1.Contrastive analysis of diagnostic value between multi-slice spiral CT and MR pulmonary artery angiographies in acute pulmonary embolism

Objective:To investigate the comparison of diagnostic value between multi-slice spiral CT (MSCT) and MR pulmonary angiographies in the diagnosis of acute pulmonary embolism.Methods: 86 patients with acute pulmonary embolism who underwent imaging examination were randomly divided into control group (43 cases) and observation group (43 cases) according to random number table method. The patients in the control group were analyzed with MSCT for enhanced scan under pulmonary angiography. The patients in the observation group were analyzed with MR for pulmonary artery imaging and pulmonary embolism. These results were compared depended on above data.Results: The differences of sensitivity and Youden index between control group and observation group were statistically significant (x2=10.052,x2=10.052,P<0.05), and the sensitivity of observation group was higher than that of control group, and both the diagnostic specificity of the two groups were 100%. For embolism of different part, the accuracy of observation group was higher than that of control group. And for the comparison of left multiple between the two groups, the difference were significant (x2=4.168,P<0.05). While for the comparisons of single, bilateral and multiple for right side, the differences between the two groups were no significant (x2=2.355, x2=0.917,x2=2.912,P>0.05). For the diagnosis of various indirect symptom, the accuracy of observation group was higher than control group. For the comparison of diagnostic accuracy of pulmonary segments infarct between the two groups, the difference of them was significant (x2=5.630,P<0.05). While the comparisons of pericardial effusion, pleural effusion and pulmonary artery hypertension, the differences of them between the two groups were no significant (x2=3.214,x2=3.733,x2=3.281;P>0.05).Conclusion: For patients with acute pulmonary embolism, the diagnosis by using MR imaging technology has series advantages, such as convenient operation and higher diagnostic accuracy, and it can accurately determine the severe degree of the disease.
2.Survey of Infective Disease in Community

OBJECTIVE To investigate the status of community health services and offer a scientific basis for making a direction of community to control the infective disease.METHODS Totally 1503 outpatients were investigated retrospectively for infective diseases in community.RESULTS The average age of these patients was 65,the number of females was much more than males.The most frequently infective diseases were upper respiratory tract infection,bronchitis,periodontitis,etc.CONCLUSIONS It′s necessary to strengthen the community health service and to establish an effective system of health service.The patients must cultivate a health habit of life,they not only need in time diagnosis and treatment,but also enhance own knowledge about health and disease prevention.
3.Effect of geniposide on proliferation of pancreatic βcells and its mechanisms

Objective To explore the role of geniposide (Gen) in the proliferation of pancreatic βcells and the cell signaling transduction pathway .Methods After INS-1 pancreatic βcell line was treated with indicated concentrations of Gen for 24 hours, the number of viable cells was measured with MTT assay , and the cell proliferation was evaluated with BrdU labeled method .Additionally , Western blotting was used to probe the phosphorylation of Akt 308 and FoxO1 induced by Gen.Results The number of INS-1 cells and BrdU labeled positive cells was increased by Gen .Moreover, the phospho-rylation of Akt308 and FoxO1 was induced , and the phosphorylation of FoxO 1 induced by Gen could be prevented in the presence of LY294002, an inhibitor of PI3K kinase.Conclusion Gen induces the proliferation of INS-1 cells in a dose-dependent manner, and PI3K signal pathway plays an essential role in the phosphorylation of FoxO 1 induced by Gen.
4.Restoration of endothelial function after reperfusio injury by tetrahydrobiopterin

Objective To evaluate the influence of tetrahydrobiopterin on NO and MDA concentration and SOD activity as well as the myocardial infarction area. Methods Twenty-four rabbits were divided into three groups randomly: control group,MI/R group and BH 4 group. After occlusion of the left anterior descending coronary artery of a rabbit for 60 minutes followed by 90 minutes of reperfusion (ischemia/reperfusion),NO and MDA and SOD concentrations were measured and the myocardial infarction area was caculated. 5.Relationship between the pulse pressure and the changes of cerebrovascular hemodynamic index in health examination population

Objective To investigate the effect of the increased pulse pressure on cerebrovascular hemodynamic index.Methods From January 2014 to October 2016,the application of the method of cross-sectional study,6 006 consecutive health subjects examined and detected with ultrasound noninvasive cerebral hemodynamic index (CVHI) at the Physical Examination Center,Pu′er People′s Hospital,Yunnan Province were enrolled.In order to control the influence of age and gender on the results,according to age and gender in the increased pulse pressure group (≥60 mmHg),and the same sex and age difference of not more than 1 years old were selected as the inclusion criteria in the normal pulse pressure group (60 mmHg).Finally,1 115 subjects were enrolled as a control group for comparative analysis.The study population was divided into an increased pulse pressure group (n=590) and a normal pulse pressure group (n=1 115) in a mixed manner.The differences of CVHI index and its integral score of both groups were compared.The correlation of the increased pulse pressure level and changes of cerebrovascular hemodynamic accumulative scores were analyzed.Results The minimum velocity,mean velocity,and critical pressure in the increased pulse pressure group were significantly lower than those in the normal pulse pressure group (P<0.05),while the differences of the peripheral vascular resistance,characteristic impedance,pulse wave velocity,dynamic resistance,diastolic blood pressure,and critical pressure were significant higher than those in the normal pulse pressure group.There were significant differences (P<0.01).The integral values of cerebral hemodynamics in the increased pulse pressure group and the normal pulse pressure group were 76±22 and 88±19 respectively.There was significant difference (P<0.01).The proportion of the integral value <75 was 34.4% (203/590) and 15.6% respectively (174/1 115,P<0.01).With the increase of pulse pressure level,the integral value of cerebral hemodynamics showed a significant downward trend,there was a significant negative correlation between the two (r=-0.312,P<0.01).Conclusion The increased pulse pressure may result in the changes of cerebral hemodynamic indexes and its integral values.There is a significant negative correlation between the pulse pressure level and the integral value of cerebral hemodynamics.
6.Radiotherapy of prostate cancer

