1.Study on Allergen Detection and Immune Molecules in the Patients with Bronchial Asthma

Objective To investigate the allergens and study the clinical significance of detecting serum levels of TNF-α,IL-4, IL-6 and IL-8 in patients with bronchial asthma.Methods 62 cases of patients with bronchial asthma were selected from August 2011 to August 2013 in Baoji Central Hospital,enzyme-linked immunosorbent assay were carried out in patients u-sing standardized allergens.62 cases of healthy people were selected as control group,and detected serum levels of TNF-α, IL-4,IL-6 and IL-8 of the two groups.Results Dust mites were the most important and the strongest allergens.The levels of TNF-α,IL-4,IL-6 and IL-8 in observation group was significantly higher than those in control group (230.87±27.82 ng/ml vs 152.14± 23.27 ng/ml;25.32 ± 12.04 ng/L vs 8.57 ± 3.64 ng/L;152.45 ± 24.56 μg/L vs 98.56 ± 18.63 μg/L;218.79±32.91μg/L vs 142.68±23.76μg/L,P<0.05).Conclusion TNF-α,IL-4,IL-6 and IL-8 levels in patients with bronchial asthma j udgment can be used as an important predictor,and it is worthy of clinical application.
2.Outcome and safety analysis of colonoscopy in 1 249 patients aged 80 years and over

Objective To investigate the value and safety of colonoscopy in patients aged 80 years and over.Methods Clinical and endoscopic data of 1 249 patients aged 80 years and over collected from December 2005 to December 2015 at PLA army General Hospital were analyzed retrospectively.Results The average age was 83.03 years.Reasons for receiving colonoscopy included constipation with abdominal distension(19.38 %)and hematochezia(10.57%).The completion rate of colonoscopy for the entire length was 94.50 %.There were no abnormal findings in 492 cases (39.39 %).Colonic polyps (31.62 %) and colorectal cancer (14.25 %) were among the major lesions detected with colonoscopy.Colorectal neoplasms were found in 58.33% of the 132 patients with hematochezia.The total complication rate from colonoscopy and treatment was 0.72%,with the complication rate from treatment at 1.32%.Conclusions Constipation with abdominal distension and hematoehezia are the main reasons for undergoing colonoscopy for very elderly patients.Colon polyps and colorectal cancer are common disorders in elderly patients over 80 years of age and people with hematochezia in this age group are at high risk of having colorectal cancer.Colonoscopy is a safe and effective procedure and an important examination method for very elderly patients with hematochezia.
3.Acute gout attack during upper gastrointestinal bleeding:a report of 186 cases

The clinical data of 186 patients with acute gout attack during upper gastrointestinal bleeding were analyzed retrospectively.The ratio of male to female was 1.7∶1, smokers and alcohol drinkers accounted for 66.1% ( 123/186 ) and 62.4% ( 116/186 ) of patients were had underlying diseases.All patients had different degree of joint painful and fever , the blood uric acid levels were higher.Gastroscopic examinations were performed in 166 patients , of whom 88 cases received endoscopic intervention.The symptoms of gout were improved after treatment with dexamethasone and celecoxib ; and the medication did not induce or increase gastrointestinal bleeding.
4.Palpitation- hepatosplenomegaly- ascites: a case report and literature review

This report presented a case of 62-year-old woman who was admitted to our hospital for cirrosis, but she had a variety of clinical manifestations,such as abdominal distension,diarrhea,ascites, hepatosplenomegaly,anemia, palpitation, flushing, low blood pressure, arrhythmia and so on. Upper gastrointestinal endoscopy showed thicken mucosa at gastric fundus. Colonoscopy revealed nodular, pseudopolypoid. Ascitic fluid test suggested a transudate. Endoscopic abnormal mucosa biopsy showed chronic inflammation. Many mast cells were seen in bone marrow and liver biopsies,and liver tissue immunophenotype was CD117, and CD68. Thus the patient's diagnosis was systemic mastocytosis. It is rare that a patient only has the gastrointestinal tract symptoms complicated with ascites without skin lesion. The goal for treatment was to reduce hypersecretion of mast cells.
5.Maternal and fetal outcomes in women complicated with lupus nephritis

