3.CT Differential Diagnosis of Thyroid Neoplasm

Objective To investigate the value of CT scans in differential diagnosis of thyroid neoplasm.Methods CT findings ofthyroid neoplasm in 29 cases including 8 cases thyroid adenoma,4 cases nodular goiter and 17 cases thyroid carcinoma identified by operation and pathology were analyzed retrospectively.Results The contour of thyroid adenoma was distinct,a completely enhanced ring in 5 cases and enhanced nodules in 3 cases were showed on contrast-enhanced scan.4 cases of nodular goiter showed homogenous cystic low density the lesions were localized and distinct in contour,the cystic wall could be seen.Thyroid carcinoma appeared unhomogenous density,the edges contour of lesion was not distinct and infiltration on adjacent tissue might be seen.Conclusion CT scan might play an important role in diagnosis and differential diagnosis of thyroid neoplasm.
4.Coverage the soft tissue defect in the foot and ankle with peroneal artery perforator-based sural neurocutaneous island flap

Objective To study the methods and therapeutic effect of coverage the soft tissue defect in the foot and ankle with neurocutaneous sural artery island flap.Methods 30 patients had been reconstructed foot and ankle soft-tissue defects with neurocutaneous sural artery island flap,the area of soft tissue defect from 2.0 cm × 3.5 cm~ 8.0 cm× 16.5 cm,the flaps were designed with the size measuring 2.5 cm × 4.0 cm to 8.5 cm × 18cm.Results 26 flaps survived completely and 4 with partial or margin necrosis,the wound infection occurred in 6 patients and were treated with dressing,shin graft and antibiotic drug.Conclusions Sural neurocutaneous island flaps are easy to separate.Major arteries are not injured.It is the ideal flap to repair the soft tissue defect around ankle and foot,the texture,color and contour of the flap is similar to recipient area.
5.Relationship between depression and the function of cognition in patients with obstructive sleep apnea-hypopnea syndrome

Objective To assess the depression and cognition function in patients with severe obstructive sleep apnea (OSAHS) , and to study the relationship between the depression and severity of OSAHS, cognition disorders. Methods A total of 74 OSAHS patients (OSAHS group) and 39 normal controls ( control group) which all diagnosed by polysomnography (PSG) were chosen, and they were assessed the state of depression by self-rating depression scale ( SDS) and for cognition disorders by mini-mental state examination ( MMSE ). Results The score of SDS in OSAHS group was significantly higher than that in the control group(P<0.01). In patients with OSAHS , the score of SDS was positively correlated with apnea hyponea index ( AHI) , which was negatively correlated with mean level of SaO_2 ( MSaO_2 ) and the lowest SaO_2 ( LSaO_2 ) (P<0.01 ). The score of MMSE in OSAHS group was significantly lower than that in the control group, which was negatively correlated with AHI(P<0.01) and positively correlated with MSaO_2 and the lowest LSaO_2(P<0.01 ). The score of SDS was negatively correlated with MMSE( r= -0.505, P<0.01). Conclusion OSAHS patients exit depression and cognition disorders, which was correlation to intermitten hypoxemia in frequently.
6.Combination chemotherapy with paclitaxel and oxaliplatin in 30 patients with advanced gastric cancer

Purpose: To evaluate the efficacy and toxicity of combination chemotherapy with paclitaxel and oxaliplatin (two new drugs) in patients with locally advanced and(or) metastatic gastric adenocarcinoma ( M/AGC). Methods: Between May 2001 and May 2002, 30 patients (22 male and 8 female) with a median age of 58 years (range35-80) were consecutively enrolled in this study. Oxaliplatin and paclitaxel were administered as a two-hour infusion every one or two weeks, respectively, and DDP or FUDR were infused in abdominal cavity every one week. Results: Thirty patients were evaluable for activity, with 2 complete and 17 partial responses, objective response rate( RR): 63% ( 95% CI: 46% -80%). Twenty-three of thirty patients (77%) experienced WHO grade Ⅰ-Ⅳ bone marrow suppression, which was the most common and serious toxicity. WHO Grade Ⅰ-Ⅲ side effect ( non-hematological toxicity) of gastrointestinal tract, liver, peripheral neuropathy, kidney , mucositis and heart occurred in 40%, 30%, 13%, 10%, 10% and 7% of patients, respectively. No patients withdrew because of treatment-related toxicity. Conclusions: Our data suggest that the combination of paclitaxel and oxaliplatin has promising therapeutic activity in patients with advanced gastric cancer. This regimen shows good efficacy and an acceptable safety profile in M/AGC patients, and may prove to be a suitable alternative regimen in this indication, especially for the patients with bad function of the heart , liver and kidney or old, physically weak patients.

