1.Therapeutic effect of Edaravone on the patients with acute cerebral infarction

Objective To verify the efficacy and safety of Edaravone,a novel hydroxy-free radical scavenger,on acute cerebral infarction(ACI).Methods We performed a randomized,placebo-controlled,parallel-group,double-blind study on ACI patients. 63 patients,enrolled within 48h of onset,were allocated to Edaravone group( n = 31) or the placebo group( n = 32) randomly. Edaravone was infused at a dose of 30 mg,twice a day,for 14 days. Meanwhile,Hydroxyethylrutin and Aspirin were taken as basic treatment. The therapies of the placebo group were similar to those of Edaravone group except for Edaravone.Before treatment and at 7th,14th and 21th day after treatment,the neurological deficits and activities of daily living (ADL) were evaluated using European Stroke Scale (ESS) and Barthel Index respectively. 3 months later,all the patients were followed up for survival and ADL.Results At 21th day the increase rate of ESS was evaluted.There were significan difference between Edaravone group[(60.3?28.2)%] and the placebo group[( 35.1? 23.6)%]( P
2.Clinical analysis of 30 patients with sporadic Creutzfeldt-Jakob disease

Objective To investigate the features of clinical,electroencephalogram(EEG)and imaging in the patients with sporadic Creutzfeldt-Jakob disease(sCJD).Methods The clinical data of 30 patients with sCJD were analyzed retrospectively.Results 6 patients were acute onset,18 patients were subacute onset,which followed by conscious disturbance,myoclonus and decorticate rigidity in 1~3 months.6 patients were chronic onset,which followed by above described typical symptoms in 1~2 years.All the patients showed generally persistent slow waves on EEG in the early stage.18 patients showed typical triphasic wave on EEG and 8 patients presented atypical triphasic wave,which were almost in the intermediate-advandced stage.There were 10 patients in bilateral basal ganglia,and 1 patient in right lenticular nucleus showed hypersignal in MRI T2WI.17 patients carryed out MR diffusion-weighted images(DWI).All the images showed the hypersignal in frontal,temporal,occipital cortex,and 8 patients of them accompanied symmetric hypersignal in bilateral basal ganglia.Conclusions Almost of sCJDs are subacute onset.DWI show typical hypersignal lesions in occipital,frontal,temporal cortex and /or basal ganglia in their early course of disease,which are useful in early diagnosis of CJD.All the patients in the intermediate-advandced stage shows conscious disturbance,myoclonus and decorticate rigidity.The character of EEG is typical triphasic wave.

Objective To investigate the clinical features,diagnosis and treatment of spontaneous rupture of renal angiomyolipoma (RAML). Methods A total of 23 cases of spontaneous rupture of renal angiomyolipoma were studied retrospectively.The main clinical manifestations were summarized as follows.Sudden and unexpected lumbar and abdominal pain occurred in 21 cases and lumbar swollen pain in 2.Of them 3 cases concomitantly had hematuria.Nausea and vomiting occurred in 2 cases,shock in 3 and fever in 1.All cases underwent B-ultrasonic examination and 21 had CT examination. Results Diagnosis of RAML was established preoperatively in 16 cases,but 4 cases were misdiagnosed for carcinoma of kidney,1 case for renal infectious disease.Two cases were diagnosed by frozen-section sample during operation.Nephrectomy was performed in 10 cases and nephron sparing nephrectomy in 12,and digital subtraction angiography (DSA) embolization in 1. Conclusions Spontaneously ruptured RAML often changes its clinical symptoms and imaging features,which may result in a high misdiagnosis rate and subsequent nephrectomy.Comprehensive analysis of clinical manifestations and image findings can improve preopreative diagnosis rate,and frozen slice sample examination during operation can avoid unnecessary nephrectomy.
6.Gemcitabine plus cisplatin in treatment of recurrent ovarian cancer

