To study the relationship of the polymorphism of the insulin receptor substrate-1(IRS-1) gene 5′-flanking regulatory sequence and Type 2 diabetes,the IRS-1 gene 5′-flanking regulatory sequence was scanned by PCR-SSCP in 78 healthy control subjects and 76 Type 2 diabetic subjects. Applying PCR-denatured polyacrylamide gel electrophoresis and silver staining, the insertion/deletion polymorphism of the CAG-rich region was analyzed. The genome DNA of the normal and variant subjects was amplified with high-fidelity pfu DNA polymerase. The purified and digested target fragments were then subcloned into the pCAT Basic vector. Each allele was identified according to the mobility by the restrictive endonuclease digestion of the recombinant combined with denatured polyacrylamide gel electrophoresis and silver staining, and finally the constructive plasmids containing different alleles were analyzed by DNA sequencing. Firstly, we found several insertion/deletion variations in the CAG-rich region of IRS-1 gene. Secondly,7 genotypes and 6 alleles(T1～T6) in this site were detected.Moreover, T5 and T6 were only observed in Type 2 diabetic group.

Objective To summarize the clinical experience of intracranial wide-necked aneurysms using intracranial stent technique or balloon-assisted technique. Methods Thirty-three ruptured intracranial wide-necked aneurysms were treated by stent-assisted or balloon-assisted technique combined with the use of Microplex coils. The lesions included 8 anterior communicating artery aneurysms, 19 posterior communicating artery aneurysms, 3 middle cerebral artery aneurysms and 3 basilar artery aneurysms. Results Of 19 patients treated with stent-assisted technique and Microplex coils, complete occlusion of the aneurysm was achieved in 13, over 90% occlusion in 4 and (70-90) % occlusion in 2. Among another 14 patients treated with balloon-assisted technique and Microplex coils, complete occlusion of the aneurysm was achieved in 9, over 90% occlusion in 3 and (70-90) % occlusion in 2. Conclusion For the treatment of intracranial wide-necked aneurysms, both the stent-assisted and balloon-assisted techniques are safe and effective. In treating intracranial wide-necked aneurysm, the selection of proper technique should be based on the patient's individual condition.

Objective To investigate the influence of deoxyschizandrin (Deo) on P-glycoprotein (P-gp).Methods The effect of P-gp on Deo (20,40,80 μg·mL-1) was studied in the Caco-2 cell model in vitro,and the apparent permeability coefficient (Papp) of Deo (20-160 μg·mL-1) on a P-gp substrate,rhodamine123 or cyclosporine A,was calculated.Healthy male Sprague-Dawley rats were randomly divided into five groups:blank control group,verapamil group,low-,medium-and high-dose Deo group (8 rats in each group).Rats in the low-,medium-and high-dose Deo group were intragastrically administered once daily with Deo at 8,16 and 32 mg·kg-1 for 3 consecutive days,while rats similarly received gavagewith verapamil (4 mg·kg-1) in the verapamil group and equal volume of purified water in the blank control group.Thirty minutes after the rats were treated with their respective drugs,rhodamine123 (5 mg· kg-1) was orally administrated.Then the pharmacokinetic profiles of rhodamine 123 were analyzed to evaluate the inhibitory ability of Deo on P-gp in vivo.Results The bidirectional transport rates of Deo (20,40,80 μg·mL-1) were similar,with non-selectivity.Deo (20-160 pg·mL-1)significantly inhibited the basolateral→apical(BL→AP) directional transports of rhodamine 123 and cyclosporine A in Caco-2 cell model (P ＜ 0.05) in a concentration-dependent manner.And Deo (8-32 mg· kg-1) also dose-dependently decreased the peak concentrations (Cm.) and the area under the plasma concentration-time curve (AUC0-t) of Rho123.Conclusion Deo can inhibit P-gp in vitro and in vivo,but it is not a P-gp substrate.

OBJECTIVE　To evaluate the availability and safety of a new carbapenem meropenem.METHODS　The randomized study was conducted in 60 patients with respiratory tract infections.They were divided into two groups:meropenem or imipenem/cilastatin.RESULTS　The cure rate,effective rate and bacterial clearance rate for meropenem were 80.0%,93.3%,92.3%,respectively and for those of imipenem/cilastatin were 76.7%,90.0% and 91.3%,respectively.The rate of adverse reactions for both meropenem/cilastatin was 3.3%.CONCLUSION　Meropenem appeared to be a highly effective and safe agent in treatment of respiratory tract infections.Its efficacy and safety are similar to that of imipenem/cilastatin.

