Feeding adult male rats with gossypol acetic acid at a dose of 15 nig/kg/day for 50 days led to infertility. Spermatogenesis and sperm motility were impaired ( table 1 ) . However, testosterone, K+ and Na+ concentsrations in serum and RTF were found to be unchanged (table 2, 3 ) . It suggests that at low antifertility doses gossypol disrupts spermatogenesis in the seminiferous epithelium without affecting testosterone, K+ and Na+ secretion and translation in testis.

Objective To investigate the correlation between tumor metastasis with T cell subsets and cytokines in the pa tients with primary hepatocellular carcinoma (HCC).Methods Ninety-seven cases of primary HCC were prospectively collected from January 2014 to January 2016 and assigned into the metastasis group (38 cases) and non-metastasis group(59 cases) according to whether suffering from metastasis.Surgical specimens were obtained from all pauents and flow cytometry was used to detect the CD4+,CD8+ T cell proportion in HCC tissue,paracancerous tissues and peripheral blood.Moreover,ELISA was adopted to detect the peripheral blood IL-6,IL-10,TNF-α and IFN-γ levels.Results Compared with the non-metastasis group,the CD4+ T cell proportion of HCC tissue in the metastasis group was significantly increased(P=0.02),while the CD8+ T cells were significantly decreased (P=0.015).There was no statistical difference in CD4+ T cells proportion in the paracancerous tissue between the two groups (P=0.328).However the CD8+ T cells proportion of paracancerous tissue in the metastasis group was significantly higher than that in the non-metastasis group (P=0.021).There was no statistically significant difference in the proportion of CD8+ T cells in peripheral blood of the two groups (P=0.362).The proportion of CD4+ T cells in peripheral blood of the metastatic group was significantly lower than that in non-metastasis group (P=0.032).When compared with non-metastasis group,the metastasis group got a decreased level of peripheral blood IL-6 (P =0.012);while the IL-10 level was significantly increased (P =0.006);the TNF-α level and IFN-γ level were significantly decreased(P=0.000,P=0.035).Conclusion The patients with primary HCC have obvious T cell subsets and cytokines imbalance.

Objective To observe the changes of peripheral blood CD27+B cells percentage in patients with HBV-related disease of different severity and the clinical significance.Methods 20 cases of chronic viral hepatitis B,23 cases of HBV-related liver cirrhosis,21 cases of liver cancer were selected,25 cases of healthy controls in the corresponding period who had the physical examination were set as the healthy control group.The peripheral blood in the healthy controls and the patients with HBV-related disease was collected.The cellular immune level changes and CD27+ B cells percentage was detected by flow cytometry,the humoral immunity was detected by immune transmission turbidty method:IgM,IgD.Results (1)Compared with the healthy control group,CD27+B cells percentage significantly decreased in the three groups of patients with HBV-related disease [(5.16 ± 0.36) ％ vs.(4.52 ± 0.22) ％,(2.24 ± 0.15) ％,(0.58 ± 0.02) ％,F =4.32,P ＜ 0.05],and the downward trend gradually obvious as the disease degree exacerbated.nnnnn(2)IgM,IgD in the three groups of HBV related disease patients rised obviously,and the increase range became more obvious as the disease degree exacerbation(F =3.29,5.23,P =0.02,0.03).Conclusion CD27+B cells has a close relationship with HBV-related disease,and rebuilding the body's immune defense system is great importance for evaluating prognosis and the clinical guidance in HBV-related disease.

