1.Biomechanical effect of sacroiliac screw fixation with different needling, points on extorsion unsta-hie pelvis

Objective To observe the biomechanical effect of sacroiliac screw fixation with dif-ferent needling points in treatment of extorsion pelvic instabihty so as to provide experimental basis for clinical application. Methods Ten fresh adult pelvis specimens were fixated by sacroiliac screw with needling points B (2.5 cm anterior to posteroinferior iliac spine and 40. cm posterior to greater sciatic notch), A (0.1 cm anterior to the same horizontal plane of point B) and C ( 1.0 cm posterior to the same horinzontal plane of point B). The shifting of the pubic symphysis diastasis and the sacroiliac joint crack under the same extorsion force was measured in integrity ligament group, point A group, point B group and point C group to compare the stability of different needling points. Results In confronting extor-sion force, integrity ligament group and point C group were obviously inferior to point A group and point B group (P<0.05 ) but there was no statistical difference between point B group and point A group (P > 0.05). As for the stability of the sacroiliac joint, there was statistical difference among all groups ( P < 0.05 ), while there was no statistical difference between integrity ligament group and point B group ( P > 0.05). Conclusion Point B is the most stable, safe and effective needling point.
2.Effect of Combined Pentoxifylline and Albendazole against Echinococcus multilocularis Infection in Mice

Objective To observe the effect of pentoxifylline(PTX),albendazole(ABZ),and a combination of PTX and ABZ in mice infected with Echinococcus multilocularis(E.M).Methods The first part of the experiment was to observe the in vitro effect of PTX on the cultured E.M protoscolex.In the second part,mice were infected by abdominal inoculation of E.M and divided into groups given by ABZ 50 mg/kg?d,PTX 360 mg/kg?d,PTX 180mg/kg?d,and a combined regimen ABZ 50 mg/(kg?d)+PTX 180 mg/(kg?d).Another infected group and a uninfected group served as controls which received normal saline only.100 days post-treatment,the mice were sacrificed for further observation.Indicators included wet weight of the cyst,cyst inhibition rate,level of serum cytokines TGF-? determined by ELISA,IL-2 and IL-10 determined by radio-immunoassay(RIA).Results The inhibition rate on cysts of the combined ABZ and PTX was 88%,considerably higher than 58 % of the group ABZ.The serum TGF-? and IL-10 decreased and IL-2 increased after treatment in comparison to the controls.Conclusion The PTX and ABZ combination shows better effect on E.multilocularis infection than that of single ABZ.PTX might help increase immunity of the mice.
3.Biomechanical study on bone marrow-derived mesenchymal stem cells promoting tendon-bone healing following anterior cruciate ligament reconstruction

Objective To observe the effect of bone marrow-derived mesenchymal stem cells (bMSCs) on graft healing within a bone tunnel after anterior cruciate ligament (ACL) reconstruction in rabbits.Methods The study involved 24 New Zealand white rabbits undergone ACL reconstruction with an autologous ipsilateral gastrocnemius tendon graft.Both hindlimbs were included.In one hindlimb,graft coated with fibrin glue compound by bMSCs was employed (bMSCs group).Whereas in the contralateral hindlimb,graft coated with fibrin glue without cells was employed (control group).At postoperative 2,4,6 and 8 weeks,specimens were harvested to have a biomechanical test of tensile strength and stiffness of tendon-bone interface.Results Tensile strength and stiffness of tendon-bone interface in both experiment and control groups presented a rising trend with the prolong of repair time.In contrast,significantly higher tensile strength and stiffness of tendon-bone interface were observed in experiment group since the 6 weeks (P < 0.05).Conclusion bMSCs transplantation significantly enhances the early tensile strength and stiffness at tendon-bone interface after ACL reconstruction in rabbits and improves the graft healing within a bone tunnel.
4.Interventional therapy for left iliac vein stenosis or occlusion

Objective To evaluate interventional therapy for left iliac vein stenosis or occlusion (left iliac vein compressed syndrome or Cokkett syndrome).Method In this study 316 cases received interventional therapy for left iliac common vein lesions,189 cases underwent staged saphenous vein high ligation and stripping,and 82 cases received external prosthetic sleeve valvuloplasty.Two hundred and thirty-one cases were followed up from 6 to 120 months(average 52 months),with all cases being evaluated by colour ultrasound,and among those 116 cases received venography.Results The stenosis or occlusive segments of left iliac vein were successfully dilated in 305 cases,of which 272 cases received stents implanting therapy.There was no inhospital mortality nor pulmonary embolism.According to following up results.the symptom of varicose vein disappeared in about 95.8%patients.and the symptom of leg swelling disappeared or dramatically relieved in about 95.7%cases.among which 74%leg ulcers were completely healed.Stents were occluded by thrombus during venography in about 5.4%. Conclusions The long term result of interventional therapy is satisfactory for left iliac vein stenosis or occlusion caused by Cockett syndrome.
5.The trend and value of 18F-FDG PET/CT included in the criteria for liver transplantation in hepatocellular carcinoma

