1.Advance in research of CDX2 and CDH17 in gastric cancer

Caudal-related homeobox transcription factor CDX2 is a specific nuclear transcription factor. It seems to play a key role in intestinal development and differentiation in normal and cancer tissues, and it is important for the maintenance of intestinal epithelium cells, liver-intestine cadherin(CDHIT) as one protein has intimate correlation with invasion and metastasis of tumor, has as well as highly expresses in intestinal-mucosa. Recent studies indicate that CDX2 and CDHI7 is closely associated with tumorigenesis,differentia-tion and metastasis. They might act as a new biomarker of gastric cancer diagnosis and a new target of tumor gene therapy.
2.Expression and significance of Cdx2 in digestive and urogenital system tumors

Cdx2 plays an important role in the proliferation and differentiation of normal intestinal cells. It has been suggested that Cdx2 is a master regulator of the intestinal differentiation program and is therefore not usually expressed outside the intestinal organs. Loss of Cdx2 expression leads to focal gastric differentiation in the colon, whereas aberrant expression of Cdx2 in the upper digestive tract may induce the initial trans-differentiation to intestinal metaplasia. The detection of a strong nuclear immunoreactivity for Cdx2 in metaplastic cells suggests that the protein is a major transcription factor in the development of intestinal metaplasia.Abnormal expression of Cdx2 is also correlated with early stage of adenocarcinoma in different organs, indicating that the protein may play an anti-oncogenic role in cancer development. The Inflammation-metaplasia-cancer'sequence is a frequent event in the lower esophagus, stomach, uterine cervix and the bladder. In all these specific anatomical sites, the noxious stimuli leading to metaplasia may have direct and/or indirect carcinogenic effects on chronically injured cells and Cdx2 plays a key role in the cascade process. Detection of Cdx2 expression in adenocarcinoma may be useful in the evaluation of malignancy and prognosis.
3.Laparoscopic Therapy for Esophageal Hiatus Hernia and Reflux Esophagitis Complicated with Cholecystolithiasis:Report of 27 Cases

Objective To explore the feasibility of laparoscopic surgery for esophageal hiatus hernia and reflux esophagitis complicated with cholecystolithiasis.Methods Five trocars were used for the patients to perform laparoscopic hiatal hernia repair,followed by fundoplication(Nissen fundoplication in 21 cases and Toupet fundoplication in 6),and then LC.Results The procedures were completed in all of the 27 cases.The postoperative mean esophageal pressure of the patients was significantly higher than that before the operations [(19.32?4.11) mm Hg vs(7.30?1.36) mm Hg,t=-16.407,P=0.000],while the 24-hour pH value were decreased markedly(9.20?2.15 vs 160.16?50.30,t=16.387,P=0.000).No hemorrhage,intra-abdominal infection,biliary leakage occurred in this series.The patients were followed up for 1 to 24 months(1-6 months in 7 cases,and 7-24 months in 20 cases),during which no esophageal stenosis or incisional hernia were detected.Conclusions It is feasible to perform primary laparoscopic surgery for patients suffering from esophageal hiatus hernia and reflux esophagitis complicated with simultaneous cholecystolithiasis,if no contraindications.
4.Relationship and research progress of the nutrition inositol and the pediatric developmental disorders

Inositol,also called cyclohexanehexol,is a water-soluble Vitamin (Vitamin B8),which is widely distributed in plants and animals.It is an essential and indispensable nutrient for human and animals to maintain normal physiological functions with multiple bioactivities.Inositol can promote the maturity of a variety of ingredients in pulmonary surfactants,participating in the regulation of cell growth and surviving,biofilm formation,as well as the information transmission processes of a variety of signaling molecules.Therefore,inositol plays a key role in the growth and development of children and is closely related to pediatric developmental diseases.In order to provide a new way of thinking for prevention and treatment of inositol and its molecular mechanisms in pediatric developmental diseases,this article reviews the relationship between inositol and pediatric clinical diseases and its research progresses combined with the results of our laboratory studies.
5.Total subadventitial cystectomy for the treatment of 30 patients with hepatic hydatid cyst

