2.Comparative evaluation of laparoscopic and open surgical nephroureterectomy for upper tract transitional cell cancer

Objective To compare the clinical outcomes of urethral resectoscopy and retroperitoneal laparoscopic nephroureterectomy with open surgical nephroureterectomy. Methods A total of 44 patients with renal pelvic and ureteral neoplasms were included.Urethral resectoscopy and retroperitoneal laparoscopic nephroureterectomy were performed in 15 cases (Group A) and open surgical nephroureterectomy in 29 cases (Group B).The therapeutic effectiveness,postoperative recovery,relevant cost and complications were compared between Group A and Group B. Results The analysis showed that the intra-operative bleeding volum [(75.1?29.5)ml],the postoperative intesinal function recovery [(24.1?12.6)h],time to ambulation [(24.3?10.5)h],use of antalgesic [(3.0?0.8)d],intravenous antibiotic [(7.2?3.1)d],hospital stay [(6.3?1.2)d],convalescene of normal activities [(28.0?7.8)d] and incidence of complications with Group A were significantly superior to those with Group B(P
3.Preliminary experimental study of in vivo endothelialization of viable homograft valve

Objective To determine an optimal method of in vivo endothelialization when homograft valve (HV) leaflets are treated with different methods. Methods Twenty five fresh HV leaflets were divided into five groups randomly. The leaflets of GroupⅠ were fresh HV ones without any treatment; GroupⅡwere cryopreserved in liquid nitrogen for 1 month; Group Ⅲ was treated by DOA; GroupⅣ was treated by 25 g?L -1 glutaraldehyde; Group Ⅴ was first treated by hypotonic/hypertonic, then with an enzyme-based solution. Each HV leaflet was trimmed to 2 mm one which was cultured with lymphocyte from the recipient. In order to compare the antigenicity of HV leaflets, 3H-TdR incorporation was observed after tissue-cell coculture and stimulous index (SI) was calculated. The HV leaflets of different groups were implanted into the sheep's femorofemoropopliteal for three months; then the following procedures were done. Immunohistochemistry of Ⅷ factor correlative antigen was used to identify the cells property; observation of growth condition of EC and superficial morphosis of HV leaflets was made with scanning election microscope (SEM). Results Multiple comparison showed that SI of Groups Ⅱ, Ⅲ, Ⅳ, and Ⅴ differed significantly from Group Ⅰ(P0.05), but Group Ⅲ, Ⅳ, and Ⅴ all had significant differences from GroupⅡ (P
4.Laparoscopic adrenalectomy for adrenal tumors via transperitoneal or retroperitoneal approaches:A report of 56 cases

Objective To summarize the clinical experience of retroperitoneal and transperitoneal laparoscopic adrenalectomy for adrenal tumors.Methods Laparoscopic adrenalectomy was carried out in 56 cases of adrenal tumors via transperitoneal(10 cases) or retroperitoneal(46 cases) approaches.For transperitoneal laparoscopic adrenalectomy,the operation was performed through 3 abdominal trocars.After the peritoneum and the Gerota's fascia were opened,the adrenal gland or tumor were disconnected near the upper polar of the kidney.Then the tumor was removed by using the LigaSure system and a harmonic scalpel.For retroperitoneal laparoscopic adrenalectomy,a self-made balloon was used to dilate the retroperitoneal space.Then the Gerota's fascia was opened and the tumor was removed by using the LigaSure system and a harmonic scalpel. Results Of the 10 cases of transperitoneal adrenalectomy,conversions to open surgery were required in 3 cases because of extensive abdominal adhesion;of the 46 cases of retroperitoneal adrenalectomy,a conversion to open surgery was needed in 1 case because of massive hemorrhage of the inferior vena cava.Of the remaining 52 cases,the intraoperative blood loss was 20~200 ml(mean,70 ml) and no blood transfusion was needed.The operation time was 40~200 min(mean,110 min).The postoperative hospital stay was 3~8 d (mean,5.4 d).Follow-up for(6~36) months(mean,8 months) in 52 cases showed no tumor recurrence and metastasis.Conclusions Laparoscopic adrenalectomy has advantages of reliable effects,minimal invasion,rapid recovery,and short hospital stay.This procedure can be used as the first choice for the treatment of adrenal tumors.
5.Pathological analysis of the induced breast tumor models in tree shrew

Objective To establish a tree shrew mode of breast tumor.Method Forty-five 3 to 4 month-old female tree shrews were orally gavaged with 20 mg 7,12-dimethylbenz(a)anthracene (DMBA) or peanut oil per animal for three times.Following that, fifteen DMBA administrated tree shrews were implanted 90 day-release medroxyprogesterone acetate ( MPA) pellets.The tree shrews were palpated once weekly to detect mammary tumors for 45 weeks after first DMBA administration.Results DMBA were able to induce breast tumors (12.5%) in tree shrews, and MPA increased the tumor incidence (50%) while no breast tumors were observed in the control group.Three induced breast tumors were intraductal papillary carcinomas and one was IDC by H&E stain.Conclusion All induced tumors are similar with spontaneous tumors in structure and molecular markers.
6.Effects of Gastrodin on BDNF Expression in AD Tree Shrew