Objective To investigate the effect of the locking plate fixation with anti-osteoporosis drug in the treatment of older patients with proximal humeral fractures. Methods Sixty-two patients with proximal humeral fractures who were treated in Yuzhou Mining Company Hospital of Kailuan Group of Hebei Province from Feb. 2011 to Oct. 2013 were divided into observation group(n=30)and control group(n=32). Patients in both groups were given locking plate fixation,and patients in the observer group were also received anti-osteoporosis treatment. The information including hospitalization periods,efficacy and bone mineral density of the two groups was recorded. Results The hospitalization periods of the observation group and control group were (6. 9 ± 1. 0)d,(9. 5 ± 1. 1)d,and the difference was statistically significant(t=8. 426,P﹤0. 05). The efficacy in observer group was excellent in 17 cases,good in 10 cases,fair in 2 cases and poor in 1 case,and the efficacy of the control group was excellent in 14 cases,good in 10 cases,fair in 5 cases and poor in 3 cases. The difference was statistically significant(χ2 =4. 617,P ﹤0. 05 ). After the anti-osteoporosis treatment,the bone mineral density of the observer group and control group were( 0. 79 ± 0. 13)g/cm2 and(0. 66 ± 0. 05)g/cm, the difference was statistically significant( t =10. 251,P ﹤0. 05 ). Conclusion The effect of locking plate fixation with anti-osteoporosis drug is better with shorter hospital stay,safe and reliable,and it is worthy of clinical application.

Objective To investigate the reversal effect of verapamil on the drug resistance of NCI-H 446 small cell lung cancer cell line induced by fractionated radiation.Methods Exponentially grown NCI-H 446 cells were exposed to total radiation dose of 50 Gy,which was given in 25 fractions of 2 Gy per fraction.The survival rate of NCI-H 446 cell line,before and after fractional radiation,was observed after adding different concentrations of mitomycin C(MMC).The change in survival rate of NCI-H 446 cell line was also observed while verapamil was added additionally to the above.Results The survival rate of the radiated cell line was higher than that of the unradiated cell line under the same concentration of MMC(P0.05),or the survival rate of radiated cell line was obviously lower than that of unradiated cell(P

Objective To explore the change of platelet activation before and after stent implantation in acute coronary syndrome. Methods Detection of positive expression of platelet membrane glycoprotein CD62p,CD63 and thrombin-sensitive protein (TSP) were performed using flow cytometry (FCM) in 82 patients which included 45 unstable angina pectoris (UAP) and 37 acute myocardial infarction (AMI). Another 45 angiographically normal patients served as the control. Results Expression of CD62p, CD63 and TSP after stent implantation in ACS group. On the other hand, the expression of the above factors was higher in ACS group compared with the control group. Conclusion Acute thrombogenesis induced by platelet activation is a basic pathophysiologic process in ACS. Endothelial injury and plaque rupture produced by stent implantation may be a cause of platelet activation.

Objective:To investigate the effects of schizandrin B on P- glycoprotein by a classical in vitro cell model. Methods:Caco-2 cell model was used as the carrier,and rhodamine 123 and cyclosporin A were employed as the P-gp substrates, the transmembrane transportation of schizandrin B,rodamine 123 and cyclosporin A were detected by HPLC and a liquid scintillation counting assay,and the apparent permeability coefficient and permeability directional ratio were calculated. Results:The bidirectional transportation rates of schizandrin B(20 μg·ml -1 ,40 μg·ml -1 and 80μg·ml-1 )were similar,and showed non-selective difference. Schizandrin B(20-160 μg ·ml -1 )significantly inhibited the BL → AP directional transportation of rhodamine 123 and cyclosporine A in Caco-2 cell model(P < 0. 05) in a concentration-dependent manner. Conclusion:Schizandrin B is a P-gp inhibitor,while it isn’t a P-gp substrate.

OBJECTIVE To discuss the common problems and their management in the endoscopic nasal dacryocystorhinostomy(DCR) for recurrent dacryocystitis.METHODS A total of 44 patients with recurrent dacryocystitis were performed in our hospital from July,2003 to July,2008.The clinical data of all the cases were studied.All the cases were followed up for over 6 months.RESULTS The common problems met in the operation according to the occurrence frequency were bleeding,difficult for localizing the lacrimal sac,the size and the location of the bony ostium,the size of lacrimal ostium and sinonasal diseases.The cure rate was 93.18%(41/44). CONCLUSION Special instruments,skillful handling and familiar with intranasal anatomy of the lacrimal sac area are the keys to improve the successful rate of endoscopic nasal dacryocystorhinostomy.
