Objective To discuss clinical curative effect and value of valsartan combined with Huangkui capsule on the chronic glomerulonephritis proteinuria.Methods 70 cases of patients with chronic glomerulonephritis proteinuria were selected in our hospital,and randomly divided into two groups,35 cases in control group with valsartan oral,80 mg per time,once per day,8 weeks as a course of treatment;35 cases in experimental group, combined with Huangkui capsule on the basis of conventional treatment orally,5 capsules per time,3 times per day,8 weeks for a period of treatment. After 8 weeks treatment,changes of serum creatinine,serum urea nitrogen,the urine protein and clinical therapeutic effect were compared before and after treatment in two groups.Results Contents of serum urea nitrogen,serum creatinine,urine protein were improved in two groups after treatment, and serum creatinine contents of experimental group (153.69 ±13.78)μmol/L was lower than that of control group (179.19 ±23.49)μmol/L,and the difference was statistically significant (P<0.05);serum urea nitrogen contents of experimental group (7.09 ±1.16)mmol/L was lower than that of control group (9.29 ±2.38)mmol/L,and the difference was statistically significant (P<0.05);urine protein contents of experimental group (0.57 ± 0.32)g/L was lower than that of control group (0.89 ±0.26)g/L,and the difference was statistically significant(P<0.05);clinical curative effect of experimental group was better than that of control group,and the difference was statistically significant(P<0.05 ).Conclusion Valsartan combined with Huangkui capsule could effectively reduce albuminuria symptomsin the treatment of patients with chronic glomerulonephritis proteinuria,which has a remarkable effect,and has a positive significance for the renal function recovery.

目的探讨推拿加腰背肌训练治疗非特异性下腰痛的作用机理。方法200例非特异性下腰痛(NLBP)患者分为4组:空白组、训练组、推拿组、推拿加训练组,各50例。在治疗开始和治疗4周后,对4组患者进行肌酸激酶(CK)、乳酸脱氢酶(LDH)、超氧化物歧化酶(SOD)活性、丙二醛(MDA)含量的测定及目测类比定级法(VAS)评定。结果4组患者的CK、LDH、SOD和MDA含量以及VAS指数均有不同程度的改善。组间比较,推拿加训练组的CK、LDH、SOD和MDA含量以及VAS指数均明显优于其他3组(均P<0.01)。结论中医推拿技术结合主动功能训练治疗NLBP具有明显的康复疗效。

ObjectiveTo determine the role of mammary ductoscopy in the evaluation and intraductal therapy for nipple discharge.MethodsIn this study 3280 patients with nipple discharge underwent mammary ductoscopy. Ductoscopy revealed space occupying lesions in 1018 patients who subsequently underwent surgery; 2260 patients with no-space occupying lesion underwent ductoscopically guided duct lavage.ResultsSpace occupying lesion was often seen in patients with bloody discharge,single duct nipple discharge,discharge in unilateral breast and lesions located in general or levelⅠ - Ⅱ duct.Non-bloody discharge,multiple duct with nipple discharge and bilateral breast involved and lesions at multi-level ducts were common in patients with no-space occupying lesions. Ductoscopically guided duct lavage was curative in 88.72％ among 2260 cases undergoing lavage.ConclusionsSpace occupying lesion should be suspected in patients with bloody discharge,single duct nipple discharge and unilateral breast involving lesion and lesions located in the general or level Ⅰ - Ⅱ duct system;Mammary ductoscopy is useful tool in the intraductal therapy for non-space occupying lesions.

