1.Effect of Continuous Passive Motion on Hand Spasm and Hand Function of Hemiplegic Patients

Objective To observe the effects of hand function training with continuous passive motion (CPM) on hand spasm and function of the hemiplegic patients.Methods 60 hemiplegic patients after stroke or brain trauma and with hand spasm were randomly divided into the treatment group and control group with 30 cases in each group. The cases in the treatment group were treated mainly with the method of CPM together with the routine rehabilitation procedures, while the those in the control group were only treated with the routine rehabilitation procedures. The changes of hand spasm and function of two groups were observed.Results After four weeks' treatment, the efficacy of the treatment group was obviously better than that of the control group ( P<0.05).Conclusion The rehabilitation training is an effective method for treatment of hemiplegia, and CPM is an important auxiliary method in the treatment. These two Methods combined together can shorten the course of treatment, and strengthen cure efficacy.
2.Expression of tumor stem cell markers Lgr5 and CD44 in esophageal carcinoma and its clinical significance

Objective To analyze the expressions of tumor stem cell markers Lgr5 and CD44 in esophageal carcinoma and its clinical pathological significance.Methods A total of 108 cases of patients with esophageal cancer, 35 cases of stage Ⅱ, 40 cases of stage Ⅲ, 4 cases of stage Ⅳ. After tumor specimens were stained, the expressions of Lgr5 and CD44 in esophageal cancer tissue were detected and its relationship with the clinical features were analyzed.Results There were relations between expressions of Lgr5 and CD44 in esophageal cancer tissue and clinical staging, the later the stageing, the higher expression rate of Lgr5 and CD44, but there was no significant differece with Chi-square test. The Spearman correlation analysis showed there was no correlation between Lgr5 or CD44 and esophageal cancer condition. Conclusion CD44 and Lgr5 are highly expressed in esophageal cancer tissue, but the correlation between CD44 and Lgr5 expression and esophageal cancer condition is uncertain.

5.Evaluation of image quality and radiation dose in maramography: a comparison among digital radiography, computed radiography and film-screen systems

Objective To compare the image quality, radiation dose and diagnostic accuracy on film-screen, digital radiography (DR) and computed radiography (CR) mammography. Methods Three different kinds of mammography machines (film-screen, DR and CR) were tested to make sure they were qualified and comparable. Radiographies were taken on Mammo-152 phantom and ACR phantom using filmscreen, DR and CR systems with the same radiation dose. Next, radiographies were taken on two phantoms using DR and CR systems with classic mode or AEC and the radiation dose was recorded. The images were numbered and scored by eight independent experienced radiologists under the same reading condition according to ACR method. Statistics was performed with the randomized complete-block design variance analysis. The diagnostic accuracy of three kinds of mammography methods were compared with Kappa test in clinical cases who had pathologic results. Results The spacial resolution of film-screen system was the highest (7.0-8.0 Lp/mm) and that of CR was the lowest (2.5 Lp/mm). When the radiation dose was increased (27. 0%-30. 0% ), the resolution was improved with DR ( +11. 0% ) but almost no change with CR ( +1.5%). The radiation dose of DR was the lowest on ACR phantom. The delineation of mass was better on film-screen than on CR, but the delineation of fibers and speck was better on CR. When the radiation dose of CR was increased (25. 0% ), the delineation of fibers and speck on CR was similar to that on DR and the delineation of mass was similar to that on film-screen. There was a good correlation between the mammography diagnosis and pathological results (film-screen is 50/56, DR is 83/90 and CR is 61/69,P < 0. 01 and Kappa value > 0. 75) . The diagnostic accuracy of three mammography systems had no statistical difference [film-screen 89. 3% (50/56), DR 92. 2% (83/90) and CR 88. 4% (61/69),Pearson X 2=0. 722 ,P =0. 697]. Conclusion Film/screen system has the highest space resolution and DR has the lowest radiation dose. Three mammography systems have the same diagnostic accuracy for breast disease.
6.Effect of nerve growth factor in promoting survival of super-length random flap

