1.Changes of coagulation and fibrinolysis function and PLT in patients with lung cancer before and after ;chemotherapy

Objective To investigate the changes of blood coagulation and fibrinolytic function and PLT in patients with lung cancer before and after chemotherapy,and to investigate the changes of the patients with lung cancer and the influence of the changes with lung cancer and the prognosis.Methods 40 patients with lung cancer(observation group)treated in our department from March 2012 to May 2014 were selected as the research subjects.The changes of coagulation and fibrinolytic activity,PT(PTINR),APTT,thrombin time(Fib),thrombin time(D -D),thrombin time (TT),D -D,and PLT were analyzed.The relationship between the parameters and the lung cancer staging was analyzed. Results Before chemotherapy,the levels of Fib,PLT,D -D,PT,PTNR,APTT,TT,PLT were (11.34 ±1.14)s, (1.01 ±0.07),(24.34 ±4.53)s,(2.54 ±0.45)g/L,(184.31 ±10.88)×109 /L,(143.35 ±23.45)ng/mL, (14.55 ±4.56)s.After chemotherapy,the levels of Fib,PLT,D -D,PT,PTNR,APTT,TT,PLT were (11.57 ± 1.36)s,(1.03 ±0.05),(24.52 ±5.32)s,(3.63 ±0.65)g/L,(210.45 ±11.24)×109 /L,(126.56 ±26.55)ng/mL, (14.34 ±4.17)s.The contents of Fib and PLT after chemotherapy were higher than before chemotherapy(t =0.024, 0.025,all P <0.05),D -D decreased(t =0.001,P <0.05),PT,PTNR,APTT,TT had no statistically significant changes(P >0.05).Compared with patients with stage Ⅰ ~Ⅱ lung cancer,Fib level in stage Ⅲ ~Ⅳ lung cancer was higher (t =0.01,P <0.05),and the rest of the patients with stage III and IV lung cancer had no significant difference.After chemotherapy,D -D decreased group(38 cases),which effective in 28 cases,10 cases were ineffec-tive.D -D increased group(n =42)effective in 12 cases,invalid in 30 cases,the difference between two groups was statistically significant (Z =-4.236,P =0.003).Chemotherapy effective group and ineffective group PT,PTINR, APTT,Fib,T -T and PLT had no statistically significant differences (all P >0.05).Conclusion PLT and Fib were increased,but D -D decreased after chemotherapy,the tumor remission rate was higher in D -D group,which indica-ted that the levels of Fib and D -D should be changed in the course of clinical chemotherapy.
2.Analysis of percutaneous nephrostolithotomy effect on patients with different body mass index

Objective To investigate the effect of percutaneous nephrostolithotomy (PCNL) on patients with different body mass index.Methods The clinical data of 84 patients with kidney stones who were treated with PCNL surgical treatment were enrolled in this study,and they were divided into different groups according to body mass index (BMI),including 16 cases of BMI < 18.5 kg/m2(low BMI group),36 cases of 18.5 kg/m2≤BMI <24.0 kg/m2 (normal BMI group),32 cases of BMI≥24.0 kg/m2 (high BMI group),and the curative effect was evaluated.Results Three groups' operation time,multichannel gravel rate,stones clearance,length of hospital stay,blood transfusion rate had no significant difference (P >0.05),the incidence of complications in low BMI group [31.25% (5/16)] was obviously higher than that in normal BMI group [5.56%(2/36)] and high BMI group [9.38%(3/32)],there was significant difference (P < 0.05).Conclusion BMI of PCNL surgery influences on certain effect,low BMI will affect patients with surgery tolerance,cause complications increase,obesity wifl increase the difficulty of the operation.
3.The role credit environment of clinical medicine in medical students'education of honesty

4.Changes of DNA content in the fibroblasts of narrow pedicled subdermal vascular network flap during its surviving

Objective To study the dynamic changes of DNA content of the fibroblasts in narrow pedicle subdermal vascular network flap (SVNP) during their surviving. Methods Eighteen pieces of surviving skin in pig back with size of 16 cm?8 cm and pedicle wideness of 4 cm were stained with acridine orange, and their DNA contents were measured with the adherent cell analysis and sorting cytometer 570. The tissue samples were taken at 3, 4 ,5 ,and 7 days after operation from different parts of the flap.Results The DNA content of fibroblast in the proximal part of the flap was slightly changed, those in the middle portion increased from 3rd to 5th day and became normal at 7th day, and in the distal portion progressively increased after operation. The results suggested that the transitional part of the flap and skin graft in the middle portion was most active in function and metabolism, and well survived, but the distal portion of the graft was poor in surviving and had a long repairing period. These indicated that local tissue hypoxia stimulated proliferation of fibroblasts and synthesis of the DNA in a short time, but as time wert by, the proliferative speed and DNA synthesis in the individual cell were heterogeneous, probably because of the degeneration or necrosis of some epidermal cells in the graft. Conclusion The wideness and length of the pedicle as well as its vascular anastomoses should be readjusted to promote the blood circulation of the distal skin graft tissues.
5.Clinical Study of Serum CA-Hb_3 Level in Patients with Colorectal Carcinoma

