1.Magnetic resonance neurography of lumbar plexus in healthy people

Objective:To Compare the difference between the data of normal lumbar plexus nerve measured through magnetic resonance neurography(MRN) with specimen. Methods:Volunteers of 30 healthy lumbar plexus nerve cases(19 males and 11 females) were selected, to measure the short shaft line and neuromuscular signal ratio of the right normal lumbar nerve root, ganglia, nerve and the femoral nerve stem, anatomy and measurements of the right lumbar plexus L2~5 nerve root,ganglia and nerve stem, femoral nerve morphology and size. And comparison of MRN measurement data and autopsy measurement data were conducted. Results:MRN could clearly show L2~5 nerve course, ganglion being circle or Oval enlargement, with the size about 3~6mm;a nerve root and stem displayed strips and smooth edge, with the size about 2~5mm, nerve Festival signal is more than the nerve root and stem. There was no significant statistical difference with the comparability between the obtained data and autopsy ones. Conclusion:MRN can clearly show lumbar plexus and major branches; its measured value can reflect changes in lumbar plexus. It is possible for diagnostic criteria of the lumbar plexus nerve diseases and dysplasia.
2.Protective effect of equivalent uiform dose optimization on organ at risk of tumor treated with intensity-modulated radiation therapy

Objective To evaluate the effect of an equivalent uiform dose optimization (EUD) based optimization algorithm in sparing the organ at risk (OAR) of tumor treated with intensity-modulated radiation therapy (IMRT).Methods 10 patients of nasopharyngeal carcinoma (NPC),10 patients of esophagus cancer,6 patients of rectum cancer were randomly selected from all patients received IMRT treatment.Then physical optimization constraints (dose/dose-volume constraints) were designed as EUD optimization,while the physical objectives for targets and other OAR were unchanged.Results There was statistically significant difference in protecting OAR in 8 NPC patients of PLANED group compared with those of PLANNO_EuD group (t values were 2.590,2.352,3.656,2.148,2.283,3.683,2.192,2.353,2.146,2.276,2.126,P < 0.05).There was no statistically significant difference in protecting OAR in 10 cases of esophageal cancer between PLANEUD group and PLANNO_EUD group (t value were 0.408,0.427,0.920,P > 0.05).There was statistically significant difference in protecting OAR in 6 cases of rectum cancer of PLANED group compared with those of PLANNO_EUD group (t values were 2.332,2.693,2.279,2.244,P < 0.05).Conclusion EUD optimization can reduce the doses to OAR which may be quite helpful to reduce the occurrence rate of OAR complications.
3.Assay and counter measure for the complications of the prosthetic disc nucleus-pulposus(PDN)replacement

[Objective]To analyze the reasons and counter treatment of complication after prosthetic disc nucleus implantation.[Method]From Feb 2002 to Aug 2004,80 patients of lumbar disc herniation were implanted with one single PDN device at our centre.65 patients of them came to the clinical,functional,and radiographic follow-up examinations,magnetic resonance images and follow-up records of all patients were reviewed carefully.They were divided into three groups.Group A:30 cases had at least 24 months follow-up data.Group B:23 cases had at least 36 months follow-up data.Group C:12 cases had at least 48 months follow-up data.[Result]After implantation,device migrations were noticed in 3 patients.24 cases experienced low back pain.In 24~48 month follow-up,the MR images of 39 patients had shown the abnormal signal pattern of the endplate where PDN-solo was implanted,even 32 of them had been found subsidence.In Group A,the height of intervertebral disc at the 24 month follow-up was decreased 4%.In Group B,at 36 month follow-up,it was decreased 12%.In Group C,at 48 month follow-up,it was decreased 18%.[Conclusion]After follow-up,the clinical effect of PDN is preferred.However,the complications of device dislocation,device subsidence remain problems that must be highlighted.This study suggested that the complications of PDN are serious.Therefore,the proper selecting patients for PDN replacement are crucial for good results.The indications for PDN must be restricted.
4.Research of the degradation of nano-nacre powder and the biocompatibility of the nano-nacre composite artificial bone

