Objective To analyze the causes of misdiagnesis and mistreatment of traumatic abdominal hernia. Methods In this study,11 cases of traumatic abdominal hernia admitted during 1971 to 2004 were retrospectively analyzed.Hernia located subcutaneously in 8 cases including incarcerated hernia in 2 csses.Hernia located intermuscularly in 2 cases with one case being incarcerated.and the other strangulated.One located retroperitoneally.Among the 11 cases,3 were simple hernia,8 were complicated with injuries of other organs. Results Eight cases were misdiagnosed,5 were operated on with wrong incision,and in one case the herniated intestinal loop,mistaken as hematoma,was incised.All these 5 cases underwent a second surgery due to failure in repairing the hernia. Condusion For traumatic abdominal wall hernia,correct diagnosis,the correct timing of operation and an appropriate incision chosen insure good therapeutic results.

Though there are many ways in culturing skin organ model,how to further improve technology to produce more similar to human's skin organ,and put it in treating dermatosis still needs further exploring. The model of skin organ provides a good platform for new drug development,toxicology experiment and physiological function research. Some tissues engineering skin products have been approved by Food and Drug Administration in America for clinical use. It is believed that following the development of tissue engineering and relative subjects,skin organ culture model in vitro will become one of the effective means on dermatosis study.

The expression of proliferating cell nuclear antigen (PCNA) was studied in 50 cases of squamous cell carcinoma of the skin using immunohistochemistry and PCNA cyclin monoclonal antibody. PCNA staining was found in the nuclei of tumour cells, two types of staining were seen, a granular pattern and a more diffuse nuclear staining. The expressions of p53 protein, PCNA and macrophage count in 50 cases of skin squamous cell carcinoma were studied by LSAB immunostaining of monoclonal antibodies (PC-10, CD68 and Do-7). The relationship between the proliferation of cancer cell and the count of stroma macrophage to the degree of differentiation was studied and observed in situ. The results showed that the positive rates of expression of both p53 protein and PCNA in the squamous cell carcinoma were 56% and 100%, respectively. The distribution and strength of the expression of p53 protein and PCNA were positively correlated to the degree of cell differentiation. Macrophoge regularly located in the margin of cancer nest. The macrophage count was reversely correlated to the degree of differentiation. It is concluded that the degree of differentiation is parallel to the expression of p53 protein and PCNA, whereas, the macrophages in the stroma to some extent inhibits the cancer cell proliferation.

Objective To investigate correlation study of serum hs-CRP,UA level and the correlation of dis-ease in elderly acute myocardial infarction patients , and provide a reference basis for clinical judgment for patients with illness development and prognosis prediction .Methods 40 aged patients with acute myocardial infarction pa-tients were selected as the research object ,and protionally random according to time admission the same period ran-domly selected were 40 cases of elderly patients with unstable angina pectoris ,40 cases of elderly patients with stable angina,40 cases of clinic medical normal healthy old people for controlled object .The levels of serum hs-CRP,UA and correlation was analyzed.Results The serum hsCRP,UA in the myocardial infarction group were (34.67 ±12.18)mg/L, (412.46 ±68.93)μmol/L,which in the unstable angina pectoris group were (17.21 ±8.39) mg/L,(274.19 ± 46.62)μmol/L,which in stable angina pectoris group were (5.25 ±1.40) mg/L,(297.41 ±37.43)μmol/L,which in the health group(4.96 ±1.24)mg/L,(279.64 ±34.78)μmol/L,the differences among the myocardial infarction group,unstable angina pectoris,stable angina pectoris group had statistical significance (P<0.05), the differences between myocardial infarction group and health group had statistical significance (P<0.05);Myocardial infarction patients prognosis group hs-CRP,UA group was obviously higher than that of the prognosis was good ,compared with statistical significance difference ( P <0.05 );In myocardial infarction group , unstable angina pectoris group , the hsCRP was associated with positive correlation exists between UA (P<0.05),stable angina pectoris group no correla-tion(P>0.05);Different prognosis in patients with myocardial infarction hsCRP is associated with UA has positive correlation between ( P<0.05) .Conclusion Elderly acute myocardial infarction patients serum hs-CRP,UA obviously increased ,and illness development correlation ,a high level of hs-CRP,UA patients often indicates poor prognosis .

Objective To evaluate the effect of montelukast combined with seretide in treatment of patients with asthma and its influence on balance between Th17 and Treg cells, and expression of IL-4 and IFN-γ.Methods 110 patients with asthma were randomly divided into observation group and control group, observation group of 55 cases, using montelukast combined with seretide, control group of 55 cases, using seretide,compared two groups of patients with pulmonary function changes, peripheral inflammatory factor changes and influence on Th17 and Treg cells balance. Results Before treatment, observation group and control group,FEV1 and FEV1 /FVC value was no significant difference.FEV1 and FEV1 /FVC after treatment were significantly improved compared with before treatment in two groups, differences were statistically significance (P<0.05).Between two groups, FEV1 values were significantly difference (t=8.1156, P=0.0001), FEV1 /FVC also had significant difference (t=4.8933, P=0.0001) .Before treatment, observation group and control group, IL-4 and IFN-γdid not have a statistically significant.After treatment, IL-4 and IFN-γdecreased significantly, with statistical difference compared with before treatment (P<0.05).Between two groups, IL-4 had significant difference (t=6.0273, P=0.0001), IFN-γalso had statistically significant (t=6.6042, P=0.0001).Before treatment, there was no significant difference between Th17 and Treg percentage, after 12 weeks of treatment and 24 weeks, Th17 in observation group were(0.53 ±0.02),(0.32 ±0.01), the control group were (0.78 ±0.03),(0.62 ±0.02), which had differences (P<0.05).After 12 weeks and 24 weeks,observation group Treg respectively(2.21 ± 0.22),(2.47 ±0.10),in control group were(1.50 ±0.11),(1.84 ±0.14), Treg percentage at different time after treatment, the differences had statistically significant (P<0.05).Conclusion Montelukast combined with Seretide in treating with asthma, pulmonary function of patients is obviously improved, and it can improve Th17, Treg cell.

