1.Treatment of equinus deformity with Ilizarov fixator

Objective To evaluate the treatment of equinus deformity with Ilizarov fixator. Methods From February 2003 to August 2005, 17 patients with traumatic equinus were managed with Ilizarov fixator in our department. A preoperatively constructed ring fixator was applied on the involved foot and leg. Three days after the fixation, the compression threaded rod in the front and the distraction threaded rod at the back were respectively shortened and lengthened by turning the screw nuts on the rods 2 to 4 rounds in four times per day in the first week, and then 1 to 2 rounds in four times per day, which resulted in gradual correction of equinus deformity. The Fixation was maintained in neutral position for 2 to 3 months after achieving sufficient equinus correction to ensure the state of dorsiflexion. An ankle-foot orthosis was applied during resting and sleeping for more than three months after removal of the fixator. Results The mean follow-up period was 10 months (range, 7 to 14). All the patients achieved 0? dorsiflexion 4 to 6 weeks after the treatment while in the Ilizarov apparatus. The fixator was maintained in patients for an average of 14.5 weeks (10 to 16 weeks). No blood vessel or nerve was injured. Three patients had one mild pin track infection which responded to conventional management. A 20? equinus contracture reoccurred to one patient six weeks after the removal of the apparatus which had been maintained only for 10 weeks because the patient noted severe pain with the apparatus. One patient chose to accept his improved but persistent 10? equinus contracture. One patient who had had reconstructive surgery for his severely damaged leg muscles and tibial defect eventually required tibiotalar arthrodesis. Entire foot weight bearing with zero angle and walking without significant footdrop were achieved in the other 14 cases. Two patients complained of pain while bearing weight. Active ankle dorsiflexion of 10? beyond the neutral position was achieved in patients with functioning ankle dorsal extension muscles. Conclusion Ilizarov fixator is an effective technique to correct the equinus deformity because of its minimal invasion.
2.Effect of heparin combined with urokinase therapy on lower limb venous thrombosis

Objective To explore and analyze effect of heparin combined with urokinase therapy on lower limb venous thrombosis.Methods 90 cases with lower extremity venous thrombosis were chosed and divided into control group of 45 cases and experimental group of 45 cases according with random number table method.Control group was given pure low molecular heparin treatment, experimental group was treated with small dose of urokinase static drop on the basis of control group, 7 days for a course, a total of 2 course of treatment.Compared two groups before and after treatment of lower limb venous patency rate changes, blood coagulation function index, adverse reactions and the recurrence rate.Results Compared with control group post-treatment.patency rate of posterior divisions, popliteal vein and posterior tibial vein of experimental group were significantly better (P<0.05).Experimental group post-treatment compared with control group post-treatment, PT, APTT, TT time had significant difference ( P <0.05 ).Patients incidence of adverse reactions were significantly lower than control group ( P <0.05 ).Patients were followed up for 1 year recurrence rate was significantly lower than control group ( P <0.05 ).Conclusion Effect of low molecular heparin combined with urokinase in the treatment of venous thrombosis of lower limbs significant and its prognosis is well.
3.Identification of Pathogens and Drug Sensitivity Analysis 415 Sputum Samples

OBJECTIVE To understand the bacterial spectrum distribution of sputum among the lung infection patients in our hospital and their resistance mechanisms for the reasonable clinical application of antibiotics to effectively prevent the occurrence of nosocomial infections. METHODS The pathogens isolated from the sputum of patients were identified with pathogen micro-biochemical identification code and the commonly used antibiotics were detected by drug sensitivity tests. RESULTS From 415 samples of the sputum,537 strains were detected out,from them Gram-positive cocci accounted for 70.8%,Gram-negative bacteria accounted for 28.1%,and Candida albicans accounted for 1.1%;MRSA was in 18 strains and ESBLs 6 strains. CONCLUSIONS It is important to sputum culturing for lung infection patients and determining the pathogen species and in their drug susceptibility results,the in order to rationally use the antibiotics and enhance the effectiveness of treatment.
4.Study on histone H3 acetylation of BDNF gene promoter in SAMP8 mice