Purpose:To evaluate the efficacy and toxicities of gemcitabine plus cisplatin for patients with relapsed ovarian cancer. Methods:Twenty-eight patients with recurrent ovarian carcinoma received gemcitabine (1000 mg/m~(2)) plus cisplatin (35 mg/m~(2)) on days 1 and 8 of each 21-day cycle. Of 28 patients, sixteen who relapsed within six months of previously platinum-based regimen were platinum-resistant and the other twelve were platinum-sensitive. Results:Of 28 patients, there were 5 (17.9%) complete and 12 (42.9%) partial responses, for an overall response rate of 60.7% (95%CI: 41.7%–79.6%). The median time to progression for objective responders was 5.5 months with a range of 2.5 to 20 months. Median overall survival for all 28 patients was 12.5 months. Among 16 platinum-resistant patients, a 56.3% response rate occurred. The median survival time was 10.5 months. Among 12 platinum-sensitive patients, a 66.7% response rate occurred. The median survival time was 14.5 months. There were leukopenia grade Ⅲ in 35.7%, grade Ⅳ in 17.9%; thrombocytopenia grade Ⅲ in 28.6 %, grade Ⅳ in 14.3% of patients. Conclusions:Cisplatin plus gemcitabine is active in patients with relapsed ovarian cancer. The adverse effects are tolerable. Hematologic toxicities are manageable with dose modifications.
7.The curative effect evaluation of refractory ascites with ascitic ultrafiltration and reinfusion into abdominal cavity

Objective To observe the curative effect of refractory ascites with ascitic ultrafiltration and reinfusion into abdominal cavity.Methods Thirty-seven patients diganosed as refractory ascites were given ultrafiltration and reinfusion into abdominal cavity.The following parameters were detected respectively:24h urine volume,abdomen circumference,heart rate,blood pressure,serum electrolytes,serum albumin,total serum bilirubin.Results After treatment,the 24h urine volume increased and abdomen circumference decreasd significantly (P < 0.01 ).There was no significant difference in heart rate,blood pressure,serum electrolytes,serum albumin and total serum bilirubin before and after treatment (P >0.05).Conclusion Ascitic ultrafiltration and reinfusion into abdominal cavity has some effects on refractory ascites.
8.Malaria endemic situation in Xuzhou City from 2010 to 2013

Objective To understand the incidence and trend of malaria in Xuzhou City so as to provide the evidence for formulating the malaria control measures. Methods The information of network reported malaria cases and epidemiological da-ta were collected and analyzed statistically in Xuzhou City from 2010-2013. Results A total of 109 malaria cases were reported by the special report systems and the network report system in Xuzhou City from 2010 to 2013 in which there were 44 cases 40.37% of vivax malaria 62 cases 56.88% of falciparum malaria 2 cases 1.83% of quartan malaria 1 case 0.92% of ovale malaria. The latter three were all imported from other countries. Totally 93 cases 85.32% were confirmed by laboratory and other 16 cases 14.68% were diagnosed clinically. There was the incidence throughout the year and there were no obvious seasonal characteristics. The positive rate of blood test in the floating population was significantly higher than that of the local res-idents χ2 868.23 P<0.01 . Conclusion The malaria endemic situation is in the steady decline period in Xuzhou City. The local infections decrease significantly but the imported falciparum malaria cases increase year by year. Therefore the manage-ment for floating population and fever patients should be strengthened.
9. Amplitude of low-frequency fluctuation in resting-state spontaneous brain activity in children with idiopathic epilepsy

Objective: To explore the features of the amplitude of low-frequency fluctuation (ALFF) in resting-state brain activity in children with idiopathic epilepsy (IE). Methods Totally 17 children with IE (IE group) and 14 matched controls (control group) were recruited. The structural and resting-state fMRI data were collected, and ALFF was computed with DPARSF software. The difference of ALFF between 2 groups was explored with the multivariate regression model, and the age and gender were considered as covariates. The correlation between ALFF and duration of disease was evaluated. Results Compared with control group, increased ALFF brain regions were observed in right brainstem, left cerebellum and parahippocampus in IE group, and reduced ALFF brain regions were observed in bilateral anterior cingulate gyrus, bilateral orbital frontal cortex, left superior frontal gyrus, left middle frontal gyrus, bilateral parietal lobule and bilateral angular gyrus (all P<0.05, 3dClustSim corrected). The multivariate regression analysis showed that ALFF in the right brainstem was positively correlated with the duration of IE (r=0.82, P<0.05, 3dClustSim corrected). Conclusion: The spontaneous brain activity in children with IE alters and associates with duration of disease. These findings may be evidences for elucidating potential neural mechanism of IE in childhood.
