Objective To explore the glycerin suppositories nonretention enema treatment and the clinical ef- ficacy on acute constipation induced by a variety of unknown reasons. Methods 164 patients were randomly divided into two groups: observed group (81 cases) and the control group (83 cases ). Observed group used enema by 80ml glycerin suppositories retention. Control group used excessive traditional liquid soap retention enema. Effects of differ- ent methods of enema were compared according to fecal condition 5～20min after operation. Results The glycerin suppositories retention enema was much better than soap water retention enema with the total effective rate of 97.5 % and 81% respectively. The glycerin suppositories retention enema solution to the overflow and the occurrence of adverse reactions were significantly lower than those of the control group. There were significantly statistical dif- ferences(P < 0. 01). Conclusion Using the glycerin suppositories retention enema for the constipation, has no side effects,skin and beds contamination and patients are more willing to accept it.

Chronic rhinosinusitis (CRS) can be controlled by a combination of conservative treatment and surgical procedures. However, there is one group of CRS endotypes, refractory CRS, such as eosinophilic chronic rhinosinusitis with nasal polyp (ECRSwNP), for which the current treatment strategies of anti-inflammatory and/or antibiotic therapy or surgical removal of the lesion to improve sinus drainage are less effective. This lack of treatment efficacy highlights the need for further fundamental and clinical research to improve or restore nasal mucosal function of CRS. The role of the immune response in the pathogenesis of mucosal remodeling, which is a key problem in refractory CRS, is of particular current interest. ECRSwNP characterized by Th17/Treg imbalance in- duced by IL-6 has a different mucosal remodeling pattern. This review intends to illustrate the role of the imbalance between Th17 (T helper 17) and Treg (regulatory T) cells in the mucosal remodeling of ECRSwNP and to suggest a novel therapeutic target on treating ECRSwNP.

Objective To evaluate the clinical application value of the PPH in the treatment of severe prolapsed hemorrhoids.Method Forty cases with severe prolapsed hemorrhoids underwent improved PPH with homemade stapler.Results The operating period of time was 10～20min and the mean hospitalization period of time was 4 days.All the 40 cases were cured,of whom bleeding in 2 cases,postoperative malaise at the lower abdomen in lO cases,urinary retention in 11 cases,slight pain in 20 cases.After following-up for 3～15 months,there were no such complication as prolapse again,and no anal stricture,fecal incontinence,etc happened.Conclusion The improved PPH is a safe and effective new method in the treatment of severe prolapsed hemorrhoids.

Objective To investigate the association of two functional polymorphisms of tryptophan hydroxylase 2(THP2) gene,rs4570625 and rs4565946,with poststroke anxiety disorders.Methods Totally 112 poststroke anxiety patients and 246 non-anxious stroke controls were included and completed Hamilton Anxiety Rating Scale.DNA was extracted from blood and genotyped for the two polymorphisms of THP2 gene by polymorphism chain reaction.Results The G allele of rs4570625 was associated with the increased risk of poststroke anxiety.Both the GG genotype and G allele were observed to be significantly higher in female case than in control.No significant difference in genotype and allele firequencies of the rs4565946 was found among the groups.Patients with the G-C haplotype had significantly increased the risk of poststroke anxiety compared to controls.Conclusions Our findings suggest that these ftmctional polymorphisms in TPH2 gene may be involved in development of poststroke anxiety.