Objective To explore the role of histone H3 acetylation modification of brain derived neurotrophic factor (BDNF) in the pathogenesis of Alzheimer's disease (AD).Methods 2 months and 8 months SAMP8 mice were used as AD model.Morris water maze was used to detect the impairment of learning and memory.Western blot was used to detect BDNF protein expression in the hippocampus,and chromatin immunoprecipitation (CHIP) was applied to study the changes of histone H3 acetylation in different BDNF promoters.Results The results of water maze test showed that the time across the target quadrant in 8 months SAMP8 mice(0.9±0.4) was significant declined compared with that of 2 months SAMP8 mice(3.7 ± ±0.9) and 8 months SAMR1 mice (3.3±0.6)(all P<0.05).Meanwhile,compared with 2 months SAMP8 mice ((23.9±4.0) s) and 8 months SAMR1 mice ((21.5± 2.3) s),target quadrant time in the 8 months SAMP8 mice((11.7±2.8) s) was also significantly reduced(both P<0.05).The western blot showed the expression of BDNF in the hippocampus of 8 months SAMP8 mice was significantly decreased compared with that of 2 months SAMP8 mice and 8 months SAMR1 mice(P<0.05).Lastly,CHIP assays showed that histone H3 acetylation of BDNF exon Ⅳ and Ⅵ in the hippocampus of 8 months SAMP8 mice were remarkably decreased(P<0.05) compared with that of 2 months SAMP8 mice and 8 months SAMR1 mice.There was no significant change of histone H3 acetylation of BDNF exon Ⅰ and Ⅲ among all groups(P>0.05).Conclusion Histone H3 acetylation of BDNF exon Ⅳ and Ⅵ is reduced during the development of AD,which may be the mechanism underlying the impairment of learning and memory in AD.
5.The clinical application of VATS lung volume reduction surgery

Objective To evaluate the efficacy of lung volum ereduction surgery(LVRS) on severe chronic obstrutive pulmonary emphysema (COPE).Methods Fifteen patients with COPE with LVRS were retrospectively analyzed.The lung volume was reduced with GIA by a degree of 20%-30%.The activity and the lung function indexes of patients before LVRS,half a year and 1 year after LVRS were analyzed.Results Fifteen patients recovered and were discharged in 8-54 (14.7 ± 8.8) days after LVRS.The forced expiratory volume in one second (FEV1),total lung capacity (TLC),residual volume (RV),and 6-minute walk distance (6MWD) of patients with LVRS were significantly improved after operation (P < 0.05).Conclusion The lung volume reduction surgery provides a quite new and effective treatment for some cases of severe chronic obstructive pulmonary disease.
6.Expression of Cx43 in human lung adenocarcinoma and its significance

Objective To investigate the expression of connexin43 (Cx43) protein and mRNA in human adenocarcinoma of lung and its clinical significance.Methods One hundred and forty-eight human adenocarcinoma of lung tissues were detected by immunohistochemistry for Cx43 expression and fluorescence in situ hybridization for mRNA.The relationship between Cx43 expression and clinicopathologcal factors was analyzed.Results The positive rates of Cx43 expression in well/moderately differentiated and poorlydifferemiated/indififerentiated lung cancer tissues were 48.7 % (38/78) and 30.0 % (21/70),respectively (P < 0.05).The positive rates of Cx43 expression in negative lymph node metastasis and positive lymphnode metastasis lung cancer tissues were 56.3 % (18/32) and 34.5 % (40/116),respectively (P < 0.05).The positive rates of Cx43 expression in stage Ⅰ-Ⅱ and Ⅲ-Ⅳ lung cancer tissues were 51.5 % (49/95) and 34.0 %(18/53),respectively (P < 0.05).The positive rates of Cx43 mRNA expression in adenocarcinoma of lung adenocarcinoma and normal lung tissues were 32.4 % (48/148) and 60.0 % (12/20) (P < 0.05).Conclusion The expression of Cx43 is closely related with well differentiation,invasion,status of lymphnode,clinical stage and prognosis of human lung adenocarcinoma.It may participate in the development and progression of human lung adenocarcinoma,and may be used to predict the prognosis of human lung adenocarcinoma.
7.Flk-1~+ bone marrow mesenchymal stem cell transplantation upregulates interleukin-6 level: Whether it simultaneously aggravates collagen-induced arthritis in mice?

BACKGROUND: Immunoloregulation of mesenchymal stem cells (MSCs) is commonly approved. Previous studies have confirmed the ability of Flk-1~+ bone marrow MSCs (BMSCs) to inhibit T/B lymphocyte proliferation in vitro. OBJECTIVE: To investigate the therapeutic effect of Flk-1~+ BMSCs in collagen-induced arthritis mice.METHODS: A total of 18 healthy male DBA-1(H-2K~q) mice aged 10 weeks were randomly divided into 3 groups. All the mice were injected at the base of the tail with bovine type II collagen (CII), and received a booster injection of CII on day 21 to establish the CIA mice model. DBA-1(H-2K~q)mouse Flk-1~+ BMSCs were isolated in vitro by the density gradient centrifugation and adherence screening. Following initial immunity, mice in the cell transplantation group were infused with Flk-1~+ BMSCs (1-2)×106 cells/mouse via the caudal vein. Mice in the cell transplantation group were injected with the same volume of Flk-1~+ BMSCs during booster. Mice in the model control group were injected with an equal volume of saline 0 or 21 days following initial immunity. Following initial immunity and booster immunization, claw pad thickening and clinical score were observed, changes of joint pathology and dynamic changes in serum factor mass concentration were determined in mice. RESULTS AND CONCLUSION: Compared with the model control group, no significant difference in claw pad thickening and mean clinical score was detected in the cell transplantation group following initial immunity (P > 0.05), with the presence of obvious damage to synovial membrane and inflammatory cell infiltration. Mass concentration of each serum cell factor was similar. The claw pad was significantly thickened (P < 0.01), mean clinical score reached 3.35 points, with severe damage to synovial membrane, proliferation of blood capillary in the cell transplantation group following booster immunization. Interleukin-6 levels were greatly increased at day 28 following initial immunity (P < 0.1), but decreased at day 35 following initial immunity (P < 0.1). Results indicated that in the collagen-induced arthritis mouse models, Flk-1~+ BMSC transplantation did not obtain prospective therapeutic efficacy, but aggravation of arthritis was observed in the cell transplantation group following booster immunization. Upregulation of interleukin-6 concentration could aggravate the behavior symptom of rheumatoid arthritis mice.
8.Research progress on hemodynamic monitoring in critically ill children

