1.Evaluation of rabbit corneal endothelial decompensation model established with Descemetorhexis technique

Objective Corneal endothelial decompensation is caused by many corneal diseases. It often results in severe clinical complications. Endothelial keratoplasty (EK) is a new therapy for corneal endothelial decompensation. This study aimed to investigate a new approach to establishing corneal endothelial decompensation animal model with Descemetorhexis technique in order to better understand the tissue response to EK. Methods Thirty New Zealand white rabbits were randomly divided into three groups according to different surgical procedures; corneal endothelial cells (CEC), Descemet's membrane and corneal endothelial cells (DM + CEC) as well as Descemet' s stripping with endothelial keratoplasty(DSEK) group and 10 eyes for each. The right eyes of rabbits were as surgery eyes. Other 10 rabbits were as DSEK donors. Corneal transparency, anterior chamber response and graft location were examined once per day for two weeks under the slit lamp. Comeal thickness was measured by ultrasound biomicroscope. Corneal endothelial cells were analyzed using vital staining with alizarin red and trypan blue in 2, 4 and 8 weeks after operation. Results The cornea in DM + CEC group remained opaque throughout the observation period. In CEC and DSEK group, corneal clarity was gradually restored and corneal thickness was significantly less than that in the DM + CEC group during the postoperative 8 weeks. There were significant differences in corneal thickness between the DM + CEC group and CEC group or DSEK group during the postoperative 8 weeks (P <0. 05). The vital staining showed that most Descemetorhexis area was not covered by endothelial cells even 2 months after surgery. Conclusion A new corneal endothelial decompensation model is successfully established for the study of corneal endothelial keratoplasty, which is helpful for understanding the wound-healing of rabbit corneal endothelium after Descemel' s membrane damage.
2.Influences of modified constraint-induced movement therapy on lower-extremity walking ability and blood flow of femoral artery among elderly patients with stroke

Objective To observe the influences of modified constraint-induced movement therapy (mCIMT)on lower-extremity walking ability and femoral artery blood flow among elderly patients with stroke. Methods Totally 67 patients with stroke were randomly divided into mCIMT group(n =35) aged ( 73.2 ± 5.2 ) years and neurodevelopmental treatment (NDT) group ( n =32) as control aged(76.4 ± 3.8) years.Patients in control group exercised by NDT 2 h/time,2 times/d,5 d/week for 6 weeks. Patients in mCIMT group exercised including: up and down exercise,100-120 times/d; movement flatbed exercise for 16-20 min/d; upstairs and downstairs exercise,balance training,standing in a single leg exercise,mandatory exercise time of lower-extremity about 4 h/d,5 d/week for 6 weeks.The patients were assessed for lower-extremities motor function using maximum walking speed (MWS),Berg balance scale (BBS),timed up to go test (TUGT) and Fugl-Meyer(FMA-L) at pre-treatment and post-treatment.The change of femoral artery blood flow velocity and lumen diameter on the affected lower limb were observed by color Doppler. Results There were no differences in the above scores,lumen diameter and blood flow velocity before treatment between the two groups (P> 0.05).After treatment,the scores of MWS (56.68 ± 6.57vs.45.61 ± 5.34),BBS(46.84 ± 4.05vs.29.84 ± 4.05),TUGT ( 14.55 ± 8.25vs.25.35 ± 8.70)were higher in mCIMT group than in NDT control group (t=15.09,17.38,15.25,all P=0.001)while no difference in FMA-L score between the two groups was found (35.24 ± 7.62 vs.31.32 ±3.28,t=19.99,P>0.05).Lumen diameter of femoral artery [(9.05±1.15) mm vs.(8.05±0.68)mm,t=6.72,P=0.001] and blood flow velocity[(92.55±18.25)cm/s vs.(69.35 8.7)cm/s,t=6.83,P=0.001] were increased in mCIMT group as compared with NDT group. Conclusions The mCIMT therapy is better in improving the lower-extremity walking function and blood flow velocity of femoral artery.
3.Working experiences of rehabilitation specialized nurses:a qualitative research

Objective To explore the working experiences of rehabilitation specialized nurses and provide references for the development of rehabilitation nursing. Methods Through phenomenological study of qualitative research, 8 rehabilitation specialized nurses in our hospital were selected by purpose sampling method in September 2015, and were interviewed about the current status of work and existing problems. Interview data was analyzed by Colaizzi method. Results Interview results showed that: working experiences with positive effect were mainly reflected in the following themes:change of cognition of rehabilitation concept, improvement of rehabilitation nursing ability,increased job satisfaction,a more clear role. On the contrary, the negative aspects were:difficulties of patient′s evaluation,lack of time engaging in rehabilitation nursing, difficulty of charge,weak strength of rehabilitation team.Conclusions Nursing managers should attach great importance to the cultivation and use of rehabilitation specialist nurses, modify the related system, and promote the development of rehabilitation nursing. Nursing managers also should pay attention to nurses′emotional experience; provide development platform for rehabilitation specialist nurses; and provide interventions and support in many aspects to perfect the related system, so as to promote the development of rehabilitation nursing.