Objective To investigate the effect of gastrodin on brain-derived neurotrophic factor (BDNF) expression in Alzheimer's disease ( AD) tree shrew. Methods AD tree shrew model was established by intracerebroventricular injection with amyloid-βpeptide (Aβ) . Since the eighth day after injection, the tree shrews were treated with gastrodin for thirty days. The expression of BDNF in tree threw hippocampus was detected by reverse transcription polymerase chain reaction (RT-PCR) . Results The expression of BDNF mRNA in the treatment group was higher than that in the model group ( <0.05) . Conclusion Gastrodin can increase some degree of the expression of BDNF.
7.Exploration of the relationship between serf-efficacy sense and psychological health of nursing staff

Objective To explore the relationship between general self-efficacy sense and psycho-logical health of nursing staff. Methods 230 healthy nursing staff were investigated with self-rating de-pression scale, serf-rating anxiety scale and general serf-efficacy scale. Correlation analysis and t test were adopted to analyze the data. Results There was a negative correlation between the scores of general self-efficacy scale and the scores of self-rating anxiety and depression scale. Moreover, the scores of anxi-ety and depression of nursing staff with higher self-efficacy were higher than those with lower self-effica-cy. Conclusions Self-efficacy sense has positive effects on the level of psychological health of nursing staff. The higher self-efficacy sense will help maintain higher psychological health state.
8.Diagnosis value of expression of ICAM-1 and LFA-1 in acute rejection of liver transplantation in rats

Objective To investigate the relationship between the intercellular adhesion molecule-1(ICAM-1) and lymphocyte function associated with antigen-1( LFA-1) and acute rejection of liver transplantation in rats. Methods SD rats and Wistar rats were randomly divided into normal control group, isograft group, and allograft group. Liver biopsies from the recipients were obtained on the day 2,4, and 7 postoperatively to examine the expression level of ICAM-1 and LFA-1 in liver grafts. Samples were firstly stained with specific monoclonal antibodies using immunohistochemistry stain and then applied to image semiquantitive analysis. Results The expression levels of ICAM-1 and LFA-1 in allograft group were obviously higher than those in both isograft group and normal group. Moreover, the expression levels of ICAM-1 and LFA-1 were correlated positively with the degree of acute liver rejection. Conclusions Detecting the expressions of ICAM-1 and LFA-1 in liver graft tissue may be helpful to the diagnosis of acute rejection after liver transplantation.
9.Effect of STAT3 knockdownon the sensitivity of breast cancer cells to adriamycin

Objective To investigate the effect of STAT3 knockdown on the sensitivity of breast cancer cells with drug-resistant to adriamycin (MCF-7/ADR). Methods Levels of STAT3 and p-STAT3 in MCF-7/ADR and MCF-7 cells were detected by Western Blot. The MCF-7/ADR cells were infected with lentivirus expressing STAT3-shRNA and the negative control vectors in the STAT3-RNAi group and NC group, respectively, wihle the cells in the blank group received no treatment. The transfection efficiency was observed with fluorescence microscope, the mRNA level of STAT3, protein levels of STAT3 and p-STAT3 were detected by qRT-PCR and Western Blot, respectively. MCF-7/ADR cells were treated with different concentrations of adriamycin for 48 hours, cell proliferation was detected by MTT assay and cell apoptosis was detected by flow cytometry. Results Levels of STAT3 and p-STAT3 in MCF-7/ADR cells were significantly higher than those in the MCF-7 cells (P < 0.05). The levels of STAT3 mRNA, STAT3 and p-STAT3 in the STAT3-RNAi group were significantly lower than those in the Con group and the NC group (P<0.05, respectively). The Adriamycin IC50 in the Con group, NC group and STAT3-RNAi group was (56.1 ± 3.00)ug/mL,(54.9 ± 11.9)ug/mL and (7.6 ± 0.2)ug/mL, respectively. The flow cytometry results showed that the cell apoptosis in the Con group, the NC group and the STAT3-RNAi group was (10.5+0.7)%, (11.7+0.7)%and (34+3.1)%, respectively. Conclusion LV-shRNA-STAT3 can significantly inhibit STAT3 expression and enhance the sensitivity of breast cancer cells to adriamycin, and the underlying mechanism may be related to cell apoptosis.
10.Application of Medical thoracoscopy on 68 cases of elderly patients with pleural effusions

Objective To evaluate the value and security of the use of medical thoracoscopy in the diagnosis of elderly patients with un‐known pleural effusion and to analyze the finally pathological results .Methods STORZ medical thoracoscopy was used to retrospectively study the 68 patients with unknown pleural effusion .Multiple punch biopsy was taken in the questionable diseased regions under direct vi‐sion .The changes of patients′condition in intraoperative and postoperative ,and the causes of pleural effusion were observed .Results Sixty‐three cases were confirmed in 68 cases of patients with unexplained pleural effusion ,the diagnostic rate was 92 .6% .In all cases ,26 cases (38 .2% ) were tuberculosis ,31 cases (45 .6% ) were malignant tumor ,3 cases (4 .4% ) were chronic pus chest and Aspergillus infection in 1 case(1 .5% ) ,a large number of aspergillus hyphae were found in biopsy tissue .Non‐specific inflammatory changed in 7 cases (10 .3% ) ,in which 2 patients were eventually diagnosed with pleural effusion caused by pulmonary schistosomiasis ,and the rest cases were still un‐known .All cases had no serious complications .Conclusion Medical thoracoscopy have a good effect in diagnosing elderly patients with un‐explained pleural effusion ,which could be easily and safely performed .The primary cause of unexplained pleural effusion in elder patients may be malignant tumor ,followed by tuberculosis .