[Objective]To study the degradation of nano-nacre powder and the biocompatibility of the composite artificial bone made from this nano-scale powder in vivo.[Method]The nano-nacre powder made through mechanical grinding and the artificial bones made from this powder and PDLLA(poly-D,L-lactide acid) were implanted into the femoral holes and the femoral muscle sacs of rats respectively.Normal micron-nacre powder and the artificial bones made from this powder and PDLLA were implanted as control group,and blank group was set up without implanting anything.X-ray photography was performed immediately and at 2,4 and 8 weeks after operation.The animals were injected with tetracycline for fluorescent mark before being killed.Then,the gross specimens,histological and SEM(scanning electron microscope) observations were performed.[Result]All the observations showed that nano-nacre powder degraded faster than micron-nacre powder,meanwhile,the bone defect recovery was the fastest in this group.Both of the composite artificial bones made from those two kinds of nacre powder had the good connection with the adjacent tissue.[Conclusion]The degradation of nano-nacre powder is faster than that of micron-nacre powder in vivo,and it can promote the growing of new bone.Nano-nacre composite artificial bone also has good biocompatibility.It is a kind of better bioactive degradable material.
5.Perioperative nursing for patients receiving second stent implantation due to malignant airway stenosis

Objective To discuss the perioperative nursing measures and their effectiveness for patients with malignant airway stenosis who are receiving airway stent implantation once more due to airway re-stenosis.Methods The clinical data of 7 patients,who received airway stent implantation once more due to airway re-stenosis during the period from May 2014 to July 2016,were retrospectively analyzed.The effectiveness of nursing intervention for stent implantation was assessed.Results The symptoms of dyspnea were relieved immediately after the stent re-placement procedure in all the 7 patients,and no severe complications,such as massive hemorrhage,respiratory failure or asphyxia occurred.Conclusion The key points to ensure a successful second airway stent implantation include following measures and preparations:cooperation mode of medical care integration,preoperative individualized psychological intervention,guiding the patients to cooperate with the treatment,ready for in-operation emergency rescue,rapid and effective assistant cooperation for stent placement,and postoperative close observation for the occurrence of respiratory tract infection,bleeding and other complications.
6.Propylene glycol mannate sulfate down-regulated the expression of adhesion molecules(ICAM-1,VCAM-1)in kidney of diabetic rats

Objective To observe the effects of propylene glycol mannate sulfate(PGMS) on the expression of ICAM-1 and VCAM-1 in kidney of diabetic rats, and explore the protective effect of PGMS on kidney. Methods Diabetes was induced by injecting STZ. Then the rats were randomly divided into four groups: normal control group, diabetic modal group, treatment group with high-dose and low-dose of PGMS. 8 weeks later, the blood glucose concentrations, HbA1c, the ratio of kidney weight to body weight, urinary albumin excretion rate (UAE ) in 24 hours were examined. The ICAM-1 and VCAM-1 located were detected by immunohistochemistry, while the expression of ICAM-1 and VCAM-1 were detected by Western blot method and the levels of ICAM-1 and VCAM-1 mRNA by RT-PCR.Results PGMS could reduce kidney hypertrophy and lower urinary albumin excretion rate (UAE)of 24-hour significantly. Immunohistochemical study with light microscopy demonstrated that the levels, both ICAM-1 and VCAM-1 in the kidney of test groups decreased obviously. PGMS down-regulated the mRNA and protein levels of ICAM-1 and VCAM-1 significantly . A positive correlation between expression of ICAM-1,VCAM-1 and kidney lesion was observed. Conclusion: PGMS decreased the expression of ICAM-1 and VCAM-1 in diabetic rats.
7.The effect of glycosylated hemoglobin on serum level of plasminogen activator inhibitor-1 in patients with early diabetic nephropathy

Objective To investigate the effect ofglycesylated hemoglobin(GHbA,c) on tissue-type plasminngen activator (t-PA)and level of serum plasminogen activator inhibitor-1 (PAI-1) in patients with early diabetic nephropathy (DN).MethodNinety patients with early DN from April 2004 to May 2005 were divided into 3 groups according to the level of GHbA1c,which was respectively less than 7% (group A),between 7% and 9% (group B),and more than 9% (greup C).Thirty healthy adults were chosen as control group.The levels of serum GHbA1c,t-PA and PAI-1 were detected on empty stomach in the morning.Results The level of serum t-PA was lower,the activity of PAI-1 was higher in groups of DN than those in control group (P<0.05 or<0.01),and those were changed with the level of GHbA,c.There were significant differences between group C and group A[t-PA:(0.14± 0.06),(0.28± 0.11) U/ml; PAI-1 (3.25 ±1.01),(1.90q± 1.09) U/ml](P<0.05).Conclusions The fibrinolytic system exists unbalance in patients with early DN.Continuous hyperglycemia in patients with early DN make unbalanced fibrinolytic system more serious.Controlling the level of GHbA1c strictly can play an important role in delaying DN progress.
8.The expression of interleukin-25 in blood and nasal tissue of patients with chronic rhinosinusitis with nasal polyps.

