1.Intravesical instillation of epirubicin combined with BCG to prevent the recurrence of bladder cancer after surgical management

Objective To evaluate intravesical instillation of epirubicin plus BCG to prevent recurrence of transitional cell cancer of bladder after surgical management. Methods A total of 44 cases of bladder cancer who underwent TURBT or partial cystectomy were divided into 2 groups.①Group 1 received intravesical instillation of epirubicin plus BCG (24 cases).Within 1 week following operation single dose of intravesical epirubicin instillation was given and from the second week regular intravesical BCG was instilled afterwards.②Group 2 received BCG alone (20 cases).Within 1 week after surgical management regular intravesical BCG instillation was given. All the patients were followed up for 24 months. Objective To evaluate the expression of endothelial Per-ARNT-Sim domain protein-1(EPAS-1)/hypoxia-inducible factor-2?(HIF-2?) and its relationship with vascular endothelial growth factor (VEGF) in bladder transitional cell carcinoma (BTCC). Methods The expression of EPAS-1/HIF-2? and VEGF by immunohistochemical staining was examined in 60 cases of BTCC (Grade Ⅰ in 28 cases,Grade Ⅱ in 12,Grade Ⅲ in 20) and 8 subjects with normal urothelium. Of the 60 cases 29 had superficial (≤pT 1) bladder cancer and 31 had invasive (≥pT 2 bladder cancer).?2 test was used to assess the relationship between EPAS-1/HIF-2? and VEGF expression versus tumor grade and stage. Results The expressions of EPAS-1/HIF-2? and VEGF were negative in all normal bladder tissue but high in BTCC.Of the 60 cases,34 (56.6%) were positive and 26 ( 43.4%) were negative for EPAS1/HIF-2?.Four cases (14.3%) of Grade Ⅰ,11 (91.7%) of Grade Ⅱ and 19 (95.0%) of Grade Ⅲ were positive for EPAS-1/HIF-2?. Objective:To investigate the effects of washed platelets at low concentration(WPLT)on the proliferation of human dental pulp cells(HDPCs).Methods:WPLT was prepared from a male venous blood,cultured HDPCs were exposed to the WPLT at 50 ml/L,100 ml/L and 200 ml/L for 0.5~24 h respectively.Cell proliferation and prostaglandin E_2 production were examined by cell counting and Prostaglandin E_2 EIA Kit-Monoclonal respectively.Cyclooxygenase-2(COX-2)mRNA expression was determined by real time RT-PCR.Results:WPLT at 50 ml/L promoted HDPC proliferation and increased COX-2 mRNA expression at 3-hour-exposure.That at 50~200 ml/L increased the production of PGE_2 dose-depandently.Indomethacin or dexamethasone reduced WPLT-induced HDPC proliferation,and this inhibitory effect was reversed by exogenous PGE_2.PGE_2 promoted HDPC proliferation at 5-100 ng/ml.Conclusion:WPLT may induce prostaglandin E_2 production,thereby promote HPDC proliferation.
4.Diagnostic value of ultrasonography in gynecomastia

Objective To summarize and analyze characteristics and diagnostic value of ultrasonography in the diagnosis of 46 cases of gynecomastia sonographic image.Methods 46 cases of male breast lumps were tested by high frequency ultrasound probe The mammary glands position,thickness,border,shape,internal echo,the internal flow signals,with or without axillary lymph nodes were observed.Results There were 32 cases of unilateral disease, including 18 cases of left side,14 cases of right side,14 cases of bilateral disease.Sonographic indicated 28 cases of mammary gland tissue echo to see beneath the nipple with low echo,the boundary more clearly,more uniform internal echo;and 12 cases showed glandular tissue thickening,echogenic and showed cord -like,similar to the female gland hyperplasia sonogram;and 6 cases showed patchy irregularly echogenic area.Color Doppler performance was that the 33 cases showed glandular tissue without significant blood flow signals,10 cases showing 1 -2 strip -shaped blood flow to see in the glandular tissue,3 cases showing more abundant flow signals.Conclusion Ultrasonography has no pain,with higher diagnostic yield reproducible operation,so gynecomastia is the preferred method.
5.The effects of toadpoison and arsenic on the ultrastructure of dental pulp in dog

