1.An experimental study of the diagnosing value to nude mice model of transplanted human gastric cancer with folate-receptor MR contrast agent

Objective To evaluate the tumor targeting characteristic by observing signal varying of human gastric cancer transplanted nude mice (SGC-7901) using Folate-Receptor MR contrast agent. Methods As a Folate-Receptor MR contrast agent, Gd-DTPA-Folate was obtained by conjugation of DTPA-Folate and GdCl3 under specific conditions. Nude mice of subcutaneously transplanted human gastric cancer (SGC-7901) were used as animal models, 12 mice were divided into experimental group(n=6) and control group(n=6) randomly. Both were injected with Gd-DTPA-Folate and Gd-DTPA (contained same gadolinium) via abdominal cavity respectively. Tumor signal varying was observed by T1WI after injection of contrast agent immediately,1,2,3,4,6,12 and 24 h, and tumor signal changing of experimental group was compared with that of control group. CNR (contrast noise ratio) was regarded as evaluating mark. Results Tumor signal intensity of experimental group was increased evidently between 1-2 hours after injecting Gd-DTPA-Folate. Comparison with pre-injection, there was a significant difference(evaluating mark is CNR: q1=5.80, q2=4.64; P0.05). Conclusion Gd-DTPA-Folate shows definite characteristic of tumor targeting effect to nude mice of subcutaneously transplanted human gastric cancer (SGC-7901).
2.The investigation on the microvessel density and lymphonode micrometastasis as the supplement parameter of TNM stages in esophageal cancer

3.Treatment of perioperative complications in patients with cervical cord injury

[Objective]To investigate the clinical treatments of perioperative complications in patients with cervical cord injury.[Method]Totally 155 cases of cervical cord injury associated with fracture of the cervical spine were enrolled in this study from January 1997 to December 2007.Eighty-five cases were treated with anterior internal fixation,66 with expansive open-door laminoplasty of the cervical spine,and 4 with one stage anterior and posterior approach operations.[Result]Forty-five cases were complicated with hyperpyrexia,31 with respiratory dysfunction,43 with pulmonary infection,70 with hyponatremia,25 with urinary system infection,3 with stress ulcer,10 with deep venous thrombosis,5 with palsy of the C5 nerve root,3 with injury of superior laryngeal nerve or recurrent nerve,1 with leakage of cerebrospinal fluid,2 with cervical hematoma,1 with loosening of interal fixation,1 with esophageal fislula,and 10 died.[Conclusion]There are many perioperative complications from cervical cord injury.To pay attention to the treatments of perioperative complications will bring a satisfactory clinical effect.
4.Therapeutic analysis of non-fracture and dislocation injury of cervical spinalcord

Objective To explore the differences between the conservative treatment and the operative treatment in the management of cervical spinal cord injury, and the timing of the operation. Methods The therapeutic effectiveness o f the 45 cases who were treated from October 1996 to January 2002 was analyzed. 21 of them underwent conservative treatment and 24 operative treatment. The corr elation between the therapeutic effects of the operation and its timing was expl ored. Results The myeloid functional recovery was poor in the patients who had b een treated with conservative means but good in those who had been given an oper ation. The earlier the operation, the more effective it could be, and the higher the probability of the myeloid functional recovery. Conclusions The operative treatment promises better outcome than the conservative treatment. To get the be st effects, the operation should be done within one month after the injury, for the longer the lapse between the injury and the operation, the worse the effect.
5.Isolation of cancer stem cells from pancreatic cancer cell line and identification of their biological characteristics

Aim To isolate cancer stem cells from human pan-creatic cancer cell line L3. 6pl and to identify their biological characteristics. Methods L3. 6pl cells were cultivated in com-mercial low adhesion plate with serum-free stem cell culture me-dium ( MEM/F12 1:1 ) supplemented with B27. The cancer stem cells reformed into floating spheres were isolated. The method of tumor sphere formation was used to isolate/enrich and characterize the cancer stem cells in pancreatic carcinoma cell line L3. 6pl. Cancer stem cell spheres were collected and sorted using magnetic cell sorting ( magnetic activated cell sorting, MACS) technology, with the cell surface markers of CD24 +CD44 + ESA+. Self-renewal and EMT-related oncogene expres-sion were measured with Western blot. Cancer stem cells differ-entiation potential and the expression of cancer stem cell related signs were checked with Immunofluorescence assay. To deter-mine tumorigenesis in vivo, Xenograft assay in NOD-SCID mice were performed respectively, then immunohistochemistry proto-oncogene c-Met and RON expression were also checked. West-ern blot was used to detect the changes of stemness relative tran-scriptional factors and epithelial markers expressed in spheres before and after differentiation. Drug resistance of pancreatic cancer stem cells to gemcitabine or paclitaxel was measured with MTT assay. Results CD24 + CD44 + ESA+ cells were signifi-cantly tumorigenic, and cultured in serum-free conditions to form spheroids, which had the characteristics of stem cells with self-renewal, EMT and drug-resistant capabilities, and had a posi-tive correlation with the c-Met, RON protein expression. Con-clusion Human pancreatic stem cells are successfully isolated, which provides a useful model for individualized therapy and e-valuation of the therapeutic efficacy for pancreatic cancer pa-tients.
6.Comparison of therapeutic effects among three intramedullary fixation system for treatment of tibial fracture in 79 cases