OBJECTIVE:
To explore the expression of interleukin-25 (IL-25) in chronic rhinosinusitis with nasal polyps (CRSwNP) and its potential significance in pathogenesis.
METHOD:
IL-25 expression in blood was detected by enzyme-linked immunosorbent assay (ELISA). IL-25 expression in tissue was detected by immunohistochemistry (LSAB method) from polyps (68 CRSwNP patients) and 55 inferior turbinate mucosa from patients with deviation of nasal septum served as control. Complete blood count and HE staining of blood and tissue eosinophil infiltration degree.
RESULT:
IL-25 expression in CRSwNP group were significantly higher than the control group, the difference was statistically significant (P < 0.01). IL-25 expression in local organizations was positively correlated with the number of eosinophil infiltration in CRSwNP group (r = 0.679, P < 0.01).
CONCLUSION
The expression of IL-25 in CRSwNP patients mutually reinforcing and might increase eosinophil infiltration and play an important role in the development of CRSwNP.

9.Quality and quantity classification models of Fructus Amomi applying electronic nose with multiple mathematical statistics methods

Objective Fructus Amomi(Sharen) is derived from the dry ripe fruit of Amomum villosum Lour., A.villosum Lour. var. xanthioides T.L. Wu et Senjen and A.longiligulate T.L.Wu, which is widely utilized for its clinic effects on digestive system. However, Fructus Amomi from different species and habitats, possessing different quality, is difficult to identify. In this study, we aim to develop a simple, rapid and reliable method for authentication of Fructus Amomi. Methods Twenty-five batches of samples of Fructus Amomi were collected and electronic nose was introduced into analyzing their odor with multiple mathematical statistics methods. Na?ve bayes network (NBN), radical basis function (RBF) and random forest (RF) were applied to establish different classifiers while BestFirst+CfsSubsetEval (BC) was used to screen the attributes for searching sensor array with higher contributions. Results Firstly, after attribute-screening via BC, the established discriminative models via NBN, RBF and RF could successfully identify genuine and non-genuine samples, presenting correct judging ratios of 78% and 84% through ten-fold cross validation and external test set validation, respectively. Besides, quantity predictive models were constructed as well. In case of content of bornyl acetate, one of the effective components in Fructus Amomi, values were higher than 3.5 mg/g and lower than 1.8 mg/g with sensor response of 0.04 and 0.03, respectively. Conclusion In this paper, quality discriminative model and quantity predictive model of Fructus Amomi were established via electronic nose and multiple mathematical statistics methods. It indicates that electronic nose could be a promising method for quality evaluation of Chinese material medica.

Objective To study the novel methods of VMAT planning based dose volume histogram ( DVH) optimization, evaluated the dosimetry and planning efficiency in VMAT planning for Esophageal Carcinoma. Methods Twelve Esophageal carcinoma patients were enrolled in this study. The conventional VMAT planning as the reference, using multi?criterion optimization DVH ( MCO?DVH ) and overlapping volume histogram prediction DVH ( OVH?DVH ) two different methods to get ideal objectives function for informing VMAT plans, Then evaluate the dosimetric, planning efficiency for all new VMAT plans. The difference between the paired t?test groups. Results The two VMAT plans based DVH objective function can meet the clinical needs. Compared with the conventional VMAT plan, Conformity index ( CI ) and Homogeneity index ( 0. 77 vs. 0. 72, P=0. 017 and 0. 10 vs. 0. 12, P=0. 047 ) is better in DVH informed plans;lung V5 and spinal cord V50 are better in MCO?DVH informed plan (54. 66 vs.60. 23,P=0. 013 and 0. 98 vs.0. 49,P=0. 037).Furthermore,the DVH informed plans had higher planning efficiency (8. 2 vs. 19. 5,P=0. 023) . Conclusions DVH Objective informed VMAT Planning can achieve clinical needs with much uniform dose to target,lower OAR dose and higher planning efficiency.
