1.Association between Mycoplasma penetrans infection and clinicopathology of IgA nephropathy

Objective To study the association between Mycoplasma penetrans (Mpe) infection and clinicopathology of IgA nephropathy (IgAN). Methods Blood samples of 118 IgAN patients, 90 patients with chronic kidney disease (CKD) and 89 healthy people were collected. Mpe DNA in serum was detected by PCR and positive samples were confirmed by Southern blotting. According to Mpe infection, IgAN patients were divided into positive and negative groups. Association between clinicopatholgical features of IgAN and Mpe infection was examined.Results Significantly higher Mpe positive rate was found in IgAN group as compared to CKD and healthy groups (16.0% vs 2.2% and 1.1%, P<0.01). In Mpe positive group, 42.1% patients presented macroscopic hematuria, which was significantly higher than that in Mpe negative group (P<0.01). While Mpe negative group had greater proteinuria, higher serum creatinine level, higher Lee grading of pathology compared to Mpe positive group. There were no differences of tubulointerstitial lesions and arteriole hypertrophy between two groups. Conclusions IgAN patients have higher Mpe infection rate than CKD patients and healthy people. Mpe positive IgAN patients have more macroscopic hematuria. Mpe infection may be associated with the pathogenesis of IgAN.
2.Effects of Bushen Jiedu Formula combined with auricular pressure by coxherb seeds on cancer-induced pain and quality of life in patients with advanced colorectal cancer

Objective To explore the effects of Bushen Jiedu Formula combined with auricular pressure by coxherb seeds on cancer pain and quality of life in patients with advanced colorectal canc-er.Methods A total of 120 patients with advanced colorectal cancer admitted to our hospital from June 2019 to May 2023 were included in the study,and were randomly divided into two groups.The control group(60 cases)received routine nursing care and the"three-step"analgesic method to in-tervene pain,while the observation group(60 cases)received treatment with Bushen Jiedu Formula combined with auricular pressure by coxherb seeds on the basis of the control group.Both groups were treated for one month.The Numerical Rating Scale(NRS)score,the therapeutic effect,the quality of life(QOL)score and the occurrence of adverse reactions were compared between the two groups before and after treatment,and the levels of substance P(SP),β-endorphin(β-EP)and prostaglan-din E2(PGE2)of the two groups before and after treatment in serum were detected.Results After treatment,the NRS score and the levels of SP,β-EP and PGE2 in the observation group were(2.23±0.64),(11.51±2.64)ng/mL,(78.64±9.46)ng/L and(41.90±6.32)ng/L,re-spectively,which were lower than(2.87±1.25),(15.64±2.42)ng/mL,(101.29±11.26)ng/L and(78.63±10.17)ng/L,respectively of the control group(P<0.05).The total effective rate and QOL score of the observation group were 90.00%and(51.92±5.66),which were higher than 75.00%and(43.58±5.12),respectively of the control group(P<0.05).There was no signifi-cant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclu-sion The combination of Bushen Jiedu Formula and auricular pressure by coxherb seeds can obvi-ously improve the cancer-induced pain of patients with advanced colorectal cancer,improve their quality of life,and have good safety.

4.Effect of surgery combined with aminolevulinic acid-based photodynamic therapy on the recurrence of extramammary Paget's disease

Objective To evaluate the effect of surgery combined with aminolevulinic acid (ALA)-based photodynamic therapy (PDT) on the prognosis of extramammary Paget's disease.Methods A prospective open-labelled controlled trial was conducted.A total of 38 patients with pathologically comfirmed extramammary Paget's disease were enrolled from Shanghai Dermatology Hospital,and divided into 2 groups to be treated with surgery alone (surgery alone group,n =21) or surgery combined with ALA-PDT (combination group,n =17).Patients in the combination group received ALA-PDT after the surgery once every two weeks for 3 sessions.All the patients were followed up once every three months for more than 12 months,and the incidence of relapse was evaluated and compared between the 2 groups after the treatment.Results During the follow-up of 12-58 months (mean,35.45 ± 16.98 months),7 patients in the surgery alone group experienced relapse,and the median time to relapse was 9 months,with an upper quartile of 18 months and a lower quartile of 6 months.However,relapse only occurred in 1 patient in the combination group,and the time to relapse was 18 months after the end of treatment.The recurrence rate was significantly lower in the combination group than in the surgery alone group (P < 0.05),and the time to relapse was also longer in the combination group than in the surgery alone group.Furthermore,ALA-PDT after the surgery was well tolerated in all the patients.Conclusion Surgery combined with ALA-PDT can reduce the recurrence rate of extramammary Paget's disease,and improve its prognosis.
