1.Application of hyperbaric oxygen in the treatment of refractory peptic ulcer

Objective To explore the application effect of hyperbaric oxygen ( HBO ) in the treatment of refractory peptic ulcer .Methods Clinical data of 259 patients with refractory peptic ulcer were retrospectively ana-lyzed.According to whether received HBO therapy , these patients were divided into the HBO group ( n=127 ) and control group(n=132).The total effective rate,Helicobacter pylori(Hp) eradication rate and relapse rate of the two groups were compared .Results The total effective rate ,Hp eradication rate of the HBO group were 93.7%,92.1%, respectively,which were significantly higher than 65.2%,56.1% of the control group (χ2 =31.97,7.32,all P<0.05).The relapse rate of the HBO group was 8.4%,which was significantly lower than 33.3%of the control group (χ2 =12.51,P<0.05).Conclusion HBO combined with conventional drug in the treatment of refractory peptic ulcer has significant effect .
2.The clinical evaluation of circumferential pulmonary vein isolation in cases with persistent atrial fibrillation

Objective To evaluate the safety and effectiveness of circumferential pulmonary vein(PV) isolation in cases with persistent/permanent atrial fibrillation(AF) guided by the EnSite-NavX system.Methods Sixty cases(43 males,mean age 59.2?13.6 years) with persistent/permanent AF were enrolled from September 2004 to August 2005.The mean duration of AF was 4?6 years(6 months-24 years).The mean left atrium diameter was 43.2?6.4(42-58)mm.Circumferential PV isolation was performed guided by the EnSite-NavX system,combined with linear ablation of the superior-posterior wall of left atrium and mitral isthmus.Results All 60 cases underwent the procedure successfully,with the mean procedural time 170?34(150-240)min and the mean fluoroscopic time 23?10(12-45)min.The pulmonary vein isolation rate for the left ipsilateral PVs was 83.3% and was 78.3% for the right PVs.Segmental ostial ablation was applied for the rest cases.AF was terminated in 7 cases and was converted to atrial tachycardia(AT) or atrial flutter(AFL) in 5 cases.Sinus rhythm was restored by cardioversion in 48 cases.Post-ablation AT/AFL was documented in 10 cases (16.7%) which diminished automatically in 8 cases,and was abolished by re-ablation in 2 cases.After a mean of 6.5?3.2(4-11)months of follow-up,43 cases (71.7%) were free of AF(18 cases with anti-arrhythmic drugs).Complications: pseudo-aneurysm of femoral artery was found in 1 case and was cured after proper treatment.Conclusion Circumferential PV isolation and combination with left atrial linear ablation guided by the EnSite-NavX system was safe and effective for eliminating persistent/permanent AF.
3.The investigation on the correlation between plasma D-dimer with delayed encephalopathy in patients with acute carbon monoxide poisioning

Objective To investigate the correlation between plasma D-dimer with delayed encephalopathy (DE) in patients with acute carbon monoxide poisioning(ACOP).Methods According to the severity of disease,100 patients with ACOP were divided into mild group (27 cases),moderate group (31 cases),severe group (42cases).And depending on whether DE occured,all patients were divided into DE group(20 cases) and no DE group (80 cases).4 venous blood samples were obtained separately from each subject in 1,7,14 days after their admission to hospital by using latex immunoturbidimetry.The levels of blood plasma D-dimer of patients in each group were detected by immune turbidimetry and analyzed,1th,7th and 14th day after admission.Results The plasma D-dimer level of the mild group at different time points was in the normal range,and the difference was not statistically significant(all P > 0.05) ;The plasma D-dimer level of the severe group and moderate group was significantly higher than that of the mild group(all P <0.01),and it reached a peak in one week(7 days),and showed a downward trend after two weeks(14 days).The DE all occurred in the moderate and severe group.The plasma D-dimer levels of the DE group were higher than those of no DE group from admission to the 7 th and 14 th day (all P < 0.01).Conclusion The dynamic monitoring plasma D-dimer is helpful in assessing to the severity and prognosis of ACOP,and could surpport positive clinical significance for the prevention and treatment of DE.
4.The value of MELD score in predicting risks of laparoscopic cholecystectomy in cirrhotic patients