Objective To examine the association of arteriovenous fistula (AVF) blood flow (Qa) dynamics with inflammation state and its effect on cardiovascular diseases (CVD) in maintenance hemodialysis (MHD) patients.Methods Thirty MHD patients with AVF and twelve healthy people were enrolled in the study.Qa and cardiac output (CO) were measured by Transonic Hemodialysis Monitor HD 02.In MHD patients,pre-dialysis blood samples were taken before Qa monitoring.High-sensitivity C-reactive protein (hsCRP) was measured by immunoturbidimetry (Kyoma,Japan).Inflammatory factors IL-2,IL-6,IL-10,TNF were measured by Cytometric Bead Array (BDTM).Cardiovascular diseases morbidity was monitored prospectively within nineteen months follow-up period.Results There were no significant differences in age and sex between MHD patients and healthy people.The serum IL-6,IL-10,TNF and hsCRP were significantly higher in MHD patients than those in healthy controls [2.38 (1.86-4.69) vs 1.14 (0.27-1.18) ng/L,P＜0.01; 1.47 (1.19-2.10) vs 1.04 (0.00-1.23) ng/L,P＜0.01; 1.33 (1.05-1.56) vs 0.54 (0.00-1.24) ng/L,P＜0.05; 4.90 (1.58-7.45) vs 1.50 (0.63-1.90) mg/L,P=0.01].During the follow-up period,6 patients (20.0％) developed at least one episode of cardiovascular event.Qa,serum IL-6 and hsCRP levels were significantly higher in patients with CVD as compared to those without CVD [(1120±192) vs (893±189) ml/min,P＜0.05; 4.86 (2.96-7.85) vs 2.20 (1.80-3.10) ng/L,P＜ 0.01;11.75 (3.83-31.53) vs 4.45 (1.05-6.68) mg/L,P＜0.05].Binary Logistic regression analysis demonstrated that serum IL-6 was an independent and stronger risk factor for CVD morbidity [HR=1.943,95％CI (1.110-3.402),P=0.02].Spearman rank correlation analysis and liner regression analysis showed that Qa was positively correlated with serum IL-6 (β=0.492,P＜0.01).Path analysis suggested that Qa contributed to CVD mortality via the increase of serum IL-6.Conclusions AVF blood flow monitoring is important for MHD patients.IL-6 is an independent risk factor of CVD in MHD patients.AVF blood flow increases cardiovascular diseases morbidity in MHD patients via its promotion of IL-6 production.

Objective To study the diagnostic and predictive values of amino-terminal pro-B-type natriuretic peptide (NT-proBNP) in patients presenting acute dyspnea. Method A total of 533 patients with dyspnea were studied. According to the clinical characteristics and echocardiographic findings, the patients were divided into two groups, namely acute congestive heart failure group (ACHF) and non-ACHF group. NT-proBNP levels were assayed in all patients in order to evaluate the diagnostic value, and predictive value in patients of ACHF group by following up study to know the clinical destination of patients to be cardiac death or re-admission into hospital. Results There were 272 ACHF patients and 261 non-ACHF patients, and the levels of NT-proBNT were significantly different between two groups ( 2683.4±86.9) pg/mL vs. (238.6+8.7) pg/mL, P＜0.01) . A total of 220 patients were followed for 158 ±32. 8 days. The level of NT-proBNT in myocardium of patients in re-admission group was 2683 + 86. 9 pg/mL and in death group was 3283.4 + 105.7 pg/mL which both were higher than that in patients without cardiac events ( 1123. 5 + 72. 1 pg/mL) ( P ＜0. 01 ) . By using multivariate Cox analysis, log NT-proBNT was ( r = 0. 987, P = 0. 002), and atrial fibrillation (r = 0. 876, P = 0. 005 ) and ventriculat tachycardia ( r=0. 891, P =0. 005) were the valid predictors of cardiac events. Conclusions Routine determination of NT-proBNT in Emergency Department should be useful for quickly sorting patients with acute dyspnea. The NT-proBNT could be used as a good prognostic indicator of ACHF. In addition, log NT-proBNT and atrial fibrillation, ventricular tachycardia were the independent risk factors of cardiac events.