Hemodynamic instability is one of the main causes of death in critically ill children.Real-time and dynamic monitoring of hemodynamic changes is an important part of the work in intensive care units and operating rooms.For the past 50 years, invasive pulmonary catheter thermal dilution has been recognized as the "gold standard" for hemodynamic monitoring, but its clinical use has declined in recent years due to its patient injury, complications, and difficulty in operation.Therefore, hemodynamic monitoring techniques that are minimally or completely noninvasive, simple to perform, and highly correlated in accuracy with invasive methods are receiving increasing attention from pediatric clinicians.This review summarized the measuring principle and application status of various monitoring technologies, in order to provide evidence for the selection of appropriate monitoring methods in clinical practice.
9.Clinical study of comprehensive Traditional Chinese Medicine treatment for limb dysfunction after ischemic stroke

Objective:To evaluate the clinical effect of Limb dysfunction after the comprehensive treatment of ischemic stroke by Traditional Chinese Medicine (TCM).Methods:A total of 160 patients with limb dysfunction after ischemic stroke. Who in line with the inclusion criteria, were randomly divided into 2 groups by random number table, 80 cases each. These patients were treated in Multicenter Union Hospital from June 2017 to Janunry 2019. The patients in the control group were given basic Western medicine treatment and rehabilitation training, while the patients in the observation group were given a comprehensive treatment combined with traditional Chinese medicine based on the control group (Chinese herbs, herbal fumigation and ear point press). Both groups were treated for 4 weeks and follow-up for 2 months. The Fugl-Meyerscale were used to evaluate the degree of limb dysfunction and balance dysfunction, the National Institutes of Health Stroke Scale were used for assessing the damage extent of nerve function, the Barthel index for evaluating the mobility of daily life. The clinical efficacy was evaluated according to the method of Brunnstrom assessment.Results:After treatment, the Limb strong spasm (1.57 ± 0.36 vs. 1.98 ± 0.53, t=5.724), Hemianesthesia (1.37 ± 0.31 vs. 1.80 ± 0.36, t=8.096), Inhibited bending and stretching (1.31 ± 0.25 vs. 1.84 ± 0.46, t=9.055) in the observation group were significantly lower than those in the control group ( P<0.01). The Fugl-Meyer activity and balance rating in the observation group were significantly higher than those in the control group ( t values were 2.739, 4.705, respectively, P<0.05 or P<0.01). The total effective rate of observation group was 93.3% (78/80), the control group was 73.3% (71/80), and there exist statistical significance ( χ2=4.783, P=0.028) in the two group’s comparative difference. Conclusion:The comprehensive TCM treatment can improve the hemiplegia syndrome of ischemic stroke patients and their limb activity and balance function, promote their neural functional recovery, enhance the activity of their daily life and clinical efficacy.
10.Clinical study on MEAD regimens for relapsed or refractory adult patients with acute lymphocyte leukemia

Objective To study the clinic effect and safety of MEAD chemotherapy regimen for adult patients with relapsed or refractory acute lymphocyte leukemia. Methods Between July 2006 and July 2009,twenty-two adult patients with relapsed or refractory acute lymphocyte leukemia received MEAD regimen (mitoxantrone 6 mg/d dl-3 iv drip,cytarabine 100 mg/d dl-5 iv drip,etoposide 100 mg/d dl-5 iv drip,dexmethasone 10 mg/d dl-8 iv drip). Results The complete remission (CR) rate of adult patients with relapsed or refractory acute lymphocyte leukemia was 31.8 %,the partial remission(PR) rate was 22.7 % and the overall response (OR) rate 54.5 %. The cumulitive CR rate was 50.0 %,and the PR rate 40.9 % after two times MEAD chemotherapy regimen. The main adverse effect was different level of myelosuppression,and other toxicity of vital organ was mild. Conclusion MEAD regimen is effective and can be tolerated for adult patients with relapsed or refractory acute lymphocyte leukemia,and its side effect is mild.