Objective To evaluate the maternal and fetal outcomes of pregnant women with lupus nephritis (LN) and the risk factors.Methods Ninety-three patients with 97 pregnancies from January 1st,1990 to December 31st,2012 in Peking Union Medical College Hospital were evaluated retrospectively.Objects of study were divided into three groups:stable lupus before pregnancy (stable group,52 cases),active lupus before pregnancy (active group,26 cases),and newly diagnosed LN during pregnancy (19 cases).Adverse maternal outcomes included exacerbated disease during pregnancy,preeclampsia,increased proteinuria and impaired renal function during pregnancy or postpartum,maternal death,thrombocytopenia and hypocomplementemia.Adverse fetal or neonatal outcomes included therapeutically termination of pregnancy,fetal loss,neonatal death,preterm labor,small gestational age and asphyxia.Statistical analysis was performed by Chi-square test or Fisher's exact test.A binary logistic regression model was used to evaluate the risk factors for adverse maternal and fetal outcomes.Results (1) Adverse maternal outcomes:There was no significant difference between exacerbated cases during pregnancies in stable group and that in active group [53.8 % (28/52) vs 61.5 % (16/26),x2 =0.417,P>0.05].After deleting abortions before 20 weeks of gestation (5 cases in stable group and 4 cases in active group),there was no significant difference between preeclampsia incidence in stable group and that in active group [36.2% (17/47) vs 59.1% (13/22),x2 =3.204,P>0.05].In nineteen newly diagnosed LN women,eighteen cases were over 20 weeks of gestation,during which preeclampsia incidence was 6/18.(2) Adverse fetal or neonatal outcomes:Therapeutically termination of pregnancy rate was higher in active group than that in stable group[42.3%(10/26) vs 7.7%(4/52),Fisher's exact test,P<0.01].After deleting patients who required termination of pregnancy (three cases in stable group) and therapeutically termination of pregnancy (four cases in stable group and ten cases in active group),the rate of fetal loss and neonatal death was higher in active group than that in stable group [5/16 vs 6.7%(3/45),Fisher's exact test,P<0.05].The rate of adverse fetal or neonatal outcomes was higher in active group than that in stable group [92.3%(24/26) vs50%(26/52),x2=13.483,P<0.001].Among the nineteen newly diagnosed LN cases during pregnancy,the numbers of therapeutically termination of pregnancy and fetal loss were five and three cases respectively; among eleven live birth cases,two newborns died from severe asphyxia,and nine cases were preterm birth.(3) Binary logistic regression analysis showed that the independent risk factors for exacerbated lupus during pregnancy were hypocomplementemia (OR =0.300,95% CI:0.104-0.863) and thrombocytopenia (OR =0.054,95%CI∶0.007-0.439).The independent risk factors for preeclampsia in LN pregnant women were thrombocytopenia (OR=0.151,95%CI:0.046-0.499) and LN recurrence or first diagnosed during pregnancy (OR=0.135,95%CI:0.027-0.679).The independent risk factors for adverse fetal or neonatal outcomes were preeclampsia (OR=0.134,95%CI:0.028-0.637) and lupus active during pregnancy (OR =0.026,95 % CI:0.005-0.138).Conclusions Active lupus before pregnancy is associated with poor maternal and fetal outcomes in lupus nephritis pregnancy.All pregnancies with LN should be planned,preferably after more than six months of quiescent disease.Blood pressure,renal function,proteinuria and level of platelet and serum complements should be closely monitored.
6.Efficacy of L-Arabinose for bowel preparation before colonoscopy

Objective To evaluate the efficacy of L?Arabinose for bowel preparation before colonos?copy. Methods A total of 170 patients who underwent colonoscopy were randomized into 2 groups. The ex?perimental group (n=85) used L?Arabinose for bowel preparation, while the control group (n=85) used polyethylene glycol electrolyte solution ( PEG?ELS ) . The degree of comfort, adverse effects, and the visibility during colonoscopy were observed. Results Premedication of L?Arabinose for bowel preparation yielded to more comfort ( U=-4?349,P=0?000) , less adverse effects (χ2=29?27,P=0?000) , and similar visibility during colonoscopy ( U=-0?875,P=0?381) compared with PEG?ELS. Conclusion L?Arabinose is safe, comfortable, and effective for bowel preparation before colonoscopy.
7.The diagnostic value of endoscopic ultrasonography for gastrointestinal submucosal tumors

Objective To investigate the diagnostic value of endoscopic ultrasonography( EUS)for gastrointestinal submucosal tumors. Methods EUS was performed on 378 patients with submucosal tumor detected by endoscopy. The shape,number,origin,and property were recorded under endoscopic ultrasonogra-phy. The diagnostic consistent rate was calculated with pathology as the golden standard. Results Among 378 patients,there were 131 cases of gastrointestinal leiomyoma,mostly low echo( n=87 ),homogeneous echo (n=119),and originated from muscular layer of mucosa(n=92). There were 111 cases of gastrointestinal stromal tumor,mainly low echo(n=51),medium low echo(n=51),homogeneous echo(n=78),originated from muscularis propria(n=85). There were also 50 cases of carcinoid tumor,mainly low echo(n=36), originated from submucosa(n=27). Forty-five cases of lipoma were detected,also originated from submuco-sa,mainly hyperecho(n=40),homogeneous echo(n=41). Heterotopic pancreas(n=19),schwannoma (n=4),and granular cell tumor(n=4)were relatively rare. Overall diagnostic consistent rate of EUS in submucosal tumor was 78. 6%( 297/378 ),diagnostic consistent rate of the shape of the tumor was 91. 8%(347/378),diagnostic consistent rate of the number was 95. 5%(361/378),and the consistent rate of origin was 96. 8%(366/378). Conclusion All kinds of submucosal tumor have certain features under endoscopic ultrasonography. Endoscopic ultrasonography can display echo,size,origin of gastrointestinal submucosal tumors,as well as their relations with the wall layer in gastrointestinal tract. It plays an important role in the diagnosis,differential diagnosis and treatment options,but there are still some limitations.
8.Effect of pregnancy and spontaneous delivery on the morphology of levator ani muscle and expression f vaginal nerve fibers

