell viability and glucose metabolism,in hypoxia and normoxia human hepatoma HepG2 cells were investigated.Hypoxia cells were achieved using DMEM containing high concentration of glucose without serum and pre-incubating of CoCl_2 (final concentration 150 μmol/L) for 24 h.The antiproliferation effect of DHAA-urea was measured by colorimetric MTT assay.The cellular ATP concentration,the lactate dehydrogenase(LDH) and glucose-6-phosphate dehydro genase (G6PD) activity were detected by their kits.It was shown that DHAA-urea markedly inhibited cell viability,cellular ATP level,LDH and G6PD activity in either aerobic or anaerobic circumstance in a dose-and time dependent manner.This suggested that DHAA-urea possibly inhibited HepG2 cells growth via the inhibition of glucolysis and glucolysis-dependent ATP depletion.DHAA-urea could be a promising candidate in the development of a novel class of agents used for human hepatocellular carcinoma.

Objective To summarize the features of computed tomography (CT) and magnetic resonance imaging (MRi) of hepatic cystic echinococcosis, and investigate the key points of identification and diagnosis.Methods The clinical data of 58 patients with hepatic cystic echinococcosis who were admitted to the Affiliated Hospital of Inner Mongolia Medical University from August 2011 to August 2014 were retrospectively analyzed.Patients received plain and enhanced scan of CT and MRI.Hepatic cystic echinococcosis was divided into the 5 types according to the literatures, including unilocular echinococcasis in type Ⅰ, multivesicular hydatid cysts in type Ⅱ, anechoic content with detachment of laminated membrane from the cyst wall in type Ⅲ, calcification of lesions in type Ⅳ and mixed echinococcosis in type Ⅴ.Patients who were diagnosed as with definite or suspected hepatic cystic echinococcosis underwent surgery.The follow-up including observing the recurrence of hepatic cystic echinococcosis was performed by outpatient examination and telephone interview at postoperative month 3, 6, 12 for 1 year and then once every year up to August 2015, and was ended if there was no recurrence for more than 5 years.Results (1) The results of CT and MRI examinations: of the 58 patients, 54 received scan of CT and 21 received scan of MRI.Seventeen patients were detected in type Ⅰ with clear-boundary and low-density cystic lesions by CT examination;MRI examinations showed there were single or multiple, round or oval abnormal signal including low T1WI signal, high T2 WI signal and low T1 WI and T2WI signal of cyst wall.Thirteen patients were detected in type Ⅱ, CT examination showed the daughter cysts of multiple sizes were found in the mother cyst, arranged in honeycomb or wheel shape;MRI examination showed there were lower T1 WI signal in the daughter cyst and higher T2 WI signal in the daughter cyst compared with signal in the mother cyst, and low signal in the cyst wall of the daughter cyst and mother cyst.Six patients were detected in type Ⅲ with capsule in capsule sign and water snake sign by CT examination and ribbon sign by MRI examination.Thirteen patients were detected in type Ⅳ, CT examination showed there were irregular high-density calcified shadow with the performances for return sample or sample volume skins changes.Nine patients in type Ⅴ had more than 2 kinds of lesions.(2) Diagnosis: 4 patients were misdiagnosed by CT examination including 3 with preoperative diagnosis of hepatic cyst and 1 with preoperative diagnosis of metastatic carcinoma of liver, with an accurate rate of diagnosis of 92.6％ (50/54).Two patients with preoperative diagnosis of hepatic cystic adenocarcinoma were misdiagnosed by MRI examination, with an accurate rate of diagnosis of 90.5％ (19/21).(3) Treatment and follow-up: 58 patients underwent surgery, including 40 undergoing internal capsule removal with external capsule suturing (31 with open operation and 9 with laparoscopic operation), 10 undergoing partial hepatectomy and 8 undergoing external capsule enucleation.Of 58 patients, 3 were complicated with effusion of residual cavity, 2 with unclosed external capsule, 1 with bile leakage and then was cured after 4-8 week drainage.Fifty patients were followed up for 12.0-48.0 months with a median time of 27.1 months and a follow-up rate of 86.2％ (50/58).During the follow-up, 1 patient undergoing internal capsule removal had recurrence at postoperative month 8 and was cured by CT-guided interventional therapy using absolute alcohol, and other patients had no recurrence.Conclusions There was a higher accuracy in CT and MRI examinations for hepatic cystic echinococcosis.Honeycomb and wheel shapes are characteristic findings of hepatic cystic echinococcosis in type Ⅱ.The characteristic performances of CT examination for hepatic cystic echinococcosis in type Ⅲ are capsule in capsuleand water snake signs, and characteristic performances of MRI examination is ribbon sign.The ring-like enhancement of edge by MRI examination is an essential of identification and diagnosis between hepatic cystic echinococcosis and hepatic cyst, and irregular calcification is a differential point between hepatic echinococcosis and hepatic tumor.

