1.Clinical analysis of nine patients with laparoscopic pancreaticoduodenectomy

Objective To investigate the safety and feasibility of laparoscopic pancreaticoduodenectomy (LPD). Methods Clinical data of 9 patients who underwent LPD operation from September 2014 to February 2017 in the first hospital of Shanxi Medical University were analyzed retrospectively. Results 9 patients attempted LPD, while one required conversation to open procedure. Two cases underwent hand assisted laparoscopic surgery .The average operative time was (553 ± 86.1) minutes, the average amount of bleeding was (333.3 ± 304.1) ml, and the average hospitalization time after operation was (25.3 ± 8.9) d. Postoperative bowel sound recovery time was (3.5 ± 1) d. Postoperative complications included pancreatic leakage in 4 cases (A grade pancreatic leakage in 1 cases, B grade in 3 case), lymph leakage in 1 case, abdominal hemorrhage in 1 case , gastric emptying in 2 cases , and bile leakage in 1 case. All patients with postoperative complications were cured by non operative treatment. Postoperative pathological examination showed that 5 cases had duodenal papillary adenocarcinoma, 1 case had pancreatic ductal carcinoma, 2 cases had common bile duct disruption differentiation of carcinoma, and 1 case had pancreatic head retention cyst. Patients were followed up 3 to 24 months after operation. 1 case of pancreatic duct died 17 months after operation. One case of middle bile duct carcinoma 12 months after surgery had liver and retroperitoneal lymph node metastasis. The remaining 7 cases had no complications. Conclusions laparoscopic pancreaticoduodenectomy is minimally invasive, safe and feasible.
2.Effect of Dusuqing (毒素清) on free radical and prostaglandin metabolism of intestinal tissue in the aged rats with escherihia coli pneumonia

Objective:To study the pathogenetic mechanism of intestinal injury in the aged rat with pneumonia and the effect of Dusuqing(毒素清) on it.Methods:The models of rats with Escherihia coli pneumonia were duplicated and divided into aged control group(ACG),aged model group(AMG),high doses group(HDG) and low doses group(LDG) of Dusuqing and norfloxacin group.The pathological changes in lung tissue and intestine,the contents of intestinal 6-keto-prostaglandim F1α(6-keto-PGF1α),thromboxane B2(TXB2),nitride oxide(NO),malondialdehyle(MDA) and the activity of superoxide dismutase(SOD) were observed.Results:The lung and intestinal injury in AMG was more serious.The decrease of SOD activity and 6-keto-PGF1α content,the increase of content of TXB2,NO,MDA in the AMG were more significant than those in the ACG.In comparison with those in the AMG,the level of SOD activity,6-keto-PGF1α content were increased,otherwise the contents of MDA and NO were decreased in the HDG,the LDG and the norfloxacin group.Conclusions:The injury mediated by prostaglands and free radical is related to the occurance and development of damage of intestinal tissue with pneumonia. Dusuqing is able to improve this damage obviously in aged rats with pneumonia and the mechanisms and regulation of prostaglands metabolism.
3.Therapeutic Effect of Shenshuai Pellet on Chronic Renal Failure Patient and Its Influence on the Blood Rheology

Objective To observe the curative effect of Shenshuai Pellet on chronic renal failure patient and its influence on hemoglobin, blood plasma albumin and blood rheology. Methods 60 cases of chronic renal failure patients were randomly divided into the treatment group and control group. On the basis of high quality low-protein low-phosphorus diet, envies the vegetable protein, correction water and electrolyte balance, the acidosis and so on, the control group was treated with Oxyamyli tectus Aldehydum, the treatment group was given Shenshuai Pellet orally. Results Total effectiveness of treatment group and control group was 90% and 60% respectively. The clinical symptoms of treatment group were obviously improved. Induced pluripotent stem cell (iPS cell) opened a new way to stem cell research,iPS cells can differentiate into hepatocytes in vitro gradually,this breakthrough makes the iPS cells have become a promising clinical application source.During the process of iPS cells to hepatocytes,a variety of cytokines regulate gene expression by coordinating various liver cell growth signal to promote iPS cells into hepatocytes differentiation and maturation.This article summarizes the relevant cytokines and their role in iPS cells in hepatocytes differentiation.