Objective To investigate the effect of pregnancy and spontaneous delivery on the morphologic characteristics of the levator ani muscle and innervation of the vaginal mucosa. Methods Eight nullipara without pelvic floor dysfunction (PFD) and 64 normal primipara undergoing spontaneous delivery were enrolled in this study during July to December 2006 in Peking Union Medical College Hospital. Biopsy specimens of levator ani muscle (LAM) and anterior and posterior vaginal walls were obtained from the puerpera as well as from the 8 nullipara undergoing vaginal operation. The structures of LAM were examined with histological techniques. Vaginal mucosa specimens were examined using immunohistochemistry staining for protein gene product 9. 5 ( PGP 9. 5), vasoactive intestinal poptide (VIP) and ne uropeptide Y ( NPY),and the positive stained nerve fibers were calculated respectively. Results The LAMs of the puerpera undergoing spontaneous delivery presented myogenetic and neurogenetic changes, both acute and chronic.Type Ⅰ muscular fibers were predominant(79% )with both types increasing in diameters [ (86±9)μm and (79±15) μm]. Significantly different ( P < 0. 05 ) innervation of PGP 9. 5, VIP, and NPY nerve fiberswas observed between epithelial lamina of anterior vaginal wall(5.9±3. 3, 7. 6±3. 1 and 8. 2±3. 2, respectively) and that of posterior vaginal wall (3. 8±2. 9, 5.9±3. 1 and 6. 0±3.0, respectively), with the nerve fibers being more in epithelial lamina of anterior vaginal wall, while no difference in the innervation of nerve fibers was observed in the lamina propria. Significantly different( P <0. 05 ) innervation of PGP 9. 5 and VIP nerve fibers was observed in the lamina propria of the anterior vaginal wall in puerperal undergoing vaginal delivery (6.9±3.2 and 4.9±2. 1) compared with those in nullipara (3.9±3.6 and 3. 1±1.2). Conclusions Pathologic changes occur in LAMs and pelvic floor nerves during labor and delivery. LAM fibers become hypertrophy to adapt to the physiological changes during pregnancy. Richer innervation of PGP 9. 5 and VIP nerve fibers in the lamina propria of the anterior vaginal wall in puerpera undergoing spontaneous delivery is beneficial for dilation of the blood vessels and smooth muscles and makes preparation for delivery.
9.Therapeutic effect of endoscope-guided bougie dilatation on benign esophageal stricture in children

Objective To investigate therapeutic effect of endoscope?guided bougie dilatation on children with benign esophageal stricture. Methods Data of 71 patients with benign esophageal stricture were retrospective analyzed.Patients were divided into group A (reflux stricture),B (congenital esophageal atresia stricture) and C ( caustic injuries stricture) ,based on different causes. The expansion effectiveness and factors of the three groups were analyzed. Results A total of 885 expansions were performed on 71 patients with the total efficacy rate 94?37%( 67/71) . No statistic differences were shown in expansion effectiveness among the 3 groups; group C ( 14?9 times/case, P < 0?05 ) showed more expansion frequencies than group A (9?1 times/case,P<0?05)and group B(10?7 times/case, P<0?05),more complications than group A(1?22%VS 0,P<0?05) and group B(1?22% VS 0?31%,P<0?05). Conclusion Endoscope?guided bougie dilatation is safe and effective for childrens′ benign esophageal stricture. Caustic injuries, refractory benign esophageal stricture,need more expansions and may be accompanied with more complications.
10.Curcumin inhibits oxidative stress in BCG-infected macrophages based on Nrf2 pathway

Objective:To investigate the inhibitory effect of curcumin on oxidative stress in BCG-infected macrophages based on the Nrf2 pathway.Methods:THP-1-derived macrophages were infected.The experiment was divided into control group,BCG group,BCG+curcumin group and BCG+curcumin+ML385 group.Cellular ROS fluorescence intensity were observed under a fluores-cence microscope;Glutathione(GSH)levels were measured by Colorimetry;Western blot was used to detect the protein expressions of Nrf2,HO-1 and NQO1;MTT was used to detect the proliferation rate of macrophages.Results:BCG infection significantly enhanced ROS fluorescence intensity,reduced cell GSH content(P<0.01),inhibited protein expressions of Nrf2,HO-1 and NQO1,at the same time inhibited cell proliferation(P<0.01);curcumin significantly weakened ROS fluorescence intensity,increased GSH level(P<0.05),promoted Nrf2,HO-1 and NQO1 protein expressions and cell proliferation(P<0.01);Nrf2 inhibitor ML385 reversed the effect of curcumin.Conclusion:Curcumin can alleviate BCG-induced oxidative stress in macrophages by increasing the expression of Nrf2 and inducing the transcription of downstream antioxidant molecules.
