Objective To compare the performance of 2009 and 2010 reference standards of the United States Clinical and Laboratory Standards Institute(CLSI)in antimicrobial susceptibility for Enterobacteriaceae.Methods The breakpoints of susceptibility for ceftriaxone,ceftazidime and aztreonam in CLSI M100-S19(2009)were used to analyze results of antimicrobial susceptibility of Escherichia coli (308 isolates)and Klebsiella pneumoniae(194 isolates),as well as those revised in M100-S20(2010).Results With both the breakpoints in CLSI M100-S19(2009)and CLSI M100-S20(2010),proportions of antimicmbial resistance of ceftriaxone,ceftazidime and aztreonam for isolates of extended-spectrum β-lactamases(ESBLs)-producing E. coli were 84.2 percent vs.98.0 percent and 84.9 percent vs.98.0percent,respectively:those for isolates of ESBLs negative E. coli were 4.2 percent vs.7.7 percent and 5.6percent vs.8.4 percent,respectively;those for isolates of ESBLs positive Klebsiella spp.were 67.2 percent vs.90.2 percent and 67.2 percent vs.90.2 percent,respectively:and those for isolates of ESBLs negative Klebsiella spp.were 9.8 percent vs.10.6 percent and 10.6 percent vs.10.6 percent,respectively.Conclusions Proportions of antimicrobial resistance for ceftriaxone,ceftazidime and aztreonam with the rcference breakpeints of CLSI M100-S20(2010)increase to various extent,8.8 compared to those with the old ones in 2009.Compared with ESBLs negative isolates,ESBLs positive isolates are affected much more by the breakpeints of CLSI M100-S20(2010).Proportion of affected Klebsiella spp.by the revised breakpoints of CLSI M100-S20(2010)for ceftazidime and aztreonam is higher than that of E. coli with ESBLs positive.

Objective To investigate the effect of perinatal care on the reduction of the maternal and perinatal mortality. Methods The perinatal care and the relationship between the maternal and perinatal mortality during 1980 to 1997 were analysed. The factors that related to the morality were studied. Results The maternal mortality rate was significantly decreased from 31.7/10~5 live births in 1960 to 16.1/10~5 live births in 1992～1997. The main cause of maternal deaths in 1960' was obstetrical factors, but in 1997 it was pregnancy complications. The perinatal mortality rate was 21.2‰ in 1980～1981 ,and was 10.9‰ in 1992～1997. The factors related to the maternal and perinatal mortality including the perinatal care, high risk pregnancy,and birth weight were analysed, the specific mortality rate of each factor was calculated. Conclutions The two mortalities can be used to evaluate the result of perinatal care,and are important feedback information for making health care policy. Pernatal care play an important role in the reduction of maternal and perinatal mortality.

Objective To study the expression of homocysteine in serum of patients with common malignant tumor,and initially investigate the possibility of serum homocysteine as cancer biomarker.Methods Expression levels of homocysteine and caner biomarkers (CEA,AFP,CA125,CA199) in serum of 180 patients with established malignant tumor and 30 healthy controls (control) were measured,the results of homocysteine were compared with that of the cancer biomarkers based on the cutoff value used in clinic.Results The expression levels of homocysteine was significantly higher in patients with malignant tumor than in controls [(13.89 ± 4.95) μmol/L-(21.40 ± 9.38) μ mol/L vs (11.40 ± 3.13) μmol/L,P ＜ 0.05)].Conclusions The positive predictive rate of homocysteine is higher than the four kind of cancer biomarkers in lung cancer,breast cancer,esophageal cancer.The increase of homocysteine in tumors may be universal,and Homocysteine may be used as cancer biomarker in lung cancer,breast cancer and esophageal cancer.

