1.Expression of activated-leukocyte cell adhesion molecule in ovarian serous carcinoma and its significance

Objective To study the expression of activated leukocyte cell adhesion molecule (ALCAM/CD166) in serous ovarian tumors and its significance.Methods The expression level of CD166 protein,estrogen receptor,progestogen receptor were detected by immunohistochemical staining in 56 cases of malignant tumors,30 cases of borderline tumors and 10 cases of benign tumors,the relationship was investigated between the expression of CD166 and clinicopathologic information and its correlation with the expression of estrogen receptor (ER) and progestogen receptor (PR).Results The positive rates of CD166 protein in malignant,borderline and benign epithelial ovarian tumors were 87.5 % (49/56),56.7 % (17/30) and 10.0 % (1/10) respectively,there were significant differences (x2 =7.463,31.054,10.563,all P < 0.01).There were also significant differences in positive rates of CD166 protein among different clinical stages,pathologic grades and lymph node metastasis of epithelial ovarian tumors (x2 =41.205,Z =-4.663,Z =-2.981,all P < 0.01).There was a negative correlation between the expression of CD166 protein and the leves of ER and PR (x2 =23.36,21.441,both P < 0.05).Conclusion CD166 takes part in development and progression of serous ovarian carcinoma.The expression of CD166 increase significantly in ovarian serous carcinoma.
2.Changes of the Function of Platelet and Anticoagulation in Patients with Acute Pancreatitis

Objective:To investigate the clinical value of changes of platelet and anticoagulation function in patients with acute pancreatitis. Methods:β-TG, PF4 ,TXB2 ,GMP-140,antithrombin-Ⅲ,and protein C were measured in all patients. Results:There was no significant difference in all parameters between acute edema pancreatitis group and normal group(P>0.05).Compared with the control group, parameters of platelet significantly increased in acute necrosis pancreatitis group(P<0.01),and parameters of anticoagulation significantly decreased(P<0.01). Conclusion: The platelet system was activated and the level of anticoagulation system decreased in acute necrosis pancreatitis. Parameters are important in understanding and preventing this disease.
3.Clinical efficacy and safety analysis of long-term use of finasteride in benign prostatic hyperplasia

Objective To evaluate the safety and clinical efficacy of finasteride in treating patients with benign prostatic hyperplasia (BPH) during a 14-year period in a hospital.Methods Forty-one patients with BPH receiving finasteride 5 mg daily for the treatment from December 1994 to Febrary 2009 were included in the study. The base line and the end of study data of nocturia, prostate volume, serum creatine, complete blood count and serum prostatic specific antigen (PSA) were recorded. The acute urinary retention, surgical treatment and drug adverse reaction (prostate cancer or breast cancer) during the observation periods were recorded as well.ResultsAll the 41 cases took finasteride regularly for long-term medical therapy of BPH. At the end of this study, the average age of patients was (87.9±5.4) years old and the average duration of treatment was (141.1±27.1) months. The numbers of nocturia were 1.8±1.5 and 3.2±1.3 pre- and post- treatment, respectively (t= -4.52,P<0. 05). Before and at the end of the study, the prostate volumes were (44.9±26.6) ml and (42.8±31.3) ml, respectively(t=0. 33,P>0.05). Stratified study showed that, compared with the baseline data, the prostate volume was increased by 17.3 % in patients with prostate volume <25 ml(t= -0. 88 ,P>0. 05) ; the prostate volume was decreased by 17.2% in patients with prostate volume of 25-40 ml(t=2.59,P<0.05); the prostate volumes were (63.3±28. 9) ml and (62.6±36.5) ml pre- and post-treatment in patients with prostate volume > 40 ml, and there was no significant change(t= 0.07, P>0. 05). Before and after the treatment, the serum creatine levels were (96.8±18. 6) mol/L and (86.45±32. 3) mol/L, respectively(t= 1.79, P>0. 05) ; the white blood cell counts were (6.4±1.5) × 109 L and (6.0±1.7) ×109 L, respectively (t= 1.13,P>0. 05) ; and the PSA levels were (1.2±2.0) μg/L and (1.4±1.7) μg/L, respectively (t=-0. 49,P>0. 05). Three cases (7.3%) occurred acute urinary retention. There was no prostate cancer and breast cancer case, and no new adverse event occurred during long-time use of finasteride. Conclusions This retrospective study has demonstrated that the clinical progress of BPH can be controlled effectively by long-term administration of finasteride.
4.MRI and CT representations of intracranial dermoid cyst