Objective This study was to evaluate a new radical resection of hepatic hydatidosis. MethodSince 1999, total subadventitial cystectomy through the intraadventitial space of the hepatic hydatid cyst was performed on thirty patients with 45 liver hydatid cysts. Results The procedure was successful in all cases with no intraoperative rupture of the cysts. The average blood loss per cyst was 20?ml. There was no operative mortality, nor postoperative complications. Twenty one cases were followed up for an average period of 11 months with no recurrence. Conclusions The results indicate that the new procedure is a safe and radical alternative for the treatment of hepatic hydatidosis.
6.Medical Support for Peacekeeping Operations——Employment, Maintenance and Repair of Medical Equipment

Chinese peacekeeping medical team established a Level II Hospital on Liberia in 2004. Peacekeeping medical teams abide by varieties rules on medical service support and establish management and maintenance system of medical instrument. More than 95% medical equipment is still kept in good condition after four years and the routine rate of self-maintenance of small medical equipments can be reached 95%. All the medical equipment have passed the half-yearly verification of ORI by UN, the experience have great value for future similar missions.
7.Influence of discontinuance of methimazole and propylthiouracil on 24 h radioactive iodine uptake in normal rats

Objective To investigate the influence of discontinuance of methimazole (MMI) and propylthiouracil (PTU) on 24 h radioactive iodine uptake(RAIU) in normal rats.Methods A total of 66 female SD rats were randomly divided into 3 groups by simple random sampling method.MMI group (30 rats) received MMI with a dose of 0.6 mg·ml-1·d-1 orally for 2 weeks, PTU group (30 rats) received PTU with a dose of 6 mg·ml-1·d-1 orally for 2 weeks.At 1, 3, 5, 7 and 9 d after MMI or PTU was discontinued, 6 rats were sacrificed to measure 24 h RAIU respectively.The control group (6 rats) received normal saline with a dose of 1 ml/d orally for 2 weeks, and then they were sacrificed to measure 24 h RAIU too.Data were analyzed with two-sample t test or t′ test.Results Thyroid 24 h RAIU in normal rats was (14.6±4.3)%.The 24 h RAIU on 1, 3, 5, 7 and 9 d was (4.6±0.7)%, (24.2±7.2)%, (13.1±2.9)%, (14.1±2.6)%, (16.1±3.7)% respectively after MMI withdrawal.The 24 h RAIU was significantly lower than that in control group at day 1 (t′=5.55, P<0.01), higher at day 3 (t=-2.83, P<0.05), and returned to the normal level at day 5, 7, 9 (t=0.68, 0.24,-0.67, all P>0.05).The 24 h RAIU on 1, 3, 5, 7 and 9 d was (1.9±0.8)%, (3.3±1.2)%, (7.7±2.4)%, (29.0±4.0)%, (17.1±4.8)% respectively after PTU withdrawal.It was lower than that in control group (t=7.01, 6.10, 3.40, all P<0.05) at day 1, 3, 5, and higher at day 7 (t=-5.97, P<0.01).The 24 h RAIU of 9 d showed no difference when compared with that in control group (t=-0.95, P>0.05).Conclusions MMI and PTU can inhibit the iodine uptake of thyroid, the 24 h RAIU returns to normal level at day 5 after withdrawal of MMI and at day 9 after withdrawal of PTU in euthyroid rats.
8.Combined detection of antinuclear antibody and antinuclear antibody spectrum and its clinical significance in systemic lupus erythematosus