BACKGROUND: The choline acetyltransferase (CHAT) is the key synthetic enzyme for acetylcholine, and is the important symbol of the functional activity of the cholinergic system. There was a relationship of the cholinergic neurons damage with the pathogenesis of Alzheimer disease (AD) and the mild cognitive impairment (MCI). Whether or not the fimbria/fornix transection may affect the expressive variety of ChAT in different cerebral regions of rats (cortex, hippocammpi CA1, amygdala, Meynert nucleus) is very important for the recognition of the pathogenesis of AD and MCI and the establishment of experimental animal model of AD.OBJECTIVE: To observe the expressive variety of ChAT in different cerebral regions of rats with fimbria/fornix transection and discuss exploratorily the methods of simulative experimental AD.DESIGN: A randomized and control study.SETTING: Institute of Cerebrovascular Disease, Affiliated Hospital of the Qingdao University Medical College and the Department of Neurology, No. 1People' s Hospital of Jining.MATERIALS: The experiment was completed in the Institute of Cerebrovascular Disease, Affiliated Hospital of the Qingdao University Medical College from March to December 2003. Totally 14 adult healthy female Wistar rats aged 5 months were randomly divided into model group and control group with 7 in each group.METHODS: ① The bilateral fimbria-fornix of brain in model group were transected on the stereotaxic apparatus to set animal model. After opening the skull, at the coordinates, along the bregma posterior 2.2 mm-2.5 mm and lateral 1.0 mm according to the atlas of Paxinos and Watson, and the dorsal fornix, the anterior part of hippocampus and the fimbria were cut off with a double blade under visual inspection. Rats in the control group were not performed with fimbria/fornix transection, and the other procedures were completed as those in the model group. ② On the 28 day after the surgery, all rats were killed under anesthesia to take out the brain tissues and make coronary sections for histochemical observation in a immunohistochemical way. The expressions of the ChAT positive neurons were observed in the cortex, hippocammpi CA1, amygdala, Meynert nucleus, and the brown neurons under microscopy was the ChAT positive neurons.MAIN OUTCOME MEASURES: Expressions of ChAT positive neurons in the cortex, hippocammpi CA1, amygdala, Meynert of the basal forebrain in the model group and the control group.RESULTS: All the rats entered the final analysis without any loss. The expression of ChAT positive neurons in the cortex, hippocammpi CA1,amygdala, Meynert of the basal forebrain in the model group were distinctly decreased than that in the control group (2.97±1.45, 32.60±7.33, t=10.51,P ＜ 0.01); (6.83±2.41, 50.57±5.85, t=1 8.30, P＜ 0.01); (14.43±6.75, 35.43±10.49,t=4.47, P ＜ 0.01); (5.77±6.62, 48.77±7.10, t=1 1.72, P ＜ 0.01), and the differences were significant.CONCLUSION: Finbria/fomix transection can decrease the expression of ChAT positive neurons in many cerebral regions of rats, and can be used in a method of setting the experimental animal model of AD.

Objective To study the diagnosis and treatment of coincident vesical transitional cell carcinoma and prostatic cancer. Methods 8 cases of coincident vesical transitional cell carcinoma and prostatic cancer were evaluated clinically. Results All the 8 were diagnosed as vesical transitional cell carcinoma on cystoscopy and biopsy.Whereas on needle biopsy of the prostate,prostatic cancer was diagnosed in 7 and BPH in 1 but turned to be prostatic cancer on pathological study after cystoprostatectomy.4 patients underwent transurethral resection of the bladder cancer and bilateral orchiectomy with bladder instillation of MMC or BCG and oral flutamide for prostatic cancer.1 patient underwent partial cystectomy,bilateral orchiectomy and external radiotherapy for prostatic cancer.3 underwent radical cystoprostatectomy.2 cases were lost to follow up.In the other 6,3 survived less than 1 year because of wide metastasis.The other 3 have been followed up for 1.5 to 4.0 years,remained well and tumor free. Conclusions PSA detection together with rectal palpation,biopsy,transrectal ultrasonography and cystoscopy played the important role in the diagnosis of coincident vesical transitional cell carcinoma and prostatic cancer.Radical cytoprostatectomy yields a better outcome.