OBJECTIVE To compare the clinical characteristics between eosinophilic and noneosinophilic CRSwNP and identify the predictors of eosinophilic CRSwNP. The impact of comorbidity (allergic rhinitis-AR, bronchial asthma-BA) on the predictors was also studied. METHODS Histologic characteristics of surgical samples were analyzed in 182 CRSwNP patients, who were classified into eosinophilic CRSwNP (ECRSwNP) group and noneosinophilic CRSwNP (non-ECRSwNP) group. Factors associated with ECRSwNP were selected by correlation analyses. The optimal cutoff points of the predictors were determined by a receiver operating characteristic curve. Based upon the different comorbidity, four groups were divided to study the impact of comorbidity on the optimal cutoff points of the predictors. RESULTS Patients with ECRSwNP had significant higher rate of revision FESS, significant higher concurrent rate of allergic rhinitis and bronchial asthma than non-ECRSwNP patients (all P<0.05). There were significant differences in Ratio of Lund-Mackay scores, peripheral blood eosinophil percentage and serum total IgE between ECRSwNP group and non-ECRSwNP group except the peripheral blood neutrophilic percentage (U value were 1028.00, 1143.50, 800.00 respectively, all P<0.05). Peripheral blood eosinophilic percentage had the highest significant correlation with degree of infiltration of eosinophils in nasal polyps (r =0.538, 0.568, both P<0.05) and there also existed significant correlation between degree of infiltration of eosinophils in nasal polyps and Lun-Mackay score (r =0.419, P<0.05). Comorbidity of AR and BA could impact the peripheral blood eosinophil percentage. According to the different combination of comorbidity, the range of the optimal cutoff points of peripheral blood eosinophil percentage was 3.05%-4.8%. CONCLUSION ECRSwNP and non-ECRSwNP displayed significant differences in certain clinical features. Comorbidity of AR and BA could increase the peripheral blood eosinophil percentage, that may impact the diagnosis of the subtype of ECRSwNP.

Objective:To investigate the efficacy and safety of programmed death-1 (PD1) inhibitor combined with transcatheter arterial chemoembolization (TACE) in the treatment of huge primary liver cancer.Methods:From June 2016 to December 2019, the clinical data of 31 patients with huge primary liver cancer enrolled in the Central Hospital of Lishui were retrospectively collected and analyzed. The tumor size ranged from 10.1 to 18.8 cm, with an average of (14.2±2.3) cm. The patients were divided into TACE group (TACE treatment, 18 cases) and combined group (one week after TACE, patients receiving a dose of 200 mg PD1 inhibitor administration every 21 days, 13 cases), according to whether patients receiving PD1 inhibitors. The patients were followed up. The disease control rate (DCR) were compared between the two groups using Mann-Whitney U test. The median overall survival (OS) and progression free survival (PFS) were calculated by Kaplan-Meier method. Results:The DCR in combined group (53.8%, 7/13) was higher than that in TACE group (22.2%, 4/18), and the difference was statistically significant ( Z=-2.13, P=0.04). The median PFS (5.0 months) in combined group was longer than that in TACE group (3.0 months), the difference was statistically significant (χ2=4.39, P=0.04). The median OS (15 months) in combined group was longer than that in control group (9 months), and the difference was statistically significant (χ2=5.51, P=0.02). Conclusion:The combine PD1 inhibitors with TACE is an effective and safe therapy for huge primary liver cancer.

OBJECTIVETo establish a model bearing human lung cancer xenograft with bone metastasis in mice with normal immune function.

METHODSForty female C57BL/6J mice were randomly allocated into 4 equal groups, including a control group and 3 immunosuppression groups treated with low, moderate, and high doses of dexamethasone (50, 100, and 150 mg, respectively). Four days after immune suppression, the mice were subjected to percutaneous injection of1.0×10(9) L(-1) A549 cells into the tibial plateau, and the bone defects were assessed radiographically 28 days after modeling. HE staining and immunohistochemical staining were used to examine the tumor tissues and bone tissue damages.

RESULTSIn each of the 4 groups one mouse died during tumor cell injection. Only 1 mouse showed tumor formation in low-dose immunosuppression group, as compared to 7 and 4 in moderate- and high-dose immunosuppression groups. X-ray and microCT scan showed significant tibial bone destruction in moderate- and high-dose groups. The moderate- and high-dose groups showed similar ALP activities but both were significantly higher than those in the other two groups (P<0.05).

CONCLUSIONImmunosuppression with a moderate dose of dexamethasone results in longer survival time of the human lung cancer xenograft-bearing model mice as well as a higher tumor formation rate.

The guideline has been formulated with reference to the technical documents such as Guideline for Environmental Sanitation Disposal and Preventive Disinfection in Flooded Areas and Technical Proposal for Sanitary and Anti epidemic Measures after Flood Disasters, as well as the latest research progress at home and abroad. In order to guide the sanitary and anti epidemic measures in flooded areas, protect the health and safety of students and teachers and ensure the normal educational and teaching order, the guideline introduces the key measures that should be taken by schools, teachers and students in flood striken areas.
