Objective The correlation of union of femoral shaft fracture with selection of therapies was analyzed to explore the importance of protecting the periosteum and soft tissue,and to observe the influence of Chinese medicine.Methods A retrospective study was carried out in 478 femoral shaft fracture patients hospitalized in Zhongshan Hospital of TCM from 2001 to 2008.According to the selection of therapies,the patients were separated into conservative group(N=198) and operative group(N=280).The conservative group was then divided into two subgroups: conservative treatment alone(N=105),and conservative treatment combined with oral use of Chinese medicine(N=93).The operative group was divided into three subgroups: open reduction and fixation with dynamic compression plate(N=113),open reduction and fixation with limited contact-dynamic compression plate(N=98),and closed reduction and fixation with intramedullary nail(N=69).The correlation of union of femoral shaft fracture and selection of therapies was analyzed.Results The union rate in the conservative group was higher than that of the operative group(P

3.Transurethral Plasmakinetic Resection for Bladder Tumor Complicated with BPH:Report of 41 Cases

Objective To evaluate the efficacy and safety of transurethral plasmakinetic resection for patients with superficial bladder tumor(SBT) complicated with benign prostate hyperplasia(BPH).Methods A total of 41 patients with SBT complicated with BPH were treated by transurethral plasmakinetic resection in our hospital.Various procedures were selected basing on the location,size,quantity of the bladder tumors,size of the prostate,and degree of adhesion around the gland.Results The opertion was completed in all of the 41 cases.The mean operation time was(67.2?25.3) min for resection of the bladder tumors and(72.3?23.2) min for the prostate.During the surgery,7 cases showed obturator nerve reflex and 2 developed bladder perforation.No patients died or had transurethral resection syndrome or rectal perforation.Six months after the operation,reexamination showed a mean IPSS of 6.2?3.7 and Qmax of(20.1?4.2) ml/s,which were significantly imporved compared with those detected before the surgery [25.3?4.1,(8.9?4.1)ml/s,t=22.209 and -12.174 respectively;and both P=0.000].The patients were followed up for 1 to 4 years,during which 9 cases showed recurent bladder tumor.Conclusions Transurethral plasmakinetic resection is safe and effective for patients with SBT complicated with BPH.The surgical procedure should be determined accordingly.
4.Retroperitoneal laparoscopic adrenalectomy in 81 cases

Objective To evaluate the clinical significance of retroperitoneal laparoscopic adrenalectomy for benign adrenal tumors.Methods A total of 81 cases of retroperitoneal laparoscopic adrenalectomy from March 2002 to October 2005 was retrospectively analyzed,including 21 cases of Cushing's syndrome,57 cases of aldosterone-preducing adenoma,2 cases of adrenal pheochromocytoma,and 1 case of bilateral adrenocortical hyperplasia.Three trocars were introduced at the lumbar part and a retroperitoneal space was established by blunt dissection with the camera assembly.The tumor was removed by using a harmonic scalpel.Results The procedure was successfully completed in all the 81 cases,with an operation time of 20~140 min(mean,60 min) and an intraoperative blood loss of 20~100 ml(mean,45 ml).There was no need of blood transfusion intra-and post-operatively.Retroperitoneal hematoma occurred in 1 case post-operatively.Follow-up examinations in 37 cases for 3~18 months(mean,11 months) showed normal blood pressure,stature and laboratory findings.Conclusions Retroperitoneal laparoscopic adrenalectomy has advantages of minimal invasion,little blood loss,quick post-operative recovery,and low complication incidence.It should be considered as the first choice for treating benign adrenal tumors.
5.Postoperative Ultrasound, CT and MRI Follow-up in Alpha Fetoprotein Negative Hepatocellular Carcinoma

PurposeRecurrence limits the survival of postoperative hepatocellular carcinoma (HCC) patients. The purpose of this study is to investigate the value of ultrasound (US), CT and MRI follow-up in alpha fetoprotein (AFP) negative HCC patients.Materials and MethodsThe follow-up data of 31 pathology-confirmed, AFP negative HCC patients were analyzed retrospectively. All patients underwent US, CT and MRI. Features including tumor size, morphology, echogenicity and enhancement pattern were analyzed. The recurrent lesion detection rates of all three diagnostic modalities were compared.ResultsThere were 55 recurrent lesions. On CT and MRI, these lesions were round or ovoid in shape with long axis of 0.7-3.4 (1.7±1.1) cm. There were 16 solitary lesions and multifocal lesions in 15 cases. US showed widely distributed blood vessels within the lesions and heterogeneous flow rate. CT and MRI demonstrated significant enhancement in the arterial phase with wash out in portal phase and delayed phase. The detection rate were 60.0% (33/55), 83.6% (46/55), 89.1% (49/55) for US, CT and MRI, respectively (χ2=15.120,P<0.01). Detection rate of MRI (80.0%, 16/20) was signiifcantly higher than that of CT (65.0%, 13/20) and US (40.0%, 8/20) for lesions with long axis diameter of 0.7-1.0 cm (χ2=6.910,P<0.05). For lesions between 1.0-2.0 cm, MRI, CT and US detection rate were 91.7% (22/24), 91.7% (22/24) and 66.7% (16/24), respectively (χ2=6.792,P<0.05). ConclusionImaging follow up can detect AFP negative HCC recurrence. MRI has unique advantage in lesions <2 cm.
6.Argon-helium cryoablation combined with transcatheter arterial chemoembolization for the treatment of advanced hepatocellular carcinoma:analysis of therapeutic effectiveness