Objective To evaluate the safety and feasibility of laparoscopic cholecystectomy(LC) in patients with cirrhosis,and compare the value of model for end-stage liver disease (MELD) score and Child-Pugh classification in predicting prognosis.Methods We reviewed the records of 55 laparoscopic cholecystectomies in cirrhotic patients in our department in the recent 11 years.Indications included symptomatic gallbladder disease,cholecystitis,cystic polyps and cystic adenoma.MELD score and Child-Pugh class were preoperatively calculated and associated with postoperative results.Data regarding patients and surgical outcome were retrospectively analyzed.Results No perioperative death occurred.Total cholecystectomy was employed in 53 patients and subtotal cholecystectomy in 2 patients.Median operative time was(77?5.1)min.Median intraoperative blood loss was(51.0?3.33)mL.Median hospital stay was(5.0?1.3)days.Postoperative complications occurred in 9.09% of the patients,including hemorrhage,intra-abdominal collections and wound complications,which were all controlled conservatively.The incidence of postoperative complications in Child A patients was 7.27%,in Child B was 10.0%;in MELD score below 14 was 2.44%,and in MELD score above 14 was 28.57%.The difference between rates of postoperative complications in patients with preoperative MELD score above 14 and below 14 was significant(P0.05).Conclusions Laparoscopic cholecystectomy is a safe procedure for selected cirrhotic patients,and with controllable complications.MELD score appears to predict morbidity more accurately than Child-Pugh classification system.
5.The association between embryo damage and anxiety, depression and life events during pregnancy

Objective To explore the relationship between embryo damage and anxiety,depression and life events during pregnancy. Methods A case control study was carried out. 192 women diagnosed with embryo damage were selected as the case group,and 192 normal pregnant women or normal parturition within 7 days were selected as the control group from March to June,2014.All cases were from two 3-A-grade general hospitals of Jining. Self-rating anxiety scale ( SAS ) , center for epidemiological survey, depression scale( CES-D) and life events scale for pregnant women( LESPW) were used to evaluate the anxiety,depres-sion and life events of the two groups,so as to analyze the association between these factors and embryo dam-age. Results Multiple non-conditional logistic regression analysis showed that anxiety ( β=0. 409, OR=1.050,95%CI =1.003-1.009) ,depression( β=0.085, OR=1.088,95%CI =1.035-1.145) ,life events dur-ing pregnancy( β=0.003, OR=1.003,95%CI =1.001-1.005) were the risk factors of embryo damage,while education ( β=-0.749, OR=0.452,95%CI=0.298-0.685) ,pre pregnancy health status ( β=-1.366, OR=0.255,95%CI =0.098-0.655) and taking the initiative to acquire knowledge of eugenics ( β=-0.884, OR=0.413,95%CI =0.203-0.840) were the protective factors of embryo damage. Conclusion Anxiety,depres-sion and life events during pregnancy are related to the occurrence of embryo damage. High degree of educa-tion,pre pregnancy in good health and taking the initiative to acquire eugenics knowledge can reduce the oc-currence of embryo damage.
6.Feasible study for evaluating upper cervical reduction by the clivo-axial angle

Objective:To measure the clivo-axial angle (CAA)and cervicomedullary angle (CMA)in upper cervical deformity patients who underwent computer-assisted posterior upper cervical reduction and fixation surgery,and analyze their correlation.Methods:In the study,25 patients with symptomatic upper cervical deformity were chosen for measurement of preoperative and postoperative CMA and CAA using magnetic resonance imaging (MRI)and intraoperative C-arm.The angle between the clivus plane and the straight line parallel to that of the posterior margin of the C2 vertebral body was defined as CAA;moreover,the angle between the straight line parallel to the ventral side of the cervical spinal cord and the straight line parallel to that of the ventral side of the medulla oblongata was defined as CMA.Two ex-perienced spinal surgeons performed the measurements.The CAA and CMA were measured three times, and the mean value was considered as the result.Results:Analyses of the CAA and CMA were per-formed with Wilcoxon rank test,which showed that there was consistency between the CAAs measured with MRI and intraoperative C-arm.The result showed that the postoperative CAA and CMA increased significantly compared with preoperation (P<0.001).Through Spearman rank correlation analysis,the preoperative CAA was positively correlative with the CMA (r=0.902,P<0.001),and so was the post-operative CAA (r=0.921,P<0.001).Conclusion:Preoperative and postoperative CAA in upper cer-vical deformity patients is significantly correlative with preoperative and postoperative CMA.CAA can be measured during surgery using intraoperative CT or C-arm based three-dimension navigation,and may predict the patient’s CMA,which cannot be measured during operation,but is essential for evaluating the decompression and reduction of the spinal cord.
7.Determination of Baicalin and Geniposide in Ganxueguanliu Huoxue Capsules by HPLC

