1.Research progress in screening of early esophageal cancer in China

China is a country with high incidence of esophageal cancer. Precision screening and early diagnosis of esophageal cancer is the premise and key of precise treatment, and also an effective measure to improve the prognosis of esophageal cancer; The mode of combining key screening of high risk groups with screening of opportunistic individual, endoscopy assisted with iodine staining and indicative biopsy is feasible strategy and effective method to screen early esophageal cancer in the stage. Artificial intelligence and molecular biology technology is the research direction and has the development prospect.
2.Preparation and Clinical Use of Rosa Roxburghii Tratt's SOD Oral Liquid

OBJECTIVE:To introduce the preparation of Rosa roxburghii tratt's SOD oral liquid and its therapeutic effect on insomnia.METHODS:To mix Rosa roxburghii tratt's juice with Rhizoma Aractyloidis's decoction in proper proportion.To control the quality by detecting activity of SOD .The therapeutic effect on insomnia was observed in 34 patients .RESULTS:The quality was not changed obviously after storage for 18 months;The effective rate for insomnia was 88.2%. CONCLUSION:The preparation is safe,stable and effective.
3.OBSERVATION OF GASTRIN AND SUBSTANCE P IN THE NERVOUS SYSTEM OF INTESTINE IN PATIENTS WITH DELAYED MOTOR CONSTIPATION

Objective To study the neuropathological changes of gastrin and substance P(SP) in the intermuscular and submucous nerve plexus of the colonic walls in patients with delayed motor constipation(DMC). Methods Gastrin and rabbit SP polyclonal antibiotics were used to make an immunohistochemical staining of the samples of different segments obtained from 10 patients with DMC and 8 normal subjects(control group) for a comparative observation as well as a relative semi-quantitative analysis. Results The immune positive nerve cells of gastrin and SP in the intermuscular nerve plexus of colon with DMC were markedly reduced; no differences in the immune response of gastrin and SP in the mucous nerve plexus were found between the two groups(P<0.01). With routine HE staining, focal inflammation occurred in the mucous membrane of DMC colon and that the neuronal vacuolus of the intermuscular nerve plexus degenerated, reduced and even disappeared. Conclusion The abnormal changes of the neural structure in the immune reponse of gastrin and SP in the intermuscular nerve plexus of colon with DMC might be related to reduction of gastrin and SP peptide neuron or dysfunctional.
4.Clinical observation of blind needle synovial biopsy

Objective To investigate the optimal technology of blind needle synovial biopsy.Methotis Blind needle synovial biopsy was performed on 81 knees with pain and swelling.Twenty patients with rheumatoid arthritis (RA) received uhrasonographic (US) examination before biopsies.Results Synovium were obtained in all patients and the successful rate was (71±21)%,while the area of synovium was (1.8±0.8)mm2.The time for accomplishing the procedure was (26±6) min and the depth of trocar insertion Was (3.1±0.7) cm.The successful rate in patients with RA (80±6)% was much higher than that in non-RA patients (54±10)%.The successful rate with US guidance (85±5)% was much higher than that without US guidance (78±6)%.The positive predictive value of synovium evaluation by naked eye Was 95.0%,while the negative predictive value was 81.1%.Conclusion Blind needle synovial biopsy should be spread because it is simple and safe to perfonn and it can obtain sufficient synovium for all purposes.
6.Prime Comparative Study Between 64-slice Spiral CT Coronary Angiography and Selective Coronary Arteriography

Objective To evaluate the accuracy and feasibility of 64-slice spiral CT (64SCT) in assessing coronary artery disease.Methods A total of 30 suspected patients (male 21 cases, female 9 cases, and mean age of 54.6 years) were undergone both 64SCT and selective coronary angiography (SCA). Volume redering (VR) ,multiplanar reconstruction (MPR), maximum intensity projection (MIP) and transverse section were reconstructed. The results of coronary reconstructions were compared with SCA to analyze the accuracy of the 64SCT in detecting coronary artery stenoses.Results In the 396 segments of coronary artery(diameter≥2 mm)of 30 patients, 385 were judged to be evaluable by 64SCT. The evaluable rate was 97.2%. The sensitivity, specificity, positive predictive value and negative predictive value of the 64SCT in detecting coronary artery stenoses(≥50% of stenosis) were 96.22%, 94.56%, 89.44% and 96.88%. The accuracy rate of 64SCT in detecting ≥50% stenosis of coronary artery was 95.90%.Conclusion 64SCT has high accuracy in detecting coronary artery stenoses, as a noninvasive method,it can be used for screening patients with known or coronary artery diseases.
7.STUDIES OF THE RELATIONSHIP BETWEEN IRON STATUS AND DIETARY PATTERNS IN PRESCHOOL CHILDREN

A nutrition survey, covering a total of 4801 preschool children in 6 provinces, autonomous regions and municipalities, has been carried out. The objective was to determine the effects of various dietary patterns on the blood hemoglobin, free erythrocyte proporphyrin, and serum ferritin values. The results indicated a high prevalence of iron deficiency (average 51.9%). Iron deficiency anemia was detected in 16.4% of the children examined and the incidence rate was particularly high among infants at the age group of 6 months and one year, which was 28.7% and 22.9% respectively. The dietary patterns were different in the above geographic areas. The results of dietary survey revealed that a low incidence of iron deficiency was found in those children who consumed more animal food and protein.
8.Clinical value of narrow-band imaging magnifying endoscopy in early esophageal carcinoma

