1.Posterior approach to radical resection for thoracic tumor and spine stabilization and reconstruc-tion in one stage

Objective To discuss total vertebrectomy and spine stability and reconstruction by pos-terior ap proach for thoracic vertebra tumor and pedicle screw system fixation and intervertebral fusion.Methods Eighteen patients of 3males and15females of thoracic total vertebral tumor were operated in this group.The age of the group were from14to58years old,with the average of 23years.There were4aneurysmal bone cysts,2hemangiomas,2osteoblastomas,1neurilemomas,5giant cell tumor,1solitary myeloma and3metastatic thoracic vertebra tumors.The locations of the tumors was T 4 in1case,T 5 in1case,T 6 in2cases,T 8 in4cases,T 9 in3cases,T 10 in4cases,T 11 in2cases and T 12 in1case respectively.Pre-operative Frankel classification was grade A in6cases,B in7cases,C in3cases and E in2cases.Cir-cumferential decompression was performed with total spondylectomy through one-stage posterior ap proach.Anterior verte bral reconstruction was provided by autograft(ribs or ilium)bone or metallic cage;Posterior reconstruction was achieved by pedi cle screws system of CD or TSRH,Scofix fixation and auto graft fusion.Results The mean follow-up was8months,ranging from3months to24months.Twelve of 16cases with neu ro logical dysfunction were recovered completely,1case recovered from Frankel grade A to C,2cases from grade A to D,1case from grade B to D.The patient with neurilemmoma recurred and became ma lig nant after20months of operation and died4months later.One case of metastatic tumor of lung cancer sur vived6months after opera tion.One patient with giant cell tumor recurred.The other patients were still alive in the latest follow-up.There were no instruments breakage and spinal instability during the follow-up.Conclusion If the thoracic total vertebral tumor compress spinal cord se riously,the patients should be indi cat-ed for surgical treatment constructively.Total vertebrectomy followed by one-stage posterior approach re con-struction could relieve the com pression caused by tumor and reconstruct the stabilization of spine effectively.
2.The diagnosis of primary small intestinal lymphoma by CT

Objective To evaluate the value of CT in diagnosing primary small intestinal lymphoma. Methods CT features of 11 cases of primary small intestinal non Hodgkin′s lymphomas were retrospectively analyzed. Results The most frequent location of the diseases were at ileum (10/11). According to the main CT features from all 11 cases, it could be classified into two types: bowel wall thickening type ( n =7) and luminal polypoid mass type ( n =4). 5 of 7 cases in the first type showed aneurysmal sign, while 3 of 4 cases in the second type were combined with intussusception. 7 cases from the two types were found to have mesenteric lymph nodes enlargement, among them 1 case showed “sandwich” sign, 4 showed long range or multicentric intestinal lesions. The tumor detection rate was 81.8%, and the correct diagnosis rate was 63.6% by CT. Conclusion Bowel wall thickening and luminal polypoid mass were two different types of CT features in the primary small intestinal lymphoma, and polymorphic lesions accompanied by multifocal mesenteric lymph nodes enlargement, aneurysmal sign, “sandwich” sign, long range or multicentric intestine lesions were the main characteristic CT features, which may lead to the correct diagnosis by CT.

Objective: To discuss the feasibility of a new method to evaluate wearing simulators,and to investigate the feasibility of specimens' depth loss as the index of the materials' wearing resistance.Methods: Two commercial composite resins(SureFil and Spectrum TPH) were selected.The specimens were subjected to 100,200,300,400,500,800,1200,1 600 and 2 000 cycles wearing in the CW3-1 wearing system.Wearing was determined quantitatively by weighing the specimen and measuring the height of the specimen,volume loss(mm 3) and height loss(?m) were calculated.Previous clinical study results on SureFil and Spectrum TPH conducted at Hong Kong University were used to examine the relationship between clinical wearing and laboratory wearing.The difference of the wearing resistance between the two materials and the Pearson correlation of the height loss and the volume loss were analyzed by statistical software SPSS 15.0.Results: The wearing resistance of the two materials was significant difference(P

Objective To explore the relationship between the expression of LIGHT/HVEM in patients after allo-HSCT and the development of GVHD. Methods The expression of LIGHT and HVEM in T lymphocytes was detected by FACS Calibur. Results All patients achieved engraftment and hematopoiesis reconstitution, aGVHD occurred in 9 of 26 patients (34.6%) with grade Ⅲ-Ⅳ aGVHD in 3 cases. Seven cases developed cGVHD (26.9%). LIGHT was not expressed in T lymphocytes from healthy donors and patients without GVHD, while constitutive expression of HVEM was detected. When aGVHD occurred, the expression levels of LIGHT in T lymphocytes were significantly increased, while those of HVEM decreased. After GVHD was controlled, the expression levels of those co-stimulators went back to normal On the day 15 after transplantation, the expression of LIGHT in T lymphocytes of patients with aGVHD later was significantly higher, while that of HVEM lower than in those without aGVHD. The patients with aGVHD of grades Ⅲ-Ⅳ demonstrated higher LIGHT expression than in those with aGVHD of grades Ⅰ-Ⅱ. Conclusion The expression of LIGHT and HVEM might be involved in the development of GVHD after allo-HSCT and could be used as one of the useful indicators in predicting aGVHD.
6.Antioxidation Effect of Ginkgo Biloba Extract on Motor Function after Spinal Cord Injury in Rats