Liver transplantation is an effective method for the treatment of hepatocellular carcinoma (HCC). In order to reduce the high recurrence rate of tumor after liver transplantation for HCC, some scholars put forward the famous Milan criteria. Since the Milan criteria are too strict, some HCC patients with relatively “good biological behavior” and large lesions or multiple nodules are excluded from the waiting list for liver transplantation, and thus a large number of “expanded versions of the Milan criteria” appeared around the world. As for the histopathology of HCC, microvascular invasion (MVI) and poorly differentiated tumor tissue are significantly associated with the high recurrence rate after liver transplantation for HCC. This article reviews and summarizes the articles on the application of 18F-FDG PET/CT in liver transplantation for HCC in China and foreign countries and points out that the uptake of 18F-FDG in HCC lesions reflects the difference in the biological behavior (i.e., invasion) of tumor tissue. The intense uptake of 18F-FDG is positively correlated with MVI and poor differentiation of HCC. In addition, 18F-FDG can detect extrahepatic metastatic lesions sensitively and accurately. Preoperative 18F-FDG PET/CT findings have a high value in predicting the prognosis of liver transplantation for HCC, and it is a trend to incorporate such findings into the criteria for liver transplantation in HCC. It is also expected to unify the various expanded versions of the Milan criteria. The new criteria for liver transplantation may be defined as follows: the Milan criteria should be followed in general; as for the patients who do not meet the Milan criteria, liver transplantation can be performed for those who have lesions with negative 18F-FDG PET/CT results, without the involvement of major blood vessels or extrahepatic metastasis.
6. Role of short-term starvation in alleviating hepatic ischemia-reperfusion injury in mice and possible mechanism of action

Objective:
To investigate the role of short-term starvation (STS) in alleviating hepatic ischemia-reperfusion injury in mice and possible mechanism of action.
Methods:
Wild-type male C57BL/6 mice aged 8 weeks were randomly divided into 75% hepatic ischemia-reperfusion injury group (IR group), STS+75% hepatic ischemia-reperfusion injury group (STS group), and sirtinol+STS+75% hepatic ischemia-reperfusion injury group (SIR group), using a random number table, and sham-operation groups (IR-Sham group, STS-Sham group, and SIR-Sham group) were also established. The serum levels of alanine aminotransferase (ALT) and aspartate aminotransferase (AST) were measured, and the histomorphological changes of the liver were observed, as well as the expression of Sirt1, LC3B, and P62 proteins in liver tissue and the results of LC3B fluorescence staining. An analysis of variance was used for comparison of data between multiple groups, and the t-test was used for comparison of data between two groups.
Results:

7.Antitumor and Apoptosis Induction Effects of Paeonol on Mice Bearing EMT6 Breast Carcinoma.

Paeonol is a major phenolic micromolecular component of Moutan cortex Radicis, a traditional Chinese Medicine. It has shown antitumor effects in previous studies; however, the underlying mechanisms remain unknown. This study investigated the mechanism by giving treatments of placebo, cyclophosphamide, paeonol of 150 and 300 mg/kg to 4 groups of mice bearing EMT6 breast cancer. Apoptosis in tumor cells were confirmed by morphology analysis, including hematoxylin, eosin staining and TUNEL staining. The results showed that the weight of EMT6 breast tumor was significantly reduced in the groups treated with both 150 and 300 mg/kg of paeonol. Immunohistochemical and Western blot results showed that the expression of Bcl-2 was down-regulated while the expression of Bax, caspase 8 and caspase 3 was up-regulated respectively. These results suggest that paeonol exhibits antitumor effects and the mechanism of the inhibition is via induction of apoptosis, regulation of Bcl-2 and Bax expression, and activation of caspase 8 and caspase 3.

8. Survival time and influencing factors in multidrug-resistant tuberculosis patients in Wuhan, 2006-2014

Objective:
To investigate the survival time of multidrug-resistant tuberculosis (MDR-TB) patients and the influential factors in Wuhan.
Methods:
The relevant information were collected from TB management information system, cause of death reporting system and medical records by trained doctors. The univariate and multivariate Cox proportional hazards model were applied to analyze the factors affecting survival time of patients.
Results:
A total of 552 patients with MDR-TB were included in the analysis. After the diagnosis of MDR-TB, the cumulative survival rates from the first year to the third year were 0.94, 0.88, and 0.80, respectively. The mortality density of MDR-TB patients was 6.52/100 person-years, and the median survival time was (89.52±1.85) months. Kaplan-Meier analysis showed that the cumulative survival rate of the standardized treatment group was significantly higher than that of the non-standardized treatment group (
9.Risk factors of depression and anxiety in epilepsy patients and their relationship with quality of life

