Objective to observe the correlation between the expression and prognosis of osteosarcoma Insulin-like growth factor receptor(IGF1R),explore the factors affecting the prognosis of patients with osteosarcoma.Methods between January 2005 and April 2014 menstrual standardized comprehensive treatment of 60 cases of patients with stage ⅡB osteosarcoma were retrospectively analyzed.Using immunohistochemical method,the detection of osteosarcoma tissue and IGF1R expression rate in normal tissue adjacent to tumors and to analyze its correlation with survival of patients with osteosarcoma and factors affecting the prognosis.Results IGF1R in the positive expression rate of osteosarcoma tissues was 80.0％ (48/60),expression in normal tissue adjacent to tumor rate was 4/8,has a statistically significant difference between (P =0.032).IGF1R positive expression in osteosarcoma tissues group median disease-free survival time and the median overall survival time were significantly lower than the negative expression group (P =0.023,P =0.040) respectively.IGF1R expression level and pathological necrosis rate of preoperative chemotherapy,the time of first visit to the lung metastasis,and the survival prognosis of patients have significant correlation (P ＜ 0.05),and age,tumor location,tumor size,the serum LDH and ALP levels,is urgent to make a diagnosis and give treatment time,factors such as operation,preoperative chemotherapy times no significant correlation (P ＞ 0.05).Condusion IGF1R positive expression rate in stage Ⅱ B osteosarcoma tissue significantly negatively correlated with the prognosis of patients with osteosarcoma,is the independent prognostic factors of patients with osteosarcoma.

Objective To estimate the efficacy of super-early minimal invasive evacuation of hematoma via keyhole for treating hypertensive basal ganglionic hemorrhage. Methods 36 patients with hypertensive basal ganglionic hemorrhage(9 cases in grade Ⅱ, 17 in grade Ⅲ and 10 in grade Ⅳ) were treated by minimal invasive evacuation of hematoma via keyhole at super-early stage, and their efficacy was statistically compared with 35 patients with hypertensive cerebral hemorrhage underwent conservative therapy or lateral ventriculopuncture. Results In the 36 patients underwent minimal invasive evacuation of hematoma via keyhole, 22 cases had a good prognosis(61.1%), 9 had a poor prognosis(25.0%), and 5 died(13.9%). Among the 35 patients underwent conservative therapy or lateral ventriculopuncture, 5 cases had a good prognosis(14.3%), 22 had a poor prognosis(62.9%), and 8 died(22.9%). BACKGROUND:The template method used in cementless total hip arthroplasty is influenced by many factors,so the accuracy is poor.Studies demonstrated that femoral isthmus plays an important role in total hip arthroplasty,which affect femoral prosthesis.OBJECTIVE:To discuss the effects of femoral isthmus on cementless total hip arthroplasty.METHODS:The images of 4 cadaver femoral isthmus were obtained by computer tomography.The relations between femoral component and cortical bone contact were observed on coronal slices by sawing manully.The longest and the smallest medullary canal dimensions and the cortical thickness of femoral isthmus were measured by CT films from 50 patients (55 hips) prepared forprimary total hip arthroplasty.The size of prosthesis was predicted based on the smallest medullary canal dimension of isthmus.And the conventional preoperative template was served as control.RESULTS AND CONCLUSION:There were a press fit and good contact between the femoral component and cortical bone of isthmus in 3 cadaver femoral bones.In 50 patients,the longest medullary canal dimensions of femoral isthmus were 8.2-22.4 mm(mean 14.2±3.1 ram).The smallest medullary canal dimensions of femoral isthmus were 6.1-17.9 mm[mean (10.2±2.9) mm],and the cortical thickness of femoral isthmus were 2.7-12.7 mm[mean (7.5±1.8) mm].The results of CT-isthmus measurement method predicted sizes were excellence in the 50.9% cases.There was statistically significant difference of predicted sizesbetween the excellence rate of CT-isthmus measurement method and the rate of control (P＜0.05).Particularly,the excellence rate of CT-isthmus measurement method was 63.9% in the group aged below or equal to 65 years.When the ratio of the longest medullary canal dimensions and the smallest medullary canal dimensions of femoral isthmus was large than or equal to 1.4,the excellence rate of CT-isthmus measurement method was 70%.The press fit and good contact between the femoral component and femoral isthmus plays a very important role in stabilizing of femoral prosthesis in total hip arthroplasty.The predicted sizes of femoral prosthesis depending on the medullary canal dimensions of femoral isthmus measured by CT film has better accuracy than the conventional preoperative template.