Radiotherapy(RT) is one of the principle treatment options for prostate cancer.Modern RT and surgery show similar progression-free survival in localized prostate cancer.Adjuvant RT compared to observation significantly diminishes the risk of prostate-specific antigen (PSA) progression and local failure for patients at the highest risk for recurrence after radical prostatectomy,including with seminal vesicle invasion,extraprostatic extension,extensive positive margins,and detectable postoperative PSA.Salvage RT is effective at controlling local recurrence and reduces the risk of distant metastasis and prostate cancerspecific mortality (PCSM) for patients with PSA or local recurrence after prostatectomy.Hypofractionated radiotherapy of recent years' reports shows similar cancer control rates without an increased risk of late toxicity in comparison to conventional regimens.The addition of androgen-deprivation therapy (ADT) to radiation improves the overall survival and biochemical progression-free survival(bPFS) for intermediate-risk and high-risk patients.
7.Immunosenescence and its role in cancer

Immunosenescence is a biological process featured by the structural and functional decline of immunity in ageing people,which favors the carcinogenesis by rendering the body's immune surveillance and tumor escape.Decreasing clearance of tumor cells by macrophages and natural killer cell reflects the immunosenescence of innate immune,also,the tumor antigen presentation ability of dendritic cell is decreased.While,the decline of the CD8 + T cells cytotoxic effect and the secretion of tumor growth factor accounts for the immunosenescence of adaptive immunity.Intervention of immunosenescence may produce the novelty ideas of prevention and treatment of cancer.
8.A randomized, placebo-controlled trial on efficacy and safety of recombinant human acidic fibroblast growth factor in treatment with open tibiofibula fractures

Objective To observe the recombinant human acidic fibroblast growth factor (rh-aFGF) treatment of open fractures of the tibia and fibula efficacy and safety.Methods Seventy-two cases of open fractures of tibia and fibula patients were selected,all patients were given the C-arm X-ray machine to guide needle,debridement,the entire complex fractures,external fixa tion device postoperative were given anti-infection treatment.Treatment group were given rh-aFGF 1-5 soluble solvent cleaning wounds and skin grafts,suture given daily dressing (3-4 spray/cm2,6-8 times/d).The control group was not given medication,continuous treatment after 7-14 d assess wound healing,wound complications and infection.Results Treated the wounds healing time,healing time,the incidence of delayed fracture healing,skin graft survival area,walking function recovery time,edema score,pain score,the incidence of severe infection and late healing in treatment group were significantly better than those in control group:(27.2 ± 13.1) d vs.(34.7 ± 12.3) d,(55.4 ± 17.4) d vs.(62.2 ± 18.7) d,(99.2 ± 1.2) % vs.(92.6 ± 3.6) %,(2.2 ± 0.4) scores vs.(3.8 ± 0.4) scores,(2.1 ± 0.4) vs.(3.7 ± 0.2) socres,5.6% (2/36) vs.30.6% (11/36),2.8% (1/36) vs.19.4% (7/36),there were statistical differences (P < 0.05).rh-aFGF treatment of major adverse reactions were mild itching.Conclusion The recombinant human acidic fibroblast growth factor can accelerate wound healing and reduce post-operative complications,reducing infection.
9.Clinical application of video-assisted thoracoscopy plus minithoracotomy

Objective To discuss the value of video-assisted thoracoscopy plus minithoracotomy (VATM) in the management of thoracic diseases. Methods An 1.5 cm incision was made along the middle axillary line at the 7th costal interspace on the diseased side to introduce the thoracoscope. After the exploration of intrathoracic lesion, another incision 5~8 cm in length close to the lesion was made. The thoracic cavity was opened by way of the costal interspace. Surgical performance of exploration, dissection, hemostasis and suture was performed under thoracoscope and direct vision by using routine surgical instruments and thoracoscopic instruments. A total of 43 cases of VATM were carried out. Results The operation time was 40~150 min (mean, 67 min). The length of incision was 5~8 cm (mean, 6 cm). The chest drainage lasted 2~5 d. No postoperative complications occurred. The postoperative hospital stay was 5~8 d (mean, 6 d). Follow-up found no recurrence for 3~22 months in 18 cases of spontaneous pneumothorax and pleural effusion, and found no recurrence, distant metastasis or incision implantation for 5~20 months in 3 cases of lung cancer receiving either lobectomy or wedge resection. Simple biopsy of tumor was conducted in 6 cases of lung cancer, who obtained chemotherapy or gamma knife radiotherapy postoperatively and survived 5~21 months. Conclusions VATM is practical, minimally invasive and safe. By using routine surgical instruments it has an optimistic outlook.
10.Effect of tetrahydrobiopterin on lipid peroxidation in hypercholesterol rabbits