Objective To explore the diagnosis value of narrow-band imaging magnifying endoscopy (NBI-ME) in the early esophageal cancer and precancerous lesions while estimating the quality,depth and treatment strategy.Methods One hundred and eleven cases of patients with suspected early esophageal cancer and precancerous lesions,who underwent ESD treatment,were selected as the study subjects.To estimate the diagnosis value of NBI-ME in the quality,depth and treatment strategy with pathological histology as gold standard.Results While estimating quality,there was no low grade intraepithelial neoplasia(LGIN) cases with NBI-ME.A total of 33 cases were diagnosed as high grade intraepithelial neoplasia(HGIN),including 30 cases of HGIN and 3 cases of LGIN after ESD treatment,and the other 78 cases were diagnosed as early esophageal cancer,including 5 cases of LGIN,22 cases of HGIN,and 51 cases of early esophageal cancer.Compared with the pathology results,the quality consistency of NBI-ME is general:K=0.498.While estimating depth,there was no LGIN cases with NBI-ME.A total of 33 cases were diagnosed as HGIN,including 30 cases of HGIN and 3 cases of LGIN after ESD treatment,67 cases were diagnosed as intramucosal carcinoma,including 5 cases of LGIN,22 cases of HGIN,30 cases of intramucosal carcinoma,and 10 cases of submucosal carcinoma after ESD treatment,and 11 cases were diagnosed as submucosal carcinoma.Compared with the pathology results,the depth consistency of NBI-ME is general:K=0.469.Most of the patients shoud be treated by ESD except 8 cases of LGIN.Conclusion The study shows general diagnosis value of NBI-ME in estimating quality,depth and treatment strategy of early esophageal cancer and precancerous lesions.
9.Study on Oxidative Damage of Ultraviolet A Radiation to Human Keratinocytes and Its Protection

Objective To investigate the oxidative damage of ultraviolet A (UVA) radiation to primary cultured human keratinocytes and the protective effects of sunscreens from UVA Methods The levels of malondialdehyde (MDA) in supernatant and the activities of catalase (CAT) and superoxide dismutase (SOD) in intra keratinocytes were determined among primary cultured human keratinocytes irradiated by UVA radiation at different dosages The protective effects of sunscreens from UVA radiation were evaluated also Results With the dosages of UVA radiation increasing from 2 88 J/cm 2 up to 11 52 J/cm 2,the levels of MDA increased from 2 54 ?mol/L to 6 75 ?mol/L,the activities of CAT decreased from 15 76 U/mg(prot) down to 6 33 U/mg,the activities of SOD decreased from 25 55 NU/mg(prot) to 23 81 NU/mg(prot) After the primary cultured human keratinocytes were irridiated by UVA radiation with the same dosages as mentioned above through a piece of quartz glass surface of which was smeared by the broad spectrum sunscreens,the levels of MDA increased from 2 18 ?mol/L to 4 82 ?mol/L,the activities of CAT decreased from 17 19 U/mg(prot) to 10 35 U/mg(prot),the activities of SOD decreased from 26 52 NU/mg(prot) to 24 51 NU/mg(prot) Conclusion UVA radiation could cause significant oxidative damage to human keratinocytes Under the conditions of this study,the broad spectrum sunscreens had certain protective effects on keratinocytes from UVA radiation
10.Study on morphology of gastric mucosa in rats with portal hypertension

Objective To investigate the pathogenesis of portal hypertensive gastropathy(PHG). Methods Two rat models with portal hypertension(PHT) and a sham operation group were established to detect the pathological changes in histology and ultrastructure of the gastric mucosa as well as quantitative changes in histological morphology by graphic analysis computer system. Results Prominent edema, scattered red dots/ecchymosis were found in gastric mucosa in rats with PHT. Light and scan electron microscopy showed swelling or exfoliating of the epithelium cell, reduction of gastric gland number, thin of gastric mucosa, while infiltration of inflammatory cells, epithelium metaplasia were not found. The most characteristic findings were the changes of the mucosal capillaries and venules in both mucosal basal lamina and submucosa layer light microscopically, as well as the transmutation, stenosis and loose intercellular joining of the capillaries on electron microscopy. Ultrastructure observation revealed numerous pinocytes in epithelial cells as well as proliferation and hyperplasia of smooth muscle, collagenous fiber and extracellular matrix in venules. Quantitative analysis showed that the ratios of the cross sectional area and the vascular wall area between the gastric wall area(CSA/GWA & VWA/GWA) was higher in liver cirrhosis(LC) and portal vein stenosis(PVS) groups than that in sham operation(SO) group. There was a positive correlation between portal vein pressure and the ratio of the length of damaged mucosa and the length of mucosa(LDM/LM that was higher in LC group than in PVS group). Conclusions In rats with cirrhotic portal hypertension, distinct gastric microvascular morphological changes are the major etiological factor of PHG as a part of pathological changes in portal hypertension.