Objective To study the effect of nerve growth factor(NGF)in survival of super-length random flap.Methods The experiment employed the animal model of random flap in Wistar's rat back,applied 66.7 μg NGF locally in trial group's flap at the first day after operation with auto-control.The survival proportion,skin temperature and survival rate of random flap were observed,and the blood flow,the blood flow velocity,the blood flow volume change of random flap were measured by the laser and photic-conduct fiber means in differ time.Results The survival proportion(7.12±1.54)cm2 and survival rate(92±5)%of random flap of trial group was obviously higher(P<0.05)than those in control group[(5.23±0.19)cm2,(69±5)%]after pedicle division.The blood flow,the blood flow velocity,the blood flow volume and skin temperature in the midge and at the end of random flap in the trial group were obviously higher than those in control group 1 day after operation(P<0.05),and the blood flow velocity in the pedicle of random flap in the trial group was increased markedly(P<0.05).The blood flow volume and skin temperature in the pedicle of random flap in the trial group were higher than those in control group 3 days after operation(P<0.05).The survival of randomflapinthetrial groupwere very significantly increased than those in control group 7 days after operation(P<0.01).Conclusion NGF can increase flap's blood flow,blood flow velocity and blood flow volume,enhancing survival of the big-proportioned and super-length random flap.
7.Upper limb free mini-flap transplantation for repair of finger wounds

BACKGROUND:Recently, the clinical repair methods of irregular wound on fingers primarily include local pedicled flap and free mini-flap of upper extremities or lower extremities.
OBJECTIVE:To discuss the application of free mini-flap derived from upper limb in repairing the wound on fingers.
METHODS:From December 2010 to February 2014, 12 patients with irregular wounds on 12 fingers were selected from Department of Traumatic Orthopaedics and Hand Surgery, First Affiliated Hospital of Guangxi Medical University, China. The size of wounds ranged from 1.5 cm × 2.0 cm to 3.0 cm × 4.5 cm with different degrees of bone or tendon exposure. After the debridement, 12 patients were treated by free mini-flaps of upper limb. The donor sites were directly sutured.
RESULTS AND CONCLUSION:Among the 12 patients, al the wounds at recipient and donor sites were healed at stage I, and the free mini-flags survival completely. Al of patients were fol owed up for 3-6 months (average 4.5 months). Free mini-flags had good appearance and needn’t undergo secondary trimming. The active motion of fingers was improved dominantly. According to the criteria of Hand Surgery Association Society of Chinese Medical Association Society for the function evaluation of upper limb, three cases were excellent, eight were good, and only one was bad. The excellent and good rate was 91%. Free mini-flaps of upper limb are an ideal method for repair of wounds on fingers. It has no injury to normal tissue in hands, donor site is very secluded, and the short-term curative effect is good.
8.Diagnosis and treatment of pancreatic tuberculosis

Pancreatic tuberculosis (PTB) is a rare,chronic,specific and infectious disease which is generally secondary to tuberculosis at the common sites of pancreas,and it has a high misdiagnosis rate due to the hidden onset and nonspecific symptoms of PTB.A patient with PTB was admitted to the First Affiliated Hospital of Zhengzhou University in June 2014.Before operation,the space-occupying lesions of the head of pancreas were detected by preoperative imaging examination,and the patient was regarded as with pancreatic cancer.Intraoperative exploration showed cystic duct involvement,and the granulomatous inflammation was detected by rapid pathological examination using frozen section technique,after that the patient received granuloma resection+cholecystectomy according to suspected PTB.The diagnosis of PTB was confirmed by postoperative pathological examination,and the patient received liver-protective and anti-tuberculosis treatments after discharge.
9.Mangiferin protects bone marrow mesenchymal stem cells against hypoxia

BACKGROUND:Bone marrow mesenchymal stem cells have particularly applied prospects in tissue engineering. However, the death of transplanted cells limits the tissue regeneration. To search a new drug of anti-free radicals and protecting bone marrow mesenchymal stem cells is of great significance. OBJECTIVE:To investigate the protective effects of mangiferin on bone marrow mesenchymal stem cells against hypoxia. METHODS:Rat bone marrow mesenchymal stem cells were cultured in vitro and hypoxia cellmodel was established by cobalt chloride. cells were divided into normal control group, hypoxia group (treated with cobalt chloride), and mangiferin groups (cobalt chloride+20, 40, 80, 160 μmol/L mangiferin). After 12 and 24 hours of hypoxia, superoxide dismutase, malondialdehyde, and catalase levels in the cellsupernatant were determined. After 3, 6, 12, 24 hours of hypoxia, reactive oxygen species change was detected in each group. RESULTS AND CONCLUSION:Mangiferin significantly improved the survival rate of bone marrow mesenchymal stem cells exposed to hypoxia, increased the intracellular superoxide dismutase and catalase activities, decreased intracellular malondialdehyde and reactive oxygen species levels, thereby effectively protecting bone marrow mesenchymal stem cells against hypoxia. These findings indicate that mangiferin has effective protection against hypoxia and strong antioxidant ability, and can significantly reduce oxidative damage.