Objective To observe the antioxidative effect of Ginkgo biloba extract (EGb) on motor function after spinal cord injury (SCI) in rats. Methods 36 healthy adult Sprague-Dawley rats were divided randomly into EGb group (group A), methylprednisolone (MP) group (group B) and control group (group C) with 12 rats in each group. SCI model was made by Allen's mode (10 g×25 mm) at the level of the 9th thoracic vertebra (T9). Group A was given 40 mg/kg EGb daily by intraperitoneal injection, while group B was given 30 mg/kg MP at the first injection and then 5.4 mg/kg every 6 hours for 4 times. Group C received an equal volume of normal saline daily. On the 1st, 3rd and 7th days after SCI, 4 rats were selected randomly from each group. The motor function were assessed by Basso-Beattie-Bresnahan scale (BBB). The activity of superoxide dismutase (SOD) and the content of malondialdehyde (MDA) in serum were determined. Results At every time point after surgery, the BBB scores were significantly higher in groups A and B than in group C (P<0.05). Compared to group C,the activity of SOD increased and the content of MDA decreased significantly in groups A and B on the 3rd and 7th days after SCI (P<0.05).Conclusion EGb may reduce the level of oxidative stress and promote the recovery of the motor function in rats after SCI.
7.Information for first-visit psychology outpatients in general hospital

This study was to analyze 1400 first-visit psychology outpatients from 8 May 2004 to 30 June 2007 in Zhangzhou Hospital,Fujian Medical University.Our results showed that of all the participants,the following patients were more commonly seen:female,teenagers or mid-age less than 50,and lower education level.Regarding to mental diseases,hysteria,serious stress disorders or adjustment disorder,and neurosis were the leading diagnosis,followed by mood disorder,schizophrenia or other psychosis disorders,and apparatus psychosis disorders.
8.Somatic symptoms in patients with depressive disorder :an analysis of 736 cases

The somatic symptoms of patients with depressive disorder in a general hospital were retrospectively analyzed.From May 2004 to April 2009,736 patients in Psychological Clinic were diagnosed as depressive disorder,among them 723(98.2%)had somatic complains.The somatic symptoms were complex and involving several systems,nervous system,digestive system and cardiovascular system were the most common.It is necessary to recognize the typical somatic symptoms of depressive disorders to avoid misdiagnosis.
9.Bone graft fusion in the treatment of two-level contiguous cervical disc herniation:titanium mesh versus interbody fusion cage

BACKGROUND: Many studies have shown that different types of anterior cervical surgery in the treatment of two-level contiguous cervical disc herniation can obtain satisfactory results, but which method is the best has not yet reached a consensus. OBJECTIVE: To compare the efficacy and safety of three types of anterior cervical surgery for treating two-level contiguous cervical disc herniation. METHODS: We retrospectively analyzed clinical data of 62 patients with two-level contiguous cervical disc herniation who underwent anterior decompression and fusion. These patients were assigned to three groups. Bone graft group received anterior cervical discectomy with autogenous iliac bone graft fusion. Titanium mesh group received anterior cervical corpectomy with titanium mesh fusion. Cage group received anterior cervical discectomy with cage fusion. Fusion rate of bone graft and improvement of neurological function (Japanese Orthopaedic Association Scores) were evaluated and compared after treatment in the three groups. Cervical vertebra anteroposterior and lateral images were used to measure height of anterior and posterior margin of vertebral body and Cobb angle changes of fusion segment. RESULTS AND CONCLUSION: Al 62 patients were fol owed up and the fol ow-up time was ranged from 8 to 30 months. Operation time was significantly longer in the bone graft group than in the titanium mesh and Cage groups (P < 0.05). Intraoperative blood loss was larger in the bone graft group than in the titanium mesh and Cage groups (P < 0.05). Japanese Orthopaedic Association Scores were significantly improved after treatment (P < 0.05). No significant difference was found at different time points (P > 0.05). The fusion rate of bone graft was higher in the bone graft group than in the titanium mesh and Cage groups (P < 0.05) at 3 months after treatment, and bone union was found in the final fol ow-up. Height of anterior and posterior margin of vertebral body was significantly increased after treatment (P < 0.05). No significant difference in the increase of the height of anterior margin was detected among the three groups (P > 0.05). The increase in the height of posterior margin was higher in the Cage group than in the bone graft group and titanium mesh group (P < 0.05). Vertebral height loss of the anterior margin was higher in the bone graft group than in the titanium mesh and Cage groups, but vertebral height loss of the posterior margin was highest in the bone graft group, fol owed by titanium mesh group and Cage group at 3 months after treatment (P < 0.05). The increase value of Cobb angle was higher in the titanium mesh group than in the bone graft group and Cage group (P < 0.05). At 3 months after treatment, the altered value of Cobb angle was highest in the bone graft group, fol owed by titanium mesh group and Cage group (P < 0.05). No significant difference in the height of anterior and posterior margins of the vertebral body and Cobb angle was detectable between final fol ow-up and 3 months post-treatment (P > 0.05). These findings indicated that the three kinds of fusion method for treating two-level contiguous cervical disc herniation could obviously lessen nervous systems and improve cervical vertebra functions. In the bone graft group, operation time was long. Intraoperative blood loss was more. Postoperative height loss of the posterior margin of the vertebral body was visible. Cervical lordosis could be easily induced. Compared with the bone graft group, titanium mesh and Cage groups could better maintain the height and curvature of the cervical vertebra, but the Cage group had apparent advantages.

A study on the reform of the evaluation method for college students' innovative experiment was carried out on account of the major problems existed in the present evaluation system.A set of evaluation indicators emphasizing process evaluation was constructed based on the theory of developmental evaluation.Also the performance appraisal methods for all indicators were determined.Practice suggested that the evaluation method based on developmental evaluation was able to promote the ability of scientific research and innovation of college students.
