BACKGROUND: There is still no satisfactory therapy for thetreatment of cerebral infarction at present. Although sub-hypothermia is effective in the treatment of cerebral infarction, its side effect is relatively more. Local sub-hypothermia might have favorable effects in the treatment of cerebral infarction.OBJECTIVE: To observe the protective effects of local sub-hypothermia on the ischemic brain tissues in rats to further explore its mechanism.DESIGN: A randomized controlled basic study based on the experimental animals.SETTING: Institute of neurology of a university hospital.MATERIALS: The study was conducted in the Animal Laboratory of Neurology, the First College for Clinical Medicine, Harbin Medical University between April 2000 and January 2002. Fifty male Wistar rats(cleanness grade) with a bodyweight of(250 ± 25) g were selected.INTERVENTIONS: Ten rats were randomly selected from the 50 rats and divided into normal group and sham-operation group with 5 rats each. The rest 40 rats were randomly divided into normal temperature cerebral ischemia group and local hypothermia cerebral ischemia group with 20 rats each. Rat cerebral middle-sized arterial ischemia model was established for local sub-hypothermia disposal.MAIN OUTCOME MEASURES: Impacts on cerebral infarction volume, nerve function, and neuron-specific enolase(NSE) of rats in each group.RESULTS: The cerebral infarction volume at 48 hours after embolism in rats was ( 128.95 ± 13.42) or (84.90 ± 11.36) mm3 respectively, nerve function evaluation was( 1.60 ± 0. 24) or (0. 95 ± 0. 17), and serous NSE concentration was(13.55±4.07) or(9.19±3.42) μg/L in either sub-hypothermia group or normal temperature group, which were significantly different from each other.CONCLUSION: Local sub-hypothermia therapy has protective effects on ischemic cerebral neurons.

A 57-year-old female patient was admitted to our hospital because of sustained hyponatremia within stress dose of hydrocortisone replacement after transsphenoidal partial pituitary tumor surgery. Multiple clinical parameters investigation for 2 weeks was shown increasing serum Na+and uric acid level and decreasing blood pressure level under water restriction about 500 ml/d, and even without hydrocortisone replacement, the serum Na+ still maintain normal level. After surgeries and radiotherapy, the tumor was undetectable and serum Na+ level was within the normal range without any water restriction.

Objective To study the relationship between MRI features and pathological characteristics of the renal clear cell carcinoma, and to conduct a comparative analysis. Methods 23 patients renal clear cell carcinoma who were proved by postoperative pathology were retrospectively analyzed. The preoperative MRI findings and the postoperative pathological characteristics were compared. Results MRI plain scan showed uniform signal was seen in 2 cases, and mixed signal was found in 21 cases with cystic and necrosis. Blood was seen in 11 cases and false envelope was present in 12 cases. Dynamic enhanced scanning revealed that solid part was enhanced obviously, it was lower than the renal cortex but higher than the renal medulla in cortical phase and continued to be enhanced in medullary phase and delay phase.Postoperative pathological characteristics showed that tumor profiles were yellow-white and cystic and necrotic structures were present within tumors, blood was found in 17 cases. Endoscopic tumor cells were present in solid nests or acinar structure, and mesenchyma was rich in thin-walled vessels involved in reticular interval.14 cases were found to have false envelope. Conclusion Renal clear carcinoma MRI imaging manifestion is related to organixational srructure. MR plain scan combined with dynamic enhanced scan has very high value in the diangnodis of renal cell carcoinoma.

Objective To explore the optimal exercise way of improving the risk indicators of electrocardiogram (ECG) during exercise. Methods Twenty-four healthy male subjects aged between 40 and 49 years old were divided randomly into a control group (n=9),a low amount exercise group of 1200 kcal/wk (n=7) and a high amount exercise group of 2000 kcal/wk (n=8).The intensity of the exercise groups was 65%~ 80%VO2max training in the standard 400 m track for 12 weeks. The accelerometer and polar watch were worn to monitor the exercise process.During the exercise, the exercise amount was controlled using the distance and duration while the intensity was controlled using target heart rate. The subjects took part in the graded exercise test before and after the exercise intervention.Twelve lead electrocardiograms were used to measure the indicators of ST segment depression and corrected QT interval dispersion (QTcd) during the exercise. Results The exercise intervention did not result in significant change in ST segment depression, while the QTcd indicator in both the exercise groups decreased significantly after the intervention (P<0.05).Moreover,the decrease of QTcd indicator in the low amount exercise group was significantly higher than that of the control group (P<0.05),while the decrease of QTcd indicator in high volume group was higher than that of the control group (P<0.01).No significant differences were found in the ST segment depression among the 3 different groups. Conclusion The Twelve-weeks exercise interventions with intensity of 65% ~ 80% VO2max and exercise amount of 1200 kcal/wk and 2000 kcal/wk have no effect on the ST segment depression during the exercise. However, they can result in significant decrease in the QTcd of ECG during exercise, reducing the risk of exercise-induced myocardial ischemia.