Objective idiopathic hemothorax .Methods 46 patients wifh idiopathic hemothorax To explore the treatment method of were first closed thoracic drainage catheter ,of which 12 patients underwent emergency explor-atory thoracotomy,with video-assisted thoracic surgery in 10 cases,non-surgical treatment of 24 cases.Results 46 patients were cured without complications ,were followed up 3 months to 5 years.without recurrence .Conclusion Idi-opathic hemothorax once diagnosed , treatment principles that should be placed closed thoracic drainage , and timely surgical treatment,especially video-assisted thoracic surgery ,patients can reduce the amount of blood transfusion ,ef-fective,less invasive,faster recovery,less bleeding,the patient easy to accept .

Objective To explore the effect of thrombolysis with anticoagulation treatment for early stage of posttraumatic and postoperative deep vein thrombosis of lower extremity. Methods The clinical data of 38 palients with early stage of posttraumatic and postoperative deep vein thrombosis(DVT) of lower extemity treated by thrombolysis with anticoagulation and dispersion drugs were analyzed retrospectively. Results The thrombolytic effect was significant. After the treatment, the deep veins were recanalization without regorging in 71.0% of patients. The total effective rate was 100%. Only one patient had hemorrhagic complication, two patients had symptomatic pulmonary embolism,but none of the patients died. Conclusions Thrombolysis with anticoagulation is an effective and safe method for posttraumatic and postoperative DVT in the early stage.

Objective To discuss the clinical efficacy of compound gastritis mixture (CGM) in treating precancerous lesions of gastric carcinoma (PLGC).Methods Totally 85 PLGC patients were randomly divided into treatment group and control group. The treatment group took CGM and the control group took Vitacoenzyme tablets. One therapeutic course was three months, and the treatment lasted for two courses. The clinical symptoms, electronic gastroscopy presentation, and pathological tissues before and after treatment were observed, the clinical efficacy was evaluated.Results There was statistical significance in TCM syndrome between the two groups (P<0.05), and the effective rate in the treatment group was more obvious than that in the control group (P<0.01). The symptoms of the two groups were significantly improved (P<0.05,P<0.01), but the improvements of the main symptoms in the treatment group were superior to those in the control group (P<0.05,P<0.01). The total effective rate of electronic gastroscopy presentation was 80.0% (36/45) in the treatment group, which was better than that of the control group (P<0.05). Pathological curative effects of chronic atrophic gastritis, intestinal metaplasia, and dysplasia in the treatment group were also better than those in the control group (P<0.05).Conclusion CGM has definite clinical efficacy in treating PLGC.

Aim To investigate Jianpi Qinghua Chinese herbal compound( JQCC) on the expressions of the rel-evant proteins of TLR4 and its downstream MyD88-de-pendent pathways, and on the inflammatory factor TNF-α in the animal model of chronic atrophic gastritis ( CAG) in rats, so as to discuss the molecular mecha-nism of JQCC in the treatment of CAG. Methods 53 Wistar rats were randomly divided into the blank con-trol group(n=8) and the CAG model group(n=45), and the animal model of CAG in rats was replicated by the “ammonia + sodium deoxycholic acid + ethanol”method. After the successful modeling was confirmed, the rest of the 40 CAG rats in the CAG model group were divided into the model group, the vitacoenzyme-tablet group, the low dose of JQCC group, the medium dose of JQCC group, the high dose of JQCC group ( each group n =8 ) . The experimental animals of all the groups were given intragastric administration of medication continuously for 30 days. Then the patho-logical histological changes were observed by HE stai-ning. The protein expressions of TLR4, MyD88, NF-КB and COX-2 were tested by Western-blot assay. And the serum TNF-α level was measured by ELISA. Results The protein expressions of TLR4 , MyD88 , NF-КB and COX-2 and the serum TNF-α level in the rats in the model group were increased evidently ( P<0. 01). Compared with the model group, the gastric mucosa lesions were improved in the low dose of JQCC group, the medium dose of JQCC group, the high dose of JQCC group, together with significant decreases of the protein expressions of TLR4 , MyD88 , NF-κB and COX-2 and the serum TNF-α level ( P <0. 05 or P <0. 01 ) . Conclusion JQCC could effectively improve the pathological and histological changes in the gastric mucosa in CAG rats, and the therapeutic mechanism might be related to the expressions of the relevant pro-teins of TLR4-MyD88-dependent pathways and the ex-pressions of anti-inflammatory cytokines.