Objective:To build an HPLC method for the determination of baicalin and geniposide in Ganxueguanliu Huoxue cap-sules. Methods:An Agilent Eclipse XDB-C18 (150 mm × 4. 6 mm, 5μm) column was chosen for the analysis. The mobile phase con-sisted of 0. 2% phosphoric acid (A) and methanol (B) with gradient elution. The flow rate was 1. 0 ml·min-1. The detection wave-length was set at 238 nm and the column temperature was 25℃. Results:A good linear relationship was observed within the range of 0. 418-8. 360 μg·ml-1 for baicalin (r=0. 999 9) and 0. 226-4. 524μg·ml-1 for geniposide (r=0. 999 9). The average recovery of baicalin and geniposide was 99. 79% (RSD=1. 55%, n=6) and 100. 2%(RSD=2. 05%, n=6), respectively. Conclusion: The method is rapid, convenient and specific, which can be used for the content determination of baicalin and geniposide in Ganxueguanliu Huoxue capsules.

Including early composition, mismatch negativity (MMN), N2 family, P3 family, and other related components of language processing. These components are closely related to human cognitive function,the research of ERP can lead to a deeper understanding of cognitive function of human brains and has important application value of forensic medicine.
9.Early Analgesic Effect of Continuous Femoral Nerve Block for Total Knee Arthroplasty

Objective To compare the early analgesic effects of continuous femoral nerve block analgesia (CFNB) and patient controlled intravenous analgesia (PCIA) and their effects on functional recovery after total knee arthroplasty.Methods Retrospective analysis was conducted on 60 ASA Ⅰ and ASA Ⅱ patients with unilateral total knee arthroplasty in our hospital from January 2014 to January 2015.The patients were divided into two groups,CFBN group (treatment group) and PCIA group (control group),with 30 patients in each group.VAS scores of postoperative ipsilateral knee resting and movement pain,range of postoperative knee joint motion (ROM),muscle strength of femoral quadriceps,incidence rates of adverse reaction within 7 days after operation,HSS score 3 months before and after the operation,and discharge time were observed.Results Postoperative VIS score in CFNB group was lower than that in PCIA group.ROM and muscle strength of femoral quadriceps in CFBN group were better than those in PCIA group on day 1,2,3,5,and 7 after the operation.Postoperative adverse reaction rate within 7 days in PCIA group was significantly higher than that in CFNB group.HSS score three months after the operation in CFNB group was higher than that in PCIA group.Discharge time of CFNB group was significantly earlier than that of PCIA group (P<0.05).Conclusion Compared with PCIA analgesia,CFNB analgesia shows better analgesic effect.It can alleviate early knee joint pain after TKA,increase joint motion,significantly shorten the hospitalization time,promote joint exercises which are effective in functional rehabilitation,and decrease the incidence of adverse reactions like nausea and vomiting.
10.Expression of Advanced Glycation End Products in a Rat Model of Polycystic Ovarian Syndrome

Objective To investigate the expression of advanced glycation end products(AGEs)in a rat model of polycystic ovarian syndrome (PCOS)induced by dehydroepiandrosterone. Methods The PCOS rat model was established using dehydroepiandrosterone . After confirming that the model rats were set up successfully,serum AGEs concentrations were measured using competitive ELISA and the expression of AGEs in the ovarian tissues of these rats was detected using immunohistochemistry. Results The serum AGEs level in the experimental group were signifi?cantly higher than that in the control group(P<0.05). Furthermore,AGEs expression were significantly higher in the experimental group than in the control group(P<0.01). Conclusion AGEs are highly expressed in the rat PCOS model,signifying their importance in the diagnosis and pathogenesis of PCOS.