Objective To evaluate argon-helium cryoablation combined with transcatheter arterial chemoembolization (TACE) in treating advanced hepatocellular carcinoma (HCC). Methods The clinical data of 66 patients with pathologically-proved HCC were retrospectively analyzed. Based on the therapeutic scheme the patients were divided into TACE group (n=31) and combination group (TACE+argon-helium cryoablation, n=35). All the patients were followed up for 5-35 months. The complete remission rate, total effective rate and survival time were evaluated. The short-term and the long-term effectiveness were compared between the two groups. Results Both the complete remission rate and total effective rate of the combination group were significantly higher than those of TACE group (P<0.05). The median survival time of the combination group was significantly longer than that of TACE group (P=0.038). The half-year, one-year and 2-year overall survival rates of the combination group were higher than those of TACE group, although the differences were not statistically significant (P>0.05). Conclusion For the treatment of advanced hepatocellular carcinoma, argon-helium cryoablation combined with TACE can improve the short-term effect and prolong the progression-free survival time, although its exact effectiveness still needs to be confirmed by large sample, multi-central and randomized controlled studies.

8.Expression and significance of DNA-dependent protein kinase in human laryngeal squamous cell carcinoma.

OBJECTIVE:
To study the expression of DNA-dependent protein kinase (DNA-PK) in human laryngeal squamous cell carcinoma (LSCC) and normal laryngeal mucosa (NLM), and to analysize the relationship between the expression and the clinicopathologic parameters of LSCC.
METHOD:
Immunohistochemical technique (Envision) was used to detect the expression of DNA-PK in 64 cases of LSCC and 15 cases of NLM. To investigate an investigation was conducted on the relationship between the expression and clinico-pathological features of LSCC.
RESULT:
DNA-PK was lowly expressed in NLM and highly expressed in LSCC,the positive rate of DNA-PK expression was 26.67% (4/15), 78.13% (50/64), respectively, and there was significant different difference between the two groups (P < 0.05). Its expression was correlated with the level of histodifferentiation (P < 0.05), but not with TNM stages and neck lymph node metastasis (P > 0.05).
CONCLUSION
DNA-PK may be involved in disease development of LSCC.

9.Therapeutic Effect of Guxi Recipe 2 Combined with Joint Loosening Training for Postoperative Ankylosis in Tibial Plateau Fracture Patients

Objective To observe the effect of Guxi Recipe 2 combined with joint loosening training for the treatment of postoperative ankylosis in tibial plateau fracture patients.Methods Thirty-two patients with postoperative ankylosis had active exercise combined with small-range continuous passive motion(CPM) 2 weeks after operation,and received steaming and washing with Guxi Recipe 2(mainly composed of Herba Menthae,Radix Saposhnikoviae,fermented soybean,Rhizoma et Radix Notopterygii,Ramulus Cinnamomi,Radix Clematidis,Radix Pterospermi Heterophylli,Radix Angelicae Pubescentis,Olibanum,Cortex Acanthopanacis,and Myrrha) combined with CPM and manipulation for loosening joint 3 weeks after operation.The function of knee joint was assayed with the criteria of knee joint function evaluation issued by the Hospital for Special Surgery(HSS) 6 and 12 months after operation.Results The results of knee joint function scoring showed that the therapeutic effect was excellent in 21 patients,good in 8,average in 3,and the excellent and good rate was 90.6%.Conclusion Guxi Recipe 2 combined with joint loosening training is effective on the recovery of knee joint function of tibial plateau fracture patients with postoperative ankylosis.
10.Research in length of stay, early complications and treatment cost in radical cystectomy patients with different Charlson Comorbidity Index score

Objective To assess the length of stay,early complication and cost of treatment in radical cystectomy (RC) patients with different Charlson Comorbidity Index Score (CCI).Methods A prospective study of a total of 102 patients who underwent RC between March 2012 and August 2014 in Center for Research of Urology in Yunnan Province,China.They were divided into three groups:69 cases in CCI=0 or 1group,19 cases in CCI=2 group,14 cases in CCI≥3 group.Comorbidities were graded according to CCI,and each patient was followed-up for 3 months after RC.Length of stay,early complications and treatment cost were analyzed by statistics.Results A total of 102 patients were analyzed.There were obvious differences with respect to length of stay,early complication and cost of treatment comparing patients in each CCI group,H=20.722,6.025,and 7.047,P<0.05.The incidence of diversion-related early complications in patients with different CCI showed significant difference,H=7.100,P<0.05,however,the non diversionrelated early complications did not show significant difference between patients with different CCI,H=2.590,P>0.05.Conclusions Patients with different CCI showed difference in the length of stay,early complication and cost of treatment in RC patients with different CCI.CCI≥3 might help to identify patients at risk for early complications after RC,especially in the orthotopic urinary diversion operation.Patients' complication should be evaluated standardized before operation and these should be included in patients' consideration.
