1.Clinical analysis of 31 cases of intra-abdominal hernia
Journal of Third Military Medical University 1983;0(04):-
Objective To analyze the causes and the scheme of diagnosis and treatment of intra abdominal hernia. Methods Clinical data from 31 cases of intra abdominal hernia from 1977 to 2001 were analyzed retrospectively. Among the patients were 26 male and 5 female, and 4, 6, 19, and 2 patients were at the age range of 16 25, 26 40, 41 55, and over 56 years old, respectively. Results The causes of congenital intra abdominal hernia were related to malrotation of intestine and abnormality of intestine fixation during embryogenesis, to erroneous juncture of visceral layer peritoneum with parietal peritoneum, and to partial degeneration and weakness of mesentery. The causes of acquired intra abdominal hernia were related to operation or trauma. Conclusion As to patients with intermittence or episode of stomachache, touched painful sausage like mass, intestinal obstruction by X ray, and displacement of aortic arch of small intestines shown by selective arteriography, diagnosis of intra abdominal hernia and immediate operation should be considered. When reduction of the twisted intestines and repair of the hernial neck or hiatus are performed, important blood vessels such as the superior mesenteric arteries should not be injured.

AIM: To establish a new sensitive microplate-based method to determine alpha-glucosidase inhibiting activity and provide a reliable high-throughput way for screening alpha-glucosidase inhibitors in vitro.METHODS: The fitting combination of enzyme and substrate in a certain reaction was tested.Acarbose, the most popular alpha-glucosidase inhibitor in clinical use was used to validate the established method.Calibration curve, wavelength fidelity and kinetic analysis, together with the effect of altered incubation time, temperature and pH were then studied.RESULTS:The details of assay procedure and evaluation of factors affecting the measurement are described. As little as 160 μl assay system was performed in a 96-well plate. The optimal action was finally achieved by incubated at 37 ℃,pH 7.0 for 15 min and measured at 400 nm.Results from the validation exercises by Acarbose strongly demonstrated the accuracy and reliability of the proposed approach.CONCLUSION: This method reported in the current paper makes it possible to rapidly examine large numbers of samples for the presence of alpha-glucosidase inhibitors in very small sample volumes.Such action may help to pace the development of potential oral medications from natural products protecting patients against postprandial hyperglycemic toxicity and therefore treating diabetes mellitus and the related complications.

BACKGROUND: Tranexamic acid has been used to reduce bleeding after total knee arthroplasty in patients for the reason of big trauma and blood loss. Diabetes mellitus patients may have the poor ability to resist infection and heal tissue and vascular lesions. There are still no relevant literature reports about whether the application of tranexamic acid will achieve hemostasis and does not increase the risk of venous thrombosis of lower limbs.OBJECTIVE: To evaluate the efficacy and safety of tranexamic acid on perioperative blood loss in osteoarthritis or rheumatoid arthritis patients complicated with type 2 diabetes mellitus during total knee arthroplasty.METHODS: One hundred patients with the diagnosis of osteoarthritis or rheumatoid arthritis patients complicated with type 2 diabetes mellitus were selected between January 2013 and January 2015. Among all the subjects, 46 patients who received the operation before January 2014 served as the control group and 54 patients who received the operation after January 2014 were selected as the treatment group. Patients in the treatment group received 15 mg/kg tranexamic acid dissolved in 250 mL normal saline by fast intravenous infusion before the end of the operation. The patients in the control group just received 250 mL normal saline. Perioperative bleeding, blood transfusion, hemoglobin, hematocrit and coagulation index level were compared between the two groups, and deep venous thrombosis of lower limbs was observed.RESULTS AND CONCLUSION: (1) The postoperative drainage, hidden blood loss, total blood loss, transfusion volume, and transfusion rate in the treatment group were lower than that in control group (P < 0.05). (2) The levels of hemoglobin and hematokrit in the two groups were not significantly different, but decreased at 3 hours, 1 and 3 days after the surgery, and increased at 5 days postoperatively, but still lower than preoperatively. The levels of hemoglobin and hematokrit in the treatment group were significantly higher than that in the control group at different time points postoperatively (P <0.05). (3) Prothrombin time, activated partial thromboplastin time, and fibrinogen were not significantly different preoperatively, during tourniquet removal, at 3 hours, 1 and 5 days postoperatively between the two groups. D-dimer levels were not significantly different preoperatively and during tourniquet removal in both groups, but increased at 3 hours, 1 and 5 days postoperatively; moreover, D-dimer levels were significantly lower in the treatment group than in the control group (P < 0.05). D-dimer levels were not significantly different between the two groups at 5 days after surgery. (4)Deep venous thrombosis of lower limbs was not visible in double lower limb venous ultrasonography in both groups at 5 days and 1 month postoperatively. (5) To decrease the blood loss, intravenous infusion of 15 mg/kg of tranexamic acid during total knee arthroplasty before tourniquet removel is effective and safe in osteoarthritis or rheumatoid arthritis patients complicated with type 2 diabetes mellitus.