Objective To explore the risk factors of depression and anxiety in adult patients with epilepsy and their relationship with quality of life.Methods From May 2022 to January 2023,patients diagnosed with epilepsy(aged≥18 years)in the department of neurology of our hospital were collected.General demographic data and disease-related information were recorded.Quality of life,depression and anxiety scales were measured in all patients.SPSS26.0 software was used for multiple linear regression analysis,multiple ordered Logistic regression analysis,rank sum test,Pearson correlation analysis,etc.Results Among the 111 patients,49.5%had depression and 43.2%had anxiety.Depression score and anxiety score were correlated with attack type,attack frequency,quality of life and right temporal lobe,and there was a significant negative correlation between life quality score and anxiety and depression score(P<0.01).Seizure frequency,seizure type and right temporal lobe were common risk factors for depression and anxiety in patients with epilepsy(P<0.05).Conclusion Epileptic depression and anxiety were affected by seizure frequency and seizure type,and this bad mood further affected the quality of life of patients.No clear link has been found between the lateralization of seizures and the presence of depression and anxiety states,and further research is needed.
10.Modified Shuyuwan Ameliorates Depressive Behaviors and Promotes Myelin Regeneration in Mouse Model of Vascular Dementia Complicated with Depression by Regulating Energy Metabolism of Myelin Axons via MCT1

ObjectiveTo explore the mechanism of modified Shuyuwan in treating vascular dementia (VaD) complicated with depression in mice. MethodThe VaD model was established by bilateral carotid artery stenosis (BCAS) in seven 3-month-old male C57/BL6 mice. The regional cerebral blood flow (rCBF) of mice was measured by laser speckle imaging before and after BCAS surgery. Then, the BCAS method was combined with chronic unpredictable mild stress (CUMS) to establish a mouse model of VaD complicated with depression. BCAS/CUMS mice were assigned into BCAS/CUMS, fluoxetine (0.01 g·kg-1), and high-, medium-, and low-dose (20, 10, 5 g·kg-1, respectively) modified Shuyuwan groups. The shame group underwent sham operation without CUMS (n=10). The tail suspension test and sucrose preference test were carried out to examine the depressive behaviors of mice. The distribution and expression of myelin-associated glycoprotein (MAG), myelin basic protein (MBP), neurofilament heavy polypeptide (NF200), and anti-non-phosphorylated neurofilament epitope antibody (SMI32) in the corpus callosum (CC) were detected by the immunofluorescence assay. Western blot was employed to determine the protein levels of monocarboxylate transporter 1 (MCT1), MBP, MAG, oligodendrocyte glycoprotein (MOG), amyloid precursor protein (APP), NF200, contactin-associated protein (Caspr), and voltage-gated sodium channel (Nav1.6) in the corpus callosum. The level of lactic acid in the serum was measured by the lactic acid assay kit, and the ultrastructure of myelin was observed by ultraprojective electron microscope. ResultLaser speckle imaging showed that rCBF decreased immediately 10 min after BCAS surgery (P<0.01), and the rCBF was still cerebral hypoperfusion and did not return to the preoperative level 2 weeks after surgery. Behavioral test results showed that compared with the sham group, the BCAS/CUMS group presented decreased percentage of sucrose preference (P<0.01) and prolonged immobile time in the tail suspension test (P<0.01). Compared with the BCAS/CUMS group, fluoxetine and modified Shuyuwan increased the percentage of sucrose preference (P<0.01) and shortened the immobile time in the tail suspension test (P<0.01). The level of lactic acid was the highest in the BCAS/CUMS mice (P<0.01), and modified Shuyuwan lowered the lactic acid level (P<0.01). Immunofluorescence results showed that compared with the sham group, the BCAS/CUMS group presented decreased fluorescence intensity of MAG, MBP and NF200 and increased fluorescence intensity of SMI32 in the corpus callosum, and such changes were reversed by modified Shuyuwan at different doses and fluoxetine. Western blot results showed that compared with the sham group, the BCAS/CUMS modeling down-regulated the protein levels of MCT1, MBP, MOG, MAG, NF20, and Caspr (P<0.05, P<0.01) and up-regulated the protein levels of APP and Nav1.6 in the corpus callosum, and the above trends were reversed by modified Shuyuwan (P<0.05, P<0.01). Compared with the sham group, BCAS/CUMS modeling led to myelin ultrastructure damage and axon atrophy, which were alleviated by modified Shuyuwan. ConclusionModified Shuyuwan can ameliorate the transport disorder of lactic acid between myelin sheath and axon by upregulating the expressin of MCT1, promote the regeneration of myelin sheath in the corpus callosum, and improve the integrity of myelin sheath structure, thereby alleviating depression in VaD mice.
