1.Quality of life of elderly person in aged-care at home and influencing factors in Shanghai Minhang district

ObjectiveTo understand the quality of life and influencing factors of elderly person in agedcare at home.MethodsThe elderly person in aged-care at home in 3 towns and a street of Shanghai Minhang District were enrolled by stratified cluster sampling.To investigate quality of life for elderly person by adopting SF-36 form.Meanwhile,to find out the principal influencing factors on quality of life by adopting both single-factor and multiple-factor analysis.ResultsThe score of life quality of senior citizen in local was as following:43.63 ± 30.04 (Physical Function),55.57 ± 48.96 (Role Physical),81.75 ± 20.49 (Bodily Pain),42.07 ± 18.84 ( General Health),58.57 ± 18.96 (Vitality),66.94 ± 26.79 ( Social Function),73.33 ± 43.33 ( Role Emotional),70.92 ± 19.02 ( Mental Health),61.60 ± 21.06 ( Weighted average).There were two aspects of the factors that influencing elderly persons'the quality of life:disease-related (coronary heart disease,cerebrovascular accident) and non-disease-related ( whether to go out for a walk every day,loneliness,living space,gender and on the current life satisfaction etc).ConclusionQuality of life of elderly person in aged-care at home is relatively low,special in the general health and physical function dimensions.The controllable factors are diseases,whether to go out for a walk every day,loneliness,on the current life satisfaction etc.

3.Activities of Daily Living and Related Factors of the Elderly Homecare Receivers

Objective To understand the activities of daily living and related factors of the elderly homecare receivers. Methods The elderlyhomecare receivers in 3 towns and 1 street of Shanghai Minhang District were enrolled by stratified cluster sampling, 830 elderly personswere investigated with Activities of Daily Living Scale (ADL). Results The score of the elderly in local areas was as following: ADLscore was (25.59 ± 11.16), Physical Self-maintenance Scale (PSMS) score was (8.74 ± 4.29), Instrumental Activity of Daily Living Scale(IADL) score was (16.85±7.53). PSMS impairment or dysfunction accounted for 55.19%, and IADL accounted for 79.64%. ADL score >16points had 593 people, accounting for 71.45%, ≥22 points had 458 people, accounting for 55.18%. Conclusion The elderly homecare receivershave relatively poor ability of daily living. Related factors were whether to go out for a walk every day, monthly income, cerebrovascularaccident, medical insurance, educational level, diabetes, the current life satisfaction, disease and loss of spouse.
4.Insight on core human resource management of a hospital

Core human resource serves as the principal capital of carrier of knowledge and skills of a hospital,which is the most decisive factor of a hospital' s competitiveness and adaptability for its initiative and creativity.Their stability and development plays a key role in hospital development.This study analyzed the characteristics and capacity demand of such human resource,and proposed that their management should differ from the rest,recommending reforms in their job description,development and training,performance appraisal,remuneration design and incentive mechanism.
5.The change of cellular growth factor receptor and its effect on the proliferation and migration of vascular smooth muscle cells under electrical fields

Objective To study the change of the expression of cellular growth factor receptor and its effect on the proliferation and migration of vascular smooth muscle cells (VSMCs) under electrical field. Methods 1) A electrophoresis apparatus was designed for applying the direct current electric fields to the cultured VSMCs, which were separated from the aorta of rats. Interval photographs were used to analyse the direction and distance of VSMCs migration. Cellular growth factors such as platelet derived growth factor receptor (PDGFR), angiotensin II type 1 receptor (AT1R) and its type 2 receptor(AT2R) were studied by immunohistochemistry or immunofluorescence to show the mechanism relation to the migration of VSMCs. Results Under the direct current electric field, the expression of PDGFR was increased obviously and distributed asymmetricly on the membrane of VSMCs. The receptor mainly focused on the cathode side of cellular membrane in electric field. The AT1R were also enhanced in the cells after exposure to the electric field, but the expression of AT2R was uncharged. The longer exposure time to the 150 mV/mm electric field, the more electrotaxis the cells showed. The distance of cellular migration increased obviously with the time to exposure the electric field comparing with the control group. There was no significant difference in the expression of proliferating cell nuclear antigen (PCNA) between two groups. Conclusion External electric fields applying to the rat VSMCs can change the growth characteristic and enhance directional migration of VSMCs. These effects maybe related to the up-regulated expression and re-distribution of some cellular growth factors such as PDGFR, AT1R and AT2R.
6.The Treatment of Superficial Bladder Tumor by Transurethral Resection