Objective To investigate inner experience of junior nurses after a serious adverse event.Methods A total of 7 junior nurses undergoing a serious adverse event were interviewed from June to August 2016, using the phenomenological approach in qualitative research. The interview was analyzed by phenomenological methodology. The information was dealt with Colaizz 7 steps analysis method.Results The perception of junior nurses experiencing a serious adverse event could be classified into 3 themes: emotional changes in different periods, family attitude having an impact, the pursuit of nursing work having some changes. Conclusions The interview of junior nurses can help them undergoing a serious adverse event to express inner feelings, which shorten the time of mental health and provide the basis for the research on nursing adverse event management system.
5.Status investigation and influencing factors of post-traumatic growth in patients with urinary bladder cancer after surgery

Objective:To investigate the post-traumatic growth status of patients with urinary bladder cancer after surgery and analyze its influencing factors.Methods:Using the convenient sampling method, a total of 289 patients with urinary bladder cancer who were admitted to the First Affiliated Hospital of Zhengzhou University from January to December 2019 were selected as the research objects. The general data questionnaire, Post-traumatic Growth Inventory and Psychological Resilience Scale were used for investigation. Multiple linear regression was used to analyze the influencing factors of post-traumatic growth of patients with urinary bladder cancer after surgery.Results:Post-traumatic growth score of patients with after surgery was (61.47±10.79) . Differences in post-traumatic growth scores of urinary bladder cancer patients after surgery with different ages, residences, education levels, medical payment methods, average monthly family income, degree of self-care and metastasis were statistically significant ( P<0.05) . The total score of psychological resilience and scores of various dimensions of patients with urinary bladder cancer after surgery were positively correlated with the post-traumatic growth score ( P<0.05) . The results of multiple linear regression analysis showed that psychological resilience, education level and average monthly family income were the main factors influencing post-traumatic growth of patients with urinary bladder cancer after surgery ( P<0.05) . Conclusions:Post-traumatic growth of patients with urinary bladder cancer after surgery is at a medium level. Educational level, average monthly family income and psychological resilience are the main factors affecting post-traumatic growth of patients, suggesting that nursing managers can improve the post-traumatic growth of patients with urinary bladder cancer after surgery through intervention of influencing factors.
6.Status and influencing factors of quality of life in elderly patients with benign prostatic hyperplasia

Objective:To explore status of quality of life of elderly patients with benign prostatic hyperplasia and analyze the influencing factors.Methods:A total of 300 elderly patients with benign prostatic hyperplasia who were admitted to the First Affiliated Hospital of Zhengzhou University from September 2018 to October 2019 were selected as the research subjects by the convenient sampling method. The general information questionnaire, Benign Prostatic Hyperplasia Patients Quality of Life Scale and Health Behavior Ability Self-Rating Scale were used to investigate. Pearson correlation analysis method was used to analyze the correlation between health behavior and quality of life in elderly patients with benign prostatic hyperplasia. Multiple linear regression was used to analyze the factors affecting quality of life of elderly patients with benign prostatic hyperplasia. In this study, a total of 300 questionnaires were distributed, and 289 valid questionnaires were returned. The effective response rate was 96.33%.Results:The score of the Benign Prostatic Hyperplasia Patients Quality of Life Scale for 289 elderly patients with benign prostatic hyperplasia were (106.83±20.27) , and score of Health Behavior Ability Self-Rating Scale was (56.19±18.64) . Univariate analysis showed that there were statistically significant differences in scores of Benign Prostatic Hyperplasia Patients Quality of Life Scale for patients with different age, education background, marital status, family per capita monthly income, residence, disease course and other chronic diseases ( P< 0.05) . The results of Pearson correlation analysis showed that the health behavior of elderly patients with benign prostatic hyperplasia was positively correlated with the quality of life ( P<0.01) . The results of multiple linear regression analysis showed that educational background, healthy behavior ability and marital status were influencing factors of quality of life of elderly patients with benign prostatic hyperplasia ( P<0.01) , a total of 40.4% of the total variation was explained. Conclusions:The quality of life of elderly patients with benign prostatic hyperplasia is not high, and the educational background, health behavior ability and marital status can affect the quality of life of patients. It is recommended that clinical nursing workers take targeted and effective measures to improve the quality of life of elderly patients with benign prostatic hyperplasia.
7.Analysis of risk factors for early and late outcomes in heart transplantation patients: experience in heart transplantation from a single centre