Objective:To observe the effects of toadpoison and arsenic on the ultrastructure of dental pulp,Methods :Dental pulp of 36 teeth in 3 dogs was exposed by operation and then direct dressing with toadpoison paste or arsenic paste was performed. 1,2,6 and 24 hours after operation ,the pulps were processed and examinedunder a trasmission electron microscope (JEM-2000EX). Results:In toadpoison treated group ,the deformityof the basernembrane of pulp nerve ,swelling of metochondrion,degeneration of nerve axite were observed 1 h after dressing,and were more obvious in 2, 6 and 24 h. Damage of capillary and pulp cells were alsoobserved.In arsenic treated group ,swelling and break of capillaries ,deformity and degeneration of mitochondrion and endoplasmic reticulum in endoepithelial cells were observed,and were more obvious following time lasting. Damages of pulp nerve and pulp cells were also observed. Conclusion:Toadpoison and arsenic are toxic to pulp tissues. But the angiotoxicity of arsenic is stronger than that of toadpoison,the neurotoxicity and protoplasmic toxicity of toadpoison are stronger than those of arsenic.
6.Effects of Thymalfasin for Injection on the Apoptosis of Humun Lung Cancer A549 Cells

OBJECTIVE:To study the effects of Thymalfasin for injection on the apoptosis of human lung cancer A549 cells. METHODS:After treated with 0(blank control),25,50,100,200 and 400 mg/L Thymalfasin for injection for 24,48 and 72 h, the cell proliferation inhibitory rate was analyzed with MTT and calculated. After treated with 0(blank control),50 and 100 mg/L Thymalfasin for injection for 48 h,cell apoptosis was detected by flow cytometry,and the expression of Caspase-3,Bcl-2 and Bax and the phosphorylation level of Akt were deteced by Western blot. RESULTS:Compared with blank control group,proliferation in-hibitory rate of A549 cells increased after treated with Thymalfasin for injection,in concentration and time-dependent manner(P<0.05). The apoptotic rate of A549 cells increased after treated with Thymalfasin for injection 50,100 mg/L for 48 h (P<0.05). The expression of Caspase-3 increased while the Bcl-2/Bax and phosphorylation level of Akt decreased in A549 cells after treated with Thymalfasin for injection 100 mg/L (P<0.05). CONCLUSIONS:Thymalfasin for injection can inhibit the proliferation of A549 cells by activating Caspase-3,decreasing Bcl-2/Bax ratio,inhibiting Akt signal pathway and induce the apoptosis of A549 cells.
7.Risk factor analysis of intervention for the treatment of neurological complications in the elderly patients with ruptured intracranial aneurysm

Objective To evaluate the risk factors for interventional treatment of neurological complications in elderly patients with rupture intracranial aneurysms.Methods Form December 2004 to December 2014,520 consecutive old patients (≥ 60 years) with ruptured intracranial aneurysm treated at the Department of Neurosurgery,Changhai Hospital,the Second Military Medical University were enrolled retrospectively.The imaging and clinical follow-up results at day 30 after procedure were documented.According to whether the occurrence of interventional therapy-related neurological complications (including intraoperative aneurysm rupture,interventional therapy-related thromboembolic events,early postoperative aneurysm rebleeding,new postoperative subdural hemorrhage or intraparenchymal hemorrhage),they were divided into a complication group (n =68) and a non-complication group (n =452).Univariate statistical analysis and multivariate Logistic regression analysis were used to screen the independent risk factors for the occurrence of neurologic complications after interventional therapy.Results The incidence of neurological complications in 520 patients with aneurysm was 13.1% (n =68).Multivariate Logistic regression analysis showed that the history of hypertension (OR,2.207,95 % CI 1.149-4.240,P < 0.05),Hunt-Hess grade Ⅳ-Ⅴ (OR,4.287,95% CI 2.048-8.971,P < 0.01),Fisher grade Ⅲ-Ⅳ (OR,2.686,95% CI 1.483-4.865,P < 0.01),wide-neeked aneurysm (OR,2.884,95 % CI 1.511-5.505,P < 0.01),aneurysm bleb (OR,4.560,95 % CI 2.500-8.321,P < 0.01),and aneurysm < 3 mm (OR,5.748,95 % CI 2.122-15.570,P < 0.01) were the independent risk factors for treatment-related neurological complications in the interventional treatment of intracranial ruptured aneurysms in the elderly.Conclusion The history of hypertension,high Hunt-Hess grade,high Fisher grade,wide-neck aneurysm,aneurysm bleb,and micro-aneurysm are the independent risk factors for treatment-related neurological complications in interventional treatment of intracranial ruptured aneurysms in the elderly.
8.Feasibility of lateral-position pedicle screw placementvia combined anterior-posterior approaches in spinal surgery