Objective: To explore the effects of different proportions of Fructus Cnidii (Shechuangzi) and Psoralea corylifolia (Buguzhi) on highly metastatic human breast cancer cell line MDA-MB-231BO and bone marrow stromal cell line ST-2 in vitro. Methods: Thirty-six female SD rats were randomly divided into 6 groups to prepare the drug-medicated sera by administering with different proportions of Fructus Cnidii and Psoralea corylifolia, including 4:0 group, 3:1 group, 1:1 group, 1:3 group, 0:4 group and control group. MDA-MB-231BO cells and ST-2 cells were cultured in Dulbecco's modified Eagle's medium containing drug-medicated serum. Inhibition rates of MDA-MB-231BO cells and ST-2 cells were measured by methyl thiazolyl tetrazolium (MTT) method; migration ability of MDA-MB-231BO cells was tested by a cell migration experiment; alkaline phosphatase activity (ALP) of ST-2 cells was measured by using 4-nitrophenyl phosphate disodium salt, and mineralized nodule formation of ST-2 cells was measured by alizarin red staining. Results: Sera contaning different proportions of Fructus Cnidii and Psoralea corylifolia inhibited the migration activity of MDA-MB-231BO cells as compared with the blank serum, and serum contaning Fructus Cnidii and Psoralea Corylifolia at proportion of 1:1 had the best function (P<0.01). Fructus Cnidii and Psoralea corylifolia at ratio of 1:1 also enhanced the ALP activity of ST2 cells (P<0.05) and increased the number of mineralized nodules of ST2 cells (P<0.01). Conclusion: Kidney-warming recipe of Fructus Cnidii and Psoralea corylifolia can inhibit proliferation and migration of MDA-MB-231BO cells and increase the activity of ST-2 cells.

Using cyproheptadine ( CYP) as template molecule, methacrylic acid ( MAA) as monomer, ethylene glycol dimethacrylate (EGDMA) as cross-linker, molecularly imprinted polymers (MIP) with high selectivity to cyproheptadine (CYP) were prepared by the optimization of porogen, monomer, and the mole ratio of monomer to template. The specific surface area of the prepared polymers was 24. 9 m2 / g. The recovery of CYP was above 94. 0% when the following procedure was applied to the cartridge of MIP as adsorptive material: conditioning with methanol and water, loading with water, washing with water and methanol, and eluting with methanol-ammonia (95: 5, V/ V). As a control, the recovery of CYP on non-imprinted polymers cartridge (NISPE) was only 38. 9% . The binding capacity of the molecularly imprinted solid phase extraction (MISPE) towards CYP found to be about 8. 8 mg of CYP/ g polymers and the imprinting factor (IF) was about 2. 32. Under optimal conditions, a mixed standard solution of CYP, amitriptyline, sulfadiazine and trimethoprim (10 mg / L each) was uploaded on the MISPE and NISPE for selectivity experiment. The gradient elution was used by using 0. 05% sodium pentanesulfonate solution (A)-acetintrile (B) as a mobile phase. The recoveries on the MISPE for sulfadiazine and trimethoprim (different structure with CYP) were less than 10% , however, the recovery for the similar structural amitriptyline was more than 70% , and the recovery more than 90% for CYP. All the recoveries on the NISPE for four analytes were less than 30% . This new MISPE cartridge was applied to extract and enrich CYP in livestock drinking water sample, and the recoveries of CYP ranged from 80. 5% -97. 7% , and the limit of detection (LOD) was 0. 01 mg / L.