A total of 79 tibial fracture patients (47 males and 32 females; 16 to 73 years old) were selected from Department of Orthopedics, Huangbu Hospital of Huidong City, including 19 cases of open fractures and 60 of closed fractures. Forty-eight fractures in 79 patients were fixed with interlocking nails, 19 by revolving self-locking nail, and 12 by auto compressive locking nail. Operations were all successful. The average operation time was (102 ?10) minutes in the interlocking group, (65?10) minutes in the revolving self-locking nail group and (100?10) minutes in the auto compressive locking nail group. The patients were followed up for 6-10 months. In the interlocking group, one breakage of the screws was found, with one superficial infection and the other 3 achieved delayed bone fracture. The healing time of interlocking group was (21.0?3.2) weeks and the excellent rate of function recovery was (85.42?5.20)% by Johner-Wruh. In the revolving self-locking nail group, one developed delayed bone facture and the other one achieved dislocation; the healing time was (20.0?3.6) weeks and the excellent rate of function recovery was (84.21?4.10)%. In the auto compressive locking nail group, one had loosening screw and the other one achieved superficial infection. The healing time was (17.0?2.8) weeks and the excellent rate of function recovery was (91.67?3.70)%. 7.Early application of tissue expansion in repair of traumatic soft tissue defects in the head and face

Objective To explore a novel strategy to repair traumatic soft tissue defects in the head and face by tissue expansion in the early stage.Methods Eighteen patients with traumatic soft tissue defects were treated with thorough debridement leaving the wound unclosed or simply closed with thin split-thickness scalp skin grafting,and adjacent expander implantation in the early stage and expanded flap transposition in the second stage.Results There were 11 male patients and 7 female patients ranging in age from 3.5 to 40 years [mean,(19.4±12.2) years],with average 15 months follow-up (range,3-67 months).The average expansion time was 74.3 days (range,53-96 days).The total of 18 patients with 22 expanders were treated with satisfactory results.All the flaps survived and the skin color,texture and contour well matched to those of the peripheral tissue.Only one complication of infection happened in the 18 cases (5.56%) and total 22 expanders (4.55%),which was similar to the rate reported in the literature.There were no any other complications related to the expander.Conclusions Debridement and tissue expansion in the early stage have been proved a more effective strategy to repair traumatic soft tissue defects in the head and face,which can not only achieve satisfactory color,unbulky and well matched texture similar to normal,but also can avoid unnecessary donor site injuries and does not increase the infection rate of tissue expansion.
8.Application of low concentration methylene blue dye in sentinel lymph node biopsy of breast cancer

Objective To observe the effect and the skin flap necrosis situation of low concentration methylene blue dye in sentinel lymph node biopsy of breast cancer compared with normal concentration methylene blue dye.Methods One hundred and thirty-eight cases patients with early breast cancer who were treat by sentinel lymph node biopsy surgery in Breast surgery Department of Sichuan ProvincialTumor Hospital from June 2016 to February were selected,and randomly divided into low concentration methylene blue dye group (n =69) and common concentration methylene blue dye group (n =69).Observed and recorded the sentinel lymph node detection and skin flap necrosis of relevance ratio(1% and 0.1% concentration methylene blue dye) concentration methylene blue dye in both two groups.Results There were no significant differences in terms of number of sentinel lymph node detection,the detection rate and false negative rate in two groups,while the rate of cutaneous necrosis in low concentration methylene blue dye group was lower (5.8% vs.0%,P =0.025).Conclusion The effect of low concentration methylene blue dye in sentinel lymph node biopsy of breast cancer is the same as that of common concentration while the low concentration group with fewer side effects.
9.Changes of regulatory T cells in patients with hepatocellular carcinoma after percutaneous cool-tip radiofrequency ablation and its influence on the prognosis