BACKGROUND:Traditional internal fixation for thoracolumbar vertebral fractures easily causes poor vertebral reduction, and postoperative vertebral height loss easily appears.
OBJECTIVE:To analyze the feasibility of lateral-position pedicle screw placement in spinal surgeryvia combined anterior-posterior approaches.
METHODS: A total of 36 cases undergoing spinal surgical operation in the Baoji City Chinese Medicine Hospital from February 2013 to October 2014 were enroled in this study. The patients were divided into two groups according to the method of repair. 18 patients in the control group received prone-position pedicle screw placement, and 18 patients in the experimental group received lateral-position pedicle screw placementvia combined anterior-posterior approaches. Postoperative CT and X-ray examinations compared the adverse rate of screw placement and the rate of nerve root injury between the two groups. Operation time, length of stay, time of beginning movement, intraoperative blood loss and American Spinal Injury Association impairment scale score were compared between the two groups.
RESULTS AND CONCLUSION:Significant differences in the adverse rate of screw placement and the rate of nerve root injury were detected between the two groups (P < 0.05), and the adverse rate of screw placement and the rate of nerve root injury were better in the experimental group than in the control group. Operation time, length of stay, and time of beginning movement were significantly better in the experimental group than in the control group (P < 0.05). Intraoperative blood loss was significantly lower in the experimental group than in the control group (P < 0.05). American Spinal Injury Association impairment scale scores were significantly higher in the experimental group than in the control group (P < 0.05). These findings confirm that lateral-position pedicle screw placementvia combined anterior-posterior approaches in spinal surgery can reduce intraoperative trauma, shorten the length of hospital stay, and promote functional recovery of spinal cord.
9.Improved platelet-rich plasma effects on the proliferation and immunogenicity of human bone marrow mesenchymal stem cells

BACKGROUND:Previous experiments have shown that improved platelet-rich plasma can promote the proliferation of human dental pulp cells in a concentration-dependent manner, particularly when the concentration is 10%.
OBJECTIVE:To investigate the effect of platelet-rich plasma at different concentrations on the proliferation and immunogenicity of human bone marrow mesenchymal stem cells.
METHODS:Human bone marrow mesenchymal stem cells from healthy donors were cultured and passaged for 3-4 passages, identified by flow cytometry and differentiation inductions. Platelet-rich plasma samples which were manufactured from the venous blood of the same donor were used for culturing human bone marrow mesenchymal stem cells. The proliferation of human bone marrow mesenchymal stem cells was measured by cellcounting kit-8 method and the growth curves were drawn. The most suitable concentration of platelet-rich plasma was selected to culture human bone marrow mesenchymal stem cells for three generations and the Stro-1 expression rate on the surface of human bone marrow mesenchymal stem cells was determined through flow cytometry.
RESULTS AND CONCLUSION:Platelet-rich plasma at the concentration of 5%-10%evidently promoted the proliferation of human bone marrow mesenchymal stem cells on the 6th and 8th days. The most effective concentration to promote the proliferation was 10%. Platelet-rich plasma at the concentration of 10%stil promoted the proliferation of human bone marrow mesenchymal stem cells on the 10th day, and maintained a better immunogenicity of human bone marrow mesenchymal stem cells compared to the control group. These findings indicate that platelet-rich plasma can promote the proliferation of human bone marrow mesenchymal stem cells in a concentration-dependent manner, and 10%platelet-rich plasma is better to maintain the immunogenicity of human bone marrow mesenchymal stem cells.
10.Expression of periostin and MMP-2 in colorectal carcinoma and their clinicopathological significance

Objective To investigate the clinicopathological significance of periostin and MMP-2 in colorectal carcinoma.Methods Immunohistochemistry was used to detect the expression of periostin and MMP-2 in colorectal carcinoma,tubular adenoma,villous adenoma and normal colorectal tissue.The correlations between the expression of periostin and MMP-2 with clinicopathologic parameters were analyzed.Results In colorectal carcinoma,tubular adenoma,villous adenoma and normal colorectal tissue,the positive rates of periostin were 83.7 % (41/49),40.0 % (6/15),32.1% (9/28),0 (0/15),respectively,while the positive rates of MMP-2 were 71.4 % (35/49),60.0 % (9/15),64.3 % (18/28),0(0/15),respectively.There were significant differences among the groups (x2 =41.252,P =0.000; x2 =24.811,P =0.000).The expression of periostin in colorectal carcinoma tissue were not correlated with sex (x2 =0.002,P =0.961),age (x2 =2.267,P =0.132),tumor sites (x2 =1.506,P =0.220),differentiation status (x2 =0.875,P =0.350) and lymphatic metastasis (x2 =3.315,P =0.069).The expression of MMP-2 in colorectal carcinoma were correlated with lymphatic metastasis (x2 =5.800,P =0.016),whereas not correlated with sex (x2 =0.562,P =0.453),age (x2 =0.138,P =0.711),tumor sites (x2 =0.408,P =0.532),differentiation degree (x2 =1.335,P =0.248).The expression of periostin and MMP-2 were positively correlated in colorectal carcinoma (r =0.332,P =0.020).Conclusion Periostin and MMP-2 are correlated closely with the development and progression of colorectal carcinoma.It might be helpful for evaluating the biological properties and prognosis of colorectal carcinoma,and it would be more accurate to use a combined analysis of the two indicators.