[Objective]To report the result of percutaneous biopsy in bone tumor and analyze the interrelated influence factors.[Method]A total of 171 patients were subjected to trocar puncture for biopsy of bone tumor in pre-operation,and the results of all cases were reviewed.[Result]Totally 155 cases in 171 cases showed positive outcome,the positive rate was 90.64%.The accuracy of 98 cases was comfirmed in 122 postoperative pathology diagnoses,and the acuracy rate was 80.33%.There were 110 cases of tumor.Biopsy sensitivity was 95.41%,and biopsy diagnostic particularity was 100%.[Conclusion]Percutaneous biopsy is both effective and valuable for the diagnosis of bone tumor in pre-operation,but the biopsy positive rate and accuracy,and postoperative complication was highly related to the operator cognitive level of bone tumor and meticulous manipulation and so on.

[Objective]This study was aimed to research the synergistic antitumor proliferation effects and their best proportion of ursolic acid(UA) and tetrandrine(Tet), a pair of compounds isolated from Chinese herbs which showed complement inhibition on the multiple signal pathways. [Methods] The reporter assays on tumor-related signal pathways for MAPK/ERK, MAPK/JNK, NF-κB, Wnt, Notch, Cell Cycle, Myc/Max and Hypoxia were used to study the effect of five different Chinese herbal compounds on tumor proliferation,it was concluded cepharanthine(Cep), Tet, 18α-glycyrrhetinic acid(18α-Gly), UA and luteolin(Lut). MTT assay and crystal violet staining were used to study the antiproliferative effect of 15 different compounds for the tumor cells of MDA-MB-231,SW480,MG63,PC3,DU145,HCT116,143B and MDA-MB-468, which is consisted with Cep, Tet, 18α-Gly, UA and Lut for the 15 different compounds. Coefficient of drug interaction(CDI) method was used to detect the synergistic effect of the two compounds. Combination of index(CI) and isobologram method was used to screen the best ratio of compounds in their antiproliferative effects. [Results] The signal pathway reporter assay showed that UA and Tet could complementarily inhibit tumor-related signaling pathways. And the results also showed that UA and Tet could induce synergetic anti-tumor cell proliferation in vitro. Furthermore, the optimal ratio of UA and Tet was 9:1 by using isobologram and CI method. [Conclusion] UA and Tet can be inhibited and complemented by 8 tumor-related signaling pathways, and we used MTT assay and crystal violet staining or other methods to confirm the synergistic antitumor proliferation effects, furthermore, the optimal proportion for UA and Tet were screened, and it provided a new insight to develop new anticancer formula in research.

Objective To compare the phenotype of myeloid derived suppressor cells (MDSCs) separated from the bone marrow of mice 3 d and 7 d after cecal ligation and puncture ( CLP) and to elucidate its potential role in the accumulation and immuno-function of MDSCs by determining the expression of microRNA-146a(miR-146a)in order to explore the effect of miR-146 a on immonosuppression of MDSCs in sepsis .Methods A septic model was prepareol by CLP in adult male C57BL/6J mice.MDSCs(expressing cell-surface CD11b and GR-1 antigens )from bone marrow were harvested 3 and 7 days after CLP and were separated with magnetic bead sorting technique .Then,cytokines secretion and arginase-I activity were detected and the T cell proliferation in vitro and the expression of miR-146a of MDSCs (3 d and 7 d after CLP)were observed.Results MDSCs secreted mostly such promoting inflammatory factors as TNF-α, IL-6 3 days after CLP, but 7 days after CLP , they primarily secreted IL-10 and TGF-βwhich were anti-inflammatory factors . MDSCs had potent immunosuppressive properties by increasing T cell suppression in a late anti-inflammatory phase ( CLP3 d vs CLP7 d, P<0.05).In the meantime,miR-146a of the MDSCs in bone marrow was overexpressed in septic mice at 7 days(P<0.05). Moreover,the expression of miR-146a of the MDSCs in bone marrow of septic mice was higher at 7 days than at 3 days after CLP(P<0.05).Conclusion The data indicate that the phenotype of MDSCs evolves through early pro -inflammatory phase into the late anti-inflammatory phase .MDSCs have potent immunosuppressive properties in the late phase of sepsis . miR-146 a might play a crucial role in the regulation of immunosuppressive activity of MDSCs in late sepsis .