Objective To summarize risk factors for clinical outcomes in heart transplantation patients, evaluate the characters of Chinese patients by comparing with international data, and introduce new clinical strategies. Methods We performed 200 heart transplantations from Jun. 2004 to May 2010. The clinical information was recorded and all patients were followed up. By analyzing 160 patients with a follow-up period of more than one year, we summarized clinical outcomes and risk factors of early and late results of heart transplant patients. Results Of 160 patients, 8. 1 % received postoperative extracorporeal membrane oxygenation (ECMO) support and 10% continuous renal replacement therapy. In 550 cases/times of endomyocardial biopsies, the incidence of rejection with grades more than Ⅱ (concluding grade Ⅱ ) was 14. 9%. In-hospital mortality was 3. 8%. Smoking,preoperative diastolic pulmonary arterial pressure, PAWP, total serum protein level and albumin level were risk factors of peri-operative mortality, and preoperative diastolic pulmonary arterial pressure,primary heart diseases, pulmonary hypertension and implantations of ICD, MCS and ECMO were risk factors of late mortality. Postoperatively, 1-, 3- and 5-year survival rate was 94. 4%, 91.9% and 88. 8%, respectively. Compared with UNOS data, the rate of primary heart diseases, pulmonary hypertension, and implantation of ICD, MCS and ECMO were different, and the long-term survival rate of 160 patients was higher than that reported by ISHLT. Conclusion The risk factors of mortality of Chinese heart transplant patients are different with their counterparts from western countries. Our corresponding peri-operative treatments and clinical strategies have produced satisfactory clinical outcomes.
8.Effect of fall risk assessment software in electronic medical record system used by junior nurses

Objective To explore the effect of fall risk assessment software in electronic medical record system used by junior nurses. Methods By retrospective study, the fall events reported by electronic medical record system between 2014 and 2015 were analyzed. The prediction rate for high risk falls and the incidence of falls related to junior nurses were compared before and after using fall risk assessment software, and the effect of fall risk assessment software on cognition and safety attitude of junior nurses was analyzed by revised report cognitive questionnaire and patient safety culture assessing scale about clinical nurse. Results After using fall risk assessment software, the prediction rate for high risk falls of junior nurses was higher than before (54.9% vs 25. 9%), and the difference was statistically significant (P<0.01). The incidence of falls related to junior nurses reduced to 40%, and the difference was statistically significant ( P<0. 05 ) . The cognition of nursing adverse event report improved significantly. The total score of patient safety culture assessing scale and the scores of all dimensions were increased. The differences were statistically significant (P<0.05).Conclusions Fall risk assessment software in electronic medical record system can improve the junior nurses′ ability of preventing adverse events, strengthen their security beliefs, and ensure the safety of care.
9.Phenomenological study on real work experience of "online nurses" door-to-door services

Objective:To analyze the real work experience of "online nurses" door-to-door services under the Internet model so as to provide a basis for the government and management departments to carry out "Internet + nursing services" work.Methods:According to the phenomenological research method, intentional sampling and the principle of maximum differentiation were used to select 20 nurses who had provided home care services in Class Ⅲ Grade A hospital and hosted community health service center in Ningbo for the semi-structured interview from August to November 2019. The interview data was organized and analyzed using the Colaizzi data analysis method.Results:There were two themes that affected the work experience of "online nurses" nursing service. Among them, the positive factors included the obvious increase in professional values, policy support guarantee, the surge in patient demand, insurance protection and first aid safety guarantee; the negative factors included the unbalanced input and harvest, service costs not included in medical insurance, improper disposal of medical waste, lack of team support, insufficient publicity of "online" nursing services, weak multidisciplinary nursing knowledge, and lack of standardization of "online" nursing medical records.Conclusions:The "online nurses" have positive experiences such as increased professional values in their service experience. At the same time, there are also difficulties such as imbalance in income and expenditure, and insufficient team support. It is necessary to further strengthen the government-hospital-community-enterprise collaboration, gradually improve the "online" nursing service standards from multiple angles and directions, provide a good working environment and the security for online nurses so as to serve society well.
10.Qualitative research on the demand for on-site nursing service training of "network nurses"

Objective:To deeply understand needs and problems of on-site nursing service training for "network nurses", so as to provide a reference for the construction of "Internet + Nursing Service" training content system standards.Methods:Using the purposive sampling method and based on principle of maximum differentiation, a total of 12 "network nurses" in Ningbo were selected from March to May in 2020 to conduct semi-structured interviews. Braun thematic analysis method was used to analyze data and extract themes.Results:A total of 4 themes were obtained. The first was that the requirements of practicing safety education and training included risk prevention training, adverse event handling process training, emergency response plans and complication handling training. The second was the needs of humanistic care education and training, including on-site service language, code of conduct training, nurse-patient communication training and health education knowledge training. The third was the demand for diversified training forms. Fourthly, factors affecting training quality.Conclusions:"Network nurses" have a greater demand for occupational safety education and humanistic care education. It is recommended that multiple parties should cooperate to improve the "Internet +Nursing Service" emergency laws and regulations as soon as possible, provide diversified training models, reserve high-quality "Internet + Nursing Service" talents and promote continuous and standardized development of "Internet + Nursing Service".