Objective To investigate the changes of regulatory T cells (Treg) in patients with hepatocellular carcinoma (HCC) after ultrasound- guided percutaneous cool- tip radiofrequency ablation (RFA), and to discuss its influence on the prognosis. Methods A total of 30 patients with HCC were enrolled in this study. The percentage of Treg in peripheral blood was estimated with flow cytometry before RFA and one, 4, 7 and 12 months after RFA. During the follow-up period, the therapeutic effects were evaluated by contrast enhanced sonography or contrast enhanced CT scanning. By using the methods of receiver operating characteristic (ROC) curve and Kaplan-Meier survival function, the correlation of Treg dynamic changes with the progression-free survival time was analyzed. Results One month after RTA, the tumor response (TR) rate in the 30 patients was 93.3% (28/30), the tumor progression (TP) rate was 6.67%(2/30). The percentage of Treg before RFA was (9.42 ± 1.16)%, which decreased to (6.55 ± 0.97)% one month after RFA, the difference was statistically significant (t = 15.325, P <0.001). Twelve months after RFA, TR rate became 33.3%(10/30), and TP rate became 66.7%(20/30). The preoperative percentage of Treg of TR group was (8.75 ± 0.72)%, which was significantly lower than that of TP group (9.76 ± 1.20)%, the difference was statistically significant (t=-2.448, P=0.021). ROC curves indicated that the optimal cut-off value of Treg nadir was 4.82%, the sensitivity was 90.0% and the specificity was 60.0%. The optimal cut-off time to reach Treg nadir was 5.5 months, the sensitivity was 70.0% and the specificity was 85.0%. Kaplan-Meier curve analysis showed that after RFA the progression-free survival rate (PFS) of patients with Treg nadir ≤ 4.82% was significantly higher than that of patients with Treg nadir>4.82%. PFS of patients with reaching Treg nadir≥5.5 months was significantly higher than that of patients with reaching Treg nadir<5.5 months. Log-rank test results were字2=5.207, P=0.023; 字2=22.079, P < 0.001, respectively. Conclusion Percutaneous cool-tip radiofrequency ablation can decrease the percentage of Treg cells. Besides, Treg nadir and the time reaching Treg nadir can reflect the prognosis of HCC patients after RFA to a certain extent.
10.Behavior problems and influencing factors of school-age children in Zhongshan

Objective :
To investigate the behavior problems and influencing factors of school-age students from the third to the sixth grade in Zhongshan,and to provide evidence for early intervention of behavior problems in children.
Methods :
According to the proportion of population in urban area and township in Zhongshan,students of Grade Three to Six from eight primary schools(three in urban area and five in township)were recruited by stratified sampling method. The behavior problems in children were assessed by the Conners Parent Symptom Questionnaire(PSQ). Sociodemographic information,family discipline and so on was investigated by a general questionnaire. The influencing factors for behavior problems were analyzed by a logistic regression model.
Results :
A total of 2 292 questionnaires were issued,and 2 236 valid questionnaires were recycled,with an effective rate of 97.56%. The positive rate of behavioral problems was 11.72%. The results of multivariate logistic regression analysis showed that the risk factors for behavior problems were females(OR=1.594,95%CI:1.170-2.171),birth asphyxia(OR=2.372,95%CI:1.320-4.261),main family discipline(laissez-faire:OR=3.326,95%CI:1.450-7.630;doting:OR=3.244,95%CI:1.867-5.638;autocratic:OR=2.609,95%CI:1.584-4.296,mixed:OR=2.313,95%CI:1.669- 3.207),less than four hours per week for father-child communication(OR=1.551,95%CI:1.052-2.286),negative life events(OR=2.188,95%CI:1.448-3.308),living in township(OR=2.031,95%CI:1.330-3.102),academic performance (average:OR=2.786,95%CI:1.868-4.156;poor:OR=6.665,95%CI:3.236-13.727;very poor:OR=25.068,95%CI:5.786-108.617);the protective factors were occupation of mother as civil servants or professional personnel(OR=0.449,95%CI:0.238-0.844)and higher grades(Grade Five:OR=0.496,95%CI:0.339-0.727;Grade Six:OR=0.468,95%CI:0.309-0.710).
Conclusion
Females,birth asphyxia,main family discipline,less communication between father and child,occupation of mother,negative life events,place of residence,academic performance and grade were the influencing factors for behavior problems.