Anterior ischemic optic neuropathy (AION) is the ciliary artery-circulatory disorders for the supply of optic nerve head area before the sieve plate prozone and sieve plate district, result in the insufficient blood supply of the optic nerve head and ischemia hypoxia, edema. Clinical manifestations of the type with non-artery inflammatory neuropathy and artery inflammatory neuropathy, which would both eventually lead to the irreversible damage to the optic nerve. In the early phase of AION, the main method is giving a glucocorticoids intravenous drip and a periglomerular injection (or retrobulbar injection) in time, but using the glucocorticoids for a long time or in quantity can cause a series of complications. Therefore, the combination of traditional Chinese and western medicine treatment of AION has become more and more significant in clinic.

Objective To study the effect of autogeneic platelet-rich fibrin (PRF) on proliferation and adipogenic differentiation of human adipose-derived stem cells (ADSCs) in vitro.Methods ADSCs were isolated from adipose tissue obtained from donors undergoing liposuction and were cultured,and underwent identification.ADSCs at passage 3 were divided into three groups:test groups were cultured with 1PRFM and 2PRFM,and control group was cultured without PRF membrane.Then the growth of the cells was observed by inverted microscope.MTT method was used to observe cell proliferation activity at days 1,2,3,4,5,6 and 7 after culture.Adipogenic differentiation of ADSCs was observed and quantified by oil red O staining at days 3,5,7,9,11 and 14.Results Cell proliferation and adipogenic differentiation would be increased with the PRFM,There were significant differences among three groups.Conclusions PRF could significantly promote proliferation and adipogenic differentiation of ADSCs.
8.Effects of the bioactive parts of Xiongma Decoction on CGRP-CRLR/RAMP1 pathway in TCC of rats with nitroglycerin-induced migraine

Objective To explore the effects of bioactive parts of Xiongma Decoction (parts of ethyl acetate and n-butanol extract) on the CGRP-CRLR/RAMP1 signal pathway so as to clarify its therapeutic mechanism on migraine.Methods We randomly divided 36 male SD rats into 6 groups with 6 in each:blank group,model group,groups of low-,medium-and high-dose Xiongma Decoction bioactive parts,and Sumatriptan group.By giving hypodermic injection of 10 mg/kg nitroglycerin,the migraine rat model was copied;Only 18 rats were found to have positive expressions of CGRP,CRLR,and RAMP1 in TCC with immunohistochemistry staining after heart perfusion.For the remaining 18 rats,TCC was stripped directly from the whole brain and divided into two parts,one part used to detect CGRP,CRLR,RAMP1 mRNA expressions by qPCR,and the other part to detect CGRP,CRLR,RAMP1 protein expressions by Western blot.Results The number of CGRP,CRLR and RAMP1 immunoreactive cells,the mRNA and protein expressions on TCC in model group were effectively increased,compared with those in the blank group (P<0.05),indicating that the model copying was successful.Compared with those in the model group,the number of CGRP,CRLR and RAMP1 immunoreactive cells in Xiongma Decoction bioactive parts was significantlv decreased,and the expressions of CGRP,CRLR and RAMP1 mRNA and protein were reduced (P<0.05).Conclusion The bioactive parts of Xiongma Decoction can reduce the activity of CGRP-CRLR/RAMP1 signal pathway in TCC of migraine rats.

Objective:To explore the effect of Jinfeng particles on the expression of MMP-1/TIMP-1 in cartilage of gout arthritis rabbit′s articular,to explicit treatment mechanism of gouty arthritis. Methods:Perfusion of ethambutol and adenine,inject MSU into joint cavity of rabbits to copy the model of gouty arthritis. After successful mode,were randomly divided into model group,Jinfeng granules in high,medium and low dose group(19. 6,9. 8,4. 9 g/kg),allopurinol group(0. 028 g/kg),Tongfengding group(0. 448 g/kg) ,another blank group. Administered for 7 days. 2 h after the last administration,were taken and painted articular cartilage tissue,to observe the changes of the pathological morphology;and the use of immune cell detection of the expression of TIMP-1,MMP-1,MMP-1/TIMP-1 ratio calculation. Results: Jinfeng granule could obviously reduce the articular cartilage of rats with inflammatory cells infiltration,hyperemia, swelling and necrosis, could significantly inhibit the expression of MMP-1 ( P< 0. 05 ) , stimulate TIMP-1 activation(P< 0. 01),and significantly reduce the ratio of MMP-1/TIMP-1(P< 0. 05). Conclusion: Jinfeng particles can effectively inhibit the pathological damage of gouty arthritis,upside the expression of TIMP-1 and its mechanism may be related to the inhibition of MMP-1,regulated the balance of MMP-1/TIMP-1,and then exert anti-inflammatory effects.
