1.A Study on Surgical Treatment of Early-stage Breast Cancer with Breast-conserving Operation

Objective To explore the effect of breast-conserving operation for the treatment of early-stage breast cancer. Methods From November 1999 to Octorber 2001, the patients with early-stage breast cancer were treated with breast-conserving surgery followed by radical irradiation according to the patients' will. The cosmetic results and quality of life were analyzed after surgery and radiation. Results In all the patients received operation treatment from 1999 to 2001, eighteen patients were treated with breast-conserving surgery with breast conservation rate of 10.2%, of which 7 cases with quadrantetomy plus axillary lymph node dissection (ALND), 8 cases with lumpectomy plus ALND and 3 cases with lumpectomy. Pathological staging was as follow: 4 cases in stage 0, 12 cases in stage Ⅰ, and 2 cases in stage Ⅱ. All cases were followed-up. The cosmetic results were good in 94%(17/18) and fair in 6%(1/18). All the patients had not local recurrence and distant metastasis. Conclusions Breast-conserving surgery followed by radiation therapy is superior to mastectomy in terms of improving breast cosmesis and quality of life. Postoperative radiation therapy does not affect the cosmetic result.
2.Pancreaticoduodenectomy for the surgical treatment of advanced gastric carcinoma

Purpose:To investigate the choice of surgical therapy for the pancreaticoduodenal region invaded by or involved in stomach carcinoma. Methods:51 patients from Jan. 1981 to Dec. 2000 with stomach carcinoma invaded by or involved in the pancreaticoduodenal region were analyzed retrospectively. Results:All patients were primary stomach carcinoma, the operative methods of stomach carcinoma included subtotal gastrectomy (Group SG,n=29), radical Pancreaticoduodenectomy (Group PD,n=11),gastrojejunostomy (n=8) and tumour’s biopsy (n=3). In group PD, one-,and three-year survival rates was 63.6% and 36.4%, in group SG, it was 17.2% and 3.4%. The survival rate of group PD was markedly better than that of group SG respectively (P

Objective To summarize and analyze characteristics and diagnostic value of ultrasonography in the diagnosis of 46 cases of gynecomastia sonographic image.Methods 46 cases of male breast lumps were tested by high frequency ultrasound probe The mammary glands position,thickness,border,shape,internal echo,the internal flow signals,with or without axillary lymph nodes were observed.Results There were 32 cases of unilateral disease, including 18 cases of left side,14 cases of right side,14 cases of bilateral disease.Sonographic indicated 28 cases of mammary gland tissue echo to see beneath the nipple with low echo,the boundary more clearly,more uniform internal echo;and 12 cases showed glandular tissue thickening,echogenic and showed cord -like,similar to the female gland hyperplasia sonogram;and 6 cases showed patchy irregularly echogenic area.Color Doppler performance was that the 33 cases showed glandular tissue without significant blood flow signals,10 cases showing 1 -2 strip -shaped blood flow to see in the glandular tissue,3 cases showing more abundant flow signals.Conclusion Ultrasonography has no pain,with higher diagnostic yield reproducible operation,so gynecomastia is the preferred method.
4.Study on the linkage between the interluekin 4 receptor ? chain polymorphism and susceptibility of asthma and atopy

Objective To investigate the linkage between the mutation of IL 4R ? chain gene and susceptibility of asthma and atopy in Wuhan. Methods Thirty five samples from asthmatic children,twenty five samples from adult patients with asthma, twenty samples from health adult and children respectively, were analyzed. A coding region 1205 1928 of IL 4R ? chain gene was detected by reverse transcription polymerase chain reaction/Single Strand conformation polymorphism/Sequence. Result 57.8% cases from asthmatic children, one polymorphism (1902 A→G), were found exist in the IL 4R ? chain gene and only 8% cases from asthmatic adults and 5% cases from control have this mutation. Conclusion The SNP of IL 4R ? chain coding area 1902 correlates with susceptibility of asthma in Wuhan asthmatic children (? 2=12.54, P 0.05, ?=0.05). There may be another mutations in IL 4R ? chain coding area except 1902 or another changes in other gene related with allergy.
5.Rebleeding after stenting and coil embolization of an aneurysm in anterior wall of the internal carotid artery

Objective To report a case of early rebleeding of an arterial wall wide-necked aneurysm of the internal carotid artery with stenting and coil embolization. Methods Balloon-expandable stent was deployed across the aneurismal neck through endovascular approach. Microcatheter was superselectively introduced into the aneurismal sac for coil packing. Results The procedure was technically successful with stent completely covering the neck and 90% occlusion by coil packing of the aneurysm; simultaneously keeping the patency of parent artery and adjacent perforating arteries. The patient recovered well, but died for rerupture and rebleeding of the treated aneurysm on the 16th day after the procedure. Conclusions Endovascular stenting combined with coil embolization is an alternative method for treating anterior wall wide-necked aneurysm of the internal carotid artery, but attention and recognition of the risk of early rebleeding of the treated aneurysms should always be kept on mind therefore short-term follow-up and prompt treatment are critically necessary.
6.Comparison of ultrasonography and magnetic resonance imaging in diagnosis of breast cancer