AIM:Many studies have documented an anabolic effect of hydroxymethylglutaryl coenzyme A (HMG-CoA) reductase inhibitors, statins, on undisturbed bone. Reports of their effects on fractured skeleton were limited. A study was therefore conducted to check the effects of statins on fracture healing. METHODS: Simvastatin (10 mg?kg-1?d-1) was injected subcutaneously to tissue overlying the site of fractured tibiae of ovariectomized rats for a treatment period of 5 d. Vehicle reagent was used as control. Healing quality was evaluated at 1, 2 and 4 weeks after fracture. 8.Surgical management of severe polycystic liver disease

Objective To evaluate the effects of different surgical treatments on severe polycystic liver disease (SPLD). Methods A total of 22 patients with SPLD were surgically treated in our Department from December 1989 to July 1999. Of the patients, 5 were treated with the partial hepatic resection in combination with cyst fenestration (group A), 7 with laparotomic fenestration (group B), 4 with laparoscopic fenestration (group C) and 6 with puncture under the guidance of ultrasonography B (group D). The surgical outcome and long term follow up results were retrospectively analyzed. Results After the treatments, all the patients experienced immediate relief of symptoms. However, the follow up for an average of 3 years showed that 10 patients developed recurrence of the disease. The recurring rates were 0, 28.5%, 65.5% and 100% in groups A, B, C and D, respectively. Conclusions The approach of partial hepatic resection in combination with cyst fenestration is the most effective treatment for SPLD. Laparoscopic fenestration may not be an appropriate surgical way for treatment of SPLD.
9.Lomparison of the relative indicators of RBC suspension produced by different leukocyte filter in different preservation periods

Objective To comprehensively analyze the effect and various detection indicators of leucocyte depletion suspended red blood cells(RBC) prepared by different storage time and different manufacturers of leucocyte depletion instrument to provide some decision-making basis for peers .Methods Two hundreds and forty bags of suspended RBC(1 ,1 .5 ,2 U) prepared from whole blood by 4 manufacturers were randomly collected and preserved in (4 ± 2)℃ dedicated refrigerator according to different specifica-tions .Then the sterile quick-release connector was used to take 240 leucocyte depletion suspended RBC samples of same specifica-tions and different shelf lives of 0 ,7 ,14 ,21 ,28 ,35 d by different manufacturers .Their capacity ,hematocrit ,leukocyte residues ,plas-ma free hemoglobin content and hemoglobin content were detected ,and the hemolysis rate at the end of shelf life was calculated .Re-sults The effect of leucocyte depletion suspended RBC by the same manufacturer had no statistical difference among different spec-ifications (P>0 .05);the capacity and plasma free hemoglobin content in the same specification had statistical difference among difference among different manufacturers (P<0 .05);the free hemoglobin and hemolysis rate at the end of shelf life in the same manufacturer had statistical difference among different preserving time (P<0 .05) .Conclusion By comprehensive considering the results of indexes of capacity ,hematocrit ,filter time ,free hemoglobin content and hemolysis rate ,our station chose the Shandong Weigao and Fresenius disposable leucocyte depletion plastic bags for ensuring the leucocyte depletion effect ,and safety and efficacy of RBC transfusion .
10.18F-FDG PET-CT imaging for treatment evaluation in lymphoma

Objective To investigate the clinical value of PET-CT scans in lymphomatous patients in post-therapy evaluation. Methods 18F-FDG PET-CT imaging were performed in 40 lymphomatous patients whom had been received therapy. Successive PET-CT imaging were performed in 5 cases, and the treatment was evaluated with clinical evaluation. Results Successive PET-CT imaging were performed in 5 cases, in which the tumor was eradicated in 2 cases, improved in 2 cases, and relapsed in 1 case. In 35 patients PET-CT imaging was repeated after treatment, among them 30 cases with complete or partial remission, and PET-CT imaging was positive in 25 cases. PET-CT imaging was still positive in 8 cases with recurrent or remnant tumor. PET-CT imaging changed the therapeutic regime. Conclusion PET-CT imaging can detect the recurrent or remnant tumor sensitively and accurately. There is a significant value for the guidance of clinical therapeutic.