Objective To design leaf patterns for Multileaf Collimator(MLC)routine quality assurance(OA)with a 2D diode array.Methods According to the detector distribution characteristic of the 2D diode array and basillg on the"picket fence"pattern,design the"stepwise"pattern.For each diode involving MLC QA,a calibration curve of relative output versus leaf positioning error was measured through delivering a set of patterns with different intentionally introduced positioning errors.When this proposed technique was delivered,the referenced patterns were exposed,and the calibration curves were used as a mean to quantitative determination of the leaf possible positioning errors through the detector readings.Results Compared with the"picket fence"pattern,the"stepwise"pattern not only had a high detecting efficiency,but also increased the dosimetric sensidvity to leaf positioning error.A 1 mm Ieaf positioni.error corresponds to a dose variation of 25% for the"stepwise"pattern,while for the"picket fence"pattern the same positioning error just causes a 17% dose vailation.Conclusions The new"stepwise"pattern is more efficient to be carried out,and more sensitive to sub-millimeter changes of leaf positioning.

Objective To set up assessment indexes of nursing process quality in hospitals that are scientific, practical and in keeping with the times so as to provide hospital departments in charge of nursing with the basis for reasonable assessment and scientific decision-making. Methods Typical investigations were conducted to understand the content and methods of the current assessments of nursing process quality in hospitals; the basic scope of the assessment indexes was determined through meeting discussions; the indexes were selected, verified, and established by means of brainstorming and the Delphi method; field surveys were conducted to assess and analyze the nursing process quality in 25 departments. Results A system of comprehensive assessment of nursing process quality in hospitals was established. It covered four categories, viz. nursing services, nursing control, nursing skills and nursing safety, and decomposed into assessment indexes of different levels, with the overall structure consisting of 1 overall goal, 4 first-level indexes, 12 second-level indexes, 5 third-level indexes and 290 subindexes. Conclusion The new system of indexes, which stresses the idea of nursing services, highlights the quality control principle of focusing on the patients,and is characterized by clear-cut structure and easy operability, provides a reliable guarantee for scientifically and rationally assessing nursing process quality.

OBJECTIVE To discuss the reasons of causing mistakes in the nosocomial infection reports.METHODS We collected 183 cases with inaccurate reports and analyzed them by the prospective method.RESULTS The major reasons of causing the inaccurate nosocomial infection reports were the ill-defined infection time,inaccurate diagnostic standard,and the lack of clinic training.CONCLUSIONS It can reduce the inaccurate nosocomial infection reports,advance the level of nosocomical infection diagnosis,and improve the work efficiency through special training,in paralleling with the manager of nosocomial infection going deep into sickroom and communicating with clinicians.

Objective To verify the dose delivery accuracy of volumetric-modulated arc therapy plan by log-file analysis of linear accelerator that can be created when a dynamic delivery occurs.Methods Accelerator log file in binary format recorded the accelerator execution plan for each control point corresponding to the gantry angle,multi-leaf collimator leave position,cumulative machine monitor units ( MU).These information were read from the accelerator log file with Matlab7.1,then the original control points in the plan file replaced the corresponding information for the log,which generated a new plan.New plan was exported into the planning system to reculculate the dose.The volume dose histogram (DVH) and dose distribution was contrasted to determine the accuracy of the accelerator plan of implementation between two plans.Results Compared with the original plan,antry angle difference over ± 1° accounted for about 35％ of the entire arc of control points in 4 of 12 arcs and the percentage of the leave error of ±0.5 mm was about 95％.MU error of a single control point was larger,but the cumulative MU for each are was small which was located between-0.09％ to 0.11％ in the selected 12 arcs.Between the targets,the maximum dose,minimum dose,the mean dose differences were from-0.07％ to 0.42％,-0.38％ to 0.40％,0.03％ to 0.08％,respectively.The maximum dose and mean dose differences of organs at risks were located from-1.16％ to 2.51％,-1.21％ to 3.12％,respectively.Conclusions Accelerator log-file analysis to verify the VMAT plan nan be supplyed to the experimental method supplement.