OBJECTIVE: To observe the self-developed horn type of titanium clamp used for inferior turbinate resection from filling effect. METHOD: Choose the cases of inferior turbinate resection of 152 cases randomly selected 92 cases (group) in 2-4 angle type titanium clip head-tail closed wound middle turbinate, and therefore more than nasal passages in the surgical wound, just as in the nasal passages above micro tamponade, bare breathing zone, keep the ventilation, 1- 3 days to take out the angle titanium clamp; 60 cases (control group) with vaseline oil gauze or postoperative Merocel hemostatic sponge tamponade nasal bleeding. Observation of 1-3 days after nasal ventilation, headache, nasal bleeding, dry mouth, tolerance is painful, aural fullness tinnitus, a total of 7 indicators of sleep. RESULT: The team outside the there was no difference in blood loss and the control group, the rest of the indicators is better than the control group. CONCLUSION The angle of titanium clamp used in inferior turbinate resection from stuffing, patients get better comfort, avoid drawn yarn of pain, improve the quality of perioperative patients with life.

Objective To observe therapeutic effect in the near future of the operation of vertebrascope in route of retreat to treat lumbar disc protrusion and complications.Method According to symptoms,signs,myelography and CTM results to determine protrusion interspace and side type,applied the operation of vertebrascope in route of retreat to treat lumbar disc protrusion and perform functional exercises.Results 86 patients were followed up,and within these patients,58 appeared excellent (67% ),25 appeared good (29% ), 2 appeared not bad (2% ),and 1 appeared bad(1% ).Conclusion The technology has many advantages, such as little wound, little hemorrhage, little pain and fast restoration,which provide advantageous conditions for early functional exercises after operation.

Objective To explore the relationship between plasma ghrelin levels and glucose and lipid metabolisms of child and adolescent obesity.Methods 30 cases of obesity and 30 healthy normal weisht children and adolescents with same age and gender were selected.Plasma ghrelin level,serum insulin(INS),C-peptide(CPE),fasting blood glucose(FBG),total cholesterol(TC),triglyceride(TG),high-density lipoproteins(HDL),low-density lipoproteins(LDL)were measured.Results For the obesity group:Plasma ghrelin level significantly decreased [(15.49±1.98)vs(19.47 ±1.50)μg/L]compared with control(P＜0.05);BMI,waistline,FBG,INS were very significantly increased(28.19±3.83)vs(21.07±1.53)kg/m2;(88.60±6.13)vs(78.52 ±4.40)cm;(5.23±0.49)vs(4.59 ±0.63)mmol/L;(22.32 ±3.50)vs(16.98±2.50)μIU/ml,respectively compared with control(all P＜0.01);TC was significantly increased(5.87±0.88)vs(4.05±0.76)nmol/L compared with control(P＜0.05).For the plasma ghrelin level of obesity:The relationship between plasma ghrelin level and body weight,BMI,waistline,INS,TC was obviously negative(r=-0.385,-0.466,-0.336,-0.387,-0.373,respectively,all P＜0.01);The relationship between plasma ghrelin level and FBG was obviously negative(r=-0.324,P＜0.05);No relationship between plasma ghrelin level and age,body height,CPE,TG,HDLC,LDLC.Conclusion Plasma ghrelin level of children and adolescent obesity was obviously decreased,and was strongly related with disordem of glucose and lipid metabolisms.
