1.Status quo research on the information literacy of the young faculties in medical Universities and corresponding promotion strategies——Based on a survey in Peking University Health Science Center

As the new force among university faculties,the young faculties are the backbone of teaching and research activities in universities and the main information users concerned for information access.To understand the actual condition of information literacy of young faculties in Peking University Health Science Center and grasp their difficulties in getting access to information,this study analyses the information literacy of young faculties through investigating the status quo of their information capabilities,gathering their suggestions and opinions to enhance information capabilities and retrieval tests conducted among small-scale young faculties.The survey results show that 72% of young faculties don't know or know little about the four biomedical literature retrieval databases and 47% of them knowing nothing about CALIS.This suggests we should take measures to promote the information literacy of young faculties.
2.Comparison between effects of AMP579 and adenosine on Na+/Ca2+ exchanger in isolated rat ventricular myocytes

AIMTo elucidate possible mechanisms underlying the differences between 1S-[1a,2b,3b,4a(S*)]-4-[7-[[1-(3-chloro-2-thienyl)methylpropyl]propyl-amino]-3H-imidazo[4,5-b]pyridyl-3-yl]-N-ethyl-2,3-dihydroxycyclopentane carboxamide (AMP- 579) and adenosine in pharmacological and clinical effects. METHODSNa+/Ca2+ exchange current was recorded by patch-clamp technique in whole-cell configuration. RESULTSAMP579 significantly enhanced both outward and inward Na+/Ca2+ exchange currents in a concentration dependent manner. Neither infusion of an adenosine A1 receptor antagonist PD116948 30 μmol*L-1 or an adenosine A2 receptor antagonist DMPX 10 μmol*L-1 nor a protein kinase A special blocker KT 5720 0.2 μmol*L-1 or a protein kinase C special blocker GF 109203X 0.4 μmol*L-1 had effect on Na+/Ca2+exchange current increased by AMP579, suggesting that AMP579 possess a direct activating effect on Na+/Ca2+ exchange current. CONCLUSIONAMP579 possibly possesses a direct activating effect on Na+/Ca2+ exchange current.
3.Impact of anti-hypertension therapy on seasonal variability of blood pressure, urinary 8-OHdG levels in essential hypertension

Objective To investigate anti-hypertension threapy on seasonal variability of blood pressure,urinary 8-OHdG levels in essential hypertension in order to provide a basis for seasonal antihypertensive treatment.Methods Fifty hypertensive patients admitted the hospital of Erdos Clinical Medical College of Inner Mongolia Medical University at summer 2013 were selected as our subjects.The final subjects was 42 cases due to 8 lost cases.The patients were randomly divided into two groups,including 30 cases in renin angiotensin aldosterone system inhibitors(RASI) group and 12 cases in Ca2+ channel blocker(CCB) group.Epidata 3.1 software was applied to perform statistic analysis.Urinary 8-OHdG concentration was measured by enzyme-linked immunosorbent assay (ELISA).Blood pressure was measured in spring,summer,autumn and winter.Results Systolic blood pressure(SBP) in RASI group and CCB group at winter periods were (158±20) mmHg,(158 ± 20) mmHg,higher than that in summer periods ((145 ± 12) mmHg,(141 ± 9) mmHg;P< 0.05).Diastolic blood pressure(DPB) in RASI group and CCB group at winter periods were (101 ± 13)mmHg and (100±4)mmHg,significant high than that in summer periods ((93 ±7) mmHg,(90±7) mmHg;P<0.05).8-OhdG levels in RASI group at summer and autumn periods were (243.20±30.94) ng/L and (240.40±47.99) ng/L,significantly higher than that in winter and spring periods((190.80± 15.56) ng/L and (189.06± 13.56) ng/L),and the differences were significant(P<0.001).No significant differences were seen in CCB group among 4 seasons in terms of 8-OhdG levels (P > 0.05).Conclusion Blood pressure change still occur among 4 seasons in hypertensive patients after a single CCB containing RASI-based drug antihypertensive therapy.And blood pressure in winter periods is higher than that in summer,which indicates that therapy medication based on RASI might reduce the level of oxidative stress at winter periods.
4.Risk factors for mortality in patients with severe fever with thrombocytopenia syndrome

