BACKGROUND: Although bone mineral density can diagnose osteoporosis, it cannot reflect the existing bone metabolism of the subjects; bone turnover markers cannot diagnose osteoporosis, but it can timely reflect the current bone turnover rate of the subjects. OBJECTIVE: To summarize osteoporotic fracture risk predicted by bone turnover markers and bone mineral density, and to theoretically analyze the feasibility of fracture healing after operation. METHODS: With bone turnover, biochemical marker, osteoporosis in English for the search terms, Pubmed database from January 1999 to January 2009 was retrieved; With bone turnover, bone metabolism, biochemical markers, bone mineral density, osteoporosis, fractures in Chinese for the search terms, CNKI database from January 1999 to January 2009 was searched. Literatures were limited to English and Chinese languages. Inclusive criteria: the article closely related to predict fracture risk with bone turnover markers and bone mineral density; Exclusive criteria: duplicated documents. RESULTS AND CONCLUSION: Totally 631 literatures were screened out by computer, according to inclusion and exclusion criteria, 31 documents of which were involved for analysis. Osteoporosis is one of the main causes of elderly disease and death. Early prediction of osteoporotic fracture risk has a great significance. This article describes the bone metabolism, the type of bone biochemical markers, bone mineral density measurement, multi-cell basic unit and the OPG-RANKL-RANK system in bone reconstruction after a long-term fixation; focuses on the use of bone turnover markers and bone mineral density to prognosticate the fracture risk in osteoporosis or after long-term fixation by domestic and foreign academician, and theoretically analyze the feasibility of predicting postoperative fracture healing.

Objective To evaluate the effect of high volume hemofiltration (HVHF) treatment on inflammatory cytokine clearance and hemodynamics in children with sepsis. Methods Thirty-four cases of septic children were randomly divided into two groups:the control group(17 cases) was administrated with the routine therapy, while the observation group(17 cases) was underwent with HVHF treatment on the basis of routine therapy. The inflammatory cytokines and hemodynamics were analyzed before and after the treatment. Results (1) Before treatment, the levels of tumor necrosis factor (TNF)-α in the control group and observation group were (103.6 ±26.1) ng/L and (106.6 ±31.6) ng/L,and the levels of interleukin(IL)-6 were (772.8 ± 92.4) ng/L and (761.3 ± 89.2) ng/L,respectively.The concentration of TNF-α and IL-6 were markedly reduced in both two groups after treatment (P < 0.05),with TNF-α and IL-6 reducing to (85.6±23.7) ng/L,(63.6 ±26.4) ng/L and(109.7 ±38.5) ng/L,(78.1 ±24.7) ng/L,respectively.Most importantly, the reduction in the observation group were significantly higher than that in the control group(P < 0.05).(2) Before treatment,no significant differences between the observation group and the control group were observed in hemodynamics, including mean arterial pressure, saturation oxygen, and heart rate(P > 0.05),whereas after the treatment, mean arterial pressure and saturation oxygen in the observation group were significantly elevated compared with those of the control group [(100.4 ± 12.1) mmHg vs.(82.2±11.7) mmHg(l mmHg=0.133kPa),(94.2±5.3)% vs.(91.4±3.2)%,P<0.05],andheart rate in the observation group[(85.1 ± 17.3) times/min] was significantly lower than that of the control group [(98.7 ± 18.5) times/min] (P < 0.05).Conclusion HVHF treatment on septic children efficiently promotes the clearance of inflammatory cytokines and improves hemodynamic property, and thereby leads to protection of organ function and the better prognosis for septic children.

Many clinical bone nonunion or delayed union often appear in the early stage of bone healing, but they have been always found too lately. Before X-ray films of visual changes indicates salt content inbones reaching 25%,the very early changes ofclinical significance can not be observed. This paper introduces the theory of fracture healing and the definition of bone nonunionand the significance of the basic multicellular unit(BMU)and the OPG-RANKL-RANK system in bone reconstruction. Many technologies such as ultrasonic testing, vibration analysis, mechanical impedance analysis, the bone marrow cavity imaging technologiy,high-resolution CT etc.have been reported in study on the bone,as well as early diagnosis of fracture nonunion. However, the molecular level of early diagnosis is deficiency.

Objective To investigate the effect of Ulinstatin(UTI) on oxygenation and gastric intramucosal pH in dogs with sepsis. Methods Sepsis was induced by intravenous infusion of lipopolysaccharide of E.coli.055:B5 to dogs, and the twenty dogs were divided into control group and ulinastatin group.Ulinastatin was administered in the ulinastatin group. The oxygen delivery(DO2),oxygen consumption(VO2), oxygen extraction(O2ER), plasma lactate levels and gastric intramucosal pH(pHi) were monitored. Results In early sepsis dogs,DO2,VO2,O2ER and plasma lactate levels increased significantly. Gastric intramucosal pH(pHi) decreased significantly. After the treatment with ulinastatin,DO2,VO2,O2ER and plasma lactate levels decreasd(P

[Objective]To explore the indications and operative techniques for comminuted fracture of proximal humerus. [Method] Twelve patients with fracture of proximal humerus were chosen, observation was made after locking proximal humerus plate fixation with anterior acromioplasty. [Result]The average follow-up period were 12 months (6~24months). All patients' fracture in surgery group had been union and all shoulder had good functions. No incidence of avascular necrosis and nonunion was found. [Conclusion]Rigid fixation of displaced comminuted fractures of proximal humerus with anterior acromioplasty is a simple and effective technique to prevent subacromial impingement syndrome after surgery. Precise osteotomy and early postoperative exercises are the key to success.

Objective To establish a real-time quantitative PCR assay for the identification of Campylobacter jejuni in fecal samples. Methods Specific primers of the PCR were designed according to the conserved sequences of Campylobacterjeju-ni,and the real-time quantitative PCR assay was established.150 cases of fecal samples were tested by both culture and PCR methods. With the culture testing results as the reference standard, the sensitivity, specificity, accuracy and repetition of the real-time quantitative PCR were validated.Kappa test was used to estimate the difference between the two detection meth-ods.Results The standard carve of the real-time quantitative PCR assay fitted the equationY=-3.51Log(X)+37.09 (R2=0.996)well.The sensitivity, specificity, and accuracy of the established method were 92.4%,95.8% and 94%,respective-ly.The theoretical detection limit of the PCR method was 102 CFU/ml,and its reproducibility was good (CV<5%).Statisti-cal analysis demonstrated that the results of the two methods were consistent, and the consistent strength was very strong (Kappa=0.88,P<0.05).Conclusion The established real-time PCR method can assay the Campylobacterjejuni in human fecal samples rapidly and accurately.