Objective To investigate clinical effects of transurethral resection of bladder tumor (TURBT) combined with postoperative perfusion of mitomycin C to urinary bladder on superficial bladder cancer. Methods 32 patients with superficial bladder cancer were treated by TURBT and discontinuous perfusion of mitomycin C (20mg/20ml) to urinary bladder. Results 32 cases of the superficial bladder cancers all were once ablated. 8 cases relapsed in 6 months to 2 years of postoperation, which were treated by TURBT and discontinuous perfusion of mitomycin C to urinary bladder again, and relapse was not found from postoperation to present. Conclusions Treatment of TURBT combined with perfusion of mitomycin C on superficial bladder cancer is repeatable, simple, safe and effective.
7.Genetic Marker of Integron,Transposon in Escherichia coli Isolated from Elderly Patient

OBJECTIVE To investigate the existance of class Ⅰ gene integron and transposon(merA and tnpA) gene marker in Escherichia coli isolated from the elderly patients.METHODS Antibiotic sensitivity was performed by microdilution test.Integron gene IntⅠ1 and transposon marker genes were detected by polymerase chain reaction(PCR).The PCR products were purified and sequenced.RESULTS E.coli resistant to amikacin accounted for 60%,to sulfamethoxazole 95%,and to third-generation cephalosporin 100%(all ESBLs-producing strains).The resistance was related closely to the class Ⅰ integron.The positive rate of integron intⅠ1 was 80%(16/20),whereas the positive rate of gene merA was only 10%(2/20).Gene tnpA was not tested out.CONCLUSIONS The drug resistance of E.coli has close relation with transmission of integron and transposon.The transposons of Tn21/Tn501 are all firstly reported in China.
8.A Clinical Study on Specific Immunotherapy Using Allergens for Children Atopic Dermatitis

Objective To investigate the effects of specific immunotheraphy(SIT) using allergens for children atopic dermatitis(AD) to find an efficacious method to treat AD. Methods 148 children with AD were divided into SIT group and control group. All patients were given allergen intradermal tests. The patients in SIT group received SIT using positive aero-allergens for 8 to 18 months. The patients in control group received the treatment of antihistamine drugs and drugs for external use. Results Total effective rate and curative rate of SIT group were 93.2% and 68.9% respectively, and those of control group were 68.9% and 17.6% respectively, which had a significant difference between the two groups(P
9.A case of Parkes-Weber syndrome accompanied by pseudo-Kaposi's sarcoma

A girl who aged eight years and seven months presented with prunosus patches on the right buttock for 8 years,gradual unilateral enlargement of the right lower limb for more than 7 years,and multiple vegetations for 1 year.Dermatological examination showed nevus flammeus and multiple malodorous vegetations over the right lower limb with high skin temperature.The right lower limb was thicker and longer than the left lower limb.X-ray examination,magnetic resonance imaging and Doppler ultrasound examination revealed high-flow vascular malformations.Pathological examination of the vegetations showed vascular proliferation,fibroblast proliferation and erythrocyte extravasation.She was diagnosed as Parkes-Weber syndrome accompanied by pseudo-Kaposi's sarcoma.
10.Leptospira interrogans induced apoptosis of different host cells and the associated signaling pathways in vitro

Objective To determine the effect of apoptosis in different host cells induced by L. in- terrogans and the associated intracellular signaling pathway. Methods L. interrogans serogroup Icterohaem- orrhagiae serovar icterohaemorrhagiae strain Lai infected cell models in mouse mono-macrophage like cell J774A. 1, human EVC304 cells and A549 cells were established, respectively. Flow cytometry with fluores- cein labeling of FITC-Annexin V/PI was performed to examine the apoptosis or necrosis of the infected cells. Fluorometry as well as Western blot assay was applied to measure the activity of caspase-3, -8, -9 and the expression levels of apoptotic associated protein FADD in the infected cells , respectively . Results 36.70%-63.70% of the L. interrogans strain Lai infected J774A. 1 cells displayed obvious early apoptosis during the infection for 1-6 h, and then altered to later apoptosis / necrosis (53.68%) as the major injury pattern when infected for 12 h. 78.52% of the L. interrogans strain Lai infected A549 cells only showed later apoptosis / necrosis. However, no obvious apoptosis and / or necrosis could be found in the L. interrogans strain Lai infected EVC304 cells. The maximal activities of caspase-3 and -8 in the infected J774A. 1 cells were (1453.41±36.07) and (1402.15± 59.09) FU, respectively, which were the 16.38-fold and 29.99- fold of those the non-infected cells. The caspase-9 activity of the infected J774A. 1 cells slightly increased [(89.42±5.08) FU ], which significantly lower than those of caspase-3 and -8 (P <0.001). The FADD expression level of the infected J774A. 1 cells was gradually increased in an infection time-dependent man- ner. Conclusion There is a distinct diversity of apoptosis in different cells induced by L. interrogans, and FADD→caspase-8→caspase-3 is the major signaling pathway to mediate L. interrogans infection associated cell apoptosis.