Objective To analyze the clinical features and risk factors for mortality of patients with severe fever with thrombocytopenia syndrome (SFTS) in Zhoushan, the eastern coastal of China with high incidence of severe fever with thrambocytopenia syndrome bunyavirus infection, to provide reference for reducing the mortality rate of SFTS.Methods Clinical data of 107 cases of SFTS from Zhoushan Hospital during June 2011 to June 2016 were retrospectively analyzed.According to the prognosis, patients were divided into survival group and death group.The clinical features and the laboratory results were analyzed with a case-control method to analyze the prognostic factors.Normal distribution data were compared with the independent t test.Kolmogorov-Smirnov Z test were used in data with skewness distribution.Categorical data were analyze by chi-square test.The related risk factors were analyzed with the receiver-operating characteristic (ROC) curve and multivariate unconditioned logistics regression analysis.Results Seventeen cases among 107 STFs patients died, yielding the mortality rate of 15.9%.The proportion of patients suffering from two or more underlying diseases, with disorders of consciousness, activated partial thromboplastin time (APTT), the level of creatine kinase (CK), lactate dehydrogenase (LDH) as well as sepsis-related or sequential organ failure assessment (SOFA) score in death group were all significantly higher than those in the survival group (all P<0.05).The Ca2+ level and fibrinogen level in death group were significantly lower than those in the survival group (both P<0.05).Indexes mentioned above were analyzed by ROC curve, and the calculated cut-off value was set as the optimal diagnostic thresholds.These data were then included into the multivariate logistic regression analysis.It turned out that Ca2+<1.625 mmol/L, APTT >73.45 s, SOFA scores >9 were the independent risk factors for mortality of SFTS (OR=6.947, 8.459 and 11.770, respectively, all P<0.05).Conclusion Ca2+, APTT and SOFA score are the independent risk factors for prognosis of SFTS, which provide reference for prognostic evaluation of SFTS.
5.Quality assurance of computerized system for GLP laboratory

In view of the characteristics of the computerized system,the key points in the quality assurance (QA) of the computerized system was discussed and summarized combined with the requirements of the GLP laboratory in Europe and America.The validation of computerized system,the control during the use of computerized system,period maintenance and safety protection of computerized system,archives of electronic data was discussed,expecting to provide reference for the management of computerized system in Chinese GLP laboratory which is generally not high currently.The experiences were obtained as follow:Through repeated inspection and review,the problem was found and set as the risk point;a targeted QA inspection plan was made focusing on the risk-based inspection and the QA inspection plan was timely adjusted according to the problems,which ensures the pertinence and validity of the QA inspection.

There are some shortcomings of animal experiments applied in chemical toxicity testing, such as long period, large cost, species differences and dose differences, which limit the use of animal experiments' results in predicting human toxicity.Accordingly, we established a toxicity testing alternative screening system in line with the toxicity endpoints which required in chemical safety evaluation and risk assessment (genotoxicity, carcinogenicity, reproductive toxicity, acute toxicity and general toxicity) based on 3R principles (replacement, reduction, refinement) for animal experiments.This system covers most of the endpoints of toxicity assessment, and molecular biology technology was also applied to integrate the toxicity test, as well as some operation was optimized in order to shorten the experimental period, reduce experimental costs, improve animal welfare.Furthermore, the results from the screening system have higher clinical relevance because it is based on the toxicity mechanisms.
7.The role of transbronchial needle aspiration in the staging of bronchogenic carcinoma.