Objective To investigate the clinical value of liver transplantation for intrahepatic cholangiocarcinoma (ICC).Methods A retrospective analysis was performed on 68 patients with ICC who underwent hepatic resection or liver transplantation between January 2007 and September 2013.In the surgical treatment of ICC,overall survival and prognostic risk factors were analyzed to determine the clinical value of liver transplantation for ICC compared with hepatic resection.Results Of the patients with ICC,55 underwent hepatic resection and 13 were given liver transplantation.The 1 and 3-year survival rate was significantly higher in the liver transplantation group than in the hepatic resection group (76.9％ versus 52.7％,and 61.5％ versus 27.3％,P＜0.05).In addition,liver transplantation resulted in higher tumor recurrence-free survival rate than hepatic resection (69.2％ versus 41.8％,and 61.5％ versus 20.0％,P＜0.05).The multivariate analysis showed that tumor size larger than 5 cm was an independent risk factor and HBV infection was an independent protective factor of ICC.Conclusion The prognosis of patients who underwent liver transplantation was better than hepatic resection,suggesting that liver transplantation is perhaps a prior treatment in strictly selected and unresectable patients with ICC.

Objective To evaluate the correlation between CT perfusion blood volume (Lung PBV)and the degree of pulmonary embolism by CT pulmonary angiography (CTPA).Methods 1 65 patients with suspected pulmonary embolism underwent dual-ener-gy CT angiography,then the Lung PBV iodine distribution was analyzed by using a software algorithm.Consistent CTPA results evaluated by two experienced radiologists were taken as standard reference.After two months,the CTPA images were assessed by other two radiologists at random.The position and the number of perfusion defects,and (or)hypoperfusion were recorded.Results Of all 1 65 suspected patients,41 were diagnosed as pulmonary embolism with involved 1 64 segment arteries including part occlusion in 56 and complete occlusion in 108.Lung PBV showed perfusion defect in 141 pulmonary segment including 42 in patients with part occlusion and 99 in ones with complete occlusion.1 7 pulmonary segments were misdiagnosed with part occlusion in 9 and complete ones in 8.23 segments were misdiagnosed including part occlusion in 14 and complete ones in 9.The perfusion defect of Lung PBV was correlated to CTPA moderately or well (to part occlusion with r =0.508 and P =0.022;to complete ones with r =0.905 and P =0.000).Conclusion Dual-energy lung perfusion imaging is helpful for accurate assessment of abnormal blood flow distribution caused by pulmonary embolism,and improve the diagnosis.

Objective To identify the risk factors for osteoporosis in patients with hemiplegia. Methods The total body bone mineral density (BMD) of 366 patients with hemiplegia in the stage of rehabilitation was measured with dual-energy X-ray absorptiometry. The data of the patients were analyzed with multiple logistic regressions. Results The incidence of osteoporosis was 15.3 % (56/366) in the cohort. ≥60 years old (OR=5.434, 95% CI:2.734～10.801), duration of disease (OR=1.046, 95% CI:1.013～1.080), body mass index (BMI) (OR=0.746,95% CI:0.622～0.840),ADL level (OR=0.618, 95% CI: 0.444～0.858) were independently correlated with osteoporosis in patients with hemiplegia. Conclusion Patients with hemiplegia in the stage of rehabilitation were at a high risk of osteoporosis. Aged over 60 years old, lower BMI, longer duration of disease and ADL level were independent risk factors for osteoporosis, which called for supervision and prophylaxis on osteoporosis.

Objective To investigate the changes of bone mineral density (BMD) and bone mass and the influencing factors in patients with hemiplegia.Methods The total BMD, bone mass, lean mass and fat mass of 366 patients (313 stroke cases and 53 TBI cases) with hemiplegia were tested by dual-energy X-ray absorptiometry, as well as the clinical features of all patients, were studied.Results The bone masses in the paretic side (upper limb, lower limb and trunk) decreased significantly compared with nonparetic side ( P<0.01); the differences of bone masses (%) in upper limb, lower limb and trunk between paretic and nonparetic side decreased with duration of disease or trauma ( P<0.01). The multivariable stepwise regression analyses showed that the total bone mass and total BMD were negatively correlated with age and duration of disease significantly (all P<0.01), and positively correlated with body mass index (BMI) significantly ( P<0.01), the total BMD was also positively correlated with lean mass ( P<0.01) and ADL level ( P<0.05).Conclusion The bone mass deceases in the paretic side significantly, and gets worse with the duration of disease. Age, duration of disease or trauma and BMI influence total BMD and bone mass; in addition, total BMD is correlated with total lean mass and ADL level.