Objective To compare the clinical diagnostic value of ultrasound and magnetic resonance imaging (MRI) in breast cancer.Methods Ultrasound and MRI were performed in patients with breast disease.The postoperative pathological examination results were seemed as diagnostic criteria.The diagnostic accuracy was analyzed in two kinds of imaging examination.Results The diagnostic accuracy of MRI in breast cancer(86.9%) was significantly higher than the accuracy of ultrasonic diagnosis(39.3%),the sensitivity and specificity of MRI were significantly higher than those in ultrasonic diagnosis,the rate of missed diagnosis and misdiagnosis rate of MRI were lower than those of ultrasonic diagnosis,the differences were statistically significant (x2 =6.583,12.573,4.892,23.175,8.649,all P < 0.05).Conclusion Ultrasound and MRI have high ability in diagnosis of breast cancer.MRI diagnostic efficacy has more advantages,but the ultrasound is convenient to operate and repeate inspection,can be used in screening.

AIM: To study the effects of dexamethasone (DXM) on intracellular expression of TH1/TH2 cytokines and the mechanism of that during the development of asthma. METHODS: Eighteen BALB/c mice were divided into 3 groups at random: control group, asthma group, and DXM treated group, with 6 mice in each. The expressions of T-box expressed in T cells (T-bet) and GATA-binding protein-3 (GATA-3) in lung tissue were detected by Western blotting. In order to investigate whether Arg110Gln polymorphism in the coding region of the IL-13 gene is associated with asthma and total plasma IgE level in Han nationality in Hubei Chinese population, the allele frequency of 4257(g/a) site and Arg110Gln genotype of IL-13 was detected by using restriction fragment length polymorphism in Han nationality in Hubei Chinese population including 43 asthmatic children, 45 asthmatic adults, 31 control children and 46 control adults. Total plasma IgE was measured by Chemiluminescence assay. The results showed that the frequency of allele A at 4257 bp of IL-13 in children and adults was 0.39 and 0.32, respectively. The GlnGln form of Arg110Gln polymorphism of IL-13 gene was associated with susceptibility of asthma and elevated total plasma IgE in children (P=0.030 and 0.0009, respectively), but not with them in adults (P=0.219 and 0.174, respectively). Our results suggest that the Arg110Gln polymorphism of IL-13 gene is associated with susceptibility of asthma and elevated total plasma IgE in Chinese children of Han nationality in Hubei, but not with them in adults.

10.Survival and late effects of comprehensive treatment on children with neuroblastoma

Objective To investigate the organ function survival and late effects and secondary malignancy of childhood neuroblastoma (NB) with high intensity comprehensive treatment.Methods A total of 23 children with NB who received comprehensive treatment in Shanghai Children's Medical Center from December 1998 to October 2010 were enrolled.All tests were formulated according to the Children's Oncology Group (COG)'s guidelines with the approval of parents.The late side effects were graded by CTCAE v3.0,and hearing loss related to platinum was graded by Brock and Chang.Results The median follow-up was 96 (65-170) months.All the patients had at least one late side effect.The occurrence rate of 1-2 adverse effects was 100%,and 17.4% (4/23 cases) of patients had grade 3-4 adverse effects.90.5% of children (19/21 cases)developed hearing loss on both sides.Eleven children (52.4%,11/21 cases) developed dental abnormities,comprising microdontia,missing teeth,root stunting and enamel hypoplasia.9.5% of children (2/12 cases) had scoliosis.42.8% of children (9/12 cases) developed hypogonadism with the approval of parents or delayed growth.47.4% of children (9/19 cases) had abnormal respiratory function.36.8% of children (7/19 cases)had abnormal cardiac function.33.4% of children (7/21 cases)developed renal damage.23.8 % of children (5/21 cases) had abnormal liver function.66.7% of children (14/21 cases) had low level of cortisol,but adrenocorticotrophic hormone was normal.All children had normal function of thyroid.Three children developed secondary malignancy:leukemia,malignant fibroma and liver tumor.Conclusions The incidence of long-term adverse reactions of patients with neuroblastoma treated with high intensity comprehensive treatment is very high and the patients may have adverse effects,like hearing loss,dental abnormality,cardiopulmonary function abnormality,musculoskeletal problems and secondary malignancy.The upmost importance is to establish long-term follow-up program to detect the life quality of children and amend current therapeutic schemes improve life quality.
