1.Stenting for patients with high-risk and complicated carotid artery stenosis

Objective To investigate the intervention methods of patients with high-risk and complicated carotid artery stenosis. Methods Twenty-five patients with high-risk and complicated carotid artery stenosis were treated with stenting During the procedure, they used the individualized therapeutic programs, including the implantation of temporary cardiac pacing, ev3 filter, combination of predilation and postdilation, guiding catheter (coaxial technique), staged angioplasty and stenting, and multiple stent implantations. Results The technical success rate of the 25 patients was 100%. The symptoms disappeared or improved in 21 patients (84%). The intraoperative dislodgement of emboli occurred in 1 patient. Transient ischemic attack occurred in 1 patient. No patients died. Conclusions Treatment with stenting in patients with high-risk and complicated carotid artery stenosis is safe. The preoperative evaluation of risks, the use of intraoperative individualized treatment programs, and the postoperative standardized management are the preconditions for ensuring efficacy.
2.Meta-analysis of clinical nursing path on hospital stay and hospitalization expenses of patients with ovarian cyst treated with laparoscopic resection

Objective To evaluate the effect of clinical nursing path on hospital stay and hospitalization expenses of patients with ovarian cyst treated with laparoscopic resection.Methods Meta-analysis was conducted by retrieving the databases of Pubmed, Google,CNKI and Wangfan database.All randomized controlled trials about clinical pathways in patients with ovarian cyst treated with laparoscopic resection were included.RevMan 5.2 was used for statistical analysis.Results Ten papers and 1,002 patients were included.There were statistical differences in hospital stay and hospitalization expenses by meta-analysis.Conclusion The clinical nursing path can reduce hospital stay and hospitalization cost of patients with laparoscopic resection.
3.Emergency Carotid Endarterectomy Remedy for Acute Strok Induced by Internal Carotid Artery Thrombosis

Objective To evaluate the feasibility and efficacy of emergency carotid endarterectomy in acute stroke induced by internal carotid artery thrombosis.Methods Two patients with acute stroke with internal carotid artery thrombosis were treated with internal carotid artery incision,thrombous extraction,carotid endarterectomy and angioplasty.Results The cerebral ischemia symptoms of patients were obviously improved.One patient showed complete rehabilitation,another patient could take self-care.No thrombosis and restenosis in internal carotid artery were formed during one to nine month follow-up.Conclusions Emergency carotid endarterectomy in acute stroke with thrombosis occlusion in internal carotid artery is safe and feasible.

5.The changes and significance of molecular markers of prethrombotic state in coronary artery patients with diabetes mellitus undergoing coronary angioplasty

Objective To observe the changes of molecular markers of prethrombotic state (PTS) in coronary artery disease (CAD) patients with diabetes mellitus (DM) undergoing coronary agioplasty. Methods Thirty-two patients with CAD (group A) and twenty-four CAD patients with noninsulin-dependent diabetes mellitus (NIDDM) (group B) were chosen as objects undergoing coronary agioplasty, and molecular markers of PTS, lipid peroxide (LPO), endothelial function were measured before and 20 minutes, 24 hours, and 7 days after the opration respectively. Results There were significant changes of molecular markers of PTS, LPO, and endothelial function in both groups after the opration, but the changes were even more pronounced in group B. It was on the 7th day that there were still the changes in group B, whereas the changes were prone to the levels of pre-opration in group A. Conclusion There was a series of significant changes of molecular markers of PTS, LPO, and endothelial function in CAD patients after the angioplasty, and the changes were even more pronounced in those with DM.