Objective To explore the diagnostic value and clinical significance of the combined detection of antinuclear antibody (ANA) and anti-nuclear antibody spectrum (ANAs) in systemic lupus erythematosus (SLE) .Methods 110 patients with SLE ,88 patients with other autoimmune diseases (AID) and 50 individuals with healthy physical examination were selected and detected se-rum ANA by using indirect immunofluorescence (IIF);the Western blot was adopted to detect the 15 items of ANAs .Results A-mong 110 cases of SLE ,the positive rates of serum ANA ,anti-ribonucleoprotein /Smith antibody (nRNP/Sm) ,anti-Smith antibody (Sm) ,anti-Sjogren′s syndrome A antibody (SSA) ,anti-Ro-52 antibody (Ro-52) ,anti-Sjogren′s syndrome B antibody (SSB) ,anti-scleroderma 70 antibody (Scl-70) ,anti-PM-Scl antibody (PM-Scl) ,anti-cytoplasmic group acyl-tRNA antibody (J0-1) ,anti-centro-mere antibodies (CENP B) ,anti-proliferative protein antibody (PCNA ) ,anti-double stranded DNA antibody (ds-DNA ) ,anti-nu-cleosome antibody (AnuA) ,anti-histone antibody (AHA) ,anti-ribosomal P protein antibody (ARPA) and anti-mitochondrial anti-body M2 subtype (AMA M2) were 98 .2% ,59 .1 % ,39 .1 % ,71 .8 % ,68 .2 % ,21 .8 % ,2 .7 % ,3 .6 % ,0 .9% ,9 .1% ,5 .5% , 44 .5% ,38 .2 % ,27 .3 % ,38 .2% and 15 .5% respectively ,the positive rate of above 16 kinds of autoantibody in the orther AID were64.8% ,14.8% ,0% ,37.5% ,42.0% ,11.4% ,9.1% ,0% ,5.7% ,9.1% ,1.1% ,2.3% ,1.1% ,1.1% ,5.7% and2.3% respec-tively ;ANA had the highest diagnostic sensitivity (98 .2% ) and low specificity (35 .2% ) for SLE ,the antibodies with higher speci-ficity in diagnosing SLE were anti-Sm antibody (100 .0% ) ,anti-ds-DNA antibody (97 .7% ) ,anti-AnuA antibody (99 .0% ) ,anti-AHA antibody (99 .0% ) ,anti-ARPA antibody (94 .3% ) and anti-PCNA antibody (98 .9% ) and the disease control group (50 .9% ) .In detecting ANA karyotype by IIF ,the maximum was nuclear particle type (43 .5% ) and the disease control group (50 .9% ) ,no statistically significant difference was found between them (P>0 .05) ,part of the ANA-negative patients have positive ANAs .Conclusion Multiple autoantibodies can be detected in the serum of the SLE patients .The combined detection of ANA and ANAs has important clinical significance for diagnosing and differentially diagnosing SLE .
9.Establishment and genetic checkup of an inbred strain of DXB/c mouse

An inbred strain of DXB/c mouse has been established by hybridization between DBA/2 female mice and C57BL/6 male ones and subsequently by sibmating their offsprings beginning from the F2 generation.Now DXB/c mouse has been passed for 28 generations of full sibmating since 1979.Genetic checkup by means of skin grafting,mandibular morphology analysis,mixed lymphocyte cultivation,coat colour gene testing,and biochemical marker gene examination confirmed that the full homozygosity of alleles has been achieved in DXB/c mouse and DXB/c mouse comforms to the criteria of an inbred strain of mouse.In addition,the genetic background of DXB/c mouse is composed of the genes of its progenitors DBA/2 and C57BL/6 as shown by coat colour gene testing and biochemical marker gene examination.
10.Echocardioagraphic diagnosis of interrrupted aortic arch

Objective To investigate the value of echocardiography in the diagnosis of interrupted aortic arch (IAA). Methods Forty-three children that diagnosed as IAA by transthoracic echocardiography (TTE) were reviewed. The results of CTA (CT Angiography) and operation also were compared. Results Among the 43 children, 41 was admitted as IAA by operation and 2 were diagnosed as coarctation of aorta. According to the type of IAA, 25 cases were diagnosed as type A, 13 cases were diagnosed as type B, 3 cases were diagnosed as type C. Thirty-ifve cases were diagnosed by TTE correctly, 3 cases were misdiagnosed by TTE, 4 cases were suspected as IAA by TTE and ifnally conifrmed by operation. The accuracy rate was 81%(35/43). Among the 35 deifnite diagnosed cases, corrected typing cases were 30, the accuracy rate was 86%(30/35). The accuracy rate of type A, B, C were 96%(23/24), 64%(7/11) and 0. According to the results of CTA, 40 cases were diagnosed correctly, 1 case was misdiagnosed. The accuracy rate was 98%(40/41). Among the deifnite diagnosed cases, corrected typing rate was the same with operation result. Conclusions TTE is the ifrst choice for the detection of IAA. During TTE, in case the indistinct display of aortic arch, CTA should be used to improve the accuracy rate of IAA.