Objective To explore the effects of Tongfu Huayu Daotan Decoction (通化阏导痰汤)on the serum concentration of S-100β protein, neuron-specific enolase(NSE) and Prognosis in patients with severe craniocerebral injury.Methods Sixty patients with severe craniocerebral injury were randomly divided into a a'eatment group and a control group.The treated group was treated with Tongfu Huayu Daotan Decoction plus conventional treatments including dehydration,antibiotics, organ functional support, nerve nutrition, prevention of complication, etc.; the control group was treated with conventional treatments alone. The concentration of serum S-100β protein and neuron-specific enolase(NSE) in plasma at admission and at24, 36, 72huors, and 5, 7 days after treatment were determined respectively; the Glasgow outcome scale (GOS)and neurological deficits scoring at 2weeks and 4weeks after hospitalization were compared to observe the efficacy of the patients. Results The concentration of serum S-100β protein and neuron-specific enolase (NSE) in plasma at previous treatment and at 24, 36 hours after the treatment had no statistical difference in the two groups(P＞0.05 ), The concentration of serum S-100β protein and neuron-specific enolase (NSE) in plasma at 72huors, 5 and 7 days after the treatment in the Tongfu Huayu Daotan Decoction group were lower than those in the control group, the differences being significant (P＜0.01). The Glasgow outcome scale (GOS) and neurological deficits scoring at 2weeks and 4weeks after the treatment in the Tongfu Huayu Daotan Decoction group were significantly nigher than those in the control group, the differences being significant (P＜0.01) .Conclusion Tongfu Huayu Daotan Decoction can alleviate the plasma concentrations of S-100β protein and neuron-specific enolase (NSE) in patients with severe craniocerebral injury and markedly improve the clinical therapeutic effects. Combined Tongfu Huayu Daotan Decoction and western medicine can significantly reduce mortality and improve the Glasgow outcome scale (GOS), neurological deficits scoring and therapeutic effect.

Objective To evaluate the efficacy of standard large trauma craniectomy to treat the patients with severe bump brain injury. Methods Thirty-two bump brain injured patients with cerebral hernia were treated with inside and outside decompression of standard large trauma craniectomy compared with 24 cases of routine temporoparietal craniectomy between Oct 2001 and Aug 2004.Results In standard large trauma craniectomy group,21 cases got good recovery(65.6%) and 5 were dead(15.6%).In routine temporoparietal craniectomy group,9 cases got good recovery(37.5%) and 10 were dead(41.7%).There was significant difference between the two groups in good recovery and mortality(P

Objective To explore promoter methylation of HIC1 gene and the expression of HIC1/SIRT1 related to the occurrence, development, and metastasis of papillary thyroid carcinoma. Methods Using Bisulfite sequencing PCR to analyze the promoter methylation of HIC1 gene. Using quantitative real-time PCR and Western blot to analyze expression differences of HIC1 and SIRT1 genes in tissues of papillary thyroid carcinoma(40 cases) and in adjacent normal thyroid(40 cases), of which datas were analyzed by statistics. Results The degree of HIC1 gene promoter methylation was significantly higher than that in adjacent normal tissues(P<0. 01). The degree of HIC1 gene promoter methylation in papillary thyroid carcinoma was related to lymph node metastasis, age, and the tumor-node-metastasis stages(P<0. 01). Compared with the expression of HIC1 mRNA and protein in adjacent normal thyroid tissue, that in papillary thyroid carcinoma was significantly lower(P<0. 01), while the expression of SIRT1 mRNA and protein in papillary thyroid carcinoma was significantly higher(P<0. 01). The lower expression of HIC1 mRNA and protein in the tumor tissues was related to the stage of lymph node metastasis, age, and the tumor-node-metastasis stages(P<0. 05). There was a strong negative correlation between the degree of HIC1 gene promoter methylation and expression of HIC1 in papillary thyroid carcinoma(P<0. 05). The expression of HIC1 mRNA and protein between that of SIRT1 also showed a strong negative correlation(P<0. 01). Conclusion Promoter hypermethylation of HIC1 and aberrant expression of HIC1/SIRT1 in papillary thyroid carcinoma may play a significant role in the oncogenesis and progress of papillary thyroid carcinoma. HIC1 is expected to become a new marker for prevention and treatment of papillary thyroid carcinoma.