BACKGROUNDTo explore the role of transbronchial needle aspiration (TBNA) in the staging of bronchogenic carcinoma.
METHODSTo 42 cases of primary bronchogenic carcinoma with suspected lymph node metastasis by X-ray and CT scan of chest, the TBNA was performed before operation. The cytological results and c-TNM by TBNA were compared with the pathological ones and p-TNM after operation.
RESULTSThe diagnosis of 10 cases with N₂ metastasis was completely corresponding by TBNA and pathological examination after operation. Nineteen out of 22 cases with N₁ metastasis were confirmed by TBNA, and the false negative results ocurred in 3 cases. The results of TBNA in lymph nodes' size from 2 to 3 cm was completely accordant with pathological ones after operation. For 1 to 2 cm lymphnodes, the accurate rate of TBNA was 88.5% (23/26). The overall accurate rate of c-TNM by TBNA was 85.7% (36/42) compared with p-TNM. A small amount of hemoptysis ocurred in 3 cases, no pneumothorax and other serious complications were observed.
CONCLUSIONSThe TBNA for staging of bronchogenic carcinoma is a simple and economic method with high correct rate and high clinical applicable value.
8.Reconstruction of the defect of cervical trachea with the sternocleidomastoid myoperiosteal flap.

OBJECTIVE:
To study application of sternocleidomastoid myoperiosteal flap to reconstruction of the defect of cervical trachea.
METHOD:
Thirteen hospitalized patients with malignant neoplasm invading cervical trachea and primary tracheal carcinoma were analyzed retrospectively, the sternocleidomastoid myoperiosteal flap was applied to repair the defect of tracheal wall after resection of the neoplasm.
RESULT:
Among 10 tracheotomy patients, 9 cases were decannulated from 1 month to 5 months. One case needs cannulation because of tracheal stenosis. Three patients with no tracheotomy had no dyspnea after operation.
CONCLUSION
The sternocleidomastoid myoperiosteal flap is an ideal transplant for cervical tracheal reconstruction.

9.The value of transesophageal echocardiography to guide the implantation of 2 pieces of MitraClip during transcatheter mitral valve repair operation.

OBJECTIVETo investigate the value of transesophageal echocardiography to guide the implantation of 2 pieces of MitraClip during transcatheter mitral valve repair operation.
METHODSFrom October 2013 to June 2014, 6 transcatheter mitral valve repair operations were performed in our hospital for symptomatic patients with severe functional mitral regurgitation (MR), transesophageal echocardiography was applied to guide the implantation of 2 pieces of MitraClip. Clinical data are retrospectively analyzed to evaluate implantation timing and approach of the 2nd piece of MitraClip, as well as the immediate effect of the interventional therapy.
RESULTSAfter implantation of 1st piece of MitraClip, transesophageal echocardiography evidenced MR ≥ grade 2 with central regurgitation and immediate mitral average transvalvular pressure gradient < 3 mmHg (1 mmHg = 0.133 kPa) in these 6 patients and 2nd piece of MitraClip was implanted in these patients. After implantation of 2nd piece of MitraClip, it is observed via transesophageal echocardiography that mitral regurgitations were reduced by ≥ 2 grades for all 6 patients. For 3 patients, MR was reduced to grade 1. For the other 3 patients, MR is reduced to grade 2. Among the 3 patients whose MR was reduced to grade 2, 2 operations were stopped because immediate mitral average transvalvular pressure gradient equaled to 3 mmHg, and the rest 1 operation was stopped because MR was too diverse and not able to select proper position to implant the next MitraClip. All 6 operations are completed successfully.There were no myocardial infarction, death or complications requiring mitral valve surgery after the MitraClip procedure. There were also no MitraClip detachment, thrombus embolism, mitral valve apparatus injuries, mitral stenosis, pericardial tamponade post procedure.
CONCLUSIONSTransesophageal echocardiography plays an important role to guide the implantation of 2 pieces of MitraClip in transcatheter mitral valve repair operation. Mitral average transvalvular pressure gradient and initial position of regurgitation after implantation of the previous MitraClip are critical determinants for decision making if the next piece of MitraClip can be implanted or not.