Objective To evaluate intravesical instillation of mitoxantrone(MTZ) for the prevention of postoperative recurrence of bladder cancer. Methods Intravesical instillation of 12 mg MTZ dissolved in 50 ml normal saline and retained in bladder for 2 hours were given to bladder cancer patients postoperatively once a week for 8 weeks and subsequently once a month for 12 months.Kidney and liver function,blood counting,urinalysis and cystoscopy were taken periodically.The systemic and local reactions were recorded every time after the intravesical instillation. Results All of the 98 cases were followed up for 6 to 24 months with a mean of 13 months.The recurrence rate was 6.2%.There was no generalized side effect. Conclusions Intravesical instillation of MTZ was efficacious for the prevention of postoperative recurrence of superficial bladder cancer with safety and less side effect.So it can be widely used in such patients.

Objective To compare 2 approaches with different culture media which induce hyphal form of Candida albicans.Methods Induction of hyphal form was conducted for 16 C.albicans strains with either RPMI 1640 medium or DMEM medium,at 37 ℃ for 24 h,respectively.The hyphal and yeast forms were counted separately and the ratio of hyphal form to total cells was calculated.Results The ratio of hyphal form to total cells was higher in RPMI 1640 medium than that in DMEM medium at the same incubation time for the majority of strains.The ratio was above 99% for all strains after 7-day incubation with 12 times of passages in RPMI 1640 medium at 37 ℃.Moreover the ratio of hyphal form was significantly higher for fluconazole-susceptible strains than that for fluconazole dose-dependent susceptible and resistant strains in incubation with DMEM medium at 37 ℃.Conclusion Incubation with RPMI 1640 medium at 37 ℃ for 7 days seems a favorable condition to induce hyphal form of C.albicans.

Objective To discuss the surgical interference of chronic suppurative otitis media and it's long-term results.Methods In 113 cases of mastoidectomy with tympanoplasty followed up for 3 years,We discuss the results of recurrence and hearing levle of the open-mastoidectomy with tympanoplasty(OMT)and combined approach tympanoplasty(CAT).Results There is no significant difference between OMT(improved 12 dB HL)and CAT(improved 9.5 dB HL)in the improved hearing threshold leve.The recurrence rate of OMT was 5.8%,but the CAT was 24.4%,the difference is significant.Conclusion The effect of OMT is better than that of CAT for the long-term resulls in these cases.

Objective To study the effects of intrathecally administered morphine and ketamine on nitric oxide synthase (NOS) activity and nitric oxide content in the spinal cord.Methods Thirty-two male Wistar rats weighing 220-260 g were anesthetized with intraperitoneal chloral hydrate 300 mg?kg-1 . A catheter was implanted in subarachnoid space at lumbal region. Sciatic constriction injury (SCI) was produced by loose ligation of right sciatic nerve trunk with 4-0 cutgut. On the 4th postoperative day the animals were randomly divided into four groups of 8 animals :(1) control group (C); (2) morphine group (M);(3) ketamine group (K) and morphine-ketamine group (KM) . Morphine 20 ?g / ketamine 20 ?g / morphine 10 ?g + ketamine 10 ?g were injected intrathecally every day for 7 consecutive days in group M, K and KM. In control group normal saline was injected intrathecally instead of morphine and / or ketamine. The withdrawal latenvies to radiant heat focused on plantar surface were measured as radiant heat threshold before intrathecal administration of the analgesic (baseline) and 30 min after intrathecal administration of ketamine and / or morphine every day for 7consecutive days. The percentage of maximal possible effect ( MPE % ) was calculated : MPE % = (latency after intrathecal administration-baseline latency) / (radiant heat cut-off time-baseline latency) X 100% . After 7 days of intrathecal administration the animals were decapitated and the spinal cord was immediately removed and the lumbal spinal cord was dissected on ice. NOS activity and nitric oxide ( NO) content were measured by spectrophotometry. Results MPE % was significantly higher in group M and KM than in group C and K ( P