Objective To evaluate the MRI and CT representations and their valuation of intraeranial dermoid cyst. Methods 23 cases who suffered from intracranial dermoid cyst confirmed by pathology were included retrospectively. The MRI and CT representations, and the pathological and clinical characteristics were analyzed.All the cases were performed with MRI, and 20 cases were performed with CT at the same time. Results 22 cases showed inhomogeneous high signal intensity in T1WI and T2WI, 1 cases showed iso-high signal intensity in T1WI and low signal intensity in T2WI. 8 cases showed plaque-like inhomogeneous high signal intensity in DWI. 19 cases showed homogeneous low density in CT scans with standard window width and level ,amt showed inhomogeneous low density with large window width and low window level. One case showed high density in CT scans with standard window width and level. Fat drop in subrachnoid space and bilateral cerebral ventricle and interstitial brain edema were found in 8 cases with MRI and CT. Conclusion Intracranial dermoid cyst always have typical MRI and CT representations. Distinctive representations could be found when the rupture of dermoid cyst exist, and correct diagnosis could be made before operation. MRI is better than CT in the diagnosis of dermoid cyst.
5.Influencing factors of perioperative myocardial injury in patients undergone percutaneous coronary intervention

Objective To investigate influencing factors of perioperative myocardial injury in patients undergone percutaneous coronary intervention (PCI). Methods Ninty one patients with coronary heart disease (including stable angina and unstable angina) underwent PCI, the perioperative myocardial injury incidence were observed prospectively by monitoring the preoperative and postoperative high sensitivity troponin protein levels to investigate the influencing factors of perioperative myocardial injury by Logistic regression analysis. Results There were no statistically significance in perioperative myocardial injury incidence (62.5%vs 68.7%, P=0.618) and perioperative myocardial infarction incidence (29.2%vs 20.9%, P=0.411) between stable angina and unstable angina groups . These factors of perioperative myocardial injury in patients undergone PCI by Logistic regression analysis were analysed, and we found that the influencing factors were the application loading dose rosuvastatin before PCI, preoperative statin therapy more than one month, apolipoprotein A levels and total stent length. Conclusion Application loading dose of rosuvastatin before PCI, preoperative statin therapy more than one month, apolipoprotein A levels are related to perioperative myocardial injury reduction, whereas the total length of the stent is associated with an increased occurrence of perioperative myocardial injury.
6.Effect of baseline left ventricular ejection fractions impact on the outcome of cardiac resynchronization therapy in patients with heart failure

Objective To investigate the effect of baseline Left-ventricular ejection fraction (LVEF) on the cardiac resynchronization therapy (CRT). Methods A retrospective analysis of patients with heart failure was performed. Fifty-four patients [aged (59.73 ± 11.61)years, 31 males, 23 females] who underwent CRT/CRTD were divided into 2 groups according to LVEF ( group A, LVEF≤ 35%; group B, LVEF > 35% ). NYHA class, LVEF,CRT respond rates and medical adverse event (MAE) due to worsening heart failure were detected at 3 and 12 months post-surgery. Results The score of NYHA class changing wasn′t different between these two group. Compared to group B, LVEF was markedly increased (5.08 ± 2.81)% vs (2.45 ± 1.80)%, P < 0.05;(5.38 ± 2.92)% vs (2.39 ± 3.53)%, P < 0.05)at 3 and 12 months post-surgery. Similarly, group A owned high CRT response rates(66.1% vs 33.9%, P < 0.05; 81.25% vs 47.83%, P < 0.05) at the both two time points. At 12 months post-surgery , LVED decreased and the MAE was similar in all groups. Conclusion Patients with EF≤ 35% have more increases in LVEF and own high CRT response rates post-CRT.
7.Efficacy of ginsenosides combined with prednisone in patients with systemic lupus erythematosus: a prospective, randomized, double-blind, placebo-controlled trial.