Objective To investigate the expression difference of microRNA -21 (miRNA-21) in patients with chronic cardiac function and analyze its clinical significance. Methods Patients with chronic heart failure (trial group,150 cases) and the subjects without chronic heart failure (control group,45 cases) were enrolled. Patients with chronic heart failure were divided into three subgroups according to NYHA: group A (heart function classⅡ,49 cases),group B (classⅢ, 51 cases) and group C (class Ⅳ, 50 cases). miRNA-21 levels were detected by qRT-PCR method. The levels of B type natriuretic peptide urea (BNP),left ventricular end diastole diameter (LVEDD) and left ventricular ejection fraction (LVEF) were determined. Results miRNA-21 expression in patients with heart failure were higher than that of control group (P < 0.01),and miRNA-21 expression in group C was higher than that of group A.Correlation analysis showed that there was a positive correlation between expression of BNP (r = 0.763,P < 0.01), LVEDD (r = 0.691,P<0.01), and the level of miRNA-21 in patients with chronic heart failure.And a negative correlation between LVEF value and the level of miRNA-21 (r = -0.918,P < 0.01). Conclusion miRNA-21 might be a potential marker for diagnosis of heart failure and could be a basis for reference about prognosis evaluation.
8.The investigation of adefovir dipivoxil antiviral clinical efficacy on chronic hepatitis B and its effect on peripheral blood T lymphocyte subsets

Objective To investigate the clinical efficacy of adefovir dipivoxil in antiviral treatment of chronic hepatitis B and its effect on peripheral blood T lymphocyte subsets.Methods 40 cases of patients with chronic hepatitis B were selected from The Fourth People’s Hospital of Qinghai in October 2009 to June 2012,the patients were given the antiviral treatment of adefovir dipivoxil.Before treatment,3 months,6 months and 12 months after treatment,the changes of liver function indicators of alanine transaminase(ALT),aspartate transaminase(AST)and total bilirubin(TBiL),and hepatitis B virus-DNA(HBV-DNA)were recorded;the total effective rate was observed;the peripheral blood T lymphocyte subsets changes was tested and analyzed by flow cytometry.Results The ALT,AST and TBiL of the patients all improved good,and the indicators of ALT,AST and TBiL after 12 months decreased significantly than before treatment,the HBV-DNA testing of the patients recovered negative,the difference was statistically significant (P<0.05 );the total effective rate of hepatitis B patients is 92.5%;the peripheral blood T lymphocyte subsets has no significant improvement in patients after antiviral treatment for 3 months.After the treatment for 6 months and 12 months,the CD4 +T and CD4 +/CD8 +T were significantly higher than those before treatment,there was no significant changes in CD3 +T and CD8 +T before and after treatment.Conclusion Adefovir dipivoxil has a good clinical efficacy in antiviral treatment of chronic hepatitis B,and can significantly improve the patients’liver function,improve peripheral blood T lymphocyte subsets imbalance.
9.Budd-Chiari syndrome treatment by splenic-jugular venous shunt:a report of 21 cases

Objective To evaluate the clinical effect of splenic internal jugular shunt for Budd Chiari Syndrome (BCS). Methods A retrospective analysis was made on the clinical data of 21 patients of BCS who underwent this operation. Results There was no death in 21 patients receiving this operation, The average post operative reduction of FPP was 0.97Kpa.2 suffered postoperative lung infection . Abdominal incision infection occurred in one patient. Tweenty cases were followed up for one month to forty five months. Lower extremity varicose veins and abdomen wall varicose veins basically disappeared a month postoperatively; ascites vanished completely 3 months postoperatively,and lower limb ulcer healed 6 months postoperatively. Seventeen patients followed by CDFI showed that no thrombus was found in their artificial vessel. Barium meal studies in 12 cases displayed that oesophageal varicose veins had basically disappeared during the follow up period. Conclusions Splenic jugular shunt bridged by artifical vessel is safe,effective and less traumatic for patients with type III and some patients with type II Budd Chiari syndrome. It is currently the best choice for treatment of BCS.
10.Effect of HBM-based home visits on blood pressure and health-related behaviors of elderly hypertensive patients

Objective To explore the effects of health belief mode (HBM)-based home visits on the blood pressure (BP) and health-related behaviors of elderly hypertensive patients.Methods One-year HBM-based home visits were paid to 114 elderly hypertensive patients over 80 years old chosen from the community.The normalization rate of blood pressure after intervention at 3,6,12 months and health-related behaviors after intervention were also compared with those of pre-intervention.Results The BP after intervention was improved significantly after intervention as compared with that of pre-intervention (P<0.01).Health-related behaviors after intervention were significantly more than those before pre-intervention (P<0.01).Conclusions The hypertensions are controlled effectively with the implementation of the HBM-based home visits to the elderly hypertensive patients.The development of disease in elderly hypertensive patients can be slowed down and their life quality can be improved.