Objective This study was designed to determine the serum CA-Hb 3 level in human colorectal carcinoma and analyze its clinical significance.Methods The level of serum CA-Hb 3 was measured by ELISA in 72 patients with colorectal carcinoma and 36 normal persons as the control group.Results The level of serum CA-Hb 3 in colorectal carcinoma was significantly higher than that of normal control .The level of serum CA-Hb 3 was positively related with tumor size,clinical pathological staging.With progression of clinical pathological staging (Dukes'stage),the level of CA-Hb 3 increased significantly,but its relation to tumor size,age,and sex was not significant.Conclusion Serum CA-Hb 3 assay may be a potentially valuable marker in assessment of pathological staging and judgement of clinical prognosis of colorectal carcinoma.
6.Effect of homocysteine on secretion and expression of interleukin-6 in cultured rat vascular smooth muscle cells

AIM: To investigate the effect of homocysteine (Hcy) on secretion and expression of interleukin-6 (IL-6), which is a multifunctional proinflammatory cytokine, in cultured rat vascular smooth muscle cells (VSMCs). METHODS: Rat VSMCs were stimulated with Hcy. Cell ELISA was performed to measure the expression of IL-6 protein and semiquantitative RT-PCR was used to dectect the IL-6 mRNA expression. RESULTS: Compared with control, treatment of 0.25 mmol Hcy for 6 h could increase IL-6 production. In addition, Hcy concentration-dependently increased the expression of IL-6 protein in these cells. 0 1 mmol/L, 0 25 mmol/L Hcy increased IL-6 production 1 4-fold and 3 4-fold, respectively Furthermore, RT-PCR analysis demonstrated that homocysteine also enhanced IL-6 mRNA expression in a concentration- and time-dependent manner.CONCLUSION: Homocysteine can induce IL-6 expression in VSMCs and elicit vascular inflammatory response, which may thereby influence the pathogenesis of atherosclerosis.
7.Role of mitogen-activated protein kinase in myocardial hypertrophy

Myocardial hypertrophy is an independent risk factor for cardiac events. Mitogen-activated protein kinases (MAPK), including extracellular signal-regulated kinases, C-jun N-terminal kinases and P38-MAPK, are the common intracellular pathway of transducing hypertrophic signs. All three MAPK subfamilies play an important role in development of myocardial hypertrophy.
8.Influence of Smoking on the Risk of Pulmonary Tuberculosis Among Type 2 Diabetics

Objective To investigate the relationship between smoking and the occurrence of pulmonary tuberculosis among type 2 diabetics(DM-PTB).Methods A hospital-based case-control study was carried out at three hospitals in Tianjin from 2001 to 2002.87 cases of newly-diagnosed pulmonary tuberculosis with type 2 diabetes mellitus and 129 controls by frequency-matched by gander and age were recruited and interviewed.Univariate and multivariate unconditional logistic regression analysis was conducted.The adjusted odds ratio(OR) and 95% confidence interval(CI) were estimated.Results Higher smoking index(OR=1.399,95%CI=1.098~1.782)?beginning smoking at younger(OR=2.028,95%CI=1.076~3.821)?large consumption of cigarets per day(OR=1.349,95%CI=1.087~1.674)?no smoking cessation after diabetes diagnosis(OR=5.327,95%CI=1.703~16.661) were significantly associated with DM-PTB after adjusted for other risk factors.Among female diabetics large consumption of cigarets per day? beginning smoking at younger higher smoking index were generally associated with increased risk of pulmonary tuberculosis;while without smoking cessation after diabetes diagnosis might be risk factor among the male;higher smoking index might increase the risk of pulmonary tuberculosis among younger(youger than 60y) diabetics.Conclusion Smoking control or cessation among type 2 diabetics might decrease the risk of pulmonary tuberculosis.
9.Diabetes self-management skills and their effects on glycemic control among outpatients

ObjectiveTo study diabetes self-management skills among outpatient diabetics and their influence on glycemic control.MethodsData were collectedfrom 281 diabetes patients recruited in Tianjin Medical University Metabolic Hospital using a questionnaire covering their diet, physical exercise and glycemic self-monitoring during the last three months.Additional HbA1c and blood lipid information were retrieved from medical records.Logistic regression modified by factor analysis was then conducted.ResultsMore than half patients did well in diet control, physical exercise and drug treatment.23% patients exceeded physician-recommended intervals for self-monitoring.Self-monitoring was primarily conducted through hospital testing or family-owned glucometers.Abnormal blood pressure (53%) or lipid (47%) levels were also common.Overall, self-management practices among men and the younger(<60yr)were worse than that among women and elderly people.The significant related factors for good glycemic control included higher socioeconomic status and positive treatment, but Abnormal cholesterol and inadequate self-monitoring were related with poor glycemic control with the adjusted odds ratios equaling to 059, 039, 189 and 183, respectively.ConclusionsMore attention should be taken on men and the younger (<60yr)to improve their skills of self-management.Self-monitoring, physical exercise, diet control, etc. independently influence blood glucose control.
10.Effect of Triptolide on Content of Adreno-Corticotropic Hormone in Pituitary of Rats

Objective To Study the effect of long-term triptolide use on the content of the normal rat plasma ACTH.Methods 20 male SD rats were divided into 2 groups randomly,and gavaged for 7 weeks.The control group was 10 with 2%propylene glycol 2ml/kg,bid;the group of Triptolide was 10 with 2% the mixture of propylene glycol and Triptolide 20?g/kg,bid.We used radioimmunoassay to detecet the content of ACTH in rat's plasma and immunocytochemistry to detecet the expression of ACTH in pituitary. Results The content of ACTH in the group of the Triptolide had significant difference as compared with control group.The expression of ACTH in the group of the Triptolide was higher than that of control group. Conclusion These results suggested that one of the possible mechanism of the Triptolide in the HPAA might be exciting pituitary ACTH neurons,thus causing the content of ACTH increasing.