Background: The side effects of glucocorticoid in treatment of systemic lupus erythematosus (SLE) have been the focus of debate, and our preliminary study indicates that ginsenosides can enhance the efficacy of dexamethasone. Objective: To observe the effects of ginsenosides combined with prednisone in SLE patients. Design, setting, participants and interventions: A total of 60 SLE patients from Department of Rheumatology and Immunology, Changhai Hospital, Second Military Medical University, were randomly divided into treatment group and control group, with 30 patients in each group. Patients in the treatment group were given routine treatment with prednisone plus ginsenosides, while those in the control group were given routine treatment with prednisone plus placebo. They were all treated for 3 months. Main outcome measures: After three-month treatment, syndrome score in traditional Chinese medicine (TCM), total response rate and symptom improvement rate were measured and evaluated. Results: Twenty-eight cases in treatment group and twenty-seven cases in control group were included in analysis. The total response rates in the treatment group and control group were 89.28% and 66.67% respectively, and there was a significant difference between the two groups (P<0.05). After treatment, the TCM syndrome scores in the two groups were lower than those before treatment (P<0.01), and prednisone plus ginsenosides was better in decreasing the TCM syndrome score than prednisone plus placebo (P<0.05). The symptoms were improved in the treatment group as compared with the control group (P<0.05). Conclusion: Prednisone combined with ginsenosides can increase the clinical effective rate and improve the clinical symptoms of SLE patients.
8.Determination of ibotenic acid and muscimol in wild mushroom by dansyl chloride derivatization-liquid chromatography-tandem mass spectrometry

Objective :
To develop an analytical method of ibotenic acid (IBA) and muscimol (MUS) in wild mushroom by dansyl chloride (DNSCl) derivatization-liquid chromatography-tandem mass spectrometry (LC-MS/MS), and to provide technical support for etiological identification of mushroom poisoning events.
Methods :
The sample was extracted with hydrochloric acid solution, derived by bimolecular DNSCl, diluted and inorganic salts precipitated with acetonitrile. The extract was separated by a waters XBridgeTM BEH C18 column and measured by LC-MS/MS.
Results :
The limits of detection for IBA and MUS in wild mushroom were 0.15 mg/kg and 0.1 mg/kg, respectively. Good linear relationship was obtained for IBA and MUS at the range of 0.5-250 mg/kg with the correlation coefficient of 0.997 and 0.999, respectively. The average recoveries at three spiking levels were 84.5%-102.0% with relative standard deviations (RSDs, n=6) of 4.7%-8.6% for IBA. The average recoveries were 88.6%-95.4% with RSDs (n=6) of 4.9%-7.5% for MUS.
Conclusion
The optimized sample extraction and bimolecular DNSCl derivatization conditions can achieve rapid and accurate analysis of IBA and MUS in wild mushroom poisoning sample.
9.Rapid detection of clinical pathogenic bacteria and antimicrobial-resistant genes using DNA chip

Objective To detect the clinical pathogenic bacteria and antimicrobial-resistant genes quickly and sensitively using DNA chip.Methods Based on the analysis of 23S rRNA gene se- quences and other genes sequences associated with antimicrobial resistance(SHV<CTX_M),oligo nucleotide microarray was designed according to different bacteria and antimicrobial-resistant genes. The DNA fragments were amplified by labeling Cy5 fluorescence and detect clinical pathogenic bacte- rias and antimicrobial-resistant genes by hybridization.Results The result of detection(10~3-10~6 bac- teria/ml)was consistent with that of some documents in domestic and overseas under ideal circum- stances of detecting bacteria genomic DNA by the Reagent Box.And it was specific and reproducible when the detection system were evaluated with some clinical isolates and drug-resistant standard strain.DNA chip could identify 16 species and 7 generics including common diverse clinical pathogenic bacteria,and could detect the drug-resistant of extended spectrum?lactamase gene simultaneously. Conclusions The methods that we have established DNA chip is a sensitive,specific and reproducable tool for supplying routine methods.

OBJECTIVE:To establish a method for the content determination of rosin acid in Rheumatoid arthritis tablet. METH-ODS:HPLC was performed on the column of ZORBAX SB-C18 with mobile phase of acetonitrile-0.1% formic acid(82∶18,V/V) at flow rate of 1.0 ml/min ,detection wavelength was 241 nm ,column temperature was 30 ℃ and volume injection was 10 μl. MS/MS column was ZORBAX SB-C18 with mobile phase of acetonitrile-0.1% formic acid(80∶20,V/V)at flow rate of 0.2 ml/min;column temperature was 30 ℃;volume injection was 0.5 μl. Ionization mode was ESI+,atomization gas pressure was 25 psi,gas flow as 8.0 L/min,capillary voltage was 4 000 V,capillary outlet voltage was 120 V,precursor ion was 303 m/z,scan range was 50-500 m/z and the gas temperature was 350 ℃. RESULTS:The linear range of rosin acid was 2.5-100.0 μg/ml. RSDs of preci-sion,stability and reproducibility tests were lower than 2.0%,recoveries was 96.75%-98.11%(RSD=0.53%,n=6). CONCLU-SIONS:The method is simple,accurate and reproducible,and suitable for the content determination of rosin acid in Rheumatoid arthritis tablet.
